Immunreaktionen gegen Antigene können eine zelluläre Immunantwort, eingeleitet durch TH1-Zellen induzieren oder aber eine antikörpervermittelte humorale Immunantwort induzieren, die von Zellen des TH2-Typs bestimmt sind. Diese unterschiedlichen Immunreaktionen regulieren sich gegenseitig, indem sie sich wechselseitig unterdrücken. Dadurch stellt sich ein Gleichgewicht zwischen den TH1- und TH2-Immunantworten ein. Die Allergie vom Soforttyp ist TH2-dominiert und wird ausgelöst durch die Produktion spezifischer IgE-Antikörper gegen eigentlich harmlose Antigene. Durch die frühe Ausschüttung des Zytokins IL-4 von T-Zellen differenzieren naive allergenspezifische T-Zellen zu TH2-Zellen aus. Diese sezernieren dann ebenfalls IL-4 und auch IL-13, was die B-Zellen zu einer vermehrten Produktion von IgE-Molekülen stimuliert. IgE bindet an den hochaffinen IgE-Rezeptor auf Mastzellen, Basophilen und Eosinophilen. Bei weiterem Allergenkontakt kommt es durch Kreuzvernetzung der Fc-Rezeptoren zur Ausschüttung vo

Navigation through a previously deployed and deformed stent is a difficult interventional task. Inadvertent navigation through the struts of a stent can potentially lead to incomplete secondary stent extension and vessel occlusion. Better visualisation of the pathway through the stent can reduce the risks of the procedural complications and reduce the reluctance of the interventionalist to navigate through a previously deployed stent. We describe a technique of visualisation of the pathway navigated by a guidewire through a previously deployed deformed and fractured carotid stent by the use of DynaCT. Three-dimensional reconstruction of the stent/microwire allows excellent visualisation of the correct pathway of the microwire within the stent.

Glucocorticoids (GC) have important anti-inflammatory and pro-apoptotic activities. Initially thought to be exclusively produced by the adrenal glands, there is now increasing evidence for extra-adrenal sources of GCs. We have previously shown that the intestinal epithelium produces immunoregulatory GCs and that intestinal steroidogenesis is regulated by the nuclear receptor liver receptor homolog-1 (LRH-1). As LRH-1 has been implicated in the development of colon cancer, we here investigated whether LRH-1 regulates GC synthesis in colorectal tumors and whether tumor-produced GCs suppress T-cell activation. Colorectal cancer cell lines and primary tumors were found to express steroidogenic enzymes and regulatory factors required for the de novo synthesis of cortisol. Both cell lines and primary tumors constitutively produced readily detectable levels of cortisol, as measured by radioimmunoassay, thin-layer chromatography and bioassay. Whereas overexpression of LRH-1 significantly increased the expression of steroidogenic enzymes and the synthesis of cortisol, downregulation or inhibition of LRH-1 effectively suppressed these processes, indicating an important role of LRH-1 in colorectal tumor GC synthesis. An immunoregulatory role of tumor-derived GCs could be further confirmed by demonstrating a suppression of T-cell activation. This study describes for the first time cortisol synthesis in a non-endocrine tumor in humans, and suggests that the synthesis of bioactive GCs in colon cancer cells may account as a novel mechanism of tumor immune escape.
