OBJETIVO: Analisar coeficientes de internações por causas sensíveis à atenção primária. MÉTODOS: Foram utilizados dados do Sistema de Informações Hospitalares do Sistema Único de Saúde no Distrito Federal em 2008. O diagnóstico principal da internação foi analisado com base na Classificação Internacional de Doenças e foram calculados frequência absoluta, proporção e coeficiente segundo causas, faixas etárias e sexo. RESULTADOS: As causas sensíveis à atenção primária (CSAP) representaram cerca de 20% das internações no Sistema Único de Saúde. As causas mais frequentes foram: gastroenterites (2,4%), insuficiência cardíaca (2,3%) e infecção do rim e trato urinário (2,1%). Constataram-se coeficientes de internações por causas sensíveis à atenção primária relevantes no grupo infantil (< 1 ano), redução importante nos grupos etários seguintes (um a 29 anos) e aumento gradativo até as idades mais avançadas. Comparados aos dos homens, os coeficientes de internações foram discretamente maiores em mulheres jovens (20 a 29 anos) e menores em mulheres com mais de 49 anos. CONCLUSÕES: As internações por CSAP representaram 19,5% do total de internações ocorridas no Distrito Federal (2008), e as principais causas de internações foram gastroenterites, insuficiência cardíaca e infecção do rim e trato urinário. A efetividade da atenção primária em saúde no Distrito Federal para a prevenção desses eventos é discutida.

Si3N4 tools were coated with a thin diamond film using a Hot-Filament Chemical Vapour Deposition (HFCVD) reactor, in order to machining a grey cast iron. Wear behaviour of these tools in high speed machining was the main subject of this work. Turning tests were performed with a combination of cutting speeds of 500, 700 and 900 m min−1, and feed rates of 0.1, 0.25 and 0.4 mm rot−1, remaining constant the depth of cut of 1 mm. In order to evaluate the tool behaviour during the turning tests, cutting forces were analyzed being verified a significant increase with feed rate. Diamond film removal occurred for the most severe set of cutting parameters. It was also observed the adhesion of iron and manganese from the workpiece to the tool. Tests were performed on a CNC lathe provided with a 3-axis dynamometer. Results were collected and registered by homemade software. Tool wear analysis was achieved by a Scanning Electron Microscope (SEM) provided with an X-ray Energy Dispersive Spectroscopy (EDS) system. Surface analysis was performed by a profilometer.

Purpose: Because walking is highly recommended for prevention and treatment of obesity and some of its biomechanical aspects are not clearly understood for overweight people, we compared the absolute and normalized ground reaction forces (GRF), plantar pressures, and temporal parameters of normal-weight and overweight participants during overground walking. Method: A force plate and an in-shoe pressure system were used to record GRF, plantar pressures (foot divided in 10 regions), and temporal parameters of 17 overweight adults and 17 gender-matched normal-weight adults while walking. Results: With high effect sizes, the overweight participants showed higher absolute medial-lateral and vertical GRF and pressure peaks in the central rearfoot, lateral midfoot, and lateral and central forefoot. However, analyzing normalized (scaled to body weight) data, the overweight participants showed lower vertical and anterior-posterior GRF and lower pressure peaks in the medial rearfoot and hallux, but the lateral forefoot peaks continued to be greater compared with normal-weight participants. Time of occurrence of medial-lateral GRF and pressure peaks in the midfoot occurred later in overweight individuals. Conclusions: The overweight participants adapted their gait pattern to minimize the consequences of the higher vertical and propulsive GRF in their musculoskeletal system. However, they were not able to improve their balance as indicated by medial-lateral GRF. The overweight participants showed higher absolute pressure peaks in 4 out of 10 foot regions. Furthermore, the normalized data suggest that the lateral forefoot in overweight adults was loaded more than the proportion of their extra weight, while the hallux and medial rearfoot were seemingly protected.
Avaliação do II Curso de Saúde Pública da Secretaria de Saúde do Distrito Federal: um estudo de caso

Os cursos de especialização em Saúde Pública atualmente existentes entre nós, com seu formato diferencial de curta duração e alta abrangência, constituem uma realidade recente em nosso país, contando com pouco mais de dez anos. No Distrito Federal, realizaram-se nove deles, dos quais seis com a atuação diretiva e docente dos autores. Com vistas a incorporar tal experiência acumulada como contribuição para com futuras iniciativas a respeito, apresentam-se os resultados obtidos com avaliação terminal procedida em um deles, executada segundo o modelo atualmente recomendado pela Escola Nacional de Saúde Pública. Foi ela aplicada em amostra randômica de alunos, docentes e coordenadores, na busca de identificação de apreciações positivas e negativas, sobretudo dos aspectos operacional, administrativo, metodológico e institucional do mesmo. Em meio a percepções e formulações heterogêneas e pluralistas, obtiveram maiores freqüências em suas respectivas distribuições, por um lado, a adequação do trabalho de campo, baseado na utilização de quatro regionais, acompanhadas pelos alunos após cada área temática e, por outro, a preocupação para com a fragilidade de decisão institucional quanto a oferta e manutenção do curso, bem como quanto ao aproveitamento dos egressos para consolidação do sistema de saúde do Distrito Federal. Tais aspectos merecem, ao final, destacada discussão.

Três novos casos humanos de infecção por Lagochilascaris minor são apresentados. Todos os pacientes eram oriundos do Estado do Pará, Brasil, e viviam em zona rural ou próximo à mata. As lesões, no momento da consulta, localizavam-se no pescoço, mastóide e ouvido, estando já fistulizadas; em dois casos havia também envolvimento do sistema nervoso central. Amostras de tecidos, retiradas das lesões, revelaram, ao exame histológico, ovos e vermes em vários estádios de sua evolução. A identificação da espécie foi confirmada através do estudo de larvas e adultos eliminados, pelas fístulas cutâneas, de mistura com pus. O tratamento consistiu em excisão cirúrgica das tumorações e administração de antihelmínticos, com bons resultados. Em dois pacientes, no entanto, houve recidiva das lesões, após cura aparente, indicando que as drogas utilizadas não destroem os ovos do parasito.

Starting from theoretical perspectives on globalisation, the following article analyses how current working conditions are affected by globalisation processes. For this purpose, recent developments in the German clothing sector are traced back to the power of economic globalisation processes. Characterising the German clothing sector as pioneer in economic globalisation, we use empirical findings to illustrate how current processes of globalisation influence the work place: At organisational level, corporate strategies aim at rationalisation, standardisation and flexibilisation of work in order to response to the economic pressure of global markets. At individual level these strategies, in turn, speed up working processes and intensify working processes for the employees. Although these developments form strong trends, we conclude that the local embeddedness of companies is still of high importance with regard to organisational and individual consequences of globalisation.

Nowadays 70% of the world's rubber supply is synthesized artificially. The process involved in its manufacture is vulcanization which requires many chemical substances for speeding the process, as antioxidants to prevent deterioration of rubber, or others. These substances may constitute important sensitizers and thus be responsible for dermatological diseases like contact dermatitis. The objective of this study is to search for the main sensitizers among these rubber chemicals in a population mostly composed by women of a tropical country and compare the results with the ones obtained from previous studies which tested populations mainly composed by men and on different climates.

The Federal District of Brazil (DF) lies within the Cerrado biome, where open shrubland (savannas) is interspersed with riverside gallery forests and permanent swamps (veredas). Trypanosoma cruzi-infected native triatomines occur in the area, but the enzootic transmission of trypanosomatids remains poorly characterized. A parasitological survey involving sylvatic triatomines (166 Rhodnius neglectus collected from Mauritia flexuosa palms) and small mammals (98 marsupials and 70 rodents, totaling 18 species) was conducted in 18 sites (mainly gallery forests and veredas) of the DF. Parasites were isolated, morphologically identified, and characterized by PCR of nuclear (mini-exon gene) and kinetoplast DNA (kDNA). Six R. neglectus, seven Didelphis albiventris and one Akodon cursor were infected by trypanosomes; wild reservoir infection is documented for the first time in the DF. kDNA PCR detected T. cruzi in five R. neglectus and mini-exon gene PCR revealed T. cruzi I in isolates from D. albiventris. Parasites infecting one bug yielded T. rangeli KP1+ kDNA amplicons. In spite of the occurrence of T. cruzi-infected D. albiventris (an important wild and peridomestic reservoir) and R. neglectus (a secondary vector displaying synanthropic behavior), a low-risk of human Chagas disease transmission could be expected in the DF, considering the low prevalence infection recorded in this work. The detection of T. rangeli KP1+ associated with R. neglectus in the DF widens the known range of this parasite in Brazil and reinforces the hypothesis of adaptation of T. rangeli populations (KP1+ and KP1-) to distinct evolutionary Rhodnius lineages.

Biomechanical gait parameters—ground reaction forces (GRFs) and plantar pressures—during load carriage of young adults were compared at a low gait cadence and a high gait cadence. Differences between load carriage and normal walking during both gait cadences were also assessed. A force plate and an in-shoe plantar pressure system were used to assess 60 adults while they were walking either normally (unloaded condition) or wearing a backpack (loaded condition) at low (70 steps per minute) and high gait cadences (120 steps per minute). GRF and plantar pressure peaks were scaled to body weight (or body weight plus backpack weight). With medium to high effect sizes we found greater anterior-posterior and vertical GRFs and greater plantar pressure peaks in the rearfoot, forefoot and hallux when the participants walked carrying a backpack at high gait cadences compared to walking at low gait cadences. Differences between loaded and unloaded conditions in both gait cadences were also observed.

Hepatitis C virus (HCV) genotypes and subtypes were determined in hemodialysis patients in the Federal District, Brazil, by sequencing of the 5' noncoding (NC) and nonstructural 5B (NS5B) regions. From 761 patients, 66 anti-HCV-positive samples were tested for HCV RNA. All 51 HCV RNA-positive samples by PCR of the 5' NC region were genotyped as genotypes 1 (90.2%) and 3 (9.8%). Subtype 1a (82.3%) was the most prevalent, followed by subtypes 3a (9.8%), 1b (5.9%) and 1a/1b (2.0%). Forty-two samples could be amplified and genotyped in the NS5B region: 38 (90.5%) as genotype 1, subtypes 1a, and 8 (9.5%) as genotype 3, subtype 3a. For the 42 samples sequenced in both regions, the genotypes and subtypes determined were concordant in 100% and 95.2% of cases, respectively. Two samples presented discrepant results, with the 5' NC region not distinguishing correctly the subtypes 1a and 1b. These findings indicate that the HCV genotype 1, subtype 1a, is the most prevalent among hemodialysis patients in the Federal District, Brazil.

With the aim of identifying the etiology of acute febrile illness in patients suspected of having dengue, yet with non reagent serum, a descriptive study was conducted with 144 people using secondary serum samples collected during convalescence. The study was conducted between January and May of 2008. All the exams were re-tested for dengue, which was confirmed in 11.8% (n = 17); the samples that remained negative for dengue (n = 127) were tested for rubella, with 3.9% (n = 5) positive results. Among those non reactive for rubella (n = 122), tests were made for leptospirosis and hantavirus. Positive tests for leptospirosis were 13.9% (n = 17) and none for hantavirus. Non reactive results (70.8%) were considered as Indefinite Febrile Illness (IFI). Low schooling was statistically associated with dengue, rubella and leptospirosis (p = 0.009), dyspnea was statistically associated with dengue and leptospirosis (p = 0.012), and exanthem/petechia with dengue and rubella (p = 0.001). Among those with leptospirosis, activities in empty or vacant lots showed statistical association with the disease (p = 0.013). Syndromic surveillance was shown to be an important tool in the etiologic identification of IFI in the Federal District of Brazil.

Vaccination of health care workers is an efficient way to reduce the risk of occupational infection and to prevent nosocomial transmission to vulnerable patients. Despite this, achieving high immunization rates among those professionals is a challenge. We assessed the immunization status of Residents in Pediatrics at the Federal University of São Paulo from June to December 2008. Their immunization records were checked and evaluated according to the Brazilian Immunization Schedule for health care workers. Considering all required vaccines, only 3.1% of the 64 Residents were up-to-date with their immunizations. Influenza was the vaccine with the lowest uptake (3.1%) and measles and rubella were diseases with the highest evidence of immunity (62.5% each). Only 37.5% of Residents had received three hepatitis B vaccine doses with a subsequent serology confirming seroconversion. Moreover, the vast majority of Residents in Pediatrics who were not up-to-date were unaware of the fact. Both medical schools and Pediatric Residence programs should not only offer information but also check vaccination records in an effort to keep their healthcare workers´ vaccinations up-to-date.

Sôros coletados no Território Federal do Amapá, localidade de Ferreira Gomes, foram, testados para anticorpos neutralizantes de enterovírus Poliovírus 1, 2 e 3 e Coxsackie B1 a B6, em população autóctone. Os resultados apresentados na Tabela 1, indicam alta circulação do vírus da poliomielite na região atingindo-se níveis tão elevados como em escolares do Estado da Guanabara. Em relação a Coxsackie B, alcançaram-se também resultados semelhantes nas duas populações exceto com Coxsackie B., o qual apresentou-se em valôres bem mais altos nos sôros coletados na região norte. Os autores chamam atenção da necessidade de vacinação contra a poliomielite de grandes segmentos da população susceptível em curto prazo, tendo em vista a ampla disseminação das infecções por poliovírus em tôdas as regiões do país onde forem pesquisadas.
