Introduction and Objectives. Laparoscopic surgery has undeniable advantages, such as reduced postoperative pain, smaller incisions, and faster recovery. However, to improve surgeons’ performance, ergonomic adaptations of the laparoscopic instruments and introduction of robotic technology are needed. The aim of this study was to ascertain the influence of a new hand-held robotic device for laparoscopy (HHRDL) and 3D vision on laparoscopic skills performance of 2 different groups, naïve and expert. Materials and Methods. Each participant performed 3 laparoscopic tasks—Peg transfer, Wire chaser, Knot—in 4 different ways. With random sequencing we assigned the execution order of the tasks based on the first type of visualization and laparoscopic instrument. Time to complete each laparoscopic task was recorded and analyzed with one-way analysis of variance. Results. Eleven experts and 15 naïve participants were included. Three-dimensional video helps the naïve group to get better performance in Peg transfer, Wire chaser 2 hands, and Knot; the new device improved the execution of all laparoscopic tasks (P < .05). For expert group, the 3D video system benefited them in Peg transfer and Wire chaser 1 hand, and the robotic device in Peg transfer, Wire chaser 1 hand, and Wire chaser 2 hands (P < .05). Conclusion. The HHRDL helps the execution of difficult laparoscopic tasks, such as Knot, in the naïve group. Three-dimensional vision makes the laparoscopic performance of the participants without laparoscopic experience easier, unlike those with experience in laparoscopic procedures.

Objectivo: Identificar os factores psicossociais que influenciam a percepção da dor pós-operatória em doentes submetidos a cirurgia de revascularização do miocárdio (CRM). Material e Métodos: Estudo exploratório correlacional de 91 doentes (71 homens e 20 mulheres) submetidos a CRM (pontagem aortocoronária) por esternotomia). A idade média era de 63,8 ± 9,6 anos (entre 39 e 84). Foram utilizados os seguintes instrumentos: Escala Analógica Visual às 24, 48 e 96 horas do pós-operatório; Questionário de Caracterização Demográfica; Mental Health Inventory de 5 itens; Percepção de Saúde Geral (SF-36); Escala de Expectativas de Dor; Escala de Percepção de Apoio; Escala de Expectativas de Auto-eficácia; Satisfação com o tratamento, médicos e enfermeiros (American Pain Society Questionnaire) aplicados às 96 horas após a cirurgia. Resultados: Os doentes que apresentaram expectativas elevadas de dor, percepcionaram maior apoio, apresentaram níveis elevados de auto-eficácia para lidar com a dor ou, se pertenciam ao sexo masculino, sentiram menos dor. De igual modo, os doentes que apresentaram melhor saúde mental, percepcionaram a sua saúde como boa e os doentes que expressaram maior satisfação com o tratamento sentiram menos dor. A dor não foi influenciada pela idade, grau de escolaridade ou pela satisfação com a conduta de médicos e enfermeiros. Conclusão: Após as primeiras 48 horas do pós-operatório, a experiência de dor é influenciada por factores psicossociais, em oarticular pela expectativa de dor, expectativa de auto-eficácia, apoio percebido, percepção da saúde geral, percepção de saúde mental e satisfação com o tratamento para a dor. Perante os resultados, evidencia-se a necessidade de conjugar conhecimentos no sentido de dar respostas mais alargadas e de carácter multidisciplinar no tratamento da dor pós-operatória em CRM devendo, a par de outros aspectos, focar-se na gestão das expectativas dos doentes. ABSTRACT - Objective: To identify the psychological factors that influence post-surgery pain perception in patients undergoing coronary artery bypass graft surgery (CABG). Methods: This was an exploratory correlational study of 91 patients (71 men and 20 women) who underwent CABG (aortocoronary anastomosis) by sternotomy. Mean age was 63.8 ± 9.6 years (between 39 and 84). The following instruments were used: visual analogical scales at 24, 48 and 96 hours of post-surgery; demographic characteristics survey; pain expectations scale; perceived support scale; self-efficacy scale, Mental Inventory (5 items); health perception scale; and satisfaction with treatment, doctors and nurses (American Pain Society questionnaire) at 96 hours after surgery. Results: Patients who had presented high expectations of pain, perceived more support, presented high levels of self-efficacy to deal with pain or were male, felt less pain. Furthermore, patients who presented better mental health, perceived their general health as being good, or expressed greater satisfaction with treatment, felt less pain. Pain was not influenced by age, level of education or satisfaction with doctors and nurses. Conclusion: After the first 48 hours following surgery, the pain experience is influenced by psychosocial factors, in particular by expectation of pain and of self-efficacy, perceived support, perception of general and mental health, and satisfaction with pain treatment. The results confirm the need to bring together different kinds of knowledge for a broad, multidisciplinary approach to postoperative CABG pain treatment, focusing, along with other aspects, on management of patients’ expectations.

Objetivo – Comparar a técnica convencional, a técnica de energias mistas e a técnica field-in-field com energias mistas, verificando a dose recebida nos órgãos de risco e no volume alvo. Metodologia – Quinze doentes com carcinoma da mama esquerda classificadas de T1-T3N0M0 foram tratadas com cirurgia conservadora da mama, seguida de radioterapia pós-operatória. Para cada doente realizaram-se 3 planeamentos dosimétricos, sendo que cada um deles diz respeito a uma das diferentes técnicas em estudo. Através dos HDV gerados avaliaram-se a Dmáx, Dmed, Dmín, D95%, D3% e a homogeneidade da dose no volume alvo, bem como a dose nos órgãos de risco. Utilizou-se o teste de Friedman para verificar a significância do estudo, com um intervalo de confiança de 95%. Resultados – Relativamente ao pulmão esquerdo e ao coração obtiveram-se, com a técnica field-in-field com energias mistas, doses inferiores em comparação com as outras duas técnicas. Para a Dmáx e a homogeneidade de dose no PTV, a técnica field-in-field com energias mistas revelou-se mais eficaz, comparativamente às outras técnicas. No entanto, verificou-se uma melhor cobertura de dose no PTV com a técnica convencional. Considerações finais – A técnica field-in-field com energias mistas permite uma redução da dose nos órgãos de risco, uma redução significativa da Dmáx no PTV e melhora a homogeneidade da dose, comparativamente com as outras técnicas. Os resultados obtidos com a técnica field-in-field com energias mistas apontam para a redução dos efeitos secundários provocados pelo tratamento. ABSTRACT - Purpose – To compare the conventional technique, the technique of mixed energies and the technique field-in-field with mixed energies, checking the received dose in organs at risk and target volume. Methods – Fifteen patients with carcinoma of the left breast classified as T1-T3N0M0 were treated with breast-conserving surgery, followed by postoperative radiotherapy. For each patient were carried out three dosimetric plannings, each one of them concerns the different techniques under study. Through the DVH generated to evaluate Dmax, Dmed, Dmin, D95%, D3% and the homogeneity of the target volume dose, well as the dose in organs at risk. We used the Friedman test to assess the significance of the study, with a confidence interval of 95%. Results – For the left lung and heart were obtained with the technique field-in-field with mixed energies, lower doses compared with the other two techniques. For the Dmax and the homogeneity of the PTV dose, the technique field-in-field with mixed energies was more effective compared to other techniques. However, there was a better coverage of the PTV dose with conventional technique. Conclusions – The technique field-in-field with mixed energy allows a reduction in dose for organs at risk, a significant reduction Dmax in PTV and improves the homogeneity of the dose compared with other techniques. The results obtained with the technique field-in-field with mixed energy indicate to reduce the side effects caused by the treatment.

Introdução: A cirurgia abdominal acarreta grande risco de complicações pulmonares pós-operatórias. As alterações pós-cirúrgicas abdominais, reflectem-se na dinâmica ventilatória, de modo particular nos volumes e capacidades pulmonares, e na capacidade de tosse. Objectivos: Compreender qual a variação dos volumes e capacidades pulmonares e da capacidade de tosse antes e depois da cirurgia abdominal (estômago e cólon), e qual a correlação dessa variação com o nível de dor percepcionada. Desenho do estudo: Unicêntrico, prospectivo e observacional. Amostra: 10 indivíduos, propostos para cirurgia abdominal – estômago e cólon. Metodologia: Dois momentos de avaliação: um nas 24h préoperatórias em que se mediu a capacidade vital forçada (CVF) e o volume expirado máximo no primeiro segundo (VEMS1) com espirometria, e do pico de fluxo de tosse (PCF); e um segundo momento nas 24h pós-operatórias onde se repetiram as medições do primeiro momento com o acréscimo da avaliação da dor. Resultados: No pós-operatorio imediato há uma diminuição significativa da CVF de 44,30%±17,24 (p=0,005), do VEMS1 de 35,50%±28,47 (p=0,009) e do PCF de 38,97%±38,66 (p=0,012). Não se verificou nenhuma relação entre a dor percepcionada na realização das manobras de espirometria e tosse com diminuição a da CVF e do VEMS1 e do PCF respectivamente. O sexo apresentou uma relação significativa com a variação da CRF e do VEMS1 (p=0,046 e p=0,008 respectivamente). A frequência respiratória apresentou um aumento significativo no pós-operatório de 10±11,22 cpm (p=0,019). A saturação periférica de oxigénio apresentou uma diminuição significativa no pós-operatório de 3,52±2,47 (p=0,011) Conclusão: No estudo efectuado fica demonstrado o impacto negativo da cirurgia abdominal na dinâmica respiratória. A diminuição dos valores da CVF, do VEMS1 e do PCF podem contribuir de forma significativa para o aumento do risco de complicações respiratória pós-operatórias. No entanto seria importante a realização deste estudo com uma amostra maior.

A presente dissertação tem como propósito a definição das características de um modelo prognóstico,utilizando uma rede neuronal, em doentes com patologia Cirúrgica, internados num serviço de Cirurgia Geral. Para esse fim obtive dados clínicos, operatórios, o resultado da intervenção e o tempo de internamento pós-operatório em doentes submetidos a um leque amplo de intervenções de Cirurgia abdominal. Construí um sistema computacional baseado em redes neuronais, utilizando os paradigmas considerados mais adequados para o tipo de variável a prever. Analisei em seguida o desempenho dos modelos obtidos. Construí um programa capaz de recolher os dados clínicos e apresentar o resultado da sua avaliação pelas redes neuronais criadas, sem envolver o seu utilizador nos aspectos técnicos da manipulação das redes neuronais. Para cumprimento desta estratégia procurei atingir os seguintes objectivos: Recolher dados de identificação, manifestações clínicas, tipo de doença(s), diagnósticos, características da intervenção cirúrgica e resultado, referentes a um conjunto de doentes suficiente para a construção de uma rede neuronal, com o número de variáveis empregue. Construir uma base de dados com os elementos de informação assim obtidos. Eliminar todos os casos em que se verificou faltar um elemento de informação. Criar dois grupos de casos, mutuamente exclusivos, para construção e validação das redes neuronais. Criar, com base nos elementos diagnósticos e resultado, 7 grupos não exclusivos, para avaliação das redes criadas. Avaliar estatisticamente as características dos grupos criados, para os comparar e caracterizar. Proceder à escolha de um programa para criação de redes neuronais em função da variedade de paradigmas oferecidos, facilidade de utilização, uso diversificado em diversos ambientes e mercados e a possibilidade de aceder às redes criadas, através de uma linguagem de programação de alto nível. Construir três tipos de redes diferentes. Cada tipo de rede utilizando um algoritmo diferente e adequado ao tipo de variável que se deseja prever. Avaliar as redes no que se refere à sua sensibilidade, especificidade, capacidade discriminativa e calibração. Criar um programa,usando a linguagem de programação "Delphi"©, para captura de dados, articulação dos mesmos com as redes neuronais criadas e expressão dos resultados prognósticos; esse programa permite alterar os valores dos elementos clínicos e verificar a repercussão dessa alteração no prognóstico.

Cytomegalovirus (CMV) disease in acquired immunodeficiency syndrome (AIDS) patients most commonly presents as chorioretinitis and gastro-intestinal infection. Neurological involvement due to CMV may cause several clinical presentations: polyradiculitis, myelitis, encephalitis, ventriculo-encephalitis, and mononeuritis multiplex. Rarely, cerebral mass lesion is described. We report a 39 year-old woman with AIDS and previous cerebral toxoplasmosis. She presented with fever, seizures, and vulval ulcers. Her chest X-ray showed multiple lung nodules, and a large frontal lobe lesion was seen in a brain computed tomography scan. She underwent a brain biopsy through a frontal craniotomy, but her condition deteriorated and she died in the first postoperative day. Histopathological studies and immunohistochemistry disclosed CMV disease, and there was no evidence of cerebral toxoplasmosis, bacterial, mycobacterial or fungal infection. CMV disease should be considered in the differential diagnosis of cerebral mass lesion in AIDS patients. High suspicion index, timely diagnostic procedures (surgical or minimally invasive), and proper utilization of prophylactic and therapeutic medication could improve outcome of these patients.

INTRODUCTION: Chylothorax is a rare but serious postoperative condition in children with congenital heart disease. Conventional medical treatment consists of specific long-term dietary modification, and surgical reintervention, such as lymphatic duct ligation, may be indicated in refractory cases. In recent years, an additional conservative treatment, octreotide, a synthetic analog of somatostatin, has been used in management of congenital and postoperative chylothorax. METHODS: The objective of this work was to analyze the efficacy and safety of this treatment for chylothorax after congenital heart surgery. We reviewed the records of sixteen patients with chylothorax after surgery for congenital heart disease between January 1999 and December 2007, and collected the following data: demographic information; type of surgical procedure; onset, duration and management of chylothorax and treatment; and duration of hospital stay. To analyze efficacy we compared these parameters in children receiving conventional treatment only with those receiving octreotide. To analyze safety we compared the adverse effects of both treatments. Octreotide was administered at a dose of 4 to 10 microg/kg/hour, with monitoring of side effects. RESULTS: The incidence of chylothorax in our population was 1.6%. It occurred more often after Glenn and Fontan procedures (8 patients). Octreotide was begun three days after diagnosis of chylothorax and continued for a median of seventeen days (ranging from 4 to 26 days), until complete resolution. Side effects were frequent (in 3 of the 8 patients) but of no clinical relevance. All patients responded to the therapy and there was no indication for further surgical intervention. DISCUSSION AND CONCLUSIONS: Octreotide is safe and effective in the treatment of postoperative chylothorax in children with congenital heart disease. It is a useful adjunctive therapy to the conventional treatment of this complication.

Spinal arachnoiditis, an inflammatory process involving all three meningeal layers as well as the nerve roots, is a cause of persistent symptoms in 6% to 16% of postoperative patients. Although spinal surgery is the most common antecedent associated with arachnoiditis, multiple causes have been reported, including infection, intrathecal steroids or anesthetic agents, trauma, subarachnoid hemorrhage and ionic myelographic contrast material--both oil soluble and water soluble. In the past, oil-based intrathecal contrast agents (Pantopaque) were associated with arachnoiditis especially when this material was introduced into the thecal sac and mixed with blood. Arachnoiditis is apparently rarely idiopathic. The pathogenesis of spinal arachnoiditis is similar to the repair process of serous membranes, such as the peritoneum, with a negligible inflammatory cellular exudate and a prominent fibrinous exudate. Chronic adhesive arachnoiditis of the lower spine is a myelographic diagnosis. The myelographic findings of arachnoiditis were divided into two types by Jorgensen et al. In type 1, "the empty thecal sac" appearance, there is homogeneous filling of the thecal sac with either absence of or defects involving nerve root sleeve filling. In type 2 arachnoiditis, there are localized or diffuse filling defects within the contrast column. MRI has demonstrated a sensitivity of 92% and a specificity of 100% in the diagnosis of arachnoiditis. The appearance of arachnoiditis on MRI can be assigned to three main groups. The MRI findings in group I are a conglomeration of adherent roots positioned centrally in the thecal sac. Patients in group II show roots peripherally adherent to the meninges--the so called empty sac. MRI findings in group III are a soft tissue mass within the subarachnoid space. It corresponds to the type 2 categorization defined by Jorgensen et al, where as the MRI imaging types I and II correspond to the myelographic type 1.

Venous ulcers in patients with post thrombotic syndromes are complex situations with multiple therapeutic options. They are responsible for high morbidity rates, conservative treatment is very slow and recurrences are very common. Deep venous reconstructive surgery is an alternative, but it should be based on the morphologic and functional aspects of the venous system and only adopted after a very careful study, including venography. The authors describe a morphological "pattern", found in some of these patients and related to the competence of the saphenous femoral junction, rendering possible to perform a valvular transposition. Seven patients with post thrombotic ulcers who have been treated during the last 6 years in which the pattern already described was detect, underwent a transposition of the superficial femoral vein, to the great saphenous vein and when necessary complemented with skyn grafts. Before the operation all patients had ulcers with more than 3 cm in size (3.2-5.4 cm) and with more than 4 months duration (4-16 months). All ulcers healed in the postoperative period (mean time 28 days). All patients have been reevaluated in 2003 and it was diagnosed the thrombosis of two procedures, one of them with the recurrence of the ulcer who healed with conservative treatment. The authors consider this method as a very easy technique to perform, although rarely used, and a valid alternative in a highly selected group of patients.

OBJECTIVE: Venous aneurysms of the lower limbs are rare. When they involve the deep venous system, they can be associated with deep venous thrombosis and pulmonary embolism. The authors report the case of a 63-year-old women evaluated by duplex imaging and venography, in the context of a pulmonary embolism, and detected bilateral large aneurysms of the femoral veins (5 cm in the right femoral vein and 4 cm in the left femoral vein). The aneurysms were located at the bifurcation of the common femoral vein with the superficial and profound veins. METHOD: The right aneurysm was partially thrombosed. The patient was asymptomatic in the lower-limb extremities. After a period of anticoagulation, the authors proceeded to surgically excise the right aneurysm and replaced the venous system using a polytetrafluoroethylene (PTFE) graft with the construction of an a-v fistula. RESULT: The patient had no complications in the postoperative period and her 8-month follow-up examination revealed no symptoms, not even leg edema. The duplex scan showed patency of the graft. She continued with oral anticoagulation. The histologic examination revealed an eosinophilic inflammatory reaction of the vein wall, compatible with the diagnosis of Churg-Strauss syndrome. CONCLUSION: The authors conclude that they have probably described the first case in the international literature of a venous aneurysm with a Churg-Strauss syndrome and pulmonary embolism.

OBJECTIVE: The aim of this study was to evaluate the efficacy of post-caesarean analgesia comparing three techniques most frequently used. PATIENTS AND METHODS: For three months all pregnant women submitted to elective or urgent caesarean section, under general or regional anaesthesia, were evaluate with a total of 129 parturient. These parturient were divided into three groups with different techniques of postoperative analgesia: Group 1 (n = 26) received intravenous pethidine and paracetamol per os, group 2 (n = 58) received epidural morphine and group 3 (n = 45) epidural morphine and intravenous propacetamol. Pain was assessed at rest and during mobilisation using a scale of 0-without pain, 1-mild pain, 2-moderate pain and 3-severe pain. Overall satisfaction was assessed with a verbal qualitative scale of very good, good, sufficient and bad. Side effects were analysed. RESULTS: The records of pain at rest and during mobilisation were significantly lower with epidural analgesia compared with intravenous pethidine. There were no significant differences between groups 2 and 3. Similar results were observed in the degree of satisfaction. For 50% of parturient of epidural analgesia (groups 2 and 3) and only 4% of intravenous pethidine (group 1) the analgesic technique was very good. Propacetamol and epidural morphine (group 3) had better pain scores (very good and good) when compared with morphine alone (group 2) but there were no significant differences. Epidural morphine was associated with more pruritus. CONCLUSION: From this study we are able to conclude that epidural morphine offers a good quality of analgesia with better satisfaction and minimal side effects.
