SUMMARY The herpes simplex virus type 2 (HVS-2) is the most prevalent infection worldwide. It is a cofactor in the acquisition of human immunodeficiency virus (HIV) and the persistence of human papillomavirus (HPV). This study evaluated the prevalence of HSV-2, using the polymerase chain reaction (PCR), and associated factors in patients treated at the Federal University of Rio Grande (FURG) and Basic Health Units (BHU) in Rio Grande, Brazil. The observed prevalence of HSV-2 was 15.6%. Among the 302 women studied, 158 had received assistance in BHU and 144 were treated at FURG. The prevalence of HSV-2 in these groups was 10.8% and 20.8%, respectively, RR 1.9 and p = 0.012. Knowledge about the Pap smear, and the presence of lesions showed no association with HSV-2 infection. Multivariate analysis showed that the variable that most influenced the risk of HSV-2 infection was the presence of HIV infection, with a relative risk of 1.9 and p = 0.04. Discussion: Genital ulcers are an important entry point for HIV, and condom use is an important strategy to reduce transmission of HIV and HSV-2.

In the absence of intervention, the rate of vertical transmission of HIV can range from 15-45%. With the inclusion of antiretroviral drugs during pregnancy and the choice of delivery route this amounts to less than 2%. However ARV use during pregnancy has generated several questions regarding the adverse effects of the gestational and neonatal outcome. This study aims to analyze the risk factors for vertical transmission of HIV-1 seropositive pregnant women living in Rio Grande and the influence of the use of ARVs in pregnancy outcome. Among the 262 pregnant women studied the rate of vertical transmission of HIV was found to be 3.8%. Regarding the VT, there was a lower risk of transmission when antiretroviral drugs were used and prenatal care was conducted at the referral service. However, the use of ART did not influence the outcome of pregnancy. However, initiation of prenatal care after the first trimester had an influence on low birth weight, as well as performance of less than six visits increased the risk of prematurity. Therefore, the risk factors analyzed in this study appear to be related to the realization of inadequate pre-natal and maternal behavior.

The favorable outcome of the treatment of a disease is influenced by the adherence to therapy. Our objective was to assess factors associated with adherence to treatment of patients included in a clinical trial of equivalence between the standard and alternative treatment schemes with meglumine antimoniate (MA) in the treatment of cutaneous leishmaniasis (CL), in the state of Rio de Janeiro. Between 2008 and 2011, 57 patients with CL were interviewed using a questionnaire to collect socioeconomic data. The following methods were used for adherence monitoring: counting of vial surplus, monitoring card, Morisky test and modified Morisky test (without the question regarding the schedule); we observed 82.1% (vial return), 86.0% (monitoring card), 66.7% (Morisky test) and 86.0% (modified Morisky test) adherence. There was a strong correlation between the method of vial counting and the monitoring card and modified Morisky test. A significant association was observed between greater adherence to treatment and low dose of MA, as well as with a lower number of people sleeping in the same room. We recommend the use of the modified Morisky test to assess adherence to treatment of CL with MA, because it is a simple method and with a good performance, when compared to other methods.

We describe the rate of incidence of Clostridium difficile-associated diarrhea (CDAD) in hematologic and patients undergone stem cell transplant (HSCT) at HC-FMUSP, from January 2007 to June 2011, using two denominators 1,000 patient and 1,000 days of neutropenia and the risk factors associated with the severe form of the disease and death. The ELISA method (Ridascreen-Biopharm, Germany) for the detections of toxins A/B was used to identify C. difficile. A multivariate analysis was performed to evaluate potential factors associated with severe CDAD and death within 14 days after the diagnosis of CDAD, using multiple logistic regression. Sixty-six episodes were identified in 64 patients among 439 patients with diarrhea during the study period. CDA rate of incidence varied from 0.78 to 5.45 per 1,000 days of neutropenia and from 0.65 to 5.45 per 1,000 patient-days. The most common underlying disease was acute myeloid leukemia 30/64 (44%), 32/64 (46%) patients were neutropenic, 31/64 (45%) undergone allogeneic HSCT, 61/64 (88%) had previously used antibiotics and 9/64 (13%) have severe CDAD. Most of the patients (89%) received treatment with oral metronidazole and 19/64 (26%) died. The independent risk factors associated with death were the severe form of CDAD, and use of linezolid.

A cross-sectional study on prevalence, associated factors and genotype distribution of HCV infection was conducted among 848 HIV-infected patients recruited at reference centers in the Midwest Region of Brazil. The prevalence rate of HIV-HCV coinfection was 6.9% (95% CI: 5.2 to 8.6). In multivariable analysis, increasing age, use of illicit drugs (injection and non-injection), a history of blood transfusion before 1994, and the absence of a steady partnership were significant independent associated factors for HIV-HCV coinfection. The phylogenetic analysis based on the NS5B region revealed the presence of two major circulating genotypes of HCV: genotypes 1 (58.3%) and 3 (41.7%). The prevalence of HIV-HCV coinfection was lower than those reported in studies conducted with HIV-infected patients in different regions of Brazil, due to the fact that illicit drug use is not a frequent mode of HIV transmission in this region of Brazil. Serologic screening of HIV-patients for HCV before initiating antiretroviral treatment, a comprehensive identification of associated factors, and the implementation of effective harm reduction programs are highly recommended to provide useful information for treatment and to prevent HCV coinfection in these patients.

The identification of predictors for the progression of chronic Chagas cardiomyopathy (CCC) is essential to ensure adequate patient management. This study looked into a non-concurrent cohort of 165 CCC patients between 1985 and 2010 for independent predictors for CCC progression. The outcomes were worsening of the CCC scores and the onset of left ventricular dysfunction assessed by means of echo-Doppler cardiography. Patients were analyzed for social, demographic, epidemiologic, clinical and workup-related variables. A descriptive analysis was conducted, followed by survival curves based on univariate (Kaplan-Meier and Cox’s univariate model) and multivariate (Cox regression model) analysis. Patients were followed from two to 20 years (mean: 8.2). Their mean age was 44.8 years (20-77). Comparing both iterations of the study, in the second there was a statistically significant increase in the PR interval and in the QRS duration, despite a reduction in heart rates (Wilcoxon < 0.01). The predictors for CCC progression in the final regression model were male gender (HR = 2.81), Holter monitoring showing pauses equal to or greater than two seconds (HR = 3.02) increased cardiothoracic ratio (HR = 7.87) and time of use of digitalis (HR = 1.41). Patients with multiple predictive factors require stricter follow-up and treatment.

The colonization of the oral cavity is a prerequisite to the development of oropharyngeal candidiasis. Aims: The aims of this study were: to evaluate colonization and quantify Candida spp. in the oral cavity; to determine the predisposing factors for colonization; and to correlate the levels of CD4+ cells and viral load with the yeast count of colony forming units per milliliter (CFU/mL) in HIV-positive individuals treated at a University Hospital. Saliva samples were collected from 147 HIV patients and were plated on Sabouraud Dextrose Agar (SDA) and chromogenic agar, and incubated at 30 ºC for 72 h. Colonies with similar morphology in both media were counted and the result expressed in CFU/mL. Results: Of the 147 HIV patients, 89 had positive cultures for Candida spp., with a total of 111 isolates, of which C. albicans was the most frequent species (67.6%), and the mean of colonies counted was 8.8 × 10³ CFU/mL. The main predisposing factors for oral colonization by Candida spp. were the use of antibiotics and oral prostheses. The use of reverse transcriptase inhibitors appears to have a greater protective effect for colonization. A low CD4+ T lymphocyte count is associated with a higher density of yeast in the saliva of HIV patients.

No presente trabalho são comparados dois estudos, ambos com a duração de nove anos, versando os casos de cardite e cardiopatia reumática observados no Serviço de Cardiologia Pediátrica do Hospital de Santa Marta, tendo o primeiro estudo decorrido de Outubro de 1969 a Setembro de 1978, e o segundo de Outubro de 1978 a Setembro de 1987. Dos 38 casos do segundo estudo, 26 tiveram o primeiro surto de febre reumática em Portugal, tendo a cardite surgido como manifestação isolada em 18 casos (69%), associada a poliartrite em 5 casos (20%), e associada a coreia em 3 casos (11%). A insuficiência mitral isolada foi a lesão valvular mais frequente (80%) e 84% das crianças tiveram apresentação clínica inicial na classe funcional I e II da classificação da NYHA. A adesão à profilaxia secundária da febre reumática foi de 78% num grupo de 18 crianças, com um seguimento em média de 2,7 anos, tendo-se modificado neste grupo os sinais de lesão da válvula mitral no sentido da melhoria. Os 12 casos referenciados dos países africanos de língua oficial portuguesa são analisados em separado. Comparativamente ao primeiro estudo, verificou-se um decréscimo de 12,5 para 2,3 casos/ano de cardite reumática, uma redução de mortalidade, assim como uma diferente apresentação clínica, no sentido de um diagnóstico mais precoce, dum predomínio actual de formas menos graves, e de uma maior adesão à profilaxia secundária.

This study examined the association between oral candidiasis in elderly users and nonusers of prosthesis and its predisposing factors. To this end, we performed a cross-sectional study where saliva samples from 48 patients were collected they used prosthesis and 43 patients (control group) who did not use. Among the 91 patients, Candida spp were isolated in 40 (83.3%) who used prosthesis and in 23 (53.5%) in the control group. A statistically significant association was determined between the two groups, the isolation of yeasts and dental prosthesis (p < 0.05, OR = 4.3). The most common etiological agent was Candida albicans (37 isolates), with 23 (62.2%) in the denture group and 14 (37.8%) (control group). Among patients who presented clinical manifestations of oral candidiasis (n = 24), 83.3% (n = 20) belonged to the group that wore dentures, while only 16.7% (n = 4) belonged to the control group. Elderly patients with diabetes had 4.4 times higher estimated risk of developing oral candidiasis when compared with individuals without this condition. There was no statistically significant association between being user prostheses and have diabetes with the onset of candidiasis. No statistically significant association was determined between xerostomia, use of prosthesis and oral candidiasis. The use of prosthetics and poor oral hygiene in elderly patients predisposes to the development of oral candidiasis.

We studied the prevalence of intestinal parasites (IPs), their risk factors and associated symptoms among patients with gastrointestinal disorders. A total of 1,301 participants aged 22 days-90 years were enrolled in this study. We used a structured questionnaire to obtain socio-demographic and stool examination to investigate intestinal parasite infections. Data analysis was performed using SPSS16. The overall prevalence of intestinal parasites (IPs) was 32.2% (419/1,301). Three hundred and fifty nine cases/1,301 (27.6%) were infected with a single parasite and 60/1,301 cases (4.6%) presented polyparasitism. The most common IP was Blastocystis sp. 350/1,301 (26.9%), followed by Entamoeba coli 38/1,301 (2.92%), Giardia lamblia 30/1,301 (2.3%) and Cryptosporidium spp. 17/1,301 (1.3%). Regarding the socio-demographic variables, educational status (p = 0.001), contact with domestic animals and soil (p = 0.02), age above 15 years (p = 0.001) and seasons (p = 0.001) were significantly associated to intestinal parasitic infections. Concerning clinical characteristics, the presence of IPs was significantly associated to diarrhea (OR = 1.57; CI 95% = 1.24-1.98; p < 0.001) and dysentery (OR = 1.94; CI 95% = 1.03-3.66; p < 0.04). Our findings suggest that IPs are one of the main causal agents of gastrointestinal disorders. Improving the knowledge on local risk factors such as poverty, low level of education, poor sanitation, contact with soil and contact with domestic animal is warranted.

Com o objectivo de avaliar aspectos que se prendem com o diagnóstico das cardiopatias no período pré e neo-natal, os autores apresentam um estudo prospectivo de Janeiro a Junho de 1994, durante o qual foram avaliados 165 recém-nascidos, 138 no ambulatório (Grupo I) e 27 no internamento (Grupo II). No Grupo I foram vigiadas 91% das gravidezes, 33% tendo risco para cardiopatia, um quarto destas realizou ecocardiograma fetal. No Grupo II foram vigiadas 74% das gravidezes, havendo em 50% risco para cardiopatia, tendo 30% destas feito ecocardiograma fetal. As principais causas de envio dos recém-nascidos foram: sopro cardíaco (77% Grupo 1; 15% Grupo II), cianose (4% Grupo I;15% Grupo II) e a associação das duas (2% Grupo I; 22% Grupo II). A idade de suspeita / diagnóstico foi, em média 6/8 dias no Grupo I e 4/4 dias no Grupo II. No Grupo I, 89 recém-nascidos não tinham doença cardíaca, 34 tinham comunicação interventricular, 3 defeito do septo aurículo-ventricular e 2 tetralogia de Fallot; 10 eram portadores de trissomia 21. No Grupo II, 25 recém-nascidos tinham cardiopatia sendo as mais frequentes a transposição das grandes artérias e os obstáculos esquerdos (24% cada). Onze fizeram cateterismo cardíaco e 12 cirurgia, tendo 1 falecido. Conclui-se que, apesar da maioria dos recém-nascidos avaliados ter nascido sem diagnóstico pré-natal, o diagnóstico das cardiopatias graves fez-se na primeira semana de vida a seguir ao parto, nomeadamente o da transposição das grandes artérias, permitindo a tempo o tratamento cirúrgico mais adequado. No entanto, embora não fosse demonstrado neste estudo, continua a ser uma realidade o transporte por longas distâncias de recém- -nascidos com cardiopatia crítica, surgindo por isso alguns em condições não ideais, e outros fora do período adequado para certos tipos de tratamento. Por outro lado, a maioria dos enviados à consulta têm sopros transitórios, não se encontrando já, em cerca de metade, qualquer alteração na avaliação cardiovascular pelo especialista.

Os AA. estudaram, a incidência da cardiopatia chagásica em 15.000 necrópsias consecutivas e sua associação com os megas. Em 875 cardiopatias chagásicas houve 145 casos de megas, ou seja, 16,56%, com predominância do sexo masculino. No branco houve maior incidência (55) de megas do que no mulato (53) e no negro (31) dentre 858 cardiopatias chagásicas. Em 848 cardiopatias chagásicas, 85,78% eram de indivíduos que faleceram entre 21 e 60 anos. O maior número de cardiopatias (120 ou 14,14%) e de "megas'' (21 ou 15,78%) foi encontrado nos indivíduos entre 36 a 40 anos. Os nossos resultados mostram uma incidência diferente da associação cardiopatia-mega, em comparação da observada em outras regiões do País e em outros países da América do Sul.
