OBJECTIVE To assess the determinants of exclusive breastfeeding abandonment. METHODS Longitudinal study based on a birth cohort in Viçosa, MG, Southeastern Brazil. In 2011/2012, 168 new mothers accessing the public health network were followed. Three interviews, at 30, 60, and 120 days postpartum, with the new mothers were conducted. Exclusive breastfeeding abandonment was analyzed in the first, second, and fourth months after childbirth. The Edinburgh Postnatal Depression Scale was applied to identify depressive symptoms in the first and second meetings, with a score of ≥ 12 considered as the cutoff point. Socioeconomic, demographic, and obstetric variables were investigated, along with emotional conditions and the new mothers’ social network during pregnancy and the postpartum period. RESULTS The prevalence of exclusive breastfeeding abandonment at 30, 60, and 120 days postpartum was 53.6% (n = 90), 47.6% (n = 80), and 69.6% (n = 117), respectively, and its incidence in the fourth month compared with the first was 48.7%. Depressive symptoms and traumatic delivery were associated with exclusive breastfeeding abandonment in the second month after childbirth. In the fourth month, the following variables were significant: lower maternal education levels, lack of homeownership, returning to work, not receiving guidance on breastfeeding in the postpartum period, mother’s negative reaction to the news of pregnancy, and not receiving assistance from their partners for infant care. CONCLUSIONS Psychosocial and sociodemographic factors were strong predictors of early exclusive breastfeeding abandonment. Therefore, it is necessary to identify and provide early treatment to nursing mothers with depressive symptoms, decreasing the associated morbidity and promoting greater duration of exclusive breastfeeding. Support from health professionals, as well as that received at home and at work, can assist in this process.

OBJECTIVE To evaluate if temperature and humidity influenced the etiology of bloodstream infections in a hospital from 2005 to 2010.METHODS The study had a case-referent design. Individual cases of bloodstream infections caused by specific groups or pathogens were compared with several references. In the first analysis, average temperature and humidity values for the seven days preceding collection of blood cultures were compared with an overall “seven-days moving average” for the study period. The second analysis included only patients with bloodstream infections. Several logistic regression models were used to compare different pathogens and groups with respect to the immediate weather parameters, adjusting for demographics, time, and unit of admission.RESULTS Higher temperatures and humidity were related to the recovery of bacteria as a whole (versus fungi) and of gram-negative bacilli. In the multivariable models, temperature was positively associated with the recovery of gram-negative bacilli (OR = 1.14; 95%CI 1.10;1.19) or Acinetobacter baumannii (OR = 1.26; 95%CI 1.16;1.37), even after adjustment for demographic and admission data. An inverse association was identified for humidity.CONCLUSIONS The study documented the impact of temperature and humidity on the incidence and etiology of bloodstream infections. The results correspond with those from ecological studies, indicating a higher incidence of gram-negative bacilli during warm seasons. These findings should guide policies directed at preventing and controlling healthcare-associated infections.

OBJECTIVE To analyze temporal trends of the prevalence of alcohol and tobacco use among Brazilian students. METHODS We analyzed data published between 1989 and 2010 from five epidemiological surveys on students from the 6th to the 12th grade of public schools from the ten largest state capitals of Brazil. The total sample consisted of 104,104 students and data were collected in classrooms. The same collection tool – a World Health Organization self-reporting questionnaire – and sampling and weighting procedures were used in the five surveys. The Chi-square test for trend was used to compare the prevalence from different years. RESULTS The prevalence of alcohol and tobacco use varied among the years and cities studied. Alcohol consumption decreased in the 10 state capitals (p < 0.001) throughout 21 years. Tobacco use also decreased significantly in eight cities (p < 0.001). The highest prevalence of alcohol use was found in the Southeast region in 1993 (72.8%, in Belo Horizonte) and the lowest one in Belem (30.6%) in 2010. The highest past-year prevalence of tobacco use was found in the South region in 1997 (28.0%, in Curitiba) and the lowest one in the Southeast in 2010 (7.8%, in Sao Paulo). CONCLUSIONS The decreasing trend in the prevalence of tobacco and alcohol use among students detected all over the Country can be related to the successful and comprehensive Brazilian antitobacco and antialcohol policies. Despite these results, the past-year prevalence of alcohol consumption in the past year remained high in all Brazilian regions.

OBJECTIVE To analyze whether sociodemographic, occupational, and health-related data are associated with the use of hearing protection devices at work, according to gender. METHODS A cross-sectional study was conducted in 2006, using a random sample of 2,429 workers, aged between 18 and 65 years old, from residential sub-areas in Salvador, BA, Northeastern Brazil. Questionnaires were used to obtain sociodemographic, occupational, and health-related data. Workers who reported that they worked in places where they needed to shout in order to be heard were considered to be exposed to noise. Exposed workers were asked whether they used hearing protection devices, and if so, how frequently. Analyses were conducted according to gender, with estimates made about prevalence of the use of hearing protection devices, prevalence ratios, and their respective 95% confidence intervals. RESULTS Twelve percent (12.3%) of study subjects reported that they were exposed to noise while working. Prevalence of the use of hearing protection devices was 59.3% for men and 21.4% for women. Men from higher socioeconomic levels (PR = 1.47; 95%CI 1.14;1.90) and who had previous audiometric tests (PR = 1.47; 95%CI 1.15;1.88) were more likely to use hearing protection devices. For women, greater perceived safety was associated with the use of protection devices (PR = 2.92; 95%CI 1.34;6.34). This perception was specifically related to the presence of supervisors committed to safety (PR = 2.09; 95%CI 1.04;4.21), the existence of clear rules to prevent workplace injuries (PR = 2.81; 95%CI 1.41;5.59), and whether they were informed about workplace safety (PR = 2.42; 95%CI 1.23;4.76). CONCLUSIONS There is a gender bias regarding the use of hearing protection devices that is less favorable to women. The use of such devices among women is positively influenced by their perception of a safe workplace, suggesting that gender should be considered as a factor in hearing conservation programs.

Este estudo foi inicialmente conduzido em população adulta normal, compreendendo doadores de Banco de Sangue, estudantes universitários e parturientes, totalizando 889 indivíduos. Foi observado que cerca de 87% desta população apresentava anticorpos específicos para o sarampo, e que o mesmo porcentual de positividade observado nas gestantes, foi encontrado nos seus respectivos conceptos dada a passagem transplacentária dos anticorpos maternos. Foi verificado o declínio desses anticorpos após o 4.° mês, do recém-nato. Os resultados à vacinação contra o sarampo foi estudada em 1268 crianças divididas em três grupos: I) vacinadas aos 7 meses e revacinadas aos 15 meses; II) vacinadas aos 9 meses e III) vacinadas aos 7 meses e revacinadas aos 9 meses. Os resultados deste estudo indicam que apesar da resposta à vacinação ter sido mais eficiente no grupo de crianças maiores, é importante que se vacine aos 7 meses de idade, embora a porcentagem de soroconversão tenha sido de 50%. Esta medida deve ser levada em consideração, tendo em vista que a mortalidade por sarampo em crianças com menos de 1 ano representa a metade dos óbitos pela doença. Foi verificado que após a aplicação da 2.° dose, não houve diferença quanto à soroconversão, tanto no grupo revacinado 2 meses ou 8 meses após a 1.º dose da vacina. Portanto, a vacinação aos 7 meses é necessária, visando diminuir a mortalidade e a morbidade dentro do 1.º ano de vida, e a revacinação aos 9 meses, a fim de imunizar as crianças não beneficiadas com a 1.ª dose.

Propõe-se um novo método de marcação de planorbídeos para determinação de densidade e de outros parâmetros pertinentes ao estudo de dinâmica de população, como capacidade de migração, taxas de crescimento e potencial biótico nos habitats naturais. Consiste em marcação através de dois furos de cerca de 2 mm de diâmetro de cada lado da concha em sua extremidade anterior, usando-se um estilete metálico. Após a marcação os caramujos são imediatamente reintroduzidos no habitat, em local correspondente ao da captura processada poucos minutos antes. Nos experimentos realizados, as taxas de caramujos marcados, recapturados uma semana após a marcação, variaram de 14 a 18% e permitiram calcular a população total de B. glabrata, de 5 mm de diâmetro ou mais. O método permite estimar também a população de jovens com menos de 5 mm e fazer marcações múltiplas. É muito simples, de fácil manejo, não exige equipamentos nem pessoal especializados e provoca menos impacto no ambiente do que outros métodos.

This report describes two patients with chronic Chagas' Heart Disease who developed clinical and laboratorial signs of myocardial infarction. Both patients presented sudden oppressive chest pain, without precipitating factor. In the first case, the highest MB-CK value was 65 IU, 22 hours after the beginning of the pain. On the second case, it was 77 IU at 18 hours after the beginning of the pain. In both cases ECG changes suggesting non-transmural infarction were present. The 99mTc PYP myocardial scintigram of the first case was positive. Coronary angiograms performed on the 18th and 9th day, respectively, after the acute infarction did not display obstructive lesions. Possible mechanisms causing myocardial infarction with normal coronary arteries in Chagas' Disease may include: embolic event's, particularly when there is associated congestive heart failure; coronary thrombosis and coronary spasms.

Se estudió la evolución de la artritis por adyuvante en ratas que habían sido infectadas previamente con Trypanosoma cruzi, con el objeto de evaluar su competencia inmunológica a través de la respuesta artrítica. La artritis por adyuvante se indujo en ratas adultas, endocriadas de ambos sexos, con 0.1 mi de adyuvante completo de Freund en la almohadilla plantar, en 2 lotes: a) inyectadas 90 días antes con 1 x 10(6) T. cruzi y b) testigos normales simultáneos. Se midieron, la lesión artrítica macroscópicamente con una escala semicuantitativa, y con microscopía óptica la histopatología de la lesión local y la del corazón, a los 180 días post-infecoión. La magnitud de las lesiones artríticas en las ratas con T. cruzi fue significativamente menor (p < 0.001) que la de los testigos, en todo el período. El infiltrado inflamatorio local, formado por linfocitos, plasmocitos y macrófagos fue significativamente menor (p < 0.001) en las ratas chagásicas, con respecto al de los testigos. Se postula que en las ratas que recibieron T. cruzi la respuesta artrítica menor podría deberse a una competición antigénica con los determinantes del parásito o a mecanismos inmunosupresores que interfieren en la producción de la entidad experimental.

A new cross-sectional survey of household- associated mongrel dogs as well as follow-up of previously parasitemic individuals was carried out in 1984 toy means of xenodiagnosis and serologic techniques to get a deeper insight into the relationship of T. cruzi parasitemia and age among canine hosts in a rural area of Argentina. Persistence of detectable parasitemia was age-independent, or at most, loosely related to age, confirming the pattern observed in 1982. Similarly no significant age-decreasing effect was recorded among seropositive dogs in: a) the probability of detecting parasites in a 2-year follow-up; b) their intensity of infectiousness (=infective force) for T. infestans 3rd-4th instar nymphs, as measured by the percentage of infected bugs observed in each dog xenodiagnosis. Moreover, not only was the infective force of seropositive dogs for bugs approximately constant through lifetime, but it was significantly higher than the one recorded for children in the present survey, and for human people by other researchers. Therefore, and since T. infestans field populations show high feeding frequencies on dogs, the latter are expected to make the greatest contribution to the pool of infected vectors in the rural household of Argentina. This characteristic should be sufficient to involve canine reservoirs definitely as a risk factor for human people residing in the same house. The increased severity of parasitemia observed among dogs in this survey may be related to the acute undernutrition characteristic of canine populations of poor rural areas in our country, which is expected to affect the ability of the host to manage the infection.

Estudamos comparativamente quatro técnicas imunológicas para o diagnóstico da neurocisticercose (NC) utilizando líquido cefalorraquiano (LCR) como espécime clínico: as reações de fixação de complemento (RFC), hemaglutinação passiva (RHA), imunofluorescência indireta (RIF) e teste imunoenzimático ELISA Foram ensaiadas 125 amostras de LCR de pacientes com NC comprovada e 94 amostras de LCR do grupo controle (60 de pacientes com quadros clínicos neurológicos diversos e 34 de pacientes supostamente normais). Os índices de sensibilidade e especificidade obtidos para os testes foram, respectivamente, de 48,0% e 90,4% para a RFC; 88,8% e 96,8% para a RHA; 87,2 e 98,9% para a RIF e 97,6% e 98,9% para o teste ELISA. A diferença significativa (p < 0,05) observada entre os testes permite concluir que o melhor teste para o diagnóstico de NC foi o teste ELISA seguido das reações de HA e IF.

In order to investigate whether gentian violet exhibited "in vitro" inhibitory activity against Plasmodium falciparum, the Authors have carried out 20 sensitivity tests according to the microtechnique described by RIECK MANN et al.5. Results have shown inhibition of schizonts'maturation at the following concentration: 1/1000; 1/1500; 1/2000; 1/2500; 1/3000 and 1/4000, thus demonstrating inhibitory activity of the tested dye against asexual blood parasites. The present data suggest gentain violet may be possibly used in the prophylaxis of transfusion-acquired malaria.

The antigenicity of promastigotes of Leishmania braziliensis braziliensis (L. b.braziliensis) treated with 1% sodium desoxycholate in 10 mM Tris-Hcl pH 8.2 was analysed by immunoblot using as probes sera from American cutaneous leishmaniasis (ACL), American visceral leishmaniasis (AVL), schistosomiasis, malaria and Chagas' disease. The ACL sera reacted constantly with a 60 kD band. No reactivity to this protein was observed with sera from the other diseases above mentioned indicating that the 60 kD protein may be used in serodiagnosis for ACL.

Foi realizada uma pesquisa na região de Campinas, SP, Brasil, sobre a presença de Escherichia coli enterotoxigênica (ETEC), rotavírus e Clostridium perfringens enterotoxigênico em fezes diarréicas de crianças com até 2 anos de idade. Dos 132 espécimens fecais examinados quanto à presença de ETEC 27 (20,45%) foram positivos. Destes foram isoladas 41 amostras de ETEC, das quais 40 produziram apenas a enterotoxina termolábil (LT) detectada pelo teste de imuno hemólise radial modifi cado. Entre as 183 amostras de fezes examinadas para rotavírus, 29 (15,84%) foram positivas pelas técnicas de eletroforese em gel de poliacrilamida (PAGE) e ensaio imunoenzimático (EIE), sendo que destas, 15 (51,7% ) foram provenientes de materiais coletados nos meses de inverno. Todas as amostras pertenciam ao grupo A e, através da técnica de PAGE, pode-se observar que o tipo eletroforético mais freqüente (9 amostras) foi designado Ib, IIc, Illb, IVa, de acordo com a classificação por nós adotada. Apenas 113 amostras de fezes foram examinadas para a presença de C. perfringens enterotoxigênico. Para a detecção da enterotoxina nos sobrenadantes das culturas foram utilizadas as técnicas de hemaglutinação passiva reversa e inoculação intravenosa em camundongos, sendo encontradas 12 (10,61%) amostras entero-toxigênicas. Diante destes resultados é chamada a atenção sobre o valor apenas relativo de uma coprocultura convencional para fins de diagnóstico, ressaltando-se a importância da criação de métodos simplificados que favoreçam a detecção e identificação dos grupos de agentes enteropatogênicos estudados na presente pesquisa.

El objetivo de este trabajo fue comprobar si una de las variables medio-ambientales, la reinfección, puede modificar el comportamiento observado en un modelo de rata a nivel de parasitemia, anticuerpos séricos, manifestaciones electrocardiográficas y/o lesión miocárdica. Los grupos experimentales fueron: GI: ratas infectadas al destete con 1 x 10(6) T. cruzi; GR: igual a GI más reinfecciones cada 30 días hasta los 150 días post-infección inicial (p.i.i.); GI1 ratas de 51 días infectadas; GT: testigos. Se detectó parasitemia alta en GI y GR hasta los 20 días p.i.i. tendiendo a negativizarse al día 30. En GR no se observaron parásitos despúes del primer reinóculo, resistencia que no es debida sólo a la mayor edad del huésped pués hubo parasitemia en GI1. Los xenodiagnósticos fueron negativos en los tres grupos. Los anticuerpos séricos no se modificaron significativamente en GR respecto de GI, salvo en los anticuerpos 7S, pues los del GR presentaron títulos superiores en algunos de los días estudiados. Los ECG basales no mostraron cambios distintivos en las ratas infectadas. La prueba de ajmalina mostró una disminución de la FC independiente del tratamiento; el PR, QaT y QRS se prolongaron significativamente en todos los grupos respecto del basal (p < 0.05), salvo el QaT en el GT; además, el cambio de PR y QaT fue mayor en los infectados (p < 0.05). En los grupos infectados hubo también una amplia variación en la orientación del eje eléctrico respecto del valor basal, acompañado de cambios morfológicos más manifiestos en GR. La proporción de lesión cardíaca detectada histológicamente en los grupos infectados, fue significativamente superior a la del GT (p < 0.01). Se concluye que la reinfección no reproduce el cuadro agudo inicial y no modifica el tipo y grado de lesión cardíaca observada histológicamete. La prueba de ajmalina muestra cambios electrocardiográficos compatibles con daño miocárdico incipiente en las ratas infectadas y sugieren mayor compromiso en el grupo reinfectado.

Os efeitos do levamisole nas alterações histopatológicas, resistência do hospedeiro e quimiotaxia "in vivo" foram estudados na infecção experimental pelo Schistosoma mansoni em camundongos da linhagem C57B1/10. O tratamento profilático resultou em um aumento no número de vermes adultos obtidos pela perfusão e também em uma taxa de mortalidade maior (p < 0,05). As alterações histopatológicas (fígado e intestino) foram similares em todos os grupos. Uma redução significante da quimiotaxia "in vivo" ocorreu em camundongos controles infectados, assim como naqueles submetidos a tratamento profilático com levamisole. A atividade quimiotática atingiu os mesmos níveis dos camundongos controles normais (não-infectados e não-tratados com levamisole), quando o esquema curativo foi usado. O levamisole parece aumentar a susceptibilidade de camundongos da linhagem C57B1/10 à infecção pelo S. mansoni quando administrado antes da infecção e normaliza a atividade quimiotática, quando dado após a infecção.
