A Doença Inflamatória Intestinal atinge uma grande parcela da população mundial e é uma doença com causa multifatorial, duas formas clínicas são reconhecidas atualmente: A Doença de Crohn e a Retocolite Ulcerativa. Muitos estudos relacionam o diagnóstico de DII com um aumento na quantidade da bactéria Escherichia coli, mas ainda não é possível dizer certamente se esse aumento na população de E. coli é causa ou consequência da doença. As Escherichia coli com o padrão de adesão agregativo são encontradas em pacientes com a Doença de Crohn e Retocolite Ulcerativa e o presente estudo irá investigar esse comportamento através do teste de adesão com incubação de 3 horas. A distribuição média dos índices de aderência não apresentam uma diferença estatisticamente significativa entre os pacientes com Doença de Crohn (60%), Retocolite Ulcerativa (40%) e o grupo controle (60%). Assim, obtivemos resultados conflitantes com o esperado pela literatura, não foi observado um aumento na prevalência de testes positivos de aderência nos pacientes diagnosticados com Doença de Crohn e Retocolite Ulcerativa em relação ao grupo controle

Nitric oxide (NO) is a free radical with participation in almost all physiologic host processes; however, in high concentrations it may damage the tissues. Its immunoregulatory action is present in the inflammation and in auto-immune mechanisms, being intensively studied in the medical area. Recently, some studies have also reported that NO could play a role as etiopathogenic factor of the periodontal disease, which shows inflammatory and multifactorial course. Due to beneficial or damage effects of NO, according to its concentrations, some studies have been focused in the evaluation of inhibitor of NO-Synthase (NOS) as therapeutic agents in inflammatory processes. In this context, the aim of this study was to report the role of NO and NOS inhibition in the periodontal disease modulation process. In conclusion, it could be suggested that NO seems to play an essential role in evolution of inflammatory periodontal disease, and that the NOS inhibition may be considered as a promising therapeutic in modulation of inflammatory process.

Crohn’s disease is a chronic infl ammatory bowel disease with segmental transmural infl ammation, which complicate with formation of fi stulas and abscesses. The hidradenitis suppurativa (HS) is characterized by recurrent abscesses, with a predilection for areas rich in apocrine glands such as the axillary, inguinal and perineal. The differential diagnosis between these diseases is diffi cult and may compromise treatment. Report case: C.R.M.A., 40 year-old, female, white, ileal and colonic Crohn’s disease complicated with perianal and rectovaginal fi stula for 12 years, treated with biological therapy since May 2010. In Sep/2010 presented with an abscess in the buttock D with purulent discharge refractory to the use of ciprofl oxacin and metronidazole. USG: collection of 30 cm3 in buttock D. The diagnosis was HS and the patient underwent extensive surgical removal of the affected areas (10 x 2 cm) with healing by secondary intention. Skin graft performed unsuccessfully in Dec/2010. The patient returned in jan/2011 with a new fi stula at the site of resection, consistent with Crohn’s disease. In fev/2011 underwent drainage of abscesses and placement of setons in perianal fi stulas. Currently in therapy with good biological evolution of fi stulas. The prevalence of HS varies from 0.3 to 4% of the population in general. The axilla is the region most affected and perianal lesions are associated with greater weakness. There are published reports of association between HS and Crohn’s disease sporadically and further studies are needed to assess a common pathogenesis. The differential diagnosis should be performed in all cases planning immediate treatment, avoiding complications and worsening of the patient’s quality of life.

The occurrence of chronic inflammatory periodontal disease due to dental plaque in adults over 30 years of age was noticed in 47.2% of the 3,742 subjects studied, representing a 64.7 million people population in the 50 states of the U.S. and the District of Columbia. The methods used by the authors were uniquely precise and the results brought us to great insight on the correlation between orthodontic treatment and periodontal health, which is described below.

In the northeast of Brazil, caprine arthritis-encephalitis (CAE) is one of the key reasons for herd productivity decreasing that result in considerable economic losses. A comparative study was carried out using computed radiography (CR), histological analysis (HA), and scanning electronic microscopy (SEM) of the joints of CAE infected and normal goats. Humerus head surface of positive animals presented reduced joint space, increased bone density, and signs of degenerative joint disease (DJD). The carpal joint presented no morphological alterations in CR in any of the animals studied. Tarsus joint was the most affected, characterized by severe DJD, absence of joint space, increased periarticular soft tissue density, edema, and bone sclerosis. Histological analysis showed chronic tissue lesions, complete loss of the surface zone, absence of proteoglycans in the transition and radial zones and destruction of the cartilage surface in the CAE positive animals. Analysis by SEM showed ulcerated lesions with irregular and folded patterns on the joint surface that distinguished the limits between areas of normal and affected cartilage. The morphological study of the joints of normal and CAE positive goats deepened understanding of the alteration in the tissue bioarchitecture of the most affected joints. The SEM finding sustained previous histological reports, similar to those found for rheumatoid arthritis, suggesting that the goat infected with CAE can be considered as a potential model for research in this area.

Multiple sclerosis is the most common autoimmune inflammatory demyelinating disease of the central nervous system, and its etiology is believed to have both genetic and environmental components. Several viruses have already been implicated as triggers and there are several studies that implicate members of the Herpesviridae family in the pathogenesis of MS. The most important characteristic of these viruses is that they have periods of latency and exacerbations within their biological sanctuary, the central nervous system. The Epstein-Barr, cytomegalovirus, human herpesvirus 6 and human herpesvirus 7 viruses are the members that are most studied as being possible triggers of multiple sclerosis. According to evidence in the literature, the herpesvirus family is strongly involved in the pathogenesis of this disease, but it is unlikely that they are the only component responsible for its development. There are probably multiple triggers and more studies are necessary to investigate and define these interactions.

Die humane induzierbare NO-Synthase (iNOS) spielt bei zahlreichen Erkrankungen wie Asthma, Krebs und der rheumatoiden Arthritis eine entscheidende Rolle. Durch Fehlregulation der iNOS-Expression kommt es häufig zu massiven Gewebeschädigungen. Aus diesem Grund ist es wichtig die Mechanismen der Genregulation der iNOS-Expression zu verstehen. Bei Affinitätschromatographie-Analysen wurde das zytosolische PolyA-bindende Protein (PABP) als direkter Interaktionspartner der 3´UTR der humanen iNOS identifiziert. Weitere Bindungsanalysen konnten eine spezifische Bindestelle für PABP in der 5´UTR und zwei Bindestellen im AU-reichen Bereich der 3´UTR der humanen iNOS nachweisen. Eine siRNA-mediierte Herabregulation von PABP mit Hilfe der stabilen Expression spezifischer siRNAs in DLD-1 Zellen (siPABP Zellen) zeigte eine signifikant verringerte Expression der humanen iNOS und damit einhergehend eine verringerte NO-Produktion nach Zytokinstimulation. Promotoranalysen zeigten keine Veränderung der Induzierbarkeit des humanen 16 kb iNOS-Promotors in siPABP Zellen. RNA-Stabilitätsanalysen zeigten einen verstärkten Abbau der iNOS-mRNA in diesen Zellen, so dass davon auszugehen ist, dass die Regulation der humanen iNOS über die mRNA-Stabilität erfolgt. Reportergen-Analysen mit Plasmiden, welche die 5’ und/oder 3’UTR Sequenzen der humanen iNOS mit den identifizierten PABP-Bindestellen oder Mutationen in diesen Bindestellen enthielten, zeigten, dass PABP die iNOS-mRNA über die 5´UTR stabilisiert und anscheinend über die 3´UTR einen destabilisierenden Effekt auf die mRNA ausübt. Ebenfalls scheint PABP über die 3’UTR dieTranslation der iNOS mRNA zu hemmen. Die Ergebnisse dieser Arbeit zeigen, dass PABP, über seine allgemeinen Funktionen hinaus, eine spezifische Rolle in der Regulation der Expression der humanen iNOS einnimmt.rnDie rheumatoide Arthritis (RA) ist eine chronisch entzündliche Autoimmunerkrankung, welche überwiegend die peripheren Gelenke der Hände und Füße betrifft. Die aktuellen Therapiemöglichkeiten sind immer noch mit einer Vielzahl von Nebenwirkungen behaftet und führen nicht zur vollständigen Remission der Erkrankung, so dass die Entwicklung neuer Medikamente unerlässlich ist. In dieser Arbeit wurden die antiinflammatorischen Substanzen Gallielalacton (Gal) und Oxacyclododecindion (Oxa) im Mausmodell der kollagen-induzierten Arthritis (CIA) getestet. Leider waren beide Substanzen nicht in der Lage die Symptome der CIA zu vermindern, obwohl beide im Modell der LPS-induzierten akuten Entzündung die Expression proinflammatorischer Mediatoren senken konnten. Die Substanz S-Curvularin (SC) hat sich im CIA-Modell bereits bewährt und wurde in dieser Arbeit weiter untersucht. SC war in der Lage die Expression knorpel- und knochendestruktiver Markergene signifikant zu verrindern. rnIn der vorliegenden Arbeit wurden neue microRNAs identifiziert, die in der Pathogenese der CIA eine Dysregulation zeigen. Die Expression dieser microRNAs wurde von SC wieder auf das Normalniveau gebracht, so dass SC eine vielversprechende Substanz in der Therapie chronisch inflammatorische Erkrangungen sein könnte. Die neu identifizierten CIA-relevanten microRNAs könnten als neueRA-Marker oder als Zielstrukturen für neue Medikamente dienen.rn

How a mutualistic relationship between the intestinal microbiota and intestinal T cell compartments is established is important, as a breakdown of intestinal T cell homeostasis may cause inflammatory bowel diseases. A number of studies have shown that different bacterial species modulate the intestinal CD4+ T cell compartment in different ways. We performed mechanistic in vivo studies that demonstrated the crucial requirement for regulatory T cells (Treg) and interleukin-10 (IL-10) in the induction of intestinal T cell homeostasis even following colonization with a completely benign microbiota. In the absence of a functional Treg response or IL-10 receptor signaling, the same bacteria that induced a Treg response in wild-type animals now induced T helper type 17 responses, without intestinal inflammation. Therefore, Treg, IL-10 and Th17 are crucial regulatory mechanisms in the intestine not only for controlling inflammation, but also to establish a continuum of CD4+ T cell homeostasis upon commensal colonization.

REASONS FOR PERFORMING STUDY: The horse owner assessed respiratory signs index (HOARSI-1-4, healthy, mildly, moderately and severely affected, respectively) is based on owner-reported clinical history and has been used for the investigation of recurrent airway obstruction (RAO) genetics utilising large sample sizes. Reliable phenotype identification is of paramount importance in genetic studies. Owner reports of respiratory signs have shown good repeatability, but the agreement of HOARSI with an in-depth examination of the lower respiratory tract has not been investigated. OBJECTIVES: To determine the correlation of HOARSI grades 3/4 with the characteristics of RAO and of HOARSI-2 with the characteristics of inflammatory airway disease. Further, to test whether there are phenotypic differences in the manifestation of lung disease between families. METHODS: Seventy-one direct offspring of 2 RAO-affected Warmblood stallions (33 from the first family, 38 from the second) were graded as HOARSI-1-4 and underwent a clinical examination of the respiratory system, arterial blood gas analysis, endoscopic mucus scoring, cytology of tracheobronchial secretion (TBS) and bronchoalveolar lavage fluid (BALF), and clinical assessment of airway reactivity to methacholine chloride. RESULTS: HOARSI-3/4 animals in clinical exacerbation showed signs consistent with RAO: coughing, nasal discharge, abnormal lung sounds and breathing pattern as well as increased numbers of neutrophils in TBS and BALF, excessive mucus accumulation and airway hyper-responsiveness to methacholine. HOARSI-3/4 horses in remission only had increased amounts of tracheal mucus and TBS neutrophil percentages. Clinical phenotypes were not significantly different between the 2 families. CONCLUSIONS AND CLINICAL RELEVANCE: HOARSI reliably identifies RAO-affected horses in our population.

Intestinal macrophages, preferentially located in the subepithelial lamina propria, represent in humans the largest pool of tissue macrophages. To comply with their main task, i.e. the efficient removal of microbes and particulate matter that might have gained access to the mucosa from the intestinal lumen while maintaining local tissue homeostasis, several phenotypic and functional adaptations evolved. Most notably, microbe-associated molecular pattern (MAMP) receptors, including the lipopolysaccharide receptors CD14 and TLR4, but also the Fc receptors for IgA and IgG are absent on most intestinal Mø. Here we review recent findings on the phenotypic and functional adaptations of intestinal Mø and their implications for the pathogenesis of inflammatory bowel diseases.

The clinical impact and outcome of a rare radiographic finding of hepatic portal venous gas (HPVG) as well as the effectiveness of computed tomography (CT), CT scanogram, and conventional radiography in the detection of HPVG were retrospectively analyzed. CT scans, CT scanogram, and plain film radiographs of 11 patients with HPVG were reviewed and compared with their medical records and surgical and pathology reports. Eight of the 11 patients underwent plain film radiographs 1 day before or after the CT scan. HPVG was detected at CT in all 11 patients, on CT scanogram in three (3 of 11, 27.3%), and on plain films in one (one of eight, 12.5%). In nine of 11 patients (81.8%), CT revealed an associated pneumatosis intestinalis. In six of the 11 patients (54.6%), acute mesenteric ischemia was the underlying disease for HPVG. Seven patients (63.6%) underwent emergency exploratory laparotomy. The mortality rate for HPVG alone was 27.3% (3 of 11) and for HPVG related to mesenteric bowel disease 50% (three of six). Acute mesenteric ischemia is the most common cause of HPVG, which continues to have a predictably higher mortality. CT is superior to CT scanograms and radiographs in the detection of HPVG and its underlying diseases and, therefore, should be used as the primary diagnostic tool.
