Boron, Ca, Na, and Gd concentrations and H intensity in sediments obtained during Ocean Drilling Program Leg 192 were determined by prompt gamma neutron activation analysis. The results show strong positive correlation between B content and H intensity in carbonate samples; chalk samples have higher B contents than limestone samples. Average B content is 9.1 ppm for the chalk and 5.2 ppm for the limestone. When chert blocks or clay minerals are present in the carbonate samples, B content increases (up to 91 ppm).

Purpose of review Heart failure and diabetes mellitus are frequently associated, and diabetes appears to potentiate the clinical presentation of heart failure related to other causes. The purpose of this review is to examine recent advances in the application of tissue Doppler imaging for the assessment of diabetic heart disease. Recent findings Recent studies have documented that both myocardial systolic and diastolic abnormalities can be identified in apparently healthy patients with diabetes and no overt cardiac dysfunction. Interestingly, these are disturbances of longitudinal function, with compensatory increases of radial function-suggesting primary involvement of the subendocardium, which is a hallmark of myocardial ischemia. Despite this, there is limited evidence that diabetic microangiopathy is responsible-with reduced myocardial blood volume rather than reduced resting flow, and at least some evidence suggesting a normal increment of tissue velocity with stress. Finally, a few correlative studies have shown association of diabetic myocardial disease with poor glycemic control, while angiotensin converting enzyme inhibition may be protective. Summary Tissue Doppler imaging (and the related technique of strain rate imaging) appears to be extremely effective for the identification of subclinical LV dysfunction in diabetic patients It is hoped that the recognition of this condition will prompt specific therapy to prevent the development of overt LV dysfunction.

Psychological distress is common in cancer patients, however, it is often unrecognized and untreated. We aimed to identify barriers to cancer patients expressing their psychological concerns, and to recommend strategies to assist oncologists to elicit, recognize, and manage psychological distress in their patients. Medline, Psychlit, and the Cochrane databases were searched for articles relating to the detection of emotional distress in patients. Patients can provide verbal and non-verbal information about their emotional state. However, many patients may not reveal emotional issues as they believe it is not a doctor's role to help with their emotional concerns. Moreover, patients may normalize or somatize their feelings. Anxiety and depression can mimic physical symptoms of cancer or treatments, and consequently emotional distress may not be detected. Techniques such as active listening, using open questions and emotional words, responding appropriately to patients' emotional cues, and a patient-centred consulting style can assist in detection. Screening tools for psychological distress and patient question prompt sheets administered prior to the consultation can also be useful. In conclusion, the application of basic communication techniques enhances detection of patients' emotional concerns. Training oncologists in these techniques should improve the psychosocial care of cancer patients.

Secretion of mucins and exudation of plasma are distinct processes of importance to innate immunity and inflammatory disease. Yet, little is known about their relation in human airways. The objective of the present study was to use the human nasal airway to determine mucinous secretion and plasma exudation in response to common challenge agents and mediators. Ten healthy volunteers were subjected to nasal challenge-lavage procedures. Thus, the nasal mucosa was exposed to increasing doses of histamine (40 and 400 mu g ml(-1)), methacholine (12.5 and 25 mg) and capsaicin (30 and 300 ng ml(-1)). Fucose was selected as a global marker of mucinous secretion and alpha(2)-macroglobulin as an index of exudation of bulk plasma. All challenge agents increased the mucosal output of fucose to about the same level (P < 0.01-0.05). Once significant secretion had been induced the subsequently increased dose of the challenge agent, in the case of histamine and methacholine, failed to further increase the response. Only histamine increased the mucosal output of alpha(2)-macroglobulin (P < 0.01). We conclude that prompt but potentially rapidly depleted mucinous secretion is common to different kinds of airway challenges, whereas inflammatory histamine-type mediators are required to produce plasma exudation. Along with the acknowledged secretion of mucins, a practically non-depletable, pluripotent mucosal output of plasma emerges as an important component of the innate immunity of human airways.

Patients with advanced, non-curable cancer face difficult decisions on further treatment, where a small increase in survival time must be balanced against: the toxicity of the treatment. If patients want to be involved in these decisions, in keeping with current notions of autonomy and empowerment, they also require to be adequately informed both on the treatments proposed and on their own disease status and prognosis. A systematic review was performed on decision-making and information provision in patients with advanced cancer. Studies of interventions to improve information giving and encourage participation in decision-making were reviewed, including both randomised controlled trials and uncontrolled studies. Almost all patients expressed a desire for full information, but only about two-thirds wished to participate actively in decision-making. Higher educational level, younger age and female sex were predictive of a desire to participate in decision-making. Active decision-making was more common in patients with certain cancers (e.g. breast) than others (e.g. prostate). A number of simple interventions including question prompt sheets, audio-taping of consultations and patient decision aids have been shown to facilitate such involvement. (c) 2005 Elsevier Ltd. Conservation of U.S. coral reefs has been sidetracked by the partial implementation of management plans without clearly achievable goals. Historical ecology reveals global patterns of coral reef degradation that provide a framework for reversing reef decline with ecologically meaningful metrics for success. The authors of this Policy Forum urge action now to address multiple threats simultaneously, because the harmful effects of stressors like overfishing, pollution, poor land-use practices, and global warming are interdependent. Prompt implementation of proven, practical solutions would lead to both short- and long-term benefits, including the return of keystone species and the economic benefits they entail.

Arsenic contamination of groundwater (0.05 to 0.84 mg/L) in Kuitun, Xinjiang was first found in 1970’s. Alternative clean surface water was introduced in 1985. We aimed to assess the exposure and heath outcome since the mitigation. In 2000, we collected a total of 360 urine samples from villagers from the endemic area and a nearby control area for arsenic (As), porphyrins and malondialdehyde (MDA) measurements. The averaged urinary As level of villagers from the endemic site (117±8.3 μg/g creatinine; 4.2 to 943.8 μg/g creat) was higher than that of the control site (73.6±3.2 μg/g creat). No significant differences were found in urinary porphyrins or MDA between the endemic and control sites. However, when the urinary arsenic was higher than 150 μg/g creat, these two biomarkers were higher in the exposed group than the control. Within the exposed group, villagers with arsenic-related skin symptoms had higher arsenic, uroporphyrin and MDA compared to those who had not shown symptoms. Sine the water mitigation, villagers whose urinary arsenic levels were 270 μg/g creat dropped from 20% to 10% of the population. Population with arsenic-related skin symptoms remained unchanged at 31%. We noted that 7.8% of those who had skin lesions were born after the implementation of intervention and that some villagers still prefer to drink the groundwater. Further, in the dry season, lack of surface water and electrical power breakdowns are to blame for failure to ensure continuous supply of clean water. It is concluded that despite the prompt action and successful water mitigation program to curb arsenic poisonings, it is essential to continue to monitor the health outcome of this population.

O presente estudo buscou investigar o impacto da resiliência e da autoeficácia sobre o burnout em profissionais de enfermagem. O constructo resiliência no contexto do trabalho refere-se a capacidade de adaptação de forma positiva frente às adversidades que ocorrem no ambiente laboral. A autoeficácia no trabalho representa a percepção do indivíduo sobre as suas próprias competências na execução de tarefas. Já o burnout é compreendido como uma síndrome específica do meio laboral como consequência da cronificação do estresse ocupacional, apresentando três dimensões: a exaustão, o cinismo (despersonalização) e a baixa realização profissional. A amostra da pesquisa foi composta por 82 trabalhadores da área de enfermagem que trabalham em Unidades de Pronto Atendimento UPA s localizadas no estado do Acre. A maior parte dos participantes é do sexo feminino (78%), com idade média de 31 anos (DP=6,8). Para a mensuração das variáveis foram utilizadas a Escala de Resiliência no Trabalho, a Escala de Autoeficácia no Trabalho e a Escala de Caracterização do Burnout, e para coletar os dados sociodemográficos foi aplicado um questionário de autorresposta construído para este estudo. Os dados foram submetidos a análises exploratórias e descritivas, análise de variância (ANOVA), análise de correlação de Pearson e regressão linear múltipla padrão. Os resultados indicaram que os profissionais de enfermagem apresentaram níveis médios de resiliência, autoeficácia e dos componentes exaustão e baixa realização profissional do burnout; e, por sua vez, baixo nível de despersonalização. Constatou-se também correlação entre autoeficácia com dois dos três fatores do burnout: exaustão emocional e decepção no trabalho. Os achados ainda revelaram que resiliência e autoeficácia conjuntamente predizem significativamente ambos os fatores de burnout, exaustão e decepção. No entanto, a variável responsável por esta explicação foi autoeficácia no trabalho, pois apenas ela foi estatisticamente significante para explicar os dois componentes do burnout relatados. Conclui-se que os profissionais que apresentam maior autoeficácia são os que menos sucumbem ao burnout, essa síndrome tão devastadora que ataca de maneira impiedosa os profissionais que lidam no atendimento às pessoas.

Research about diagnosis of chronic illness indicates this is an emotional time for patients. Information provision is especially salient for diabetes management. Yet current orthodoxy suggests that too much information at the time of diagnosis is unhelpful for patients. In this study, we used in-depth interviews with 40 newly diagnosed type 2 diabetic (T2DM) patients in Scotland, to explore their emotional reactions about diagnosis, and their views about information provision at the time of diagnosis. Data were analysed using a thematic approach. Our results showed three main 'routes' to diagnosis: 'suspected diabetes' route; 'illness' route; and 'routine' route. Those within the 'routine' route described the most varied emotional reactions to their diagnosis. We found that most patients, irrespective of their route to diagnosis, wanted more information about diabetes management at the time of diagnosis. We suggest that practitioners would benefit from being sensitive to the route patients follow to diagnosis, and prompt, simple but detailed advice about T2DM management would be helpful for newly diagnosed patients. © 2004 Elsevier Ireland Ltd.
