Os sistemas de informação têm cada vez mais importância nos serviços de saúde. Sistemas de informação eficientes, permitem a maximização da gestão dos serviços promovendo a melhoria da qualidade dos cuidados. A ordem dos enfermeiros sugere a implementação de sistemas de registos que englobem necessidades de cuidados, intervenções de enfermagem e resultados dessas mesmas intervenções. A associação portuguesa de enfermeiros faz referência à importância de uma linguagem comum entre os enfermeiros. A linguagem CIPE® “é uma classificação de fenómenos, intervenções e resultados a nível da enfermagem. Pretende ser uma matriz unificadora em que as taxonomias e classificações de enfermagem já existentes se possam entrecruzar, de modo a permitir comparar dados em enfermagem” (CIE,2000:11) A adaptação da linguagem CIPE à gravidez, trabalho de parto e parto, é uma mais-valia no que concerne á realização de registos de qualidade que permitam uma linguagem comum, abrangente e facilitadora da comunicação entre os enfermeiros, e entre estes e outros profissionais de saúde. Através da consulta de bibliografia, dos manuais CIPE® disponíveis, de documentos existentes no serviço, dos processos clínicos e pelos conhecimentos adquiridos durante a especialização de enfermagem de saúde materna e obstetrícia, foi feito o levantamento dos fenómenos, diagnósticos e intervenções de enfermagem em relação aos focos gravidez, trabalho de parto e parto, para futura introdução no sistema informático, tendo em conta as actividades mais frequentes na maternidade para estes focos. A definição de uma Linguagem uniforme a ser utilizada por todos os enfermeiros do serviço, levou a um esforço conjunto da equipa de enfermagem e chefia. Este trabalho continua em curso, uma vez que as actividades que se realizam na maternidade têm que ser todas parametrizadas, o que torna o trabalho de parametrização um pouco mais moroso. Os dados só serão introduzidos no sistema depois da parametrização de todas actividades do serviço. Após a introdução dos dados no sistema informático e da linguagem CIPE perfeitamente adaptada à realidade da maternidade, é nossa convicção que a economia de tempo e a desburocratização dos registos libertará os profissionais para uma relação enfermeiro/utente que se pretende mais humana.

Sexual selection theory predicts that, in organisms with reversed sex roles, more polyandrous species exhibit higher levels of sexual dimorphism. In the family Syngnathidae (pipefish, seahorses, and seadragons), males provide all parental care by carrying developing embryos on their ventral surfaces, and females develop secondary sex characters. Syngnathids exhibit a variety of genetic mating patterns, making them an ideal group to test predictions of sexual selection theory. Here, we describe the mating system of the black-striped pipefish Syngnathus abaster, using 4 highly variable microsatellites to analyze parentage of 102 embryos. Results revealed that 1) both sexes mate multiple times over the course of a pregnancy (polygynandrous mating system), 2) eggs are spatially segregated by maternity within each brood pouch, and 3) larger females have higher mating success (Kolmogorov–Smirnov test; P < 0.05). Together with similar studies of other syngnathid species, our results support the hypothesis that the mating system is related to the intensity of sexual dimorphism.

The vitamin A is essential to animals because of its participation in a great number of biological functions. The investigation of this vitamin s concentrations is important to serve as reference to normality parameters. This study had as aim to analyse the serics and hepatics concentrations of vitamin A in two groups of bovines and to compare the hepatics concentrations to the present requeriments of vitamin A for pregnant women. It was also appraised the consume habit of bovine liver by pregnant women through of the alimentary frequency quest. Two groups of bovine were studied and the first was formad by Nelore bovine breed and the second by bovine without defined breed (WDB). It was analysed 120 samples: 60 of liver and 60 of serum. The method used to dose retinol was High Performance Liquid Cromatography (HPLC). The average (+ sd) of retinol concentrations in Nelore breed bovine and WDB liver were 16947,8 + 6866,9 and 5213,1 + 2517,2 µg of retinol/100g and at serum 39,6 + 17,9 e 28,6 + 9,4 µg of retinol/dL, respectively. No statistically significant correlation was found between hepatic and the serum retinol. The bovines in this study had adequate vitamin A levels. Independently of animal breed, the daily ingestion of bovine liver is not advised for pregnant women who show adequate support of vitamin A. A literatura tem referido que a prematuridade do bebé gera nas mães níveis mais elevados de ansiedade, sentimentos depressivos, assim como níveis mais baixos de autoestima, comparativamente com mães de bebés de termo, o que poderá conduzir a um envolvimento emocional mais negativo com o recémnascido. Essa vivência parece variar em função da paridade das mães (i.e., mãe pela primeira vez vs. multípara) não obstante não serem muito claras as particularidades desta variável. Este estudo teve como objetivo analisar num grupo de mães primíparas e multíparas com bebés prematuros, as associações entre o estado Psicoemocional, a Autoestima e o Bonding maternos. Participaram 50 mães (27 primíparas e 23 multíparas), com bebés pré-termo recrutados numa maternidade pública de Lisboa. Os resultados indicam que as mães multíparas apresentam níveis mais elevados de Sentimentos Depressivos expressando, também, uma maior Capacidade e Preparação Geral para a função materna. A Capacidade Geral para tratar do bebé surge associada de forma negativa com o seu estado Psicoemocional e de forma positiva com o Bonding. As associações negativas encontradas entre Ansiedade e Aceitação do bebé, e entre Estado Psicoemocional e Bonding não parecem variar em função da paridade, mostrando que os desafios se colocam a ambos os grupos de mães.

Objectives: As stillbirth has a devastating impact, it is imperative to understand the importance of clinical and emotional care after stillbirth and how it influences subsequent pregnancies. The aim of the study was to gain insight into the consideration and planning of a subsequent pregnancy by parents in the weeks following stillbirth. Design: A qualitative semi-structured interview format was utilized. Interpretative phenomenological analysis was employed as the analytic strategy. Participants and setting: The recruitment strategy focused on couples whereby the parents of ten stillborn babies were contacted; however, five men declined to participate in the study. The final sample of 15 parents were all Irish: ten of whom were female and five of whom were male. Results: Findings revealed two superordinate themes relating to a subsequent pregnancy after stillbirth: aspirations for future pregnancy and expectations of future care. Parents disclosed how the prospect of a subsequent pregnancy was daunting with fears about the potential loss of another child. Despite these fears, parentsâ aspirations differed in the days following stillbirth; mothers wished to plan a future pregnancy while fathers were reluctant to consider any pregnancies. Parents were unsure of what to expect in terms of the level of care that would be provided to them in a subsequent pregnancy. Additional appointments at the maternity hospital were considered crucial to provide reassurance during a subsequent pregnancy. Conclusions: These findings underscore the far-reaching and contrasting effects of stillbirth on parents. BACKGROUND: Conceptualization of quality of care - in terms of what individuals, groups and organizations include in their meaning of quality, is an unexplored research area. It is important to understand how quality is conceptualised as a means to successfully implement improvement efforts and bridge potential disconnect in language about quality between system levels, professions, and clinical services. The aim is therefore to explore and compare conceptualization of quality among national bodies (macro level), senior hospital managers (meso level), and professional groups within clinical micro systems (micro level) in a cross-national study. METHODS: This cross-national multi-level case study combines analysis of national policy documents and regulations at the macro level with semi-structured interviews (383) and non-participant observation (803 hours) of key meetings and shadowing of staff at the meso and micro levels in ten purposively sampled European hospitals (England, the Netherlands, Portugal, Sweden, and Norway). Fieldwork at the meso and micro levels was undertaken over a 12-month period (2011-2012) and different types of micro systems were included (maternity, oncology, orthopaedics, elderly care, intensive care, and geriatrics). RESULTS: The three quality dimensions clinical effectiveness, patient safety, and patient experience were incorporated in macro level policies in all countries. Senior hospital managers adopted a similar conceptualization, but also included efficiency and costs in their conceptualization of quality. 'Quality' in the forms of measuring indicators and performance management were dominant among senior hospital managers (with clinical and non-clinical background). The differential emphasis on the three quality dimensions was strongly linked to professional roles, personal ideas, and beliefs at the micro level. Clinical effectiveness was dominant among physicians (evidence-based approach), while patient experience was dominant among nurses (patient-centered care, enough time to talk with patients). Conceptualization varied between micro systems depending on the type of services provided. CONCLUSION: The quality conceptualization differed across system levels (macro-meso-micro), among professional groups (nurses, doctors, managers), and between the studied micro systems in our ten sampled European hospitals. This entails a managerial alignment challenge translating macro level quality definitions into different local contexts.

In Spain, immigrant women have high rates for initiating breastfeeding. In contrast, the case of immigrant Chinese mothers stands out, due to the low rate. In China, breastfeeding has historically been the cultural norm. An ethnographic study was conducted to explore aspects related to the low rate of breastfeeding. Field observations and informal interviews were conducted in two hospitals and a primary care center. Semi-structured interviews were performed with Chinese mothers and health workers. Among Spanish health workers, there is a belief that Chinese women do not breastfeed due to cultural reasons. The rapid return to work and the popular phenomenon of sending babies back to China for grandparents to raise constitute barriers for breastfeeding. Although in China breastfeeding is common practice, in Madrid the prioritization of productivity over reproduction and the existence of the so-called “satellite babies” and transnational maternity make the establishment and maintenance of breastfeeding difficult.

The findings in this summary are based on the Iowa Barriers to Prenatal Care project. Ongoing since 1991, the purpose of this project is to obtain brief, accurate information about women delivering babies in Iowa hospitals. Specifically, the project seeks to learn about women’s experiences getting prenatal or delivery care during their current pregnancy. Other information is included which may be pertinent to health planners or those concerned with the systematic development of health care services. This project is a cooperative venture of all of Iowa’s maternity hospitals, the University of Northern Iowa Center for Social and Behavioral Research, and the Iowa Department of Public Health. The Robert Wood Johnson Foundation funded the first three years of this project. The current funding is provided by the Iowa Department of Public Health. The Director is Dr. Mary Losch, University of Northern Iowa Center for Social and Behavioral Research. The Coordinator for the project is Rodney Muilenburg. The questionnaire is distributed to nearly ninety maternity hospitals across the state of Iowa. Nursing staff or those responsible for obtaining birth certificate information in the obstetrics unit are responsible for approaching all birth mothers prior to dismissal and requesting their participation in the study. The questionnaire takes approximately ten minutes to complete. Completed questionnaires are returned to the University of Northern Iowa Center for Social and Behavioral Research for data entry and analysis. Returns are made monthly, weekly, or biweekly depending on the number of births per week in a given hospital. Except in the case of a mother who is too ill to complete the questionnaire, all mothers are eligible to be recruited for participation. The present yearly report includes an analysis of large Iowa cities, African American mothers, and a trend analysis of the last ten years. Also presented in this report is a frequency analysis of all variables included in the 2012 questionnaire. Unless otherwise noted, all entries reflect percentages. Please note that because percentages were rounded, total values may not equal 100%. Data presented are based upon 2012 questionnaires received to date (n = 23,674). All analyses reflect unweighted percentages of those responding.

Objetivo: Identificar factores sociodemográficos y de fecundidad, asociados a ocurrencia de embarazo no deseado en mujeres colombianas en edad reproductiva en el año 2010. Métodos: Se realizó estudio retrospectivo de corte transversal, basado en los datos de la ENDS Colombia-2010, del total de mujeres en edad fértil (13-49 años) que al momento de la encuesta se encontraban en embarazo. La variable de interés fue embarazo no deseado, se describió la población a estudio y se evaluó la posible asociación con variables sociodemográficas y de fecundidad, a través de análisis bivariado y multivariado. Se realizaron los mismos análisis por grupo de edad (adolescentes vs adultas). Resultados: La prevalencia de embarazo no deseado en las mujeres colombianas en el 2010 fue de 61,4 %. De acuerdo al modelo de regresión logística, no estar en unión a una pareja (OR: 4,01 IC95%: 3,066-5,269), tener hijos (OR: 2,040 IC95%: 1,581 – 2,631), estar en el quintil de menor riqueza (OR: 2,137 IC95%: 1,328-3,440), y ser adolescente (OR: 1,599 IC95%: 1,183-2,162), son factores que aumentan la probabilidad de tener un embarazo no deseado. Se encontraron diferencias en los factores asociados al realizar segmentación por edad. Conclusiones: La prevalencia de embarazo no deseado permanece alta en Colombia respecto a años anteriores y a otros países. Los resultados pueden ser de utilidad para el desarrollo de políticas en salud sexual y reproductiva teniendo en cuenta los factores asociados identificados priorizando a la población adolescente y de menor estatus socioeconómico, para la prevención de embarazo no deseado en Colombia.

“O Reiki para Cuidar de Quem Cuida” surge de vivências hospitalares de profissionais de saúde que solicitam e recorrem a sessões do Projeto “Terapia de Reiki/Shiatsu aos profissionais de Saúde do serviço de obstetrícia e bloco de partos”, referindo dores, stress, ansiedade e mal-estar. Objetivos: dar visibilidade ao Reiki como técnica terapêutica no cuidar e avaliar o Burnout dos profissionais de saúde antes e após sessão de Reiki. Metodologia: sensibilização para Reiki e Burnout, aplicação de questionários, tratamento de dados, pesquisa bibliográfica. Resultados: os profissionais de saúde necessitam de mais informação e de mais sessões de Reiki que referem ser muito úteis e promotoras de bem-estar; encontram-se em nível Médio de Burnout, reduzindo os valores após o Reiki. Conclusões: o Reiki reduz o Burnout, a sua inclusão na prática de enfermagem traduz-se em ganhos em saúde para si e para os utentes; ABSTRACT: Title: Reiki: Caring for Those who Care “Reiki: Caring for Those who Care” arised from health personnel’s experiences who request and resort to the project’s sessions “Reiki Therapy/Shiatsu to the Health Personnel of the Maternity and of the Obstetric/Gynecological Emergency Department”, who refer pain, stress, anxiety and malaise. Objectives: Presenting Reiki as a therapeutic technique in caring and evaluate health personnel’s Burnout before and after Reiki sessions. Methods: Raising awareness to Reiki and Burnout, questionnaires, data treatment, bibliographic research. Results: Health personnel need more information and more Reiki sessions, which refer being useful and wellbeing promoters; they experience a medium level of Burnout, having this value decreased after Reiki sessions. Conclusions: Reiki reduces Burnout, and its inclusion in the nursing practice results in health personnel’s and patient’s health outcomes.

O aleitamento materno exclusivo é uma recomendação universal e o contacto pele-a-pele mãe/recém-nascido é fator de desenvolvimento para a díade. O trabalho académico desenvolvido centra-se nos indicadores de medida e evidência - amamentação na 1ª hora de vida & contacto pele a pele entre a díade. Tal enraíza-se no projeto Maternidade com Qualidade, da Ordem dos Enfermeiros Portugueses e em curso no Centro Hospitalar Barreiro-Montijo EPE. O projeto iniciou-se em setembro de 2015, seguindo-se os procedimentos que se relatam no atual documento. No diagnóstico de situação recolheu-se o estado dos indicadores. A vertente de investigação considerou como amostras profissionais e utentes do Centro Hospitalar Barreiro-Montijo e recolheu dados. A intervenção realizada produziu evolução positiva nos indicadores. O bonding em período sensível é reconhecido pelos profissionais como um elemento relevante nos cuidados. Com a implementação deste projeto contribuiu-se para a melhoria da qualidade dos cuidados na área de saúde materna; Mother-Child early relationship. Evaluating postpartum indicators from the Project Maternity with Quality at CHBM-EPE ABSTRACT: Exclusive breastfeeding is a universal recommendation and skin-to-skin mother / newborn is a development factor for the dyad. The academic work focuses on the measurement indicators and evidence - breastfeeding within the 1st hour of life & skin-to-skin between the dyad. This is rooted in the Maternity design with quality, of the Portuguese Nurses and ongoing in-Hospital Center Barreiro Montijo EPE. The project began in September 2015, following the procedures that are reported in the current document. In the situation analysis was collected status indicators. The research strand considers sample practitioners and users Barreiro-Montijo Hospital Center and collects data. The intervention performed produced positive results for these indicators. The bonding in the sensitive period is recognized by professionals as an important element in the care. With the implementation of this project contributed to improving the quality of care in maternal health.

Introduction: Pregnancy is associated with experiences that integrated socio-cultural contexts of space and time, bring changes in family and/or maternal-fetal relationship. Parenthood is a generational task, and one of the most complex in humans. Aimo: To describe the construction of motherhood. Method: Cross-sectional qualitative study. We used the interview technique. Convenience sample of 10 pregnant women at the end of 3rd trimester. Results: The analysis of the narrative reveals four categories: a) conceiving pregnancy idea, b) imagining maternity, c) claiming herself through pregnancy and d) announcing attachment Conclusion: pregnant women essentially express positive and tipical feelings of pregnancy, seeming an healthy evolution. The transition to parenthood is underway, as well as a developing performance to the best role.

São conhecidos os benefícios da aplicabilidade da Classificação Internacional para a Prática de Enfermagem, nos cuidados de enfermagem. Tivemos como objetivo, otimizar o SClinico na consulta de saúde materna da Maternidade Dr. Francisco Feitinha da Unidade Local de Saúde do Norte Alentejano, EPE. Para diagnóstico da situação foi aplicada uma entrevista que permitiu identificar as necessidades da população. Foram propostas intervenções de diagnóstico frequentes, necessárias para promover a melhoria dos cuidados prestados e a qualidade dos mesmos. Para avaliação da intervenção aplicamos um questionário. Constatou-se que a maioria dos enfermeiros considera os registos eletrónicos de enfermagem importantes e de muita utilidade, para a atividade profissional e constituem uma fonte de informação essencial que permite a comunicação entre os profissionais de saúde garantindo continuidade e qualidade dos cuidados de enfermagem; ABSTRACT: The benefits of the application of the International Classification for Nursing Practice in nursing care are known. Our purpose was to optimize the Clinical System and Nursing health consultation Maternity Dr. Francisco Feitinha Unit of the North Alentejo Health, EPE. For the diagnosis of the situation, it was applied an interview to identify the needs of the population. It was proposed more frequent diagnosis of interventions necessary to promote the improvement of care and its own quality. For evaluation of the intervention, we applied a questionnaire. Thus, we concluded that most nurses consider important electronic nursing records and that they are very useful for the professional activity. They are seen as an essential source of information and enable communication between health professionals ensuring continuity of quality care.
