Objectives: We tested two novel drug-eluting stents (DES), covered with a biodegradable-polymer carrier and releasing paclitaxel or sirolimus, which were compared against a bare metal stent (primary objective). The DES differed by the drug, but were identical otherwise, allowing to compare the anti-restenosis effects of sirolimus versus paclitaxel (secondary objective). Background: The efficacy of novel DES with biodegradable polymers should be tested in the context of randomized trials, even when using drugs known to be effective, such as sirolimus and paclitaxel. Methods: Overall, 274 patients with de novo coronary lesions in native vessels scheduled for stent implantation were randomly assigned (2:21 ratio) for the paclitaxel (n = 111), sirolimus (n = 106), or bare metal stent (n = 57) groups. Angiographic follow-up was obtained at 9 months and major cardiac adverse events up to 12 months. Results: Both paclitaxel and sirolimus stents reduced the 9-month in-stent late loss (0.54-0.44 mm, 0.32-0.43 mm, vs. 0.90-0.45 mm respectively), and 1-year risk of target vessel revascularization and combined major adverse cardiac events (P < 0.05 for both, in all comparisons), compared with controls. Sirolimus stents had lower late loss than paclitaxel stents (P < 0.01), but similar 1-year clinical outcomes. There were no differences in the risk of death, infarction, or stent thrombosis among the study groups. Conclusion: Both novel DES were effective in reducing neointimal hyperplasia and 1-year re-intervention, compared to bare metal stents. Our findings also suggest that sirolimus is more effective than paclitaxel in reducing angiographic neointima, although this effect was not associated with better clinical outcomes. Sm14 and paramyosin are two major Schistosoma mansoni vaccine candidate antigens. Recently, we have identified Sm14 and paramyosin epitopes that are recognized by T cells of resistant individuals living in endemic areas for schistosomiasis. Herein, mice were immunized with these peptides separately or in association in order to evaluate their vaccine potential. Immunization of mice with Sm14 peptides alone or mixed with paramyosin peptides was able to induce 26%-36.7% or 28%-29.2% of worm burden reduction, 67% or 46% of intestinal eggs reduction and also 54%-61% or 43%-52% of liver pathology reduction, respectively. Protection was associated with a Th1 type of immune response induced by Sm14 peptide immunization. In contrast, paramyosin peptide vaccination did not engender protective immunity or liver pathology reduction and immunization was associated with a Th2 type of immune response. Background & Aims: EPIC-3 is a prospective, international study that has demonstrated the efficacy of PEG-IFN alfa-2b plus weight-based ribavirin in patients with chronic hepatitis C and significant fibrosis who previously failed any interferon-alfa/ribavirin therapy. The aim of the present study was to assess FibroTest (FT), a validated non-invasive marker of fibrosis in treatment-naive patients, as a possible alternative to biopsy as the baseline predictor of subsequent early virologic (EVR) and sustained virologic response (SVR) in previously treated patients. Methods: Of 2312 patients enrolled, 1459 had an available baseline FT, biopsy, and complete data. Uni- (UV) and multi-variable (MV) analyses were performed using FT and biopsy. Results: Baseline characteristics were similar as in the overall population; METAVIR stage: 28% F2, 29% F3, and 43% F4, previous relapsers 29%, previous PEG-IFN regimen 41%, high baseline viral load (BVL) 64%. 506 patients (35%) had undetectable HCV-RNA at TW12 (TW12neg), with 58% achieving SVR. The accuracy of FT was similar to that in naive patients: AUROC curve for the diagnosis of F4 vs F2 = 0.80 (p<0.00001). Five baseline factors were associated (p<0.001) with SVR in UV and MV analyses (odds ratio: UV/MV): fibrosis stage estimated using FT (4.5/5.9) or biopsy (1.5/1.6), genotype 2/3 (4.5/5.1), BVL (1.5/1.3), prior relapse (1.6/1.6), previous treatment with non-PEG-IFN (2.6/2.0). These same factors were associated (p <= 0.001) with EVR. Among patients TW12neg, two independent factors remained highly predictive of SVR by MV analysis (p <= 0.001): genotype 2/3 (odds ratio = 2.9), fibrosis estimated with FT (4.3) or by biopsy (1.5). Conclusions: FibroTest at baseline is a possible non-invasive alternative to biopsy for the prediction of EVR at 12 weeks and SVR, in patients with previous failures and advanced fibrosis, retreated with PEG-IFN alfa-2b and ribavirin. Objective: To search for predictors of metformin response in women with polycystic ovary syndrome (PCOS) through a detailed analysis of clinical and laboratory parameters. Study design: We designed a prospective study to investigate clinical and laboratory parameters to search for predictors of metformin response in women with PCOS. A total of 53 PCOS patients were given metformin 850 mg twice a day for 6 months, after which patients were classified as responders or non-responders. Parameters analyzed for comparison between the two groups were: plasma fasting insulin glucose/insulin ratio; oral glucose tolerance test (OGTT) with insulin (120 min); HOMA and QUICKI tests; total cholesterol and fractions, triglycerides; LH, FSH, estradiol, progesterone, testosterone, androstenedione, 17-OH progesterone, and DHEAS. Results: From all patients, 30(56.6%) were responders and 23(43.3%) were non-responders. Multinomial analysis showed that the positive response to metformin was associated with higher levels of basal LH (p = 0.038) and lower levels of high-density lipoprotein cholesterol (HDL-C) (p = 0.015). Conclusion: In weight-matched PCOS subjects, laboratory markers might predict the metformin response. Higher levels of basal LH and lower levels of HDL-C are correlated with a positive response to metformin treatment in PCOS subjects. (C) 2011 Elsevier Ireland Ltd. All rights reserved.

Infection with GB virus C (GBV-C) or hepatitis G virus (HGV) is highly prevalent among HIV/AIDS patients. GBV-C/HGV viremia has not been associated with liver disease and seems to slow HIV disease progression. To study the GBV-C/HGV genotypes prevalence among HIV/AIDS patients and its association with HIV viral load (VL) and CD4+ lymphocyte counts. From February 2003 to February 2004, we analyzed 210 HIV-1-infected subjects who were on anti-retroviral therapy (ART). For 63 of them a PCR-nested to the non-coding 5` (5`NCR) region of the GBV-C/HGV was done, and for 49 a DNA direct sequencing was done. A phylogenetic analysis was performed by PHYLIP program. 63(30%) of the HIV-1-infected patients were co-infected with GBV-C/HGV. The phylogenetic analysis revealed the following genotypes (and respective relative frequencies): 1(10%), 2a (41%), 2b (43%), and 3 (6%). Co-infected patients presented lower HIV-1 VL and higher T CD4+ lymphocyte cells counts as compared with patients negative for GBV-C/HGV sequences (log = 4.52 vs. 4.71, p = 0.036), and T CD4+ lymphocyte counts (cells/mm(3) = 322.6 vs. 273.5, p = 0.081, respectively). T CD4+ cells counts equal to, or higher than, 200/mm(3) were significantly more common among co-infected patients than among HIV-infected-only patients (p = 0.042). The lowest T CD4+ cells counts were associated with genotype 1 and the highest with genotype 2b (p = 0.05). The GBV-C/HGV infection prevalence was 30% among HIV-1-infected subjects, and was associated with lower VL and higher CD4+ lymphocyte counts. GBV-C/HGV genotype 2b may be associated with better immunological response. Published by Elsevier B.V.

Objective. The objective of this study was to report our experience with pediatric orthotopic liver transplantation (OLT) with living related donors. Methods. We performed a retrospective chart analysis of 121 living related donor liver transplantations (LRDLT) from June 1998 to June 2010. Results. Indications were biliary atresia (BA; n = 81), primary sclerosing cholangitis (n = 5), alpha-1 antitrypsin deficiency (n = 4); cholestasis (n = 9), fulminant hepatic failure (n = 8), autoimmune hepatitis (n = 2), Alagille syndrome (n = 4), hepatoblastoma (n = 3), tyrosinemia (n = 2), and congenital hepatic fibrosis (n = 3). The age of the recipients ranged from 7-174 months (median, 22) and the weights ranged from 6-58 kg (median, 10). Forty-nine children (40.5%) weighed <= 10 kg. The grafts included the left lateral segment (n = 108), the left lobe (n = 12), and the right lobe (n = 1). The donors included 71 mothers, 45 fathers, 2 uncles, 1 grandmother, 1 grandfather, and 1 sister with a median age of 29 years (range, 16-53 ys) and a median weight of 68 kg (range, 47-106). Sixteen patients (12.9%) required retransplantation, most commonly due to hepatic artery thrombosis (HAT; n = 13; 10.7%). The other complications were biliary stenosis (n = 25; 20.6%), portal vein thrombosis (PVT; n = 11; 9.1%), portal vein stenosis (n = 5; 4.1%), hepatic vein stenosis (n = 6; 4.9%), and lymphoproliferative disorders (n = 8; 6.6%). The ultimate survival rate of recipients was 90.3% after 1 year and 75.8% after 3 years. Causes of early death within 1 month were HAT (n = 6), PVT (n = 2), severe graft dysfunction (n = 1), sepsis (n = 1), and intraoperative death in children with acute liver failure (n = 2). Causes of late deaths included lymphoproliferative disease (n = 3), chronic rejection (n = 2), biliary complications (n = 3), and recurrent disease (n = 3; hepatoblastoma and primary sclerosing cholangitis). Conclusions. Despite the heightened possibility of complications (mainly vascular), LRDLT represented a good alternative to transplantation from cadaveric donors in pediatric populations. It was associated with a high survival ratio.

The role of innate immune responses in protection against leptospirosis remains unclear. We examined the expression of the chemokines CCL2/JE (MCP-1), CCL3/MIP-1 alpha (MIP-1 alpha) and CXCL1/KC (IL-8) regarding resistance and susceptibility to leptospirosis in experimental mice models BALB/c and C3H/HeJ, respectively. A virulent strain of Leptospira interrogans serovar Copenhageni was used in this study. Twenty-five animals of each mouse strain of C3H/HeJ and BALB/c, were infected intraperitoneally with 106 cells. Five un-infected animals of each strain were kept as control. Mortality of C3H/HeJ mouse was observed while BALB/c mice were asymptomatic. The presence of leptospire DNA in tissues of infected animals was demonstrated by PCR. Chemokines were measured in serum, spleen, liver, kidney and lung of both strains of animals using immunoenzymatic assay (ELISA). Elevations in the levels of chemokines MCP-1 and IL-8 occurred in all organs and sera of C3H/HeJ and BALB/c infected mice. The levels of MIP-1 alpha were lower when compared to MCP-1 and IL-8 in all analyzed organs, with a slight increase in liver and kidney. Our results indicate that the expression of inflammatory mediators can vary greatly, depending on the tissue and mouse strains. It is possible that the resistance to Leptospira can be partially correlated to the increase of MIP-1 alpha observed in BALB/c mice, while an increasing and a sustained expression of MCP-1 and IL-8 in the lungs of C3H/HeJ mice can be correlated to the severity and progression of leptospirosis. The synthesis, spectroscopic properties, and chemical reactions of the stable (neopentylimino)-, (mesitylimino)-, and (o-tert-butylphenylimino)propadienones (6) are reported. Nucleophilic addition of amines affords the malonic amidoamidines 7 and 8. 3,5-Dimethylpyrazole reacts analogously to form 9b. Addition of 1,2-dimethylhydrazine produces pyrazolinones 10-12. Addition of N,Y'-dimethyldiaminoethane, -propane, and -butane gives diazepine, diazocine, and diazonine derivatives 13-15, respectively (X-ray structures of 13c, 14a, and 15a are available). The mesoionic pyridopyrimidinium olates IS are obtained by addition of 2-(methylamino)pyridine (X-ray structure of 18b available). Primary 2-aminopyridines afford the pyridopyrimidininones 20-29 and 31 (X-ray structure of 21a available), and 2-aminopyrimidines and 2-aminopyrazine afford pyrimidopyrimidinones and pyrazinopyrimidinones 33-35. Pyrimidoisoquinolinone 36 results from 1-aminoisoquinoline and pyridoquinolinone 40 from 8-aminoquinoline. 2-Aminothiazoline and 2-aminothiazole afford thiazolopyrimidinone derivatives 41-43 (X-ray structure of 43a available).

O manejo do solo deve ser realizado de tal forma que garanta a produção sustentável ao longo dos anos. Dentre as técnicas empregas, o manejo agroecológico e o plantio direto favorecem a manutenção da cobertura do solo e o aporte de matéria orgânica. Partindo da hipótese de que o maior aporte de resíduos culturais aumenta o conteúdo e estoque de matéria orgânica no solo, bem como reduz a emissão de C-CO2, o objetivo geral da pesquisa foi avaliar o impacto do manejo na matéria orgânica do solo e na emissão de C-CO2, nos períodos secos e chuvosos em diferentes cultivos agrícolas. O capítulo 1 foi desenvolvido na comunidade de Feliz Lembrança, Alegre–ES, onde foram avaliados sistemas de manejo em pastagem (PAST), café a pleno sol (PS) e café em sistema agroflorestal (SAF) e uma mata nativa (MN). O capítulo 2 foi desenvolvido no Incaper de Domingos Martins, onde se avaliou tratamentos de plantio direto de hortaliças sob palhada de gramínea (PD-G), leguminosa (PD-L), consórcio gramínea/leguminosa (PD-GL) e convencional utilizando enxada rotativa no pré-plantio (PC)em um delineamento de blocos casualizados. Amostras de solos em diferentes camadas foram coletadas para caracterização química e da matéria orgânica. Foram realizadas medições de emissão de C-CO2, temperatura do solo, umidade do solo e C biomassa microbiana do solo in situ. Foi utilizada análise de variância multivariada, vinculada a teste de aleatorização e aplicação de contrastes ortogonais no capítulo 1 e análise de variância aplicando teste F e teste de médias no capítulo 2. O SAF apresentou maior conteúdo de C orgânico total (19,8 g/kg) na camada de 0 a 5 cm e a PAST em subsuperfície. O menor estoque de C e N e os maiores valores de quociente metabólico foram encontrados no PS. O SAF reduziu a emissão de C-CO2 em 1,93 Mg ha-1 ano-1 em relação ao PS. O C orgânico total variou de 34,94 a 50,48 g/kg no PD-GL enquanto no sistema PC essa variação foi de 27,11 a 43,74 g/kg no perfil amostrado. A emissão média anual foi de 15,89 Mg C-CO2 ha-1 ano-1para a PD-G enquanto o PD-GL foi de 13,77; PD-L de 13,09 e PC de 11,20 Mg C-CO2 ha-1 ano-1. No PC, o balanço de C foi negativo (-2,15Mg ha-1), além de apresentar as menores médias anuais de umidade do solo e C biomassa microbiana e maior Qmet anual. Sistemas com contínuo e diversificado aporte de matéria orgânica promovem redução na emissão de C-CO2, bem como atuam no sequestro de C atmosférico.

Este artigo demonstra como foram e como são executados projetos em rede de C & T no Instituto Agronômico do Paraná (Iapar). Em relação aos procedimentos metodológicos, a pesquisa é qualitativa, descritiva e seccional, com perspectiva longitudinal, e foi desenvolvida por meio de um estudo de caso. Os dois projetos selecionados foram os que mais trouxeram visibilidade ao instituto, de acordo com informações extraídas de entrevistas com as várias pessoas que dirigiram o Iapar desde a sua fundação. Os dados da pesquisa foram coletados de forma primária (por meio de entrevistas semiestruturadas com dirigentes e pesquisadores envolvidos nos projetos) e secundária (pesquisa documental). A compreensão do fenômeno apoiou-se fundamentalmente na interpretação das falas dos sujeitos de pesquisa. A análise dos dois projetos executados em rede de C & T em épocas distintas pelo Iapar revelou que seu significado mudou ao longo do tempo. As primeiras redes foram formadas para o cumprimento do papel social do Iapar no Paraná. Mais recentemente, os projetos conduzidos em rede pelo instituto têm o objetivo prioritário de captação de recursos e obtenção de legitimidade ante o Estado e a sociedade.

A eficácia da vacina Sabin foi determinada em 106 crianças normais e subnutridas da Amazônia, após a administração de uma e duas doses de vacina oral (trivalente). Após a aplicação de uma dose de vacina, verificou-se que apenas 9% das crianças com subnutrição pregressa (crônica) e 43% das crianças normais formaram anticorpos neutralizantes (protetores) contra dois ou três tipos de poliovírus (p = 0,04). Após duas doses de vacina, os níveis de imunidade dos dois grupos de crianças estudadas acusaram, respectivamente, 32% e 75% (p = 0,001). Estes resultados mostram que a resposta imunitária à vacina Sabin foi sensivelmente inferior no grupo das crianças subnutridas, do que no das crianças normais. Em decorrência disto, será necessário administrar um número maior de doses de vacina oral àquelas crianças, a fim de que níveis satisfatórios de imunidade contra a poliomielite sejam atingidos em toda a população infantil.

OBJETIVOS: Determinar a soroprevalência de anticorpos antivírus da hepatite C (VHC) em doadores de sangue e correlacionar os resultados obtidos nos testes sorológicos de triagem e no teste confirmatório. MÉTODOS: Foram analisados os registros epidemiológicos e laboratoriais de 10.090 doadores de sangue do Hemonúcleo de Apucarana, Paraná, Brasil, do período de janeiro de 1997 a dezembro de 1999. Utilizou-se o método enzimaimunoensaio (ELISA) para detecção de anticorpos anti-VHC no soro. As amostras de soro com reatividade no ELISA foram avaliadas pelo teste confirmatório RIBA (recombinant immunoblot assay). Para análise estatística, utilizaram-se os testes qui-quadrado, teste exato de Fisher e índice de Kappa. RESULTADOS: Os resultados mostraram que 2.461 (24,4%) pessoas da amostra eram do sexo feminino e 7.629 (75,6%), do sexo masculino, com idade variando de 18 a 65 anos. Das 10.090 amostras de soro analisadas pelo ELISA, 88 apresentaram positividade, revelando soroprevalência de 0,9%, não demonstrando associação com as diferentes faixas etárias (p=0,197) e com o sexo (p=0,323). Avaliadas pelo teste confirmatório RIBA, 11 amostras (12,5%) apresentaram resultado positivo; 14 (15,9%), resultado indeterminado; e 38 (43,2%), resultado negativo. A análise estatística revelou alta concordância (índice Kappa de 0,939) entre os resultados obtidos no teste de ELISA e os obtidos no teste confirmatório. Amostras que forneceram resultado fracamente reagente no teste de ELISA apresentaram alta concordância com resultado negativo no RIBA immunoblot, e amostras que forneceram resultado fortemente reagente no teste de ELISA apresentaram alta concordância com o resultado positivo no RIBA. CONCLUSÕES: Os resultados reforçam a necessidade de confirmação de todos os resultados reagentes nos testes de triagem sorológica para pesquisa de anticorpo anti-VHC, uma vez que a confirmação da infecção pelo VHC é de extrema importância para o acompanhamento clínico, laboratorial e histológico dos doadores de sangue.

The hydrotris(pyrazol-1-yl)methane iron(II) complex [FeCl2{eta(3)-HC(pz)(3)}] (Fe, pz = pyrazol-1-yl) immobilized on commercial (MOR) or desilicated (MOR-D) zeolite, catalyses the oxidation of cyclohexane with hydrogen peroxide to cyclohexanol and cyclohexanone, under mild conditions. MOR-D/Fe (desilicated zeolite supported [FeCl2{eta(3)-HC(pz)(3)}] complex) provides an outstanding catalytic activity (TON up to 2.90 x 10(3)) with the concomitant overall yield of 38%, and can be easy recovered and reused. The MOR or MOR-D supported hydrotris(pyrazol-1-yl)methane iron(II) complex (MOR/Fe and MOR-D/Fe, respectively) was characterized by X-ray powder diffraction, ICP-AES, and TEM studies as well as by IR spectroscopy and N-2 adsorption at -196 degrees C. The catalytic operational conditions (e.g., reaction time, type and amount of oxidant, presence of acid and type of solvent) were optimized. (C) 2013 Elsevier B.V. All rights reserved.

OBJETIVO: Estimar a prevalência e fatores associados à infecção pelo vírus da hepatite C em usuários de drogas e identificar os genótipos e subtipos virais circulantes. MÉTODOS: Estudo realizado com 691 usuários de drogas de 26 centros de tratamento de uso de drogas filantrópicos, particulares e públicos de Goiânia (GO) e Campo Grande (MS), entre 2005 e 2006. Dados sociodemográficos e fatores de risco para infecção pelo HCV foram obtidos por meio de entrevistas. Amostras sangüíneas foram testadas para a detecção de anticorpos para o HCV. As amostras positivas foram submetidas à detecção do RNA-HCV pela reação em cadeia da polimerase com iniciadores complementares às regiões 5' NC e NS5B do genoma viral e genotipadas pelo line probe assay (LiPA) e por seqüenciamento direto, seguido de análise filogenética. Prevalência e odds ratio foram calculados com intervalo de 95% de confiança. Os fatores de risco com p<0,10 pela análise univariada foram analisados por regressão logística hierárquica. Valores de p<0,05 foram considerados estatisticamente significantes. RESULTADOS: A prevalência para anti-HCV foi 6,9% (IC 95%: 5,2;9,2). A análise multivariada de fatores de risco indicou que idade superior a 30 anos e uso injetável de drogas se mostraram associados à infecção pelo HCV. O RNA-HCV foi detectado em 85,4% (41/48) das amostras anti-HCV positivas. Trinta e três amostras foram do genótipo 1 pelo LiPA, subtipos 1a (63,4%) e 1b (17,1%), e 8 (19,5%) do genótipo 3, subtipo 3a. A análise filogenética da região NS5B mostrou que 17 (68%), 5 (20%) e 3 (12%) amostras foram dos subtipos 1a, 3a e 1b, respectivamente. CONCLUSÕES: Os resultados mostram uma prevalência elevada da infecção e do subtipo 1a do HCV em usuários de drogas, sendo o uso injetável de drogas o principal fator de risco para essa infecção.
