La resección oncológica completa es el único procedimiento que permite la sobrevida a largo plazo en cáncer de páncreas. La afectación de la arteria hepática, tronco celíaco o arteria mesentérica superior constituyen una contraindicación quirúrgica porque se asocia a mal pronóstico y por las dificultades técnicas que implica conseguir la resección oncológica completa. Solo un grupo seleccionado de pacientes con buena respuesta a la quimioterapia y pasibles de resección R0 se benefician de la cirugía de resección del tronco celíaco. A partir de un caso clínico de un adenocarcinoma de páncreas con infiltración de la arteria hepática común y tronco celíaco que tras una buena respuesta a la neoadyuvancia y embolización de la arteria hepática común fue sometido a una esplenopancreatectomía córporo-caudal con resección del tronco celíaco, se realiza una revisión de la literatura sobre el tema y sus aspectos técnicos relevantes. El análisis realizado permite sugerir que en casos debidamente seleccionados la pancreatectomía córporo-caudal con resección del tronco celíaco en bloque es un procedimiento factible, seguro, y con buenos resultados quirúrgicos y oncológicos. En condiciones de respuesta a la quimioterapia neoadyuvante y experiencia del equipo quirúrgico pareciera que esta cirugía podría mejorar el pronóstico y calidad de vida de estos enfermos.

Objetivo: El Real Decreto de Troncalidad (RDT) plantea modificar la formación sanitaria especializada, estableciendo 24 meses de formación troncal común. El objetivo de este estudio es valorar su potencial impacto en la formación especializada de medicina preventiva y salud pública (MPSP) y en otras especialidades del tronco común médico (TCM). Métodos: Se analizaron los programas de las 21 especialidades del TCM, recogiendo los periodos de rotación recomendados por cada especialidad y consensuando la información entre tres observadores. El impacto formativo se calculó como el porcentaje de meses que cada especialidad debería modificar para adaptarse al periodo común. Resultados: MPSP (100%, 24 meses) es la especialidad en la cual el RDT tendrá más impacto. Medicina intensiva (0%, 0 meses) y oncología médica (17%, 4 meses) son las especialidades menos afectadas. Conclusiones: El RDT va a afectar de manera diferente a las especialidades del TCM. El RDT supondrá un replanteamiento completo de sus actividades y de las competencias de los profesionales de MPSP.

Compararam-se dois sistemas de irrigacao por sulcos. Um deles constou da aplicacao de vazoes constantes, atraves de sifoes, para valores de R iguais a 0,26; 1 e 2. O outro constou de reutilizacao da agua escoada para valores de R iguaia a 0,5, 1 e 2. Observaram-se decrescimos nas perdas por percolacao, enquanto as eficiencias de aplicaca oe de distribuicao aumentaram, o mesmo acontecendo com as perdas por escoamento em ambos os sistemas de irrigacao, quando o valor de R aumentou. A eficiencia de irrigacao cresceu com o valor de R, quando se reutilizou a agua de escoamento, ocorrendo o inverso quando nao se reutilizou esta agua. Foram feitas determinacoes quanto a flutuacao da vazao de escoamento disponivel durante a irrigacao de cinco setores consecutivos, visando o dimensionamento de sistemas de irrigacao com reutilizacao desta agua.

The multiphase flow occurrence in the oil and gas industry is common throughout fluid path, production, transportation and refining. The multiphase flow is defined as flow simultaneously composed of two or more phases with different properties and immiscible. An important computational tool for the design, planning and optimization production systems is multiphase flow simulation in pipelines and porous media, usually made by multiphase flow commercial simulators. The main purpose of the multiphase flow simulators is predicting pressure and temperature at any point at the production system. This work proposes the development of a multiphase flow simulator able to predict the dynamic pressure and temperature gradient in vertical, directional and horizontal wells. The prediction of pressure and temperature profiles was made by numerical integration using marching algorithm with empirical correlations and mechanistic model to predict pressure gradient. The development of this tool involved set of routines implemented through software programming Embarcadero C++ Builder® 2010 version, which allowed the creation of executable file compatible with Microsoft Windows® operating systems. The simulator validation was conduct by computational experiments and comparison the results with the PIPESIM®. In general, the developed simulator achieved excellent results compared with those obtained by PIPESIM and can be used as a tool to assist production systems development

The petroleum production pipeline networks are inherently complex, usually decentralized systems. Strict operational constraints are applied in order to prevent serious problems like environmental disasters or production losses. This paper describes an intelligent system to support decisions in the operation of these networks, proposing a staggering for the pumps of transfer stations that compose them. The intelligent system is formed by blocks which interconnect to process the information and generate the suggestions to the operator. The main block of the system uses fuzzy logic to provide a control based on rules, which incorporate knowledge from experts. Tests performed in the simulation environment provided good results, indicating the applicability of the system in a real oil production environment. The use of the stagger proposed by the system allows a prioritization of the transfer in the network and a flow programming

Oil production and exploration techniques have evolved in the last decades in order to increase fluid flows and optimize how the required equipment are used. The base functioning of Electric Submersible Pumping (ESP) lift method is the use of an electric downhole motor to move a centrifugal pump and transport the fluids to the surface. The Electric Submersible Pumping is an option that has been gaining ground among the methods of Artificial Lift due to the ability to handle a large flow of liquid in onshore and offshore environments. The performance of a well equipped with ESP systems is intrinsically related to the centrifugal pump operation. It is the pump that has the function to turn the motor power into Head. In this present work, a computer model to analyze the three-dimensional flow in a centrifugal pump used in Electric Submersible Pumping has been developed. Through the commercial program, ANSYS® CFX®, initially using water as fluid flow, the geometry and simulation parameters have been defined in order to obtain an approximation of what occurs inside the channels of the impeller and diffuser pump in terms of flow. Three different geometry conditions were initially tested to determine which is most suitable to solving the problem. After choosing the most appropriate geometry, three mesh conditions were analyzed and the obtained values were compared to the experimental characteristic curve of Head provided by the manufacturer. The results have approached the experimental curve, the simulation time and the model convergence were satisfactory if it is considered that the studied problem involves numerical analysis. After the tests with water, oil was used in the simulations. The results were compared to a methodology used in the petroleum industry to correct viscosity. In general, for models with water and oil, the results with single-phase fluids were coherent with the experimental curves and, through three-dimensional computer models, they are a preliminary evaluation for the analysis of the two-phase flow inside the channels of centrifugal pump used in ESP systems

Este trabalho apresenta o procedimento adotado para a geração de um mapa descrevendo o potencial de infiltração e escoamento superficial da água a partir de informações de solo e declividade do terreno, utilizando um Sistema de Informação Geográfica - SIG. Este mapa e um passo intermediário de um método que vem sendo desenvolvido para a definição de áreas de risco de contaminação por agroquímicos, levando em consideração as propriedades dos produtos, características físicas locais e climáticas.

Trata-se de uma descrição e análise da questão das redes e dos dois circuitos econômicos a partir do pequeno estabelecimento rural no Recôncavo Baiano. Tem-se como referência o território da olericultura, o qual corresponde ao povoado do Tanque de Senzala, pertencente ao município de Santo Amaro – BA. O aspecto levantado nesse estudo é aquele da produção e do escoamento da olericultura promovida na agricultura familiar, mas precisamente pelo pequeno produtor rural, quando destacado em sua classe por abranger melhores condições de infra-estrutura, possibilitando a otimização dos aspectos produtivos e de distribuição. Para identificar tais agentes, foram aplicados 73 formulários como universo de pesquisa nos meses de outubro e novembro de 2008, nos quais inferiu-se os inputs, a entrada de insumos agrícolas, e outputs, a saída de mercadorias do território. Estes agentes se diferenciam dos demais por se lançarem no espaço baiano, “eliminando” a figura do atravessador com o objetivo de melhorar os seus lucros, fazendo assim o traçado das redes que ligam o território a alguns fixos pertencentes ao circuito superior e inferior da economia da Região Metropolitana de Salvador, e do interior do estado, a exemplo de Feira de Santana. Um aspecto conclusivo reside na importância da rede geográfica concreta, originada da comercialização, tendo como natureza o fluxo de pessoas e mercadorias. Sua dimensão temporal é a frequência periódica de velocidade lenta por causa de sua base rodoviária; sua dimensão espacial segue um modelo dentrítico em escala regional. Nessa senda as redes geográficas contribuem para a fluidez dos produtos originados no território da olericultura e representam a base concreta para a geração de emprego e renda para a classe dos produtores rurais.

hematopoietic stem cell transplantation (HSCT) is associated with more respiratory infections due to immunosuppression. this study aimed to verify the frequency of rhinosinusitis after HSCT, and the association between rhinosinusitis and chronic graft vs. host disease (GVHD) and type of transplantation, clinical treatment, surgical treatment, and survival. this was a retrospective study in a tertiary university hospital. A total of 95 patients with hematological diseases undergoing HSCT between 1996 and 2011 were selected. chronic myeloid leukemia was the most prevalent disease. The type of transplant most often performed was the allogenic type (85.26%). The frequency of rhinosinusitis was 36%, with no difference between the autologous and the allogenic types. Chronic GVHD occurred in 30% of patients. Patients with GVHD had a higher frequency and recurrence of rhinosinusitis, in addition to more frequent need for endoscopic sinusectomy and decreased overall survival. there was a higher frequency of rhinosinusitis in HSCT and GVHD. The type of transplant does not appear to predispose to the occurrence of rhinosinusitis. GVHD seems to be an aggravating factor and requires a more stringent treatment.

X-linked adrenoleukodystrophy (X-ALD) is an inherited disease with clinical heterogeneity varying from presymptomatic individuals to rapidly progressive cerebral ALD forms. This disease is characterized by increased concentration of very long chain fatty acids (VLCFAs) in plasma and in adrenal, testicular and nervous tissues. Affected individuals can be classified in different clinical settings, according to phenotypic expression and age at onset of initial symptoms. Molecular defects in X-ALD individuals usually result from ABCD1 gene mutations. In the present report we describe clinical data and the ABCD1 gene study in two boys affected with the childhood cerebral form that presented with different symptomatic manifestations at diagnosis. In addition, their maternal grandfather had been diagnosed with Addison's disease indicating phenotypic variation for X-ALD within this family. The mutation p.Trp132Ter was identified in both male patients; additionally, three females, out of eleven family members, were found to be heterozygous after screening for this mutation. In the present report, the molecular analysis was especially important since one of the heterozygous females was in first stages of pregnancy. Therefore, depending on the fetus outcome, if male and p.Trp132Ter carrier, storage of the umbilical cord blood should be recommended as hematopoietic stem cell transplantation could be considered as an option for treatment in the future.
