Background The reduction of exercise capacity because of fatigue and dyspnea in patients with heart failure can be improved with exercise training. We sought to examine the mechanisms of exercise training, as an adjunctive treatment strategy for patients with heart failure. Methods a reviewed the published data on the possible mechanisms of effect of exercise training in heart failure. Results Symptoms of heart failure may be explained on the basis of abnormal skeletal muscle perfusion and structure and endothelial function. Exercise training has been shown to engender changes in muscle structure and biochemistry and vascular function, although effects on cardiac function have not been detected uniformly and may require longer training periods. Conclusions A suitable, long-term program of exercise training may reverse unfavorable interactions among the heart, vessels, and skeletal muscles. These improvements may be preserved with an ongoing maintenance program.

Background Exercise testing has limited efficacy for identifying coronary artery disease (CAD) in the absence of anginal. symptoms. Exercise echocardiography is more accurate than standard exercise testing, but its efficacy in this situation has not been defined. We sought to identify whether the Duke treadmill. score or exercise echocardiography (ExE) could be used to identify risk in patients without anginal symptoms. Methods We studied 1859 patients without typical or atypical angina, heart failure, or a history or ECG evidence of infarction or CAD, who were referred for ExE, of whom 1832 (age 51 15 years, 944 men) were followed for up to 10 years. The presence and extent of ischaemia and scar were interpreted by expert reviewers at the time of the original study. Results Exercise provoked significant (>0.1 mV) ST segment depression in 215 patients (12%), and wall motion abnormalities in 137 (8%). The left ventricular response to dobutamine may be quantified using tissue Doppler measurement of myocardial velocity or displacement or 3-dimensional echocardiography to measure ventricular volume and ejection fraction. This study sought to explore the accuracy of these methods for predicting segmental and global responses to therapy. Standard dobutamine and 3-dimensional echocardiography were performed in 92 consecutive patients with abnormal left ventricular function at rest. Recovery of function was defined by comparison with follow-up echocardiography at rest 5 months later. Segments that showed improved regional function at follow-up showed a higher increment in peak tissue Doppler velocity with dobutamine therapy than in nonviable segments (1.2 +/- 0.4 vs 0.3 +/- 0.2 cm/s, p = 0.001). Similarly, patients who showed a > 5% improvement of ejection fraction at follow-up showed a greater displacement response to dobutamine (6.9 +/- 3.2 vs 2.1 +/- 2.3 mm, p = 0.001), as well as a higher rate of ejection fraction, response to dobutamine (9 +/- 3% vs 2 +/- 2%, p = 0.001). The optimal cutoff values for predicting subsequent recovery of function at rest were an increment of peak velocity > 1 cm/s, >5 mm of displacement, and a >5% improvement of ejection fraction with low-dose dobutamine. The health benefits provided by probiotic bacteria have led to their increasing use in fermented and other dairy products. However, their viability in these products is low. Encapsulation has been investigated to protect the bacteria in the product's environment and improve their survival. There are two common encapsulation techniques, namely extrusion and emulsion, to encapsulate the probiotics for their use in the fermented and other dairy products. This review evaluates the merits and limitations of these two techniques, and also discusses the supporting materials and special treatments used in encapsulation processes. Ascorbic acid or vitamin C is involved in a number of biochemical pathways that are important to exercise metabolism and the health of exercising individuals. This review reports the results of studies investigating the requirement for vitamin C with exercise on the basis of dietary vitamin C intakes, the response to supplementation and alterations in plasma, serum, and leukocyte ascorbic acid concentration following both acute exercise and regular training. The possible physiological significance of changes in ascorbic acid with exercise is also addressed. Exercise generally causes a transient increase in circulating ascorbic acid in the hours following exercise, but a decline below pre-exercise levels occurs in the days after prolonged exercise. These changes could be associated with increased exercise-induced oxidative stress. On the basis of alterations in the concentration of ascorbic acid within the blood, it remains unclear if regular exercise increases the metabolism of vitamin C. However, the similar dietary intakes and responses to supplementation between athletes and nonathletes suggest that regular exercise does not increase the requirement for vitamin C in athletes. Two novel hypotheses are put forward to explain recent findings of attenuated levels of cortisol postexercise following supplementation with high doses of vitamin C.

Evaluation of patients for rehabilitation after musculoskeletal injury involves identifying, grading and assessing the injury and its impact on the patient's normal activities. Management is guided by a multidisciplinary team, comprising the patient, doctor and physical therapist, with other health professionals recruited as required. Parallel interventions involving the various team members are specified in a customised management plan. The key component of the plan is active mobilisation utilising strengthening, flexibility and endurance exercise programs. Passive physical treatments (heat, ice, and manual therapy), as well as drug therapy and psychological interventions, are used as adjunctive therapy. Biomechanical devices or techniques (eg, orthotic devices) may also be helpful. Coexisting conditions such as depression and drug dependence are treated at the same time as the injury. Effective team communication, simulated environmental testing and, for those employed, contact with the employer facilitate a staged return to normal living, sports and occupational activities.

Borderline hypertension (BH) has been associated with an exaggerated blood pressure (BP) response during laboratory stressors. However, the incidence of target organ damage in this condition and its relation to BP hyperreactivity is an unsettled issue. Thus, we assessed the Doppler echocardiographic profile of a group of BH men (N = 36) according to office BP measurements with exaggerated BP in the cycloergometric test. A group of normotensive men (NT, N = 36) with a normal BP response during the cycloergometric test was used as control. To assess vascular function and reactivity, all subjects were submitted to the cold pressor test. Before Doppler echocardiography, the BP profile of all subjects was evaluated by 24-h ambulatory BP monitoring. All subjects from the NT group presented normal monitored levels of BP. In contrast, 19 subjects from the original BH group presented normal monitored BP levels and 17 presented elevated monitored BP levels. In the NT group all Doppler echocardiographic indexes were normal. All subjects from the original BH group presented normal left ventricular mass and geometrical pattern. However, in the subjects with elevated monitored BP levels, fractional shortening was greater, isovolumetric relaxation time longer, and early to late flow velocity ratio was reduced in relation to subjects from the original BH group with normal monitored BP levels (P<0.05). These subjects also presented an exaggerated BP response during the cold pressor test. These results support the notion of an integrated pattern of cardiac and vascular adaptation during the development of hypertension.

As diversas modalidades de ascetismo feminino que vigoraram nas comunidades cristãs ao longo dos três primeiros séculos do Império tenderão a se integrar numa nova experiência religiosa que começa a se esboçar por volta de 270 para alcançar uma expansão considerável no final do Mundo Antigo: o monacato. Mediante a análise da História Lausíaca, de Paládio de Helenópolis, obra escrita por volta de 420, investigamos a posição social das ascetas e o tipo de atuação que desempenharam no interior de um movimento dominado pelos homens, como foi o movimento monástico. Além disso, analisamos a capacidade das mulheres de exercer alguma modalidade de poder no âmbito das suas comunidades locais em comparação aos homens divinos cristãos do Baixo Império.

Trata-se de uma pesquisa-intervenção com o objetivo principal de analisar como os professores fazem exercício protagonista na atividade. Embasa-se principalmente no aporte conceitual da perspectiva ergológica de Yves Schwartz para realizar a análise da atividade no cotidiano escolar. O estudo foi desenvolvido em uma Escola Municipal de Ensino Fundamental de Serra/ES com duas professoras e um professor de Educação Física. As estratégias metodológicas utilizadas foram constituídas de acompanhamento da atividade desenvolvida pelos professores nas aulas, no Curso de Formação Permanente de Área de Educação Física, promovido pela Secretaria Municipal de Educação, participação no cotidiano escolar e a construção coletiva de um processo de formação na escola por meio de oficinas de práticas corporais. As técnicas de produção e registro dos dados foram compostas por diário de campo, fotos, conversas gravadas em áudio, entrevistas, questionário e documentos elaborados pela escola, como o Projeto Político-Pedagógico. A confrontação dos dados realizada com os docentes nas conversas produziu importantes deslocamentos nas análises e convocou autores e conceitos que não estavam previstos para o debate sobre o exercício protagonista. A produção dos dados gerou mudança na atividade docente e colocou em questão modos de gestão e organização do trabalho na Rede Serra de Educação, provocando transformações nos modelos de formação permanente dos professores de Educação Física do município. Os exercícios protagonistas empreendidos na atividade docente colocaram competências em circulação e tornaram visíveis os efeitos de trama e urdidura que atravessam e compõem a atividade docente dos professores de Educação Física no cotidiano escolar.

Esta dissertação apresenta o desenvolvimento de uma plataforma multimodal de aquisição e processamento de sinais. O projeto proposto insere-se no contexto do desenvolvimento de interfaces multimodais para aplicação em dispositivos robóticos cujo propósito é a reabilitação motora adaptando o controle destes dispositivos de acordo com a intenção do usuário. A interface desenvolvida adquire, sincroniza e processa sinais eletroencefalográficos (EEG), eletromiográficos (EMG) e sinais provenientes de sensores inerciais (IMUs). A aquisição dos dados é feita em experimentos realizados com sujeitos saudáveis que executam tarefas motoras de membros inferiores. O objetivo é analisar a intenção de movimento, a ativação muscular e o início efetivo dos movimentos realizados, respectivamente, através dos sinais de EEG, EMG e IMUs. Para este fim, uma análise offline foi realizada. Nessa análise, são utilizadas técnicas de processamento dos sinais biológicos e técnicas para processar sinais provenientes de sensores inerciais. A partir destes, os ângulos da articulação do joelho também são aferidos ao longo dos movimentos. Um protocolo experimental de testes foi proposto para as tarefas realizadas. Os resultados demonstraram que o sistema proposto foi capaz de adquirir, sincronizar, processar e classificar os sinais combinadamente. Análises acerca da acurácia dos classificadores utilizados mostraram que a interface foi capaz de identificar intenção de movimento em 76, 0 ± 18, 2% dos movimentos. A maior média de tempo de antecipação ao movimento foi obtida através da análise do sinal de EEG e foi de 716, 0±546, 1 milisegundos. A partir da análise apenas do sinal de EMG, este valor foi de 88, 34 ± 67, 28 milisegundos. Os resultados das etapas de processamento dos sinais biológicos, a medição dos ângulos da articulação, bem como os valores de acurácia e tempo de antecipação ao movimento se mostraram em conformidade com a literatura atual relacionada.

Bone weakening can occur due to the absence of load on the skeleton or even short periods of decreased physical activity. Therefore, musculoskeletal diseases that involve temporary immobilization by casts, inactivity or tension increases the risk of fractures. Physical activity is the most studied procedure both to prevent damage and to restore bone structure. The present study aimed at evaluating, by bone densitometry on rat femurs, the influence of hindlimb unloading and later running activity on treadmill or free movement. Sixty-four Wistar rats were used, aged 65 days with a mean corporal mass of 316.11g, randomly divided into eight experimental groups: group 1, the suspended control with seven animals under hindlimb unloading regimen for 28 days, then euthanized; groups 2 and 3, the trained suspended comprising of 7 and five animals, respectively, subjected to hindlimb unloading for 28 days, followed by treadmill exercise for 28 days (group 2) or 56 days (group 3), then euthanized; groups 4 and 5, designated free suspended, comprised of 7 animals each under hindlimb unloading regimen for 28 days followed by free activity in cages for 28 days (group 4) or 56 days (group 5), then euthanized; groups 6, 7 and 8, negative controls, each with 8 animals allowed to free activity in cages and euthanized at the ages of 93, 121 and 149 days, respectively. Bone mineral density (BMD) of the left femur was analyzed by bone densitometry. Unloading by tail-suspension decreased BMD while treadmill training and free activity in cages promoted its recovery in a similar way and over time.

The increasing availability of mobility data and the awareness of its importance and value have been motivating many researchers to the development of models and tools for analyzing movement data. This paper presents a brief survey of significant research works about modeling, processing and visualization of data about moving objects. We identified some key research fields that will provide better features for online analysis of movement data. As result of the literature review, we suggest a generic multi-layer architecture for the development of an online analysis processing software tool, which will be used for the definition of the future work of our team.
