Avaliou-se o efeito da radiação UV-C em abacates Hass, quanto ao conteúdo de fenólicos totais, atividade da enzima polifenoloxidase (PPO) e coloração. Os frutos selecionados foram submetidos à radiação em luz UV-C durante 5; 10; 15 e 20 minutos, sendo mantidos sob refrigeração (10 ± 1 ºC e 90±5% UR), e avaliados durante 15 dias. Para o teor fenólicos totais e PPO, não se observou diferença entre os tratamentos dos frutos nos diferentes tempos de exposição à luz UV-C. Os teores fenólicos totais e PPO diminuíram durante o período experimental. Os valores de luminosidade mantiveram-se elevados (85,4 a 88,5) no armazenamento. Os valores de cor a* e b* diminuíram com o armazenamento de forma mais intensa para os frutos submetidos à radiação UV-C. Não houve correlação significativa para a PPO, conteúdo de fenólicos totais e coloração.

BACKGROUND/AIMS: Controversies still exist regarding the evaluation of growth hormone deficiency (GHD) in childhood at the end of growth. The aim of this study was to describe the natural history of GHD in a pediatric cohort. METHODS: This is a retrospective study of a cohort of pediatric patients with GHD. Cases of acquired GHD were excluded. Univariate logistic regression was used to identify predictors of GHD persisting into adulthood. RESULTS: Among 63 identified patients, 47 (75%) had partial GHD at diagnosis, while 16 (25%) had complete GHD, including 5 with multiple pituitary hormone deficiencies. At final height, 50 patients underwent repeat stimulation testing; 28 (56%) recovered and 22 (44%) remained growth hormone (GH) deficient. Predictors of persisting GHD were: complete GHD at diagnosis (OR 10.1, 95% CI 2.4-42.1), pituitary stalk defect or ectopic pituitary gland on magnetic resonance imaging (OR 6.5, 95% CI 1.1-37.1), greater height gain during GH treatment (OR 1.8, 95% CI 1.0-3.3), and IGF-1 level <-2 standard deviation scores (SDS) following treatment cessation (OR 19.3, 95% CI 3.6-103.1). In the multivariate analysis, only IGF-1 level <-2 SDS (OR 13.3, 95% CI 2.3-77.3) and complete GHD (OR 6.3, 95% CI 1.2-32.8) were associated with the outcome. CONCLUSION: At final height, 56% of adolescents with GHD had recovered. Complete GHD at diagnosis, low IGF-1 levels following retesting, and pituitary malformation were strong predictors of persistence of GHD. © 2015 S. Karger AG, Basel.

A capacidade antioxidante e a qualidade pós-colheita do abacate 'Fuerte' foram avaliadas. Os frutos foram mantidos sob temperatura ambiente (24 ± 1 ºC) e sob refrigeração em 10 ± 1 ºC e 90±5% UR e avaliados durante 15 dias. Determinaram-se a perda de massa e a taxa respiratória. As características físico-químicas avaliadas foram acidez total titulável (AT), sólidos solúveis (SS), ratio, pH, firmeza, atividade da enzima polifenoloxidase (PPO), cor (L, a* e b*), fenólicos totais e atividade antioxidante (AA) por DPPH·. Nas duas condições de armazenamento, o pico respiratório ocorreu no 9º dia.Os frutos mantidos sob temperatura refrigerada apresentaram menor perda de massa e maior firmeza em relação àqueles em temperatura ambiente. O comportamento respiratório refletiu na redução dos valores de AT e no conteúdo de SS que diminuíram a partir do pico de respiração. Os valores de pH aumentaram como consequência da diminuição da AT. Os valores de luminosidade mantiveram-se elevados, observando-se diminuição do componente de cor verde na polpa com o armazenamento. A atividade da PPO diminuiu com o armazenamento. Os valores de AA variaram de 17,6 a 68,7%. O conteúdo de fenólicos totais diminuiu após o pico respiratório, e valores crescentes de AA foram observados mesmo após o 9º dia.

OBJECTIVE: As universal screening of hypertension performs poorly in childhood, targeted screening to children at higher risk of hypertension has been proposed. Our goal was to assess the performance of combined parental history of hypertension and overweight/obesity to identify children with hypertension. We estimated the sensitivity, specificity, negative and positive predictive values of overweight/obesity and parental history of hypertension for the identification of hypertension in children. DESIGN AND METHOD: We analyzed data from a school-based cross-sectional study including 5207 children aged 10 to 14 years from all public 6th grade classes in the canton of Vaud, Switzerland. Blood pressure was measured with a clinically validated oscillometric automated device over up to three visits separated by one week. Children had hypertension if they had sustained elevated blood pressure over the three visits. Parents were interviewed about their history of hypertension. RESULTS: The prevalence of hypertension was 2.2%. 14% of children were overweight or obese and 20% had a positive history of hypertension in either or both parents. 30% of children had either or both conditions. After accounting for several potential confounding factors, parental history of hypertension (odds ratio (OR): 2.6; 95% confidence interval (CI): 1.8-4.0), overweight excluding obesity (OR: 2.5; 95% CI: 1.5-4.2) and obesity (OR: 10.1; 95% CI: 6.0-17.0) were associated with hypertension in children. Considered in isolation, the sensitivity and positive predictive values of parental history of hypertension (respectively 41% and 5%) or overweight/obesity (respectively 43% and 7%) were relatively low. Nevertheless, considered together, the sensitivity of targeted screening in children with either overweight/obesity or paternal history of hypertension was higher (65%) but the positive predictive value remained low (5%). The negative predictive value was systematically high. CONCLUSIONS: Restricting screening of hypertension to children with either overweight/obesity or with hypertensive parents would substantially limit the proportion of children to screen (30%) and allow the identification of a relatively large proportion (65%) of hypertensive cases. That could be a valuable alternative to universal screening.

INTRODUCTION: There is conflicting evidence on the benefit of early transjugular intrahepatic portosystemic shunt (TIPSS) on the survival of patients with acute variceal bleeding (AVB). AIM: To assess the effect of early TIPSS on patient prognosis. MATERIALS AND METHODS: We carried out a meta-analysis of trials evaluating early TIPSS in cirrhotic patients with AVB. RESULTS: Four studies were included. Early TIPSS was associated with fewer deaths [odds ratio (OR)=0.38, 95% confidence interval (CI)=0.17-0.83, P=0.02], with moderate heterogeneity between studies (P=0.15, I=44%). Early TIPSS was not significantly associated with fewer deaths among Child-Pugh B patients (OR=0.35, 95% CI=0.10-1.17, P=0.087) nor among Child-Pugh C patients (OR=0.34, 95% CI=0.10-1.11, P=0.074). There was no heterogeneity between studies in the Child-Pugh B analysis (P=0.6, I=0%), but there was a high heterogeneity in the Child-Pugh C analysis (P=0.06, I=60%). Early TIPSS was associated with lower rates of bleeding within 1 year (OR=0.08, 95% CI=0.04-0.17, P<0.001) both among Child-Pugh B patients, (OR=0.15, 95% CI=0.05-0.47, P=0.001) and among Child-Pugh C patients (OR=0.05, 95% CI=0.02-0.15, P<0.001), with no heterogeneity between studies. Early TIPSS was not associated with higher rates of encephalopathy (OR=0.84, 95% CI=0.50-1.42, P=0.5). CONCLUSION: Cirrhotic patients with AVB treated with early TIPSS had lower death rates and lower rates of clinically significant bleeding within 1 year compared with patients treated without early TIPSS. Additional studies are required to identify the potential risk factors leading to a poor prognosis after early TIPSS in patients with AVB and to determine the impact of the degree of liver failure on the patient's prognosis.

In order to better understand the specificity of training adaptations, we compared the effects of two different anaerobic training regimes on various types of soccer-related exercise performances. During the last 3 weeks of the competitive season, thirteen young male professional soccer players (age 18.5±1 yr, height 179.5±6.5 cm, body mass 74.3±6.5 kg) reduced the training volume by ~20% and replaced their habitual fitness conditioning work with either speed endurance production (SEP; n = 6) or speed endurance maintenance (SEM; n = 7) training, three times per wk. SEP training consisted of 6-8 reps of 20-s all-out running bouts followed by 2 min of passive recovery, whereas SEM training was characterized by 6-8 x 20-s all-out efforts interspersed with 40 s of passive recovery. SEP training reduced (p<0.01) the total time in a repeated sprint ability test (RSAt) by 2.5%. SEM training improved the 200-m sprint performance (from 26.59±0.70 to 26.02±0.62 s, p<0.01) and had a likely beneficial impact on the percentage decrement score of the RSA test (from 4.07±1.28 to 3.55±1.01%) but induced a very likely impairment in RSAt (from 83.81±2.37 to 84.65±2.27 s). The distance covered in the Yo-Yo Intermittent Recovery test level 2 was 10.1% (p<0.001) and 3.8% (p<0.05) higher after SEP and SEM training, respectively, with possibly greater improvements following SEP compared to SEM. No differences were observed in the 20- and 40-m sprint performances. In conclusion, these two training strategies target different determinants of soccer-related physical performance. SEP improved repeated sprint and high-intensity intermittent exercise performance, whereas SEM increased muscles' ability to maximize fatigue tolerance and maintain speed development during both repeated all-out and continuous short-duration maximal exercises. These results provide new insight into the precise nature of a stimulus necessary to improve specific types of athletic performance in trained young soccer players.

109Cd was injected into the lumen of superficial proximal or distal tubules of rat kidneys, and recovery in the pelvic urine from the ipsilateral kidney was measured. Fractional recovery of labeled inulin always exceeded 90%. About 70% of injected inorganic Cd (CdCl2) was taken up by the epithelium of proximal tubules, while more than 90% of the injected amount was recovered after distal microinjection. The proximal fractional Cd uptake of a 1:1 (molar) Cd-L-cysteine complex was 82%, but was below 60% for a 5-10:1 molar ratio of cysteine:Cd. The chelate Cd-pentetic acid was recovered in final urine nearly quantitatively after proximal or distal microinjection. Fractional uptake of 109Cd from a Cd-metallothionein (Mt) complex, following proximal microinjection, ranged between 17 (Cd-Mt 0.19 mM) and 8% (Cd-Mt 1.5 mM). It is concluded that luminal Cd uptake by the tubular epithelium depends markedly on the chemical form of Cd and, when present, occurs mostly or exclusively in proximal tubules.

Assisted reproductive technologies (ART) induce vascular dysfunction in humans and mice. In mice, ART-induced vascular dysfunction is related to epigenetic alteration of the endothelial nitric oxide synthase (eNOS) gene, resulting in decreased vascular eNOS expression and nitrite/nitrate synthesis. Melatonin is involved in epigenetic regulation, and its administration to sterile women improves the success rate of ART. We hypothesized that addition of melatonin to culture media may prevent ART-induced epigenetic and cardiovascular alterations in mice. We, therefore, assessed mesenteric-artery responses to acetylcholine and arterial blood pressure, together with DNA methylation of the eNOS gene promoter in vascular tissue and nitric oxide plasma concentration in 12-wk-old ART mice generated with and without addition of melatonin to culture media and in control mice. As expected, acetylcholine-induced mesenteric-artery dilation was impaired (P = 0.008 vs. control) and mean arterial blood pressure increased (109.5 ± 3.8 vs. 104.0 ± 4.7 mmHg, P = 0.002, ART vs. control) in ART compared with control mice. These alterations were associated with altered DNA methylation of the eNOS gene promoter (P < 0.001 vs. control) and decreased plasma nitric oxide concentration (10.1 ± 11.1 vs. 29.5 ± 8.0 μM) (P < 0.001 ART vs. control). Addition of melatonin (10(-6) M) to culture media prevented eNOS dysmethylation (P = 0.005, vs. ART + vehicle), normalized nitric oxide plasma concentration (23.1 ± 14.6 μM, P = 0.002 vs. ART + vehicle) and mesentery-artery responsiveness to acetylcholine (P < 0.008 vs. ART + vehicle), and prevented arterial hypertension (104.6 ± 3.4 mmHg, P < 0.003 vs. ART + vehicle). These findings provide proof of principle that modification of culture media prevents ART-induced vascular dysfunction. We speculate that this approach will also allow preventing ART-induced premature atherosclerosis in humans.

OBJECTIVE: Prospective data on the association between resistin levels and cardiovascular disease (CVD) events are sparse with conflicting results. METHODS: We studied 3044 aged 70-79 years from the Health, Aging, and Body Composition Study. CVD events were defined as coronary heart disease (CHD) or stroke events. «Hard » CHD events were defined as CHD death or myocardial infarction. We estimated hazard ratio (HR) and 95% confidence intervals (CI) according to the quartiles of serum resistin concentrations and adjusted for clinical variables, and then further adjusted for metabolic disease (body mass index, fasting plasma glucose, abdominal visceral and subcutaneous adipose tissue, leptin, adiponectin, insulin) and inflammation (C-reactive protein, interleukin-6, tumor necrosis factors-α). RESULTS: During a median follow-up of 10.1 years, 559 patients had « hard » CHD events, 884 CHD events and 1106 CVD Events. Unadjusted incidence rate for CVD events was 36.6 (95% CI 32.1-41.1) per 1000 persons-year in the lowest quartile and 54.0 per 1000 persons-year in the highest quartile (95% CI 48.2-59.8, P for trend < 0.001). In the multivariate models adjusted for clinical variables, HRs for the highest vs. lowest quartile of resistin was 1.52 (95% CI 1.20-1.93, P < 0.001) for « Hard » CHD events, 1.41 (95% CI 1.16-1.70, P = 0.001) for CHD events and 1.35 (95% CI 1.14-1.59, P = 0.002) for CVD events. Further adjustment for metabolic disease slightly reduced the associations while adjustment for inflammation markedly reduced the associations. CONCLUSIONS: In older adults, higher resistin levels are associated with CVD events independently of clinical risk factors and metabolic disease markers, but markedly attenuated by inflammation.

Objective Comparative analysis of dosimetry in intracavitary balloon catheter brachytherapy with I-125 and in Cf-252 brachytherapy combined with BNCT for treatment of brain tumors. Materials and Methods Simulations of intracavitary balloon catheter brachytherapy with I-125 and in Cf-252 brachytherapy combined with BNCT were performed with the MCNP5 code, modeling the treatment of a brain tumor on a voxel computational phantom representing a human head. Absorbed dose rates were converted into biologically weighted dose rates. Results Intracavitary balloon catheter brachytherapy with I-125 produced biologically weighted mean dose rates of 3.2E-11, 1.3E-10, 1.9E-11 and 6.9E-13 RBE.Gy.h-1.p-1.s, respectively, on the healthy tissue, on the balloon periphery and on the I 1 and I 2 tumor infiltration zones. On the other hand, Cf-252 brachytherapy combined with BNCT produced a biologically weighted mean dose rate of 5.2E-09, 2.3E-07, 8.7E-09 and 2.4E-09 RBE.Gy.h-1.p-1.s, respectively on the healthy tissue, on the target tumor and on the I 1 and I 2 infiltration zones. Conclusion Cf-252 brachytherapy combined with BNCT delivered a selective irradiation to the target tumor and to infiltration zones, while intracavitary balloon catheter brachytherapy with I-125 delivered negligible doses on the tumor infiltration zones.

STUDY OBJECTIVES: Narcolepsy with cataplexy is tightly associated with the HLA class II allele DQB1*06:02. Evidence indicates a complex contribution of HLA class II genes to narcolepsy susceptibility with a recent independent association with HLA-DPB1. The cause of narcolepsy is supposed be an autoimmune attack against hypocretin-producing neurons. Despite the strong association with HLA class II, there is no evidence for CD4+ T-cell-mediated mechanism in narcolepsy. Since neurons express class I and not class II molecules, the final effector immune cells involved might include class I-restricted CD8+ T-cells. METHODS: HLA class I (A, B, and C) and II (DQB1) genotypes were analyzed in 944 European narcolepsy with cataplexy patients and in 4,043 control subjects matched by country of origin. All patients and controls were DQB1*06:02 positive and class I associations were conditioned on DQB1 alleles. RESULTS: HLA-A*11:01 (OR = 1.49 [1.18-1.87] P = 7.0*10(-4)), C*04:01 (OR = 1.34 [1.10-1.63] P = 3.23*10(-3)), and B*35:01 (OR = 1.46 [1.13-1.89] P = 3.64*10(-3)) were associated with susceptibility to narcolepsy. Analysis of polymorphic class I amino-acids revealed even stronger associations with key antigen-binding residues HLA-A-Tyr(9) (OR = 1.32 [1.15-1.52] P = 6.95*10(-5)) and HLA-C-Ser(11) (OR = 1.34 [1.15-1.57] P = 2.43*10(-4)). CONCLUSIONS: Our findings provide a genetic basis for increased susceptibility to infectious factors or an immune cytotoxic mechanism in narcolepsy, potentially targeting hypocretin neurons.

Introduction: L'hyperglycémie est un phénomène connu chez les patients gravement agressés, et surtout chez ceux nécessitant un séjour aux soins intensifs, alors que l'hypoglycémie est une complication menaçante. Des valeurs de glycémies anormales sont associées avec une mortalité et morbidité augmentées chez les patients de soins intensifs, y compris les grands brûlés. Des glycémies jusqu'à 15mmol/l ont longtemps été tolérées sans traitement. En 2001, une grande étude randomisée a complètement changé les pratiques du contrôle glycémique aux soins intensifs. Van den Berghe et al. ont montré qu'un contrôle glycémique strict atteint au moyen d'une « intensive insulin therapy » (HT) visant une glycémie 4.1-6.0 mmol/l réduisait la mortalité chez les patients chirurgicaux traités plus que 5. Par la suite plusieurs études contradictoires ont questionné la validité externe de l'étude de Louvain: avec la publication de l'étude « NICE-SUGAR » en 2009 enrôlant plus de 6000 patients cette hypothèse a été réfutée, aboutissant à un contrôle modéré de la glycémie (6-8 mmol/l). Bien que plusieurs études sur le contrôle glycémique aient également inclus quelques patients brûlés, à ce jour il n'y a pas de recommandation ferme concernant la gestion de la glycémie chez les patients brûlés adultes. Le but de l'étude était d'évaluer la sécurité du protocole de contrôle de la glycémie qui avait été introduit aux soins intensifs adultes chez des patients grand brûlés nécessitant un traitement prolongé aux soins intensifs. Méthodes : 11 s'agit d'une étude rétrospective uni-centrique sur des patients brûlés admis aux soins intensifs du CHUV à Lausanne entre de 2000 à juin 2014. Critères d'inclusions : Age >16 ans, brûlures nécessitant un traitement aux soins intensifs >10 jours. Critères d'exclusion : Décès ou transfert hors des soins intensifs <10 jours. Les investigations ont été limitées aux 21 premiers jours de l'hospitalisation aux soins intensifs. Variables : Variables démographiques, surface brûlée (TBSA), scores de sévérité, infections, durée d'intubation, durée du séjour aux soins intensifs, mortalité. Variables métaboliques : Administration totale de glucides, énergie et insuline/2411, valeurs de glycémie artérielle et CRP. Quatre périodes (P) ont été analysées, correspondant à l'évolution du protocole de contrôle de glycémie du service. P1: Avant son introduction (2000-2001) ; P2: Contrôle glycémie serré géré par les médecins (2002-2006) ; P3: Contrôle glycémie serré géré par lés infirmières (2007-2010); P4: Contrôle modéré géré par les infirmières (2011-2014). Les limites glycémiques ont été définis de manière suivante: Hypoglycémie extrême <2.3mmol/l ; hypoglycémie modéré <4.0mmol/l ; hyperglycémie modérée 8.1-10.0mmol/l ; hyperglycémie sévère >10.0mmol/l. Toutes les valeurs de glycémies artérielles ont été extraites depuis le système informatisé des soins intensifs (MetaVision ®). Statistiques: Wilcoxon rank test, Two- way Anova, Tuckey Kramer test, area under the curve (AUC), Spearman's test et odds ratio. STATA 12 1 ' StataCorp, College station, TX, USA and JPM V 10.1 (SAS Institute, Cary, NC, USA). Résultats: Sur les 508 patients brûlés admis durant la période étudiée, 229 patients correspondaient aux critères d'inclusion, âgés de 45±20ans (X±SD) et brûlés sur 32±20% de la surface corporelle. Les scores de sévérité sont restés stables. Au total 28'690 glycémies artérielles ont été analysées. La valeur médiane de glycémie est restée stable avec une diminution progressive de la variabilité intra-patient. Après initiation du protocole, les valeurs normoglycémiques ont augmenté de 34.7% à 65.9% avec diminution des événements hypoglycémiques (pas d'hypoglycémie extrême en P4). Le nombre d'hyperglycémies sévères est resté stable durant les périodes 1 à 3, avec une diminution en P4 (9.25%) : les doses d'insuline ont aussi diminué. L'interprétation des résultats de P4 a été compliquée par une diminution concomitante des apports d'énergie et de glucose (p<0.0001). Conclusions: L'application du protocole destiné aux patients de soins intensifs non brûlés a amélioré le contrôle glycémique chez les patients adultes brûlés, aboutissant à une diminution significative de la variabilité des glycémies. Un contrôle modéré de la glycémie peut être appliqué en sécurité, considérant le nombre très faible d'hypoglycémies. La gestion du protocole par les infirmières s'avère plus sûre qu'un contrôle par les médecins, avec diminution des hypoglycémies. Cependant le nombre d'hyperglycémies reste trop élevé. L'hyperglycémie' n'est pas contrôlable uniquement par l'administration d'insuline, mais nécessite également une approche multifactorielle comprenant une optimisation de la nutrition adaptée aux besoins énergétiques élevés des grands brûlés. Plus d'études seront nécessaire pour mieux comprendre la complexité du mécanisme de l'hyperglycémie chez le patient adulte brûlé et pour en améliorer le contrôle glycémique.

Selectivity studies for the determination of Cr(VI) using the catalytic oxidation of the o-dianisidine by hydrogen peroxide showed two distincts situations. In the first, when interferents were studied by a univariate procedure, Cr(III) and Cu(II) cause serious interferences even at the 2:1 proportion, relative to Cr(VI), while Fe(III) interfered at the 15:1 ratio and EDTA at the 10:1 ratio. On the other hand, when a multivariate investigation was performed, Cr(III) did not present any significant principal effects and its significant interaction effects were negative, in contrast to EDTA, that presented positive interaction effects although, like Cr(III), did not show significant interaction effects. In view of the interferent's action it become necessary to separate Cr(VI) by extraction with methylisobutylketone in a chloridric acid medium before its determination in vegetals and in wastewater from a cellulose industry samples. Using this procedure, the method precision is ±0,5% at the 10 ng/mL Cr(VI) concentration level. The detection and quantification limits, calculated by means of absorbance measurements of ten replicates of blank reagents were 1,1 and 3,2 ng/mL, respectively. The results obtained with real samples showed a relative standard deviation between 1,2% and 3,0% relative to their reference values.

These pieces of equipments can be obtained at the low price of US$ 50.00 and are adequate for eletrical conductivity measurements in the semiconductor range, that is from 10-1 to 10-6 S cm-1.

Two new marine metabolites, 3Z, 6Z, 9Z-dodecatrien-1-ol (1) from the ascidian Botrylloides giganteum and 4H-pyran-2ol acetate from the sponge Ircinia felix (4) are herein reported. The known bromotyrosine compounds, 2-(3,5-dibromo-4-methoxyphenyl)-N,N,N-dimethylethanammonium (2) and 2,6-dibromo-4-(2-(trimethylammonium)ethyl)phenol (3), have been isolated from the sponge Verongula gigantea. Serotonin (5) is reported for the first time from the sponge Cliona delitrix, and tambjamines A (15) and D (16) isolated as their respective salts from the nudibranch Tambja eliora. Only tambjamine D presented cytotoxicity against CEM (IC50 12.2 µg/mL) and HL60 (IC50 13.2 µg/mL) human leukemya cells, MCF-7 breast cancer cells (IC50 13.2 µg/mL), colon HCT-8 cancer cells (IC50 10.1 µg/mL) and murine melanoma B16 cancer cells (IC50 6.7 µg/mL).
