This third edition of Conflicts in the Middle East since 1945 analyzes the nature of conflict in the Middle East, with its racial, ethnic, political, cultural, religious and economic factors. Throughout the book Peter Hinchcliffe and Beverley Milton-Edwards put the main conflicts into their wider context, with thematic debates on issues such as the emergence of radical Islam, the resolution of conflicts, diplomacy and peace-making, and the role of the superpowers.

The book is brought fully up to date with events in the Middle East, covering, for instance, developments in Iraq in 2006 where a democratically elected government is in place but the insurgency show no sign of coming under control. The analysis of the Palestinian/Israeli conflict is also brought up to the present day, to include the election of the Hamas government and the 2006 conflict between Israel and Lebanon’s Hizballah.

Including a newly updated bibliography and maps of the area, this is the perfect introduction for all students wishing to understand the complex situation in the Middle East, in its historical context.

OBJECTIVES: To survey the outcomes used in Cochrane Reviews, as part of our work within the Core Outcome Measures in Effectiveness Trials Initiative.

STUDY DESIGN AND SETTING: A descriptive survey of Cochrane Reviews, divided by Cochrane Review Group (CRG), published in full for the first time in 2007 and 2011. Outcomes specified in the methods section of each review and outcomes reported in the results section of each review were of interest, in this exploration of the common use of outcomes and core outcome sets (COS).

RESULTS: Seven hundred eighty-eight reviews, specifying 6,127 outcomes, were included. When we excluded specified outcomes from the 86 reviews that did not include any studies, we found that 1,996 (37%) specified outcomes were not reported. Of the 361 new reviews with studies from 2011, 113 (31%) had a "summary of findings" table (SoF). Fifteen broad outcome categories were identified and used to manage the outcome data. We found consistency in the use of these categories across CRGs but inconsistency in outcomes within these categories.

CONCLUSION: COS have been used rarely in Cochrane Reviews, but the introduction of SoF makes the development and application of COS timelier than ever.

Background: European regional variation in cancer survival was reported in the EUROCARE-4 study for patients diagnosed in 1995-1999. Relative survival (RS) estimates are here updated for patients diagnosed with cancer of the oesophagus, stomach and small intestine from 2000 to 2007. Trends in RS from 1999-2001 to 2005-2007 are presented to monitor and discuss improvements in patient survival in Europe. Materials and methods: EUROCARE-5 data from 29 countries (87 cancer registries) were used to investigate 1- and 5-year RS. Using registry-specific life-tables stratified by age, gender and calendar year, age-standardised 'complete analysis' RS estimates by country and region were calculated for Northern, Southern, Eastern and Central Europe, and for Ireland and United Kingdom (UK). Survival trends of patients in periods 1999-2001, 2002-2004 and 2005-2007 were investigated using the 'period' RS approach. We computed the 5-year RS conditional on surviving the first year (5-year conditional survival), as the ratio of age-standardised 5-year RS to 1-year RS. Results Oesophageal cancer 1- and 5-year RS (40% and 12%, respectively) remained poor in Europe. Patient survival was worst in Eastern (8%), Northern (11%) and Southern Europe (10%). Europe-wide, there was a 3% improvement in oesophageal cancer 5-year survival by 2005-2007, with Ireland and the UK (3%), and Central Europe (4%) showing large improvements. Europe-wide, stomach cancer 5-year RS was 25%. Ireland and UK (17%) and Eastern Europe (19%) had the poorest 5-year patient survival. Southern Europe had the best 5-year survival (30%), though only showing an improvement of 2% by 2005-2007. Small intestine cancer 5-year RS for Europe was 48%, with Central Europe having the best (54%), and Ireland and UK the poorest (37%). Five-year patient survival improvement for Europe was 8% by 2005-2007, with Central, Southern and Eastern Europe showing the greatest increases (≥9%). Conclusions Survival for these cancer sites, particularly oesophageal cancer, remains poor in Europe with wide variation. Further investigation into the wide variation, including analysis by histology and anatomical sub-site, will yield insights to better monitor and explain the improvements in survival observed over time.

Background: High risk medications are commonly prescribed to older US patients. Currently, less is known about high risk medication prescribing in other Western Countries, including the UK. We measured trends and correlates of high risk medication prescribing in a subset of the older UK population (community/institutionalized) to inform harm minimization efforts. Methods: Three cross-sectional samples from primary care electronic clinical records (UK Clinical Practice Research Datalink, CPRD) in fiscal years 2003/04, 2007/08 and 2011/12 were taken. This yielded a sample of 13,900 people aged 65 years or over from 504 UK general practices. High risk medications were defined by 2012 Beers Criteria adapted for the UK. Using descriptive statistical methods and regression modelling, prevalence of ‘any’ (drugs prescribed at least once per year) and ‘long-term’ (drugs prescribed all quarters of year) high risk medication prescribing and correlates were determined. Results: While polypharmacy rates have risen sharply, high risk medication prevalence has remained stable across a decade. A third of older (65+) people are exposed to high risk medications, but only half of the total prevalence was long-term (any = 38.4 % [95 % CI: 36.3, 40.5]; long-term = 17.4 % [15.9, 19.9] in 2011/12). Long-term but not any high risk medication exposure was associated with older ages (85 years or over). Women and people with higher polypharmacy burden were at greater risk of exposure; lower socio-economic status was not associated. Ten drugs/drug classes accounted for most of high risk medication prescribing in 2011/12. Conclusions: High risk medication prescribing has not increased over time against a background of increasing polypharmacy in the UK. Half of patients receiving high risk medications do so for less than a year. Reducing or optimising the use of a limited number of drugs could dramatically reduce high risk medications in older people. Further research is needed to investigate why the oldest old and women are at greater risk. Interventions to reduce high risk medications may need to target shorter and long-term use separately.

Tendo como principal fio condutor a pergunta de partida (Qual o impacto das novas medidas de política educativa para o 1º Ciclo do Ensino Básico e Educação de Adultos na oferta local dos ambientes de aprendizagem?) procuramos encontrar uma resposta para a problemática na qual se centra esta investigação. A partir desta questão inicial delineámos o corpo da dissertação em duas partes: a primeira referente ao enquadramento teórico e a segunda respeitante ao estudo empírico. No âmbito do enquadramento teórico, procedemos a uma reflexão cruzada entre educação e território, no sentido de perceber as suas potenciais (inter) ligações, bem como à análise de alguns dos normativos que dão suporte legal a esta problemática, finalizando com a apresentação do campo de estudo. Ao nível do estudo empírico, seguimos uma metodologia partilhada (qualitativa/ quantitativa), apoiada essencialmente numa aproximação conceptual ao estudo de caso. Os dados recolhidos, por meio de inquérito por questionários aplicados, permitiram-nos conceber várias leituras do território: Cartografia Institucional do Concelho de Gavião e Cartografia Educacional do Concelho de Gavião referente aos anos lectivos de 2004-2005 e 2006-2007, respectivamente, dado o estudo ter decorrido ao longo de um período de três anos lectivos. Da análise dos dados disponíveis, centramos as nossas conclusões em, pelo menos, duas dimensões totalmente opostas. Por um lado, o novo paradigma de “escola a tempo inteiro” assume, actualmente, neste território um papel preponderante, devolvendo aos ambientes formais de educação uma clara liderança no que concerne à quantidade e diversidade de aprendizagens desenvolvidas; por outro, assistimos a um claro abandono das aprendizagens realizadas em espaços não formais, as quais eram dirigidas quase exclusivamente às faixas etárias mais avançadas, que agora vêm as várias possibilidades de aprender, o que quer que seja, como uma oportunidade cada vez mais distante. Face à problemática em estudo, a nossa dissertação termina com a formulação das sugestões e recomendações que nos parecem mais oportunas.

O presente estudo, de carácter quantitativo e qualitativo, assenta na temática do insucesso escolar. O mesmo foi orientado pela questão de partida que o perspectivou: “Quais as causas, consequências e dimensão do fenómeno do insucesso escolar nos alunos que iniciaram, em 1999/2000, a sua escolaridade, no concelho de Ourique? Baseados no quadro conceptual de um estudo de caso, seleccionámos uma “coorte” de alunos, que entrevistámos, com vista a inteirarmo-nos das experiências e situações reais que os conduziram a situações de insucesso escolar procurando dar resposta aos seguintes objectivos: 1) avaliar, quantitativamente, o insucesso escolar na população discente seleccionada; 2) identificar as, eventuais, causas do insucesso escolar verificado; 3) identificar as, eventuais, consequências do insucesso escolar verificado; 4) avaliar possíveis intervenções no sentido de mitigar o fenómeno do insucesso escolar. Para tal, analisámos o percurso escolar de um universo de 46 alunos, entre 1999/2000 e 2007/08, tendo verificado que 24 desses alunos vivenciaram episódios de insucesso escolar, ao longo do seu trajecto académico. Destes, entrevistámos 12 alunos, tendo essa abordagem possibilitado ir de encontro aos nossos objectivos. Através da análise e interpretação dos dados recolhidos, concluímos que: 1) a taxa de insucesso escolar é superior a 50%, na coorte de alunos que iniciaram a sua escolaridade, no ano lectivo 1999/00, no concelho de Ourique; 2) há uma forte participação dos pais e encarregados de educação nas vivências escolares dos alunos (80% do total da amostra); 3) o número médio de retenções, nos alunos com episódios de insucesso escolar, é de 1,5 retenções; 4) o 7.º ano de escolaridade é o ano em que se verifica um maior número de retenções, seguido do 4.º ano de escolaridade; 5) a maioria dos alunos (58,3%) encontra-se relativamente distante do seu meio familiar; 6) um número significativo de alunos com episódios de insucesso escolar (83,3%) referiu que recebe apoio da família; 7) a incapacidade individual foi a causa do insucesso escolar mais referida/inferida pelos entrevistados; 8) evidenciou-se que retenções precoces induzem a retenções posteriores; 9) a importância do 1.º Ciclo é relevante, quando os entrevistados indicam que a falta de apoio no 1.º Ciclo terá induzido a situações futuras de insucesso escolar. No final deste trabalho são efectuadas propostas que possam potenciar futuras investigações, nomeadamente de investigação acção, promovendo intervenção; ### Abstract: The schools failure: The causes of consequences in context the Agrupamento Vertical de Ourique The present study, of quantitative and qualitative content, is based on the thematic of the failure rate in schools. The study was oriented towards the fundamental issue which has been put in perspective: “What are the causes, consequences and dimension of the phenomena of the failure rate in schools that can be seen in the students that started in 1999/2000 their school education in the municipality of Ourique?” Based on the conceptual framework of a study case, we selected a group of students, which we interviewed, in order to know about the real experiences and situations that led them to situations of school failure, trying to give answers and to achieve the following goals: 1) to evaluate quantitatively the school failure of the selected students population; 2) to identify the eventual causes of the verified school failure; 3) to identify the eventual consequences of the verified school failure; 4) to evaluate the possible interventions in order to minimize the school failure phenomena. Therefore, we analysed the school path of an amount of 46 students between 1999/2000 and 2007/2008 and we reached the conclusion that 24 of those students experienced episodes of school failure. We interviewed 12 of those 24 and that approach permitted us to reach our goals. Through the analysis and the interpretation of the collected data we conclude that: 1) the rate of school failure is over 50% in the group of students that started their schooling in the school year 1999/2000 in the municipality of Ourique; 2) there is a strong participation of parents and tutors in the students’ school life( 80% of the sample); 3) the average number of retention in the students with episodes of school failure is 1, 5 retentions; 4) the 7th grade is the one in which we verify the highest number of retentions followed by the 4th grade ; 5) the majority of the students (53,8%) are far away from their family environment; 6) a significant number of students with episodes of school failure (83,3%) referred that they get support from their family; 7) the individual incapacity was the most referred/inferred cause of school failure by the ones who were interviewed ; 8) it became an evidence that early retentions lead to future retentions; 9) the importance of the basic education is relevant when the interviewed students show that the lack of support in the basic education should have led to future situations of school failure. At the end of this work we present some proposals that can lead to future investigation, namely of investigation/action promoting intervention.
