O bloqueio contínuo de nervos periféricos (BCNP), descrito inicialmente em 1946 (1), consiste na colocação e utilização de um cateter percutâneo adjacente a um ou vários nervos periféricos para administração de anestésico local. A utilização de cateteres perineurais permite prologar a analgesia associada a técnica single shot, que dura, habitualmente, 24 horas. Apesar de bem estudado para analgesia no pos-operatório, o BCNP tem actualmente várias indicações descritas como tratamento de soluços(2), de vasospasmo associado a Doença de Raynaud (3), simpaticectomia e vasodilatação após cirurgia vascular (4), reimplantação de membros amputados(5), analgesia em trauma(6) e tratamento de dor cronica (7). Apesar de estar provado que no tratamento da dor pós-operatória o uso de cateteres perineurais permite uma analgesia mais eficaz comparativamente a analgesia com opióides sistémicos (reduzindo assim a incidência de complicações associadas ao uso destes) (8) e que melhora, a curto prazo, outcomes funcionais após cirurgia da extremidade, não esta evidenciado o beneficio desta técnica regional a longo prazo (9,10,11). Considerando a eficácia da técnica loco-regional para o tratamento da dor neuropática associada a doença arterial periférica (12,13) e a segurança associada ao bloqueio de nervo periférico (8,9,10,11), relata-se um caso de utilização desta técnica num doente para tratamento da agudização de dor crónica secundária a um agravamento da isquémia do membro inferior.

Os autores propuseram-se avaliar o tratamento da dor aguda no trabalho de parto e parto, (88 casos) e no post-operatório de cesariana (53 casos). Para a avaliação da dor utilizaram a VAS e a escala descritiva de dor; o grau de satisfação materna foi inquirido às 48 horas de internamento. A forma de administração influenciou a eficácia analgésica na altura do parto. Quanto à avaliação do pós-operatório às 2 horas após cesariana 24,5% das puérperas referiram VAS >3 e às 6 horas 20,8%, apesar de terem prescrição fixa de analgésicos. Às 24 horas 40% das puérperas referiram VAS >3 e às 48 horas 30%. Das 141 puérperas, 56 consideraram-se muito satisfeitas, 84 satisfeitas e apenas 1 insatisfeita.

INTRODUCTION: Labour is considered to be one of the most painful and significant experiences in a woman's life. The aim of this study was to examine whether women's attachment style is a predictor of the pain experienced throughout labour and post-delivery. MATERIAL AND METHODS:Thirty-two pregnant women were assessed during the third trimester of pregnancy and during labour. Adult attachment was assessed with the Adult Attachment Scale ' Revised. The perceived intensity of labour pain was measured using a visual analogue scale for pain in the early stage of labour, throughout labour and post-delivery. RESULTS:Women with an insecure attachment style reported more pain at 3 cm of cervical dilatation (p < 0.05), before the administration of analgesia (p < 0.01) and post-delivery (p < 0.05) than those securely attached. In multivariate models, attachment style was a significant predictor of labour pain at 3 cm of cervical dilatation and before the first administration of analgesia but not of the perceived pain post-delivery. DISCUSSION: These findings confirm that labour pain is influenced by relevant psychological factors and suggest that a woman's attachment style may be a risk factor for greater pain during labour. CONCLUSION:Future studies in the context of obstetric pain may consider the attachment style as an indicator of individual differences in the pain response during labour. This may have important implications in anaesthesiology and to promote a relevant shift in institutional practices and therapeutic procedures.

A colecistite aguda acalculosa é uma entidade pouco frequente na idade pediátrica e uma complicação rara da enterocolite por salmonelas não tifóides. A co-infecçãocom o virus influenza A (H1N1)v nunca foi previamente descrita. Caso Clinico: Rapaz de 10 anos de idade, previamente saudável, com febre elevada, cefaleias , mialgias, vómitos e diarreia. a RT-PCR para virus influenza A, subtipo H1N1v foi postiva no exsudado nasofaríngeo mas não foi prescrito oseltamivir. Ao quinto dia da doença por persistência do quadro clinico com agravamento da sintomatologia gastrointestinal recorreu à urgência. Foi internado com sinais de desidratação grave, insuficiência pré-renal aguda e oligúria, tendo-se procedido a analgesia , antipiréticos e hidratação endovenosa. Foi isolada Salmonella Enteriditis nas coproculturas. Ao 9º dia de doença houve agravamento da dor abdominal associando-se sinal de Murphy. A ecografia abdominal revelou sinais de colecistite aguda e adenopatia a comprimir infundíbulo da vesicula biliar tendo-se iniciado antibioterapia. Os sintomas persisitiram durante cinco dias, verificando-se posterior evolução clinica favorável. Discussão: A etiologia da colecistite acalculosa foi provavelmente multifatorial para a qual contribuíram a febre, a desidratação, os analgésicos, o jejum prolongado e a infecção por salmonela, cujo ciclo envolve a via biliar. A compressão pelos gânglios hipertrofiados na sequência da infecção H1N1v foi provavelmente um fator adicional, sendo questionável se a terapêutica prévia com oseltamivir teria modificado o prognóstico.

This research addresses the problem of creating interactive experiences to encourage people to explore spaces. Besides the obvious spaces to visit, such as museums or art galleries, spaces that people visit can be, for example, a supermarket or a restaurant. As technology evolves, people become more demanding in the way they use it and expect better forms of interaction with the space that surrounds them. Interaction with the space allows information to be transmitted to the visitors in a friendly way, leading visitors to explore it and gain knowledge. Systems to provide better experiences while exploring spaces demand hardware and software that is not in the reach of every space owner either because of the cost or inconvenience of the installation, that can damage artefacts or the space environment. We propose a system adaptable to the spaces, that uses a video camera network and a wi-fi network present at the space (or that can be installed) to provide means to support interactive experiences using the visitor’s mobile device. The system is composed of an infrastructure (called vuSpot), a language grammar used to describe interactions at a space (called XploreDescription), a visual tool used to design interactive experiences (called XploreBuilder) and a tool used to create interactive experiences (called urSpace). By using XploreBuilder, a tool built of top of vuSpot, a user with little or no experience in programming can define a space and design interactive experiences. This tool generates a description of the space and of the interactions at that space (that complies with the XploreDescription grammar). These descriptions can be given to urSpace, another tool built of top of vuSpot, that creates the interactive experience application. With this system we explore new forms of interaction and use mobile devices and pico projectors to deliver additional information to the users leading to the creation of interactive experiences. The several components are presented as well as the results of the respective user tests, which were positive. The design and implementation becomes cheaper, faster, more flexible and, since it does not depend on the knowledge of a programming language, accessible for the general public.

One of the major challenges in the development of an immersive system is handling the delay between the tracking of the user’s head position and the updated projection of a 3D image or auralised sound, also called end-to-end delay. Excessive end-to-end delay can result in the general decrement of the “feeling of presence”, the occurrence of motion sickness and poor performance in perception-action tasks. These latencies must be known in order to provide insights on the technological (hardware/software optimization) or psychophysical (recalibration sessions) strategies to deal with them. Our goal was to develop a new measurement method of end-to-end delay that is both precise and easily replicated. We used a Head and Torso simulator (HATS) as an auditory signal sensor, a fast response photo-sensor to detect a visual stimulus response from a Motion Capture System, and a voltage input trigger as real-time event. The HATS was mounted in a turntable which allowed us to precisely change the 3D sound relative to the head position. When the virtual sound source was at 90º azimuth, the correspondent HRTF would set all the intensity values to zero, at the same time a trigger would register the real-time event of turning the HATS 90º azimuth. Furthermore, with the HATS turned 90º to the left, the motion capture marker visualization would fell exactly in the photo-sensor receptor. This method allowed us to precisely measure the delay from tracking to displaying. Moreover, our results show that the method of tracking, its tracking frequency, and the rendering of the sound reflections are the main predictors of end-to-end delay.

The authors propose a mathematical model to minimize the project total cost where there are multiple resources constrained by maximum availability. They assume the resources as renewable and the activities can use any subset of resources requiring any quantity from a limited real interval. The stochastic nature is inferred by means of a stochastic work content defined per resource within an activity and following a known distribution and the total cost is the sum of the resource allocation cost with the tardiness cost or earliness bonus in case the project finishes after or before the due date, respectively. The model was computationally implemented relying upon an interchange of two global optimization metaheuristics – the electromagnetism-like mechanism and the evolutionary strategies. Two experiments were conducted testing the implementation to projects with single and multiple resources, and with or without maximum availability constraints. The set of collected results shows good behavior in general and provide a tool to further assist project manager decision making in the planning phase.

É meu propósito, neste texto, discutir a Lusofonia através de um conjunto de conceitos ligados aos média digitais interativos, rompendo os limites da ligação entre produtores e recetores de conteúdos nas relações multi, inter e transculturais. Vou insistir na ideia de que os média digitais interativos significam novas práticas nas relações interculturais: por um lado, formas digitais interativas de comunicação intercultural e, por outro, textualidades multimodais (“hipertextualidades”) na produção do sentido lusófono. O percurso que proponho tem um cariz preponderantemente epistemológico. Tomando a Lusofonia como figura de interesse geoestratégico e cultural e os média digitais como objeto de análise, é minha preocupação fundamental interrogar a inovação, a hibridez e a interatividade digitais e verificar de que modo se articulam com as relações multi, inter e transculturais. Depression is an extremely heterogeneous disorder. Diverse molecular mechanisms have been suggested to underlie its etiology. To understand the molecular mechanisms responsible for this complex disorder, researchers have been using animal models extensively, namely mice from various genetic backgrounds and harboring distinct genetic modifications. The use of numerous mouse models has contributed to enrich our knowledge on depression. However, accumulating data also revealed that the intrinsic characteristics of each mouse strain might influence the experimental outcomes, which may justify some conflicting evidence reported in the literature. To further understand the impact of the genetic background, we performed a multimodal comparative study encompassing the most relevant parameters commonly addressed in depression, in three of the most widely used mouse strains: Balb/c, C57BL/6, and CD-1. Moreover, female mice were selected for this study taken into account the higher prevalence of depression in women and the fewer animal studies using this gender. Our results show that Balb/c mice have a more pronounced anxious-like behavior than CD-1 and C57BL/6 mice, whereas C57BL/6 animals present the strongest depressive-like trait. Furthermore, C57BL/6 mice display the highest rate of proliferating cells and brain-derived neurotrophic factor (Bdnf) expression levels in the hippocampus, while hippocampal dentate granular neurons of Balb/c mice show smaller dendritic lengths and fewer ramifications. Of notice, the expression levels of inducible nitric oxide synthase (iNos) predict 39.5% of the depressive-like behavior index, which suggests a key role of hippocampal iNOS in depression. Overall, this study reveals important interstrain differences in several behavioral dimensions and molecular and cellular parameters that should be considered when preparing and analyzing experiments addressing depression using mouse models. It further contributes to the literature by revealing the predictive value of hippocampal iNos expression levels in depressive-like behavior, irrespectively of the mouse strain.

Introduction: Acute intrathoracic gastric volvulus occurs when the stomach has a twist mesenteroaxial/organoaxial or chest cavity resulting in a dilatation or rupture of the diaphragmatic hiatus or diaphragmatic hernia. The purpose of this work is to show a interesting case of gastric volvulus in a patient with several comorbidities. Case Report: A 77-year-old female with past history of hiatal hernia and mental disease associated with diabetes and atrial fibrillation. Patient went to the emergency department due to vomiting associated with blood. Analytical parameters (WBC, HGB, PCR, metabolic panel and liver function), showed no significant alterations. Thoracic X-ray revealed an enlarged mediastinum due to herniation of the stomach. A computed tomography (CT) scan confirmed intrathoracic localization of the gastric antrum with twist. Patient’s symptoms were relieved by nasogastric intubation and analgesia. After six months, the patient is still asymptomatic. Conclusion: In general, the treatment of an acute gastric volvulus requires an emergent surgical repair. In patients who are not surgical candidates (with comorbidities or an inability to tolerate anesthesia), endoscopic reduction should be attempted. Chronic gastric volvulus may be treated non-emergently, and surgical treatment is increasingly being performed using a laparoscopic approach. In this case, it is a chronic form that was solved with the placement of the nasogastric tube. A nasogastric decompression is an option in the chronic form of hiatal hernia associated to gastric volvulus in patients with serious comorbidities.

The relaxivity displayed by Gd3+ chelates immobilized onto gold nanoparticles is the result of complex interplay between nanoparticle size, water exchange rate and chelate structure. In this work we study the effect of the length of -thioalkyl linkers, anchoring fast water exchanging Gd3+ chelates onto gold nanoparticles, on the relaxivity of the immobilized chelates. Gold nanoparticles functionalized with Gd3+ chelates of mercaptoundecanoyl and lipoyl amide conjugates of the DO3A-N-(-amino)propionate chelator were prepared and studied as potential CA for MRI. High relaxivities per chelate, of the order of magnitude 28-38 mM-1s-1 (30 MHz, 25 ºC) were attained thanks to simultaneous optimization of the rotational correlation time and of the water exchange rate. Fast local rotational motions of the immobilized chelates around connecting linkers (internal flexibility) still limit the attainable relaxivity. The degree of internal flexibility of the immobilized chelates seems not to be correlated with the length of the connecting linkers. Biodistribution and MRI studies in mice suggest that the in vivo behavior of the gold nanoparticles is determined mainly by size. Small nanoparticles (HD= 3.9 nm) undergo fast renal clearance and avoidance of the RES organs while larger nanoparticles (HD= 4.8 nm) undergo predominantly hepatobiliary excretion. High relaxivities, allied to chelate and nanoparticle stability and fast renal clearance in vivo suggests that functionalized gold nanoparticles hold great potential for further investigation as MRI Contrast Agents. This study contributes to understand the effect of linker length on the relaxivity of gold nanoparticles functionalized with Gd3+ complexes. It is a relevant contribution towards “design rules” for nanostructures functionalized with Gd3+ chelates as Contrast Agents for MRI and multimodal imaging.

OBJETIVO: Apresentar uma revisão sobre as características da atividade elétrica cerebral que acompanha a hipnose animal, estado induzido em laboratório em mamíferos por manipulações experimentais, bem como sobre as alterações encontradas no EEG durante o estado de hipnose, visando à discussão dos resultados encontrados na busca de evidências dos fundamentos filogenéticos que possam conduzir ao entendimento dos rudimentos neurais da hipnose humana. MÉTODO: Livros e bases eletrônicas de dados foram consultados. Critério de inclusão: artigos originais publicados entre 1966-2012. Critério de exclusão: artigos que se afastavam da visão eletroneurofisiológica da hipnose. RESULTADOS: Foram encontradas 662 referências, tendo sido selecionados os artigos e livros referenciados. Além desses artigos, foi incluído no estudo o artigo de Hoagland, publicado em 1928, que é um clássico na área de imobilidade tônica em vertebrados. CONCLUSÕES: O estado de hipnose humano resulta de processamentos em inúmeros circuitos paralelos distribuídos em uma complexa rede neuronal, envolvendo, dessa forma, uma ampla área do encéfalo. Na trajetória evolutiva, a grande ampliação dos recursos corticais pode ter tornado as respostas de imobilidade tônica passíveis de modulação consciente, respostas essas ainda presentes nos humanos e que se manifestam involuntariamente em situações de grande ameaça. Vários estudos têm evidenciado mecanismos neurofisiológicos capazes de reforçar a visão da hipnose não só como um eficiente recurso para procedimentos médicos e odontológicos, funcionando como auxiliar na analgesia e sedação, mas também como excelente ferramenta psicoterapêutica.
