The youth of Massachusetts are of primary concern to legislators and citizens. This briefing report features three essays by experts – Lisa Jones, Ramon Borges-Mendez, and Janis Wolak – who focus on three aspects of youth wellbeing: youth victimization and other indicators of psychological health, youth unemployment, and online sexual predators of youth. Although youth well-being is of primary concern, the worrisome stories about crimes against children that regularly fill the media have unfortunately obscured some more positive news from statistical reports on these same issues. Child victimizations of various types – i.e., child sexual abuse, witnessing domestic violence, child physical abuse, sexual assaults of teenagers, physical assaults and robberies of teenagers, and homicides of teenagers – have been declining nationwide and in Massachusetts since the early 1990s, in some cases declining dramatically.

The future of Brazilian children who have the protection offered by familial bonds is threatened by social inequities that force them to seek shelter and grow up in shelters. According to the Institute of Applied Economic Research, an estimated 20,000 children and adolescents are served by institutions. The majority of these children are afro-descendent males between the ages of seven and fifteen years old. Of those researched, 87.6% have families (58.2% receive visits from their families, 22.7% are rarely visited by their families and 5.8 are legally prohibited from contacting or being by their families). The percentage of children and adolescents “without families” or with “missing families” is 11.3%. There is no information available for 2% of the children and adolescents residing in shelters. The principle factors that necessitate the placement of Brazilian children in institutions that provide care and shelter include poverty (including children forced to work, sell drugs or beg, for example); domestic violence; chemical dependence of parents or guardians; homelessness; death or parents or guardian; imprisonment of their parents; and sexual abuse committed by their parents or guardians. The issue of abandoned children and adolescents and their care and shelter in the Brazilian context expresses a perverse violation of Child and Adolescent Rights.

This paper discusses how perceptions of personal security can impact on school enrolment and attendance. It mainly focuses on threats to physical harm, crime, community and domestic violence. These security fears can include insecurity that children suffer from as they go to school, maybe through the use of unsafe routes; insecurity that children feel at school; and the insecurity they suffer from in their homes. Although poverty is an indicator of insecurity, this paper does not focus solely on poverty as it is well covered elsewhere in the literature. The paper relies on qualitative data collected in Korogocho and Viwandani slum areas in Nairobi, Kenya between October and November 2004. The paper analyses data from individual interviews and focus group interviews and focuses on the narrative of slum dwellers on how insecurity impacts on educational attainment. The conclusion in this paper is that insecure neighbourhoods may have a negative impact on schooling. As a result policies that address insecurity in slum neighbourhoods can also improve school attendance and performance.

Introduction: Domestic violence (DV) affects millions of Americans each year. Yet, there is a dearth of theory-based DV curricula, resulting in physicians unprepared to screen for DV. This study utilized a survey based on constructs from the Transtheoretical Model to assess the readiness of first- and second-year medical students to screen for DV. Information from this survey will be used to help institutionalize DV education into the medical curriculum. [See PDF for complete abstract]

Purpose: to provide commentary of two review articles that discuss the role of medical professionals in combating human trafficking: “Health Care Providers' Training Needs Related to Human Trafficking: Maximizing the Opportunity to Effectively Screen and Intervene” by Isaac, Solak, and Giardino, and “Human Trafficking: What is the Role of the Health Care Provider?” by Crane and Moreno. Findings: Both articles provide a good introduction and explanation of the psychosocial and medical issues faced by many trafficking victims; however, they succeed only to varying degrees in describing all the gaps in the medical system and the vital next steps forward. Conclusion: The key next steps in the fight against human trafficking include: multidisciplinary teams need to improve coordination on all forms of human maltreatment; schools for all medical professions and social work need to significantly strengthen their curriculum on diagnosing and treating human maltreatment; and groups that provide training on human trafficking should partner with other agencies and organizations that provide training on child maltreatment and domestic violence.

OBJECTIVE The number of suicides assisted by right-to-die associations has increased in recent years in Switzerland. The aim of our study was to compare time trends in rates of assisted and unassisted suicide from 1991-2008. METHODS The Swiss National Cohort is a longitudinal study of mortality in the Swiss population; based on linkage of census data with mortality records up to 2008. The Federal Statistical Office coded suspected assisted suicides from 1998 onwards; and from 2003 onwards right-to-die associations reported the suicides they assisted. We used Poisson regression to analyse trends in rates of suicide per 100'000 person-years, by gender and age groups (15-34, 35-64, 65-94 years). RESULTS A total of 7'940'297 individuals and 24'842 suicides were included. In women, rates changed little in the younger age groups but increased in 65-94-year-olds, due to an increase in suicide by poisoning (from 5.1 to 17.2 per 100'000; p <0.001). An increase in suicides by poisoning was also observed in older men (from 8.6 to 18.2; p<0.001). Most suicides by poisoning were assisted. In men, suicide rates declined in all age groups, driven by declines in suicide with firearms. CONCLUSIONS Research is needed to gain a better understanding of the reasons for the tripling of assisted suicide rates in older women, and the doubling of rates in older men, of attitudes and vulnerabilities of those choosing assisted suicide, and of access to palliative care. Rates of assisted suicide should be monitored; including data on patient characteristics and underlying comorbidities.

Background and purpose: Breast cancer continues to be a health problem for women, representing 28 percent of all female cancers and remaining one of the leading causes of death for women. Breast cancer incidence rates become substantial before the age of 50. After menopause, breast cancer incidence rates continue to increase with age creating a long-lasting source of concern (Harris et al., 1992). Mammography, a technique for the detection of breast tumors in their nonpalpable stage when they are most curable, has taken on considerable importance as a public health measure. The lifetime risk of breast cancer is approximately 1 in 9 and occurs over many decades. Recommendations are that screening be periodic in order to detect cancer at early stages. These recommendations, largely, are not followed. Not only are most women not getting regular mammograms, but this circumstance is particularly the case among older women where regular mammography has been proven to reduce mortality by approximately 30 percent. The purpose of this project was to increase our understanding of factors that are associated with stage of readiness to obtain subsequent mammograms. A secondary purpose of this research was to suggest further conceptual considerations toward the extension of the Transtheoretical Model (TTM) of behavior change to repeat screening mammography. ^ Methods. A sample (n = 1,222) of women 50 years and older in a large multi-specialty clinic in Houston, Texas was surveyed by mail questionnaire regarding their previous screening experience and stage of readiness to obtain repeat screening. A computerized database, maintained on all women who undergo mammography at the clinic, was used to identify women who are eligible for the project. The major statistical technique employed to select the significant variables and to examine the man and interaction effects of independent variables on dependent variables was polychotomous stepwise, logistic regression. A prediction model for each stage of readiness definition was estimated. The expected probabilities for stage of readiness were calculated to assess the magnitude and direction of significant predictors. ^ Results. Analysis showed that both ways of defining stage of readiness for obtaining a screening mammogram were associated with specific constructs, including decisional balance and processes of the change. ^ Conclusions. The results of the present study demonstrate that the TTM appears to translate to repeat mammography screening. Findings in the current study also support finding of previous studies that suggest that stage of readiness is associated with respondent decisional balance and the processes of change. ^

There is increasing evidence that childhood victimization and attachment disruptions impact a child’s development. In this study, children and adolescents from an outpatient psychiatric clinic were assessed, measuring history of trauma, history of out-of-home placement, initial diagnoses, and CBCL internalizing and externalizing problem scores. Multiple regression analyses showed that both violent abuse trauma (physical/sexual abuse) and victim trauma (physical abuse/sexual abuse/witnessing domestic violence/witnessing community violence) are prevalent among patients with externalizing severity problems; concluding that diagnosis alone may not account for a history of victimization, but externalizing problem severity does. Overall, the study is consistent with past literature that it is important to acknowledge a child’s history of maltreatment and out-of-home placement when understanding their psychiatric development and diagnosis.

Understanding a population's dietary behavior is important to promote behaviors which have the most beneficial impact on health. The most recent Dietary Guidelines for Americans (2005) identifies carotenoids as a key nutrient to be consumed through increased intake of fruits and vegetables (FV). While some studies have included or focused on the Hispanic population, few have focused only on Mexican-American populations and staged its intake of FV. Stage of change behavior theory has been used to understand the adoption and promotion of healthy behaviors such as increased intake of FV. It has been shown to effectively aid interventionists' understanding of dietary behavior. Intake patterns of FV of older women, rural residents, and adolescents of Mexican American descent have been conducted but not by stages of change. This study aimed to determine the relationship between stages of change for fruits and vegetables (SOC-FV) and total carotene intake to assess the quality of SOC-FV as a surrogate measure of total carotene. ^ Data from the 2000 Qué Sabrosa Vida Community Nutrition Survey (QSV-CNS) were analyzed to identify the SOC-FV and sources of carotenes in a Mexican American population 18-60 yrs. of the Paso del Norte region. A 107 item interviewer administered food frequency questionnaire (FFQ) specifically calibrated for a Mexican American population was used to collect usual intake of total carotene. The QSV survey study population included 963 participants, 590 (61.3%) women and 373 (38.7%) men. A statistically significant mean difference in caloric intake between men and women was found (p-value = <0.01). When total carotene intake was adjusted for energy, there were significant differences between men and women (p-value = <0.0001) with women consuming a higher amount of total carotene (406 RE/kcal 1,000) than men (332 RE/kcal 1000). The food sources of total carotene for both genders included many items found in a traditional Mexican American diet. Chile, after carrots, was the highest contributor of dietary carotene. Total carotene intake was not associated with stages of change among women or men and their distributions were not linear. Mean differences of total carotene by stages of change were significant for women for pre-contemplation/contemplation (p-value = 0.04) and preparation (p-value = 0.0004) but not for men. ^ SOC-FV may serve as a surrogate measure for dietary carotene intake. This study's Mexican American population had a high carotene quality diet derived from traditional food items irrespective of their stage of change for fruits and vegetables. To better understand this population's dietary intake a measure for acculturation should be included. Interventions aimed at Mexican American populations should aim to promote traditional diets consistent with cultural practices.^ ^

Investiga as interferências da violência doméstica na fase diagnóstica, que antecede a Psicoterapia Breve, sua importância no quadro clínico apresentado pelo paciente, se a violência foi verbalizada pelo paciente e a forma como ela foi captada pelo terapeuta e levada em consideração na fase diagnóstica da Psicoterapia Breve Infantil. Foram investigados sinais e sintomas de 29 pacientes de Psicoterapia Breve Infantil. A avaliação das entrevistas iniciais que compõem a fase diagnóstica da Psicoterapia Breve, foi realizada por dois juizes independentes, com índice de concordância igual a 0,87. Foram detectados sinais e sintomas de violência doméstica em 24 dos 29 casos analisados, o que corresponde a 83% do total de pacientes. A violência doméstica foi considerada significativa para uma situação-problema a ser trabalhada em análise em 54% dos casos, o que mostra que é uma situação de limiar, em que 50% das percepções são verdadeiras e a outra metade são falsas. Dos 13 casos em que a violência doméstica foi significativa pelos juízes, apenas 54% estavam incluídas na compreensão psicodinâmica da queixa, pelo terapeuta. A inclusão da violência doméstica no foco da Psicoterapia Breve foi observada somente em 2 casos, o que corresponde a 07% da amostra geral, muito perto da incidência de mecanismos psicóticos que prejudicam a socialização (08%). Os resultados obtidos foram discutidos segundo a literatura psicanalítica, que considera a violência como a parte agressiva do self que ainda não está integrada ao self, permanecendo dissociada do conluio da família e às vezes do próprio psicoterapeuta. Tais resistências em lidar com a violência não permitem que a agressividade seja usada de forma construtiva pelo self, promovendo a socialização da criança pelo fortalecimento dos sentimentos de amizade e favorecendo o desenvolvimento de vínculos emocionais saudáveis na dupla criança e psioterapeuta.(AU)

Este artículo tiene como objetivo examinar el impacto y las consecuencias sociales contemporáneas del mantenimiento del sistema de organización social denominado «reserva indígena» en Canadá. Mediante un estudio de caso, se explora cómo la política organizacional y la estructura institucional contemporánea en las reservas indígenas canadienses están alimentando problemas psicosociales y de salud mental como, por ejemplo, abuso de sustancias psicoactivas, depresión, negligencia parental o violencia doméstica. Tres características estructurales se identifican como problemas inherentes en la estructura organizacional de las reservas contemporáneas: el desempleo estructural, la dependencia de programas de asistencia social y el faccionalismo comunitario.

A violência doméstica tornou-se um fenómeno social de grande complexidade, que não pode ser tratado de forma superficial, seja por parte daqueles que intervêm tecnicamente, seja por parte das vítimas e agressores, pois trata-se da vida real de muitos indivíduos ou sujeitos. Deste modo, tornou-se necessário analisar a violência doméstica, procurando perceber o papel dos agentes, em especial dos técnicos, que acompanham as vítimas de violência doméstica na construção do seu projecto de vida. Para a realização deste estudo, fiz uma abordagem dos diferentes conceitos de violência doméstica, fundamental neste trabalho, em virtude da complexidade e controvérsia que a sua definição implica para diferentes autores e abordagens teóricas. Naturalmente que, se o tema da violência doméstica atravessa todo o trabalho, era indispensável ouvir as vítimas desta problemática quanto ao seu sofrimento, coragem e resiliência, ou abandono do lar e acompanhamento recebido. Assim sendo, e porque pretendi fazer uma análise à forma como é realizado o acompanhamento e intervenção por parte dos técnicos, este trabalho apresenta ainda uma investigação empírica realizada através da aplicação de inquéritos por questionário vítimas e não vítimas de violência doméstica, bem como a caracterização dos dados sociodemográficos do distrito de Portalegre. O tratamento e análise dos dados permitiram confirmar, em larga medida, a fundamentação teórica apresentada na primeira parte do estudo onde se verificou haver a divergência no conceito de violência doméstica, as vítimas serem predominantemente do sexo feminino e os agressores do sexo masculino, os vários motivos que levam as vítimas a permanecer na relação e a importância do papel dos técnicos de acompanhamento na definição de projectos de vida alternativos.
