Les infraccions penals en matèria de seguretat viària constitueixen un dels grups delictius que més interès teòric i pràctic han despertat en la doctrina i la jurisprudència. Per aquest motiu, al llarg de la recerca s'ha dut a terme l'anàlisi dels diferents il·lícits penals des d'una perspectiva interdisciplinar i transversal on no tan sols han tingut cabuda les reflexions teòriques i pràctiques sobre els arts. 379 i ss. CP, sinó també la valoració dels problemes processals i criminològics que són propis d'aquests fets justiciables que es vinculen amb la seguretat viària

OBJECTIVES: To determine 1) rates of needlestick and sharps injuries (NSSIs) not reported to occupational health services, 2) reasons for underreporting and 3) awareness of reporting procedures in a Swiss university hospital. MATERIALS AND METHODS: We surveyed 6,367 employees having close clinical contact with patients or patient specimens. The questionnaire covered age, sex, occupation, years spent in occupation, history of NSSI during the preceding twelve months, NSSI reporting, barriers to reporting and knowledge of reporting procedures. RESULTS: 2,778 questionnaires were returned (43.6%) of which 2,691 were suitable for analysis. 260/2,691 employees (9.7%) had sustained at least one NSSI during the preceding twelve months. NSSIs were more frequent among nurses (49.2%) and doctors performing invasive procedures (IPs) (36.9%). NSSI rate by occupation was 8.6% for nurses, 19% for doctors and 1.3% for domestic staff. Of the injured respondents, 73.1% reported all events, 12.3% some and 14.6% none. 42.7% of doctors performing invasive procedures (IPs) underreported NSSIs and represented 58.6% of underreported events. Estimation that transmission risk was low (87.1%) and perceived lack of time (34.3%) were the most common reasons for non-reporting. Regarding reporting procedures, 80.1% of respondents knew to contact occupational health services. CONCLUSION: Doctors performing IPs have high rates of NSSI and, through self-assessment that infection transmission risk is low or perceived lack of time, high rates of underreporting. If individual risk analyses underestimate the real risk, such underreporting represents a missed opportunity for post-exposure prophylaxis and identification of hazardous procedures. Doctors' training in NSSI reporting merits re-evaluation.

The cuticle is a physical barrier that prevents water loss and protects against irradiation, xenobiotics and pathogens. This classic textbook statement has recently been revisited and several observations were made showing that this dogma falls short of being universally true. Both transgenic Arabidopsis thaliana lines expressing cell wall-targeted fungal cutinase (so-called CUTE plants) or lipase as well as several A. thaliana mutants with altered cuticular structure remained free of symptoms after an inoculation with Botrytis cinerea. The alterations in cuticular structure lead to the release of fungitoxic substances and changes in gene expression that form a multifactorial defence response. Several models to explain this syndrome are discussed.
Treatment of chronic hepatitis C genotype 1 with triple therapy comprising telaprevir or boceprevir.

Hepatitis C virus (HCV) infection is a leading cause of chronic hepatitis, liver cirrhosis and hepatocellular carcinoma worldwide. Two first-generation protease inhibitors, telaprevir and boceprevir, have recently been approved for the treatment of chronic hepatitis C genotype 1. Triple therapy comprising pegylated interferon-α, ribavirin and telaprevir or boceprevir increases sustained virological response rates to ~70% and allows to shorten treatment duration in ~½ of treatment-naïve patients with chronic hepatitis C genotype 1. Sustained virological response rates in treatment-experienced patients depend on the response to previous treatment, ranging from >80% in previous relapsers to ~30% in previous null responders. These advances come at the expense of new adverse effects and increased cost. In addition, treatment of chronic hepatitis C will become more complex. In these times of changing medical practice, the present expert opinion statement by the Swiss Association for the Study of the Liver shall provide guidance on the treatment of chronic hepatitis C with triple therapy comprising telaprevir or boceprevir.

Dans le cadre d'une consultation thérapeutique où nous recevons des familles cliniques et non-cliniques, nous observons les interactions triadiques père-mère-bébé avec des nourrissons de 8 semaines à 1 an, dans une situation de jeu semi-standardisé, le jeu triade Lausanne. Cette situation, révélatrice des modes de communication dans la famille, constitue un outil clinique d'évaluation. Après avoir décrit ce jeu à trois et notre analyse de la situation, nous présentons un exemple d'observation d'une famille où la mère a été hospitalisée, avec son enfant alors âgé de 6 mois, pour une psychose du post-partum. Nous décrivons comment la consultante, par les moyens non-verbaux, encadre cette famille pour organiser le jeu à trois. Cette expérience positive va donner des indications précieuses pour la suite du traitement.

Models predicting species spatial distribution are increasingly applied to wildlife management issues, emphasising the need for reliable methods to evaluate the accuracy of their predictions. As many available datasets (e.g. museums, herbariums, atlas) do not provide reliable information about species absences, several presence-only based analyses have been developed. However, methods to evaluate the accuracy of their predictions are few and have never been validated. The aim of this paper is to compare existing and new presenceonly evaluators to usual presence/absence measures. We use a reliable, diverse, presence/absence dataset of 114 plant species to test how common presence/absence indices (Kappa, MaxKappa, AUC, adjusted D-2) compare to presenceonly measures (AVI, CVI, Boyce index) for evaluating generalised linear models (GLM). Moreover we propose a new, threshold-independent evaluator, which we call "continuous Boyce index". All indices were implemented in the B10MAPPER software. We show that the presence-only evaluators are fairly correlated (p > 0.7) to the presence/absence ones. The Boyce indices are closer to AUC than to MaxKappa and are fairly insensitive to species prevalence. In addition, the Boyce indices provide predicted-toexpected ratio curves that offer further insights into the model quality: robustness, habitat suitability resolution and deviation from randomness. This information helps reclassifying predicted maps into meaningful habitat suitability classes. The continuous Boyce index is thus both a complement to usual evaluation of presence/absence models and a reliable measure of presence-only based predictions.

En el marc de la intervenció penitenciària amb agressors sexuals, té una especial rellevància la predicció de risc del seu comportament futur, atenent les greus repercussions que poden arribar a tenir les reincidències, tant per a les possibles víctimes com des de la perspectiva de l’impacte mediàtic i pànic moral que generen. En el context internacional, gran part de l’esforç investigador en aquesta matèria ha anat adreçat a l’estandardització de protocols per avaluar el risc de reincidència d’aquests subjectes i orientar la presa de decisions envers ells i la seva vida fora del règim penitenciari. Un dels instruments més novedosos per a la predicció del risc d’agressió sexual és el Sexual Violence Risc – 20 (SVR-20) desenvolupat per un equip d’investigadors canadencs. El treball que aquí es presenta s’inscriu en el marc de l’aplicació pràctica d’aquests procediments predictius al nostre país. L’objectiu principal és aplicar l’SVR-20, de manera retrospectiva, a una mostra d’agressors sexuals per avaluar la capacitat predictiva d’aquest instrument i, així, poder anticipar la possible reincidència dels subjectes. Per aconseguir aquest objectiu, la puntuació total del SVR-20 ha estat correlacionada amb la reincidència veritable, considerant el tractament com una variable a tenir en compte. La regressió logística ha estat utilitzada per examinar el calibratge del model. Aquesta anàlisi avalua el grau de correspondència entre les probabilitats predites (de reincidència) i la reincidència esdevinguda. L’SVR-20 prediu correctament el 55% dels no-reincidents i el 16% dels reincidents. Podem afirmar que aquest model prediu molt millor la reincidència que la no-reincidència. A més, la predicció millora quan hi incloem la variable de tractament. Aquest estudi és el primer a Catalunya que avalua la validesa predictiva d’un instrument d’avaluació del risc de violència sexual. Futures recerques haurien de centrar-se a estimar un punt de tall per a l’SVR-20 que permeti classificar els subjectes de risc de reincidència dels subjectes segurs.

Puesto que es muy importante la predicción del riesgo de reincidencia en agresores sexuales, la investigación que aquí se presenta tiene como objetivo la aplicación de uno de los instrumentos más novedosos para la predicción del riesgo de agresión sexual, el Sexual Violence Risc – 20 (SVR-20) desarrollado por un equipo de investigadores canadienses, de manera retrospectiva, a una muestra de agresores sexuales para evaluar la capacidad predictiva de este instrumento y así, poder anticipar la posible reincidencia de los sujetos

Oropharyngeal candidiasis continues to be considered the most common opportunistic disease in Aids patients. This study was designed to investigate species distribution, serotype and antifungal susceptibility profile among Candida spp. isolated from the oral cavity of Aids patients recruited from six Brazilian university centers. Oral swabs from 130 Aids patients were plated onto CHROMagar Candida medium and 142 isolates were recovered. Yeast isolates were identified by classical methods and serotyped using the Candida Check® system-Iatron. Antifungal susceptibility testing was performed according to the NCCLS microbroth assay. C. albicans was the most frequently isolated species (91%), and 70% of the isolates belonged to serotype A. We detected 12 episodes of co-infection (9%), including co-infection with both serotypes of C. albicans. Non-albicans species were isolated from 12 episodes, 50% of them exhibited DDS or resistance to azoles. Otherwise, only 8 out 130 isolates of C. albicans exhibited DDS or resistance to azoles. Brazilian Aids patients are infected mainly by C. albicans serotype A, most of them susceptible to all antifungal drugs.

In a retrospective study, we examined several determinants of basal fat oxidation in 720 healthy Caucasian volunteers. Adult men (n = 427) and women (n = 293) were characterized for resting energy expenditure and substrate oxidation by indirect calorimetry (after a 12-h overnight fast), peak O2 consumption by a treadmill test to exhaustion, body composition by hydrodensitometry, food intake from a 3-day food diary, and hormonal status by fasting hormone concentrations. Fat oxidation was negatively correlated with fat mass in men (r = -0.11; P < 0.05), but no statistical relationship was found in women. In a stepwise multiple regression analysis, fat oxidation was best predicted by peak O2 consumption and fat-free mass in men (model R2 = 0.142) and by free thyroxine, fat-free mass, and fasting insulin in women (model R2 = 0.153). Relative rates of fat oxidation (fat oxidation adjusted for differences in resting energy expenditure) were not correlated with fat mass in either gender. Women showed a lower rate of basal fat oxidation (both absolute and adjusted) than did men. Our results show that fat oxidation is not greater in individuals with a greater fat mass. Furthermore, our results support a sexual dimorphism in basal rates of fat oxidation.

Diabetes develops when the insulin needs of peripheral cells exceed the availability or action of the hormone. This situation results from the death of most beta-cells in type 1 diabetes, and from an inability of the beta-cell mass to adapt to increasing insulin needs in type 2 and gestational diabetes. We analyzed several lines of transgenic mice and showed that connexins (Cxs), the transmembrane proteins that form gap junctions, are implicated in the modulation of the beta-cell mass. Specifically, we found that the native Cx36 does not alter islet size or insulin content, whereas the Cx43 isoform increases both parameters, and Cx32 has a similar effect only when combined with GH. These findings open interesting perspectives for the in vitro and in vivo regulation of the beta-cell mass.
