Objective: To evaluate the transdentinal cytotoxicity of three different concentrations of carbodiimide (EDC) or 5% glutaraldehyde (GA) on MDPC-23 cells. Methods: Seventy 0.4-mm-thick dentin disks obtained from human molars were adapted to artificial pulp chambers. MDPC-23 cells were seeded on the pulpal surface of the disks. After 48 hours, the occlusal dentin was acid-etched and treated for 60 seconds with one of the following solutions (n=10): no treatment (negative control); 0.1 M, 0.3 M, or 0.5 M EDC; 5% GA; Sorensen buffer; or 29% hydrogen peroxide (positive control). Cell viability and morphology were assessed by methyltetrazolium assay and scanning electron microscopy (SEM), respectively. The eluates were collected after the treatments and applied on MDPC-23 seeded in a 24-well plate to analyze cell death, total protein (TP), and collagen production. The last two tests were performed 24 hours and seven days after the challenge. Data were analyzed by Kruskal-Wallis and Mann-Whitney tests (p<0.05). Results: EDC at all test concentrations did not reduce cell viability, while 5% GA did increase cell metabolism. Cell death by necrosis was not elicited by EDC or 5% GA. At the 24-hour period, 0.3 M and 0.5 M EDC reduced TP production by 18% and 36.8%, respectively. At seven days, increased TP production was observed in all groups. Collagen production at the 24-hour period was reduced when 0.5 M EDC was used. After seven days, no difference was observed among the groups. SEM showed no alteration in cell morphology or number, except in the hydrogen peroxide group. Conclusions: Treatment of acid-etched dentin with EDC or GA did not cause transdentinal cytotoxic effects on odontoblast-like cells.

BACKGROUND: Toll-like receptors (TLR) are membrane proteins that recognize conserved molecules derived from bacterial, viral, fungal or host tissues. They are responsible for promoting the production of cytokines and chemokines, increasing the expression of costimulatory molecules and influencing the T Helper response (Th) toward either a Th1 or Th2 profile, thereby modulating the regulatory T cell response and controlling the integrity of the epithelial barrier. The key factors responsible for increased susceptibility to recurrent aphthous ulceration (RAU) are unclear, and because TLRs are involved in both immune regulation and control of the epithelial barrier, a deficiency in TLR activity is likely to cause increased susceptibility. METHODS: We investigated the gene expression of TLRs one through 10 in tissue samples and peripheral blood mononuclear cells (PBMC) of RAU patients in comparison to healthy controls using real-time quantitative reverse transcription PCR. RESULTS: The analysis of mRNA expression levels in oral lesion showed significant (P < 0.01) overexpression of the TLR2(similar to 6-fold) gene and decreased expression of the TLR3 (similar to 5-fold) and TLR5 (similar to 6-fold) genes in comparison with healthy oral mucosa. The analysis of mRNA expression in PBMC indicated a down-regulation of TLR5 gene expression in the cells from RAU patients (P < 0.05; similar to 2-fold). CONCLUSION: Our results support the hypothesis that a subset of RAU patients has fewer TLR expression that have been tentatively implicated in antiinflammatory effects. This derangement of TLR gene expression may cause an overlay exuberant inflammation reaction in situations where normal individuals are resistant. J Oral Pathol Med (2012) 41: 8085

Tissue engineering is a discipline that aims at regenerating damaged biological tissues by using a cell-construct engineered in vitro made of cells grown into a porous 3D scaffold. The role of the scaffold is to guide cell growth and differentiation by acting as a bioresorbable temporary substrate that will be eventually replaced by new tissue produced by cells. As a matter or fact, the obtainment of a successful engineered tissue requires a multidisciplinary approach that must integrate the basic principles of biology, engineering and material science. The present Ph.D. thesis aimed at developing and characterizing innovative polymeric bioresorbable scaffolds made of hydrolysable polyesters. The potentialities of both commercial polyesters (i.e. poly-e-caprolactone, polylactide and some lactide copolymers) and of non-commercial polyesters (i.e. poly-w-pentadecalactone and some of its copolymers) were explored and discussed. Two techniques were employed to fabricate scaffolds: supercritical carbon dioxide (scCO2) foaming and electrospinning (ES). The former is a powerful technology that enables to produce 3D microporous foams by avoiding the use of solvents that can be toxic to mammalian cells. The scCO2 process, which is commonly applied to amorphous polymers, was successfully modified to foam a highly crystalline poly(w-pentadecalactone-co-e-caprolactone) copolymer and the effect of process parameters on scaffold morphology and thermo-mechanical properties was investigated. In the course of the present research activity, sub-micrometric fibrous non-woven meshes were produced using ES technology. Electrospun materials are considered highly promising scaffolds because they resemble the 3D organization of native extra cellular matrix. A careful control of process parameters allowed to fabricate defect-free fibres with diameters ranging from hundreds of nanometers to several microns, having either smooth or porous surface. Moreover, versatility of ES technology enabled to produce electrospun scaffolds from different polyesters as well as “composite” non-woven meshes by concomitantly electrospinning different fibres in terms of both fibre morphology and polymer material. The 3D-architecture of the electrospun scaffolds fabricated in this research was controlled in terms of mutual fibre orientation by properly modifying the instrumental apparatus. This aspect is particularly interesting since the micro/nano-architecture of the scaffold is known to affect cell behaviour. Since last generation scaffolds are expected to induce specific cell response, the present research activity also explored the possibility to produce electrospun scaffolds bioactive towards cells. Bio-functionalized substrates were obtained by loading polymer fibres with growth factors (i.e. biomolecules that elicit specific cell behaviour) and it was demonstrated that, despite the high voltages applied during electrospinning, the growth factor retains its biological activity once released from the fibres upon contact with cell culture medium. A second fuctionalization approach aiming, at a final stage, at controlling cell adhesion on electrospun scaffolds, consisted in covering fibre surface with highly hydrophilic polymer brushes of glycerol monomethacrylate synthesized by Atom Transfer Radical Polymerization. Future investigations are going to exploit the hydroxyl groups of the polymer brushes for functionalizing the fibre surface with desired biomolecules. Electrospun scaffolds were employed in cell culture experiments performed in collaboration with biochemical laboratories aimed at evaluating the biocompatibility of new electrospun polymers and at investigating the effect of fibre orientation on cell behaviour. Moreover, at a preliminary stage, electrospun scaffolds were also cultured with tumour mammalian cells for developing in vitro tumour models aimed at better understanding the role of natural ECM on tumour malignity in vivo.

Die Untersuchungen der murinen Cytomegalovirus (mCMV) Infektion im BALB/c Mausmodell konzentrierten sich bislang auf die Lunge, da diese einen Hauptort der mCMV Latenz darstellt. Da latentes CMV auch häufig durch Lebertransplantationen übertragen wird, wurde in dieser Arbeit die Leber als ein weiteres medizinisch relevantes Organ der CMV Latenz und Reaktivierung untersucht. Um zunächst die zellulären Orte der mCMV Latenz in der Leber zu ermitteln, wurden verschiedengeschlechtliche Knochenmarktransplantationen (KMT) mit männlichen tdy-positiven Spendern und weiblichen, tdy-negativen Empfängern, mit anschließender mCMV Infektion durchgeführt, um latent infizierte Mäuse mit geschlechtschromosomalem Chimärismus zu generieren. Diese Chimären erlaubten eine Unterscheidung zwischen tdy-positiven Zellen hämatopoetischen Ursprungs und tdy-negativen stromalen und parenchymalen Gewebszellen. Die Separation von Leberzellen der Chimären mittels zentrifugaler Elutriation und anschließender DNA Quantifizierung viraler und zellulärer Genome durch eine quantitative real-time PCR ergab einen ersten Hinweis, dass Endothelzellen ein zellulärer Ort der mCMV Latenz sind. Die darauf folgende immunomagnetische Zelltrennung lokalisierte latente virale DNA in der CD31-positiven Zellfraktion. Die Koexpression von CD31 mit dem endothelzellspezifischen Oberflächenmarker ME-9F1 identifizierte die sinusoidalen Endothelzellen der Leber (LSEC) als die Zellen, die latente virale DNA beherbergen. In den zytofluorometrisch aufgereinigten CD31+/ME-9F1+ LSEC waren bei gleichzeitigem Rückgang der männlichen tdy Markergene virale Genome angereichert, was darauf hinwies, dass Zellen, die virale DNA enthalten, vom Knochenmark-Empfänger stammen. Durch zytofluorometrische Analysen isolierter LSEC konnte eine vom Spender abstammende Subpopulation MHCII+/CD11b+ LSEC identifiziert werden. Anschließende Quantifizierungen viraler DNA aus latent infizierten Mäusen detektierten eine Abnahme viraler Genome mit zunehmender Menge an tdy-positiven Zellen, was beweist, dass MHCII+/CD11b+ LSEC keinen Ort der mCMV Latenz darstellen. Die limiting dilution Untersuchungen der isolierten latent infizierten LSEC ergaben eine Frequenz von einer latent infizierten Zelle unter ~1,9x104 LSEC und eine Anzahl von 7 bis 19 viralen Genomen pro latent infizierter Zelle. Nach 24 Stunden Kultivierung der LSEC konnte mittels quantitativer real-time RT-PCR mit Gesamt-RNA aus LSEC ein Anstieg der Genexpression der immediate early Gene ie1 und ie3 sowie eine Induktion des early Gens e1 gezeigt werden. Eine Erhöhung der transkriptionellen Reaktivierung durch die Inkubation der LSEC mit unterschiedlichen HDAC Inhibitoren konnte allerdings nicht erzielt werden, da sowohl die Menge der isolierten RNA aus behandelten Kulturen, als auch die Anzahl viraler Transkripte im Vergleich zu den unbehandelten Kulturen erniedrigt war. Aufgrund der kurzen Lebensdauer isolierter LSEC in vitro konnte durch Kokultivierungen latent infizierter LSEC zusammen mit murinen embryonalen Fibroblasten keine Virusreaktivierung induziert werden. Im Gegensatz dazu wurden durch den Transfer gereinigter ME-9F1+/CD31+ LSEC aus latent infizierten Spendern in immunsupprimierte Empfänger virale Rekurrenzen in Lungenexplantatkulturen des Rezipienten detektiert. Damit konnten LSEC eindeutig als zellulärer Ort von mCMV Latenz und Reaktivierung in der Leber identifiziert werden.

Neueste Arbeiten zeigen, dass Mastzellen wichtige Funktionen innerhalb des angeborenen und erworbenen Immunsystems ausüben. Von zentraler Bedeutung ist hierbei die Fähigkeit der Mastzelle, auf IgE-unabhängige Signale zu reagieren und ein breites Spektrum an Cytokinen und Chemokinen zu produzieren. Transkriptionsfaktoren der NFAT-Familie sind wichtige Regulatoren der Immunhomöostase und Cytokinproduktion. In Mausmodellen führt die Defizienz des NFATc2 zu überschießenden Immunreaktionen, was ursächlich auf die Hyperreaktivität NFATc2-defizienter T-Zellen zurückgeführt wird. Demgegenüber zeigten unsere eigenen in vitro durchgeführten Arbeiten, dass die Produktion wichtiger entzündungsfördernder Cytokine in Mastzellen abhängig ist von NFATc2. Ziel der vorliegenden Arbeit war es, die Bedeutung von NFATc2 in einem Mastzell-abhängigen Modell der transkutanen Immunisierung mit einem TLR7-Liganden als Adjuvans zu untersuchen. Experimente an NFATc2-defizienten Mäusen ergaben zunächst, dass die Schlüsselprozesse Entzündung, Auswanderung antigenpräsen-tierender Zellen, Lymphknotenhypertrophie sowie Expansion und Funktion spezifischer T-Zellen in Abwesenheit des Transkriptionsfaktors NFATc2 extrem beeinträchtigt sind. Dieser experimentell induzierte Phänotyp gleicht somit dem mastzellloser Mäuse in dieser Immunisierungsstudie. Rekonstitutionsexperimente erlaubten es, in vitro generierte Mastzellen aus NFATc2-defizienten und NFATc2-kompetenten Spendern in mastzelllose Mäuse zu transferieren und deren Reaktivität in dem angewandten TLR7-abhängigen Entzündungsmodell in vivo zu vergleichen. Hierbei zeigte sich, dass die in NFATc2-defizienten Mäusen nach transkutaner Immunisierung zu beobachtenden Beeinträchtigungen gänzlich auf die Abwesenheit des NFATc2 in Mastzellen zurückzuführen sind. Die Ergebnisse der vorliegenden Arbeit zeigen somit, dass der Transkriptionsfaktor NFATc2 für die Funktion der Mastzelle in vivo eine bedeutsame Rolle spielt. Dies betrifft sowohl Reaktionen des angeborenen als auch des erworbenen Immunsystems. Darüber hinaus könnte NFATc2 ein wichtiges Ziel bei therapeutischen Maßnahmen gegen mastzellabhängige Krankheiten darstellen.

NGAL (Neutrophil Gelatinase-associated Lipocalin ) is a protein of lipocalin superfamily. Recent literature focused on its biomarkers function in several pathological condition (acute and chronic kidney damage, autoimmune disease, malignancy). NGAL biological role is not well elucidated. Several are the demonstration of its bacteriostatic role. Recent papers have indeed highlight NGAL role in NFkB modulation. The aim of this study is to understand whether NGAL may exert a role in the activation (modulation) of T cell response through the regulation of HLA-G complex, a mediator of tolerance. From 8 healthy donors we obtained peripheral blood mononuclear cells (PBMCs) and we isolated by centrifugation on a Ficoll gradient. Cells were then treated with four concentrations of NGAL (40-320 ng/ml) with or without iron. We performed flow cytometry analysis and ELISA test. NGAL increased the HLA-G expression on CD4+ T cells, with an increasing corresponding to the dose. Iron effect is not of unique interpretation. NGAL adiction affects regulatory T cells increasing in vitro expansion of CD4+ CD25+ FoxP3+ cells. Neutralizing antibody against NGAL decreased HLA-G expression and reduced significantly CD4+ CD25+ FoxP3+ cells percentage. In conclusion, we provided in vitro evidence of NGAL involvement in cellular immunity. The potential role of NGAL as an immunomodulatory molecule has been evaluated: it has been shown that NGAL plays a pivotal role in the induction of immune tolerance up regulating HLA-G and T regulatory cells expression in healthy donors. As potential future scenario we highlight the in vivo role of NGAL in immunology and immunomodulation, and its possible relationship with immunosuppressive therapy efficacy, tolerance induction in transplant patients, and/or in other immunological disorders.

Regenerative medicine and tissue engineering attempt to repair or improve the biological functions of tissues that have been damaged or have ceased to perform their role through three main components: a biocompatible scaffold, cellular component and bioactive molecules. Nanotechnology provide a toolbox of innovative scaffold fabrication procedures in regenerative medicine. In fact, nanotechnology, using manufacturing techniques such as conventional and unconventional lithography, allows fabricating supports with different geometries and sizes as well as displaying physical chemical properties tunable over different length scales. Soft lithography techniques allow to functionalize the support by specific molecules that promote adhesion and control the growth of cells. Understanding cell response to scaffold, and viceversa, is a key issue; here we show our investigation of the essential features required for improving the cell-surface interaction over different scale lengths. The main goal of this thesis has been to devise a nanotechnology-based strategy for the fabrication of scaffolds for tissue regeneration. We made four types of scaffolds, which are able to accurately control cell adhesion and proliferation. For each scaffold, we chose properly designed materials, fabrication and characterization techniques.

Primary varicella-zoster virus (VZV) infection during childhood leads to varicella commonly known as chickenpox. After primary infection has occurred VZV establishes latency in the host. During subsequent lifetime the virus can cause reactivated infection clinically known as herpes zoster or shingles. In immunodeficient patients’ dissemination of the virus can lead to life-threatening disease. Withdrawal of acyclovir drug prophylaxis puts allogeneic hematopoietic stem-cell transplantation (HSCT) patients at increased risk for herpes zoster as long as VZV-specific cellular immunity is impaired. Although an efficient live attenuated VZV vaccine for zoster prophylaxis exists, it is not approved in immunocompromised patients due to safety reasons. Knowledge of immunogenic VZV proteins would allow designing a noninfectious nonhazardous subunit vaccine suitable for patients with immunodeficiencies. The objective of this study was to identify T cell defined virus proteins of a VZV-infected Vero cell extract that we have recently described as a reliable antigen format for interferon-gamma (IFN-γ) enzyme-linked immunosorbent spot (ELISpot) assays (Distler et al. 2008). We first separated the VZV-infected/-uninfected Vero cell extracts by size filtration and reverse-phase high performance liquid chromatography (RP-HPLC). The collected fractions were screened for VZV reactivity with peripheral blood mononuclear cells (PBMCs) of VZV-seropositive healthy individuals in the sensitive IFN-γ ELISpot assay. Using this strategy, we successfully identified bioactive fractions that contained immunogenic VZV material. VZV immune reactivity was mediated by CD4+ memory T lymphocytes (T cells) of VZV-seropositive healthy individuals as demonstrated in experiments with HLA blockade antibodies and T cell subpopulations already published by Distler et al. We next analyzed the bioactive fractions with electrospray ionization mass spectrometry (ESI-MS) techniques and identified the sequences of three VZV-derived proteins: glycoprotein E (gE); glycoprotein B (gB), and immediate early protein 62 (IE62). Complementary DNA of these identified proteins was used to generate in vitro transcribed RNA for effective expression in PBMCs by electroporation. We thereby established a reliable and convenient IFN-γ ELISPOT approach to screen PBMCs of healthy donors and HSCT patients for T cell reactivity to single full-length VZV proteins. Application in 10 VZV seropositive healthy donors demonstrated much stronger recognition of glycoproteins gE and gB compared to IE62. In addition, monitoring experiments with ex vivo PBMCs of 3 allo-HSCT patients detected strongly increased CD4+ T cell responses to gE and gB for several weeks to months after zoster onset, while IE62 reactivity remained moderate. Overall our results show for the first time that VZV glycoproteins gE and gB are major targets of the post-transplant anti-zoster CD4+ T cell response. The screening approach introduced herein may help to select VZV proteins recognized by memory CD4+ T cells for inclusion in a subunit vaccine, which can be safely used for zoster prophylaxis in immunocompromised HSCT patients.

Dendritische Zellen (DCs) nehmen eine Schlüsselrolle in unserem Immunsystem ein, indem DCs sowohl Immunität, als auch Toleranz induzieren können. Im Falle der Immunität sind DCs in der Lage die Differenzierung der verschiedenen T-Helferzellen, wie Th1-, Th2- und Th17-Zellen zu steuern und tragen so zu der Qualität einer Immunantwort bei. Auf der anderen Seite können DCs in Gegenwart von TGF-β, IDO und Retinsäure die Differenzierung von regulatorischen T-Zellen induzieren und tragen somit zur Aufrechterhaltung der peripheren Toleranz bei. Insbesondere in den Darm-assoziierten lymphatischen Geweben (GALT) müssen DCs unverhältnismäßige Immunantworten gegen harmlose Antigene aus der Nahrung und kommensale Bakterien verhindern, während gegen Pathogene schützende Immunantworten induziert werden müssen. Auf Grund dieser entgegengesetzten Funktionen der DCs wollten wir die molekularen Mechanismen der DCs untersuchen, die der Regulation von Immunität und Toleranz zu Grunde liegen. Insbesondere der Wnt-Signalweg ist für die Aufrechterhaltung der peripheren Toleranz im GALT von Bedeutung. Da die Casein Kinase 2 in diesem Signalweg entscheidend beteiligt ist, haben wir die CK2-Funktion konditionell, unter der Kontrolle des CD11c-Promotors, deletiert. Hierfür haben wir CD11c-cre Mäuse mit Mäusen verpaart, welche ein von loxP-Signalsequenzen flankiertes Ck2β Gen (CK2β-fl/fl) tragen. Die konditionelle Deletion der CK2-Funktion in DCs, führte zu einer verstärkten Expression der kostimulatorischen Moleküle (wie CD40, CD80, CD86) und der Zytokine IL-6 und IL-12 unter „steady-state“ Bedingungen. Detaillierte Untersuchungen der T-Zellen in CD11c-cre x CK2β-fl/fl Mäusen zeigte eine deutlich reduzierte naive T-Zellpopulation, einhergehend mit einer erhöhten Th1- und Th17-Differenzierung. Speziell in den mesenterialen Lymphknoten konnte eine höhere Frequenz von T-bet+ und Rorγt+ CD4+ T-Zellen gefunden werden, welche große Mengen der Zytokine IFN-γ und IL-17 nach ex vivo Stimulation produzierten. Weiterführende in vivo Versuche, hier wurde das Modell der oralen Toleranz gewählt, zeigten das eine CK2-Deletion in DCs die Induktion einer oralen Toleranz verhindert. Unsere Daten zeigen eindeutig, dass die CK2 entscheidend in der Regulation der DC Homöostase und der Aufrechterhaltung der peripheren Toleranz beteiligt ist.

Human cord blood plasmacytoid dendritic cells (PDC) react to stimulation with CPG A and CPG B with an increase in cell surface activation and maturation markers and cytokine production, similar to adult PDC. Intracellular phosphorylation in neonatal PDC did not benefit from CPG stimulation, in contrast to adult PDC. Cord blood PDC primed with CPG A, CPG B and CD40L do not promote division of autologous T cells contrary to adult PDC. Priming of neonatal PDC with CPG A or CPG B does not induce a clear bias in T helper cell response towards Th1 or Th2 while adult PDC trend towards a Th2 response.

Eine Pollenallergie geht häufig mit einer sekundären Nahrungsmittelallergie einher, die durch die Kreuzreaktivität zwischen dem Pollen- und dem Nahrungsmittelallergen verursacht wird. In der vorliegenden Arbeit wurde eine Kohorte von 20 Allergikern mit einer Typ I-Allergie gegen Birkenpollen und einer assoziierten Nahrungsmittelallergie gegen Haselnüsse und/oder Karotten in Bezug auf eine allergenspezifische IgE-Antwort, T-Zellantwort und vor allen Dingen hinsichtlich der T-Zellkreuzreaktivität zwischen den rekombinanten Allergenen Bet v 1, Cor a 1 und Dau c 1 charakterisiert. Verwendet wurde hierzu ein Zellkultursystem mit primären CD4+ T-Zellen ohne die Zugabe von exogenem IL-2 oder wiederholten Stimulationen. Zur Analyse der T-Zellkreuzreaktivität kamen zwei unterschiedliche Analyseverfahren zum Einsatz: zum einen der bewährte 3H-Thymidinassay und zum anderen eine neue durchflusszytometrische Methode, die auf der Verwendung von zwei unterschiedlichen Proliferationsfarbstoffen basiert.rnBei der Charakterisierung der T-Zellantwort konnte festgestellt werden, dass eine robuste Th2-Antwort vorliegt, die stark von dem Zytokin IL-5 dominiert wird, begleitet von einer signifikanten Produktion von IL-9 und IL-13, allerdings ohne die Beteiligung von IL-4.rnDes Weiteren konnte zum ersten Mal mit Hilfe eines dosisabhängigen Inhibitions-ELISA eine B-Zellkreuzreaktivität zwischen Bet v 1 und Cor a 1 gezeigt werden, wobei das Cor a 1-reaktive IgE prädominant ist und eine Subpopulation des Bet v 1-reaktiven IgE darstellt.rnMittels 3H-Thymidinassay konnte eine T-Zellkreuzreaktivität zwischen Bet v 1, Cor a 1 und in einem geringeren Maße zu Dau c 1 bei primären T-Zellen von Allergikern gezeigt werden. Ebenso konnte die Kreuzreaktivität zwischen Bet v 1 und Cor a 1 durch die neue durchflusszytometrische Methode bewiesen werden. Zusätzlich ist es mit Hilfe dieser neuen Methode nun möglich, zwischen den einzelnen T-Zellsubpopulationen zu unterscheiden, die sowohl nach primärer und sekundärer oder ausschließlich nach sekundärer Stimulation proliferieren. Es konnte festgestellt werden, dass die kreuzreaktiven T-Zellen aus der T-Zellsubpopulation hervorgehen, die bereits nach der primären Stimulations stark proliferiert haben. Somit kann also innerhalb des T-Zellrepertoires der allergischen Spender eine Prädominanz der kreuzreaktiven T-Zellsubpopulationen festgestellt werden. Eine monospezifische T-Zellsubpopulation konnte unter Verwendung der neuen Methode nicht detektiert werden.rnDas Ausmaß von Qualität und Quantität einer Kreuzreaktivität kann nun visualisiert werden, was dazu beitragen kann, das Protokoll einer SIT zu verbessern und optimal an den individuellen Patienten anzupassen, um möglicherweise eine größere Chance auf eine erfolgreiche Therapie in Aussicht zu stellen.

The in vitro production of recombinant protein molecules has fostered a tremendous interest in their clinical application for treatment and support of cancer patients. Therapeutic proteins include monoclonal antibodies, interferons, and haematopoietic growth factors. Clinically established monoclonal antibodies include rituximab (targeting CD20-positive B-cell lymphomas), trastuzumab (active in HER-2 breast and gastric cancer), and bevacizumab (blocking tumor-induced angiogenesis through blockade of vascular-endothelial growth factor and its receptor). Interferons have lost much of their initial appeal, since equally or more effective treatments with more pleasant side effects have become available, for example in chronic myelogenous leukaemia or hairy cell leukaemia. The value of recombinant growth factors, notably granulocyte colony stimulating factor (G-CSF) and erythropoietin is rather in the field of supportive care than in targeted anti-cancer therapy. Adequately powered clinical phase III trials are essential to estimate the true therapeutic impact of these expensive compounds, with appropriate selection of clinically relevant endpoints and sufficient follow-up. Monoclonal antibodies, interferons, and growth factors must also, and increasingly so, be subjected to close scrutiny by appropriate cost-effectiveness analyses to ensure that their use results in good value for money. With these caveats and under the condition of their judicious clinical use, recombinant proteins have greatly enriched the therapeutic armamentarium in clinical oncology, and their importance is likely to grow even further.

Background Moraxella catarrhalis, a major nasopharyngeal pathogen of the human respiratory tract, is exposed to rapid downshifts of environmental temperature when humans breathe cold air. It was previously shown that the prevalence of pharyngeal colonization and respiratory tract infections caused by M. catarrhalis are greatest in winter. The aim of this study was to investigate how M. catarrhalis uses the physiologic exposure to cold air to upregulate pivotal survival systems in the pharynx that may contribute to M. catarrhalis virulence. Results A 26°C cold shock induces the expression of genes involved in transferrin and lactoferrin acquisition, and enhances binding of these proteins on the surface of M. catarrhalis. Exposure of M. catarrhalis to 26°C upregulates the expression of UspA2, a major outer membrane protein involved in serum resistance, leading to improved binding of vitronectin which neutralizes the lethal effect of human complement. In contrast, cold shock decreases the expression of Hemagglutinin, a major adhesin, which mediates B cell response, and reduces immunoglobulin D-binding on the surface of M. catarrhalis. Conclusion Cold shock of M. catarrhalis induces the expression of genes involved in iron acquisition, serum resistance and immune evasion. Thus, cold shock at a physiologically relevant temperature of 26°C induces in M. catarrhalis a complex of adaptive mechanisms that enables the bacterium to target their host cellular receptors or soluble effectors and may contribute to enhanced growth, colonization and virulence.

Recent publications have shown that certain human leukocyte antigen (HLA) alleles are strongly associated with hypersensitivity to particular drugs. As HLA molecules are a critical element in T-cell stimulation, it is no surprise that particular HLA alleles have a direct functional role in the pathogenesis of drug hypersensitivity. In this context, a direct interaction of the relevant drug with HLA molecules as described by the p-i concept appears to be more relevant than presentation of hapten-modified peptides. In some HLA-associated drug hypersensitivity reactions, the presence of a risk allele is a necessary but incomplete factor for disease development. In carbamazepine and HLA-B*15:02, certain T-cell receptor (TCR) repertoires are required for immune activation. This additional requirement may be one of the 'missing links' in explaining why most individuals carrying this allele can tolerate the drug. In contrast, abacavir generates polyclonal T-cell response by interacting specifically with HLA-B*57:01 molecules. T cell stimulation may be due to presentation of abacavir or of altered peptides. While the presence of HLA-B*58:01 allele substantially increases the risk of allopurinol hypersensitivity, it is not an absolute requirement, suggesting that other factors also play an important role. In summary, drug hypersensitivity is the end result of a drug interaction with certain HLA molecules and TCRs, the sum of which determines whether the ensuing immune response is going to be harmful or not.
