In this article, we offer an overview of the compared quantitative importance of biotransformation reactions in the metabolism of drugs and other xenobiotics, based on a meta-analysis of current research interests. Also, we assess the relative significance the enzyme (super)families or categories catalysing these reactions. We put the facts unveiled by the analysis into a drug discovery context and draw some implications. The results confirm the primary role of cytochrome P450-catalysed oxidations and UDP-glucuronosyl-catalysed glucuronidations, but they also document the marked significance of several other reactions. Thus, there is a need for several drug discovery scientists to better grasp the variety of drug metabolism reactions and enzymes and their consequences.

O objetivo deste trabalho foi avaliar o efeito da suplementação na resposta produtiva, no pH e no nitrogênio amoniacal ruminal, e no custo de produção de ovinos em pastagem de capim-marandu no período da seca. O experimento teve a duração de 84 dias. Utilizaram-se 20 cordeiros não castrados com idade e peso corporal inicial médios de quatro meses e 24,20 kg, respectivamente, para avaliação do desenvolvimento, distribuídos em cada um dos tratamentos que são os suplementos, mineral, energético, proteico e múltiplo, com cinco animais por tatamento, em área de 0,1 ha. Para avaliação dos parâmetros nutricionais, foram utilizados quatro ovinos fistulados no rúmen com 12 meses e 55 kg de peso corporal foram distribuídos em quatro piquetes de 0,1 ha. Avaliaram-se os suplementos mineral, energético, múltiplo e proteico. Os ganhos de peso foram de 0,017, -0,008, 0,024 e 0,077 kg por dia para os suplementos mineral, energético, múltiplo e proteico, respectivamente. Quatro horas após suplementação, os valores de pH ruminal foram de 6,30, 6,40, 6,18 e 6,24 para os suplementos mineral, energético, múltiplo e proteico, respectivamente. Os valores para nitrogênio amoniacal do líquido ruminal foram de 10,57, 7,36, 21,58 e 24,50 mg dL-1 para os suplementos mineral, energético, múltiplo e proteico, respectivamente. Os suplementos mineral e proteico produziram o ganho de peso com o menor custo. O uso de suplemento energético para cordeiros submetidos à forragem com baixo teor de proteína reduz o ganho de peso.

Both neural and behavioral responses to stimuli are influenced by the state of the brain immediately preceding their presentation, notably by pre-stimulus oscillatory activity. Using frequency analysis of high-density electroencephalogram coupled with source estimations, the present study investigated the role of pre-stimulus oscillatory activity in auditory spatial temporal order judgments (TOJ). Oscillations within the beta range (i.e. 18-23Hz) were significantly stronger before accurate than inaccurate TOJ trials. Distributed source estimations identified bilateral posterior sylvian regions as the principal contributors to pre-stimulus beta oscillations. Activity within the left posterior sylvian region was significantly stronger before accurate than inaccurate TOJ trials. We discuss our results in terms of a modulation of sensory gating mechanisms mediated by beta activity.

PURPOSE: To describe a patient with metastatic choroidal paraganglioma that was locally controlled with radiotherapy. DESIGN: Interventional clinicopathologic case report. PARTICIPANT: One patient with metastatic choroidal paraganglioma. METHODS: Interventional clinicopathologic case report and systematic search of the literature. MAIN OUTCOME MEASURES: Description of clinicopathologic features, treatment methods, and outcome. RESULTS: A 50-year-old man had a nonpigmented atypical choroidal mass with secondary retinal detachment in the left eye. After incisional biopsy, the diagnosis of paraganglioma was established. Metastatic work-up revealed vertebral, mediastinal, and pulmonary metastases of a nonsecretory, malignant paraganglioma without tracer uptake. The primary tumor was not identified. The ocular tumor regressed after stereotaxic radiotherapy. Two years later, recurrent lesions developed in the contralateral eye, which also was irradiated. CONCLUSIONS: Malignant paraganglioma can metastasize in the choroid and should be included in the differential diagnosis of a nonpigmented choroidal mass. Stereotaxic radiation therapy is an effective treatment method. To the authors' knowledge, this is the first report of a patient with choroidal paraganglioma. We present measurements of hydrogen and oxygen isotopes in MORB glasses from Macquarie Island (SW. Pacific Ocean) coupled with determination of bulk H2O content by two independent techniques: total dehydration and FTIR. The incompatible trace elements in these glasses vary by a factor of 12 to 17, with K2O varying from 0.1 to 1.7 wt.%; these ranges reflect a variable degree of closed-system mantle melting, estimated from 1 to 15%. Water concentrations determined by the two techniques match well, yielding a range from 0.25 to 1.49 wt.% which correlates positively with all of the measured incompatible trace elements, suggesting that water is un-degassed, and behaves conservatively during mantle melting. Also, the agreement between the FTIR-determined and extracted water contents gives us confidence that the measured isotopic values of hydrogen reflect that of the mantle. Comparison of the range of water content with that of other incompatible trace elements allows estimation of the water partition coefficient in lherzolite, 0.0208 (ranging from 0.017 to 0.023), and the water content in the source, 386 ppm (ranging from 370 to 440 ppm). We observe a fairly narrow range in delta D and delta O-18 values of -75.5 +/- 4.5 parts per thousand and 5.50 +/- 0 .05 parts per thousand respectively, that can be explained by partial melting of normal lherzolitic mantle. The measured delta D and delta O-18 values of Macquarie Island glasses that range from nepheline- to hypersthene-normative, and from MORB to EMORB in composition, are identical to those in average global MORB. The observed lack of variation of delta D and delta O-18 with 1 to 15% degree of mantle melting is consistent with a bulk melting model of delta D and delta O-18 fractionation, in which water is rapidly scavenged into the first partial melt. The narrow ranges of delta D and delta O-18 in normal mantle are mostly due to the buffering effect of clino- and orthopyroxenes in the residual assemblage; additionally, fast ``wet'' diffusion of oxygen and hydrogen isotopes through the melting regions may further smooth isotopic differences. BACKGROUND: The diagnostic and clinical overlap between schizophrenia and schizoaffective disorder is an important nosological issue in psychiatry that is yet to be resolved. The aim of this study was to compare the clinical and functional characteristics of an epidemiological treated cohort of first episode patients with an 18-month discharge diagnosis of schizophrenia (FES) or schizoaffective disorder (FESA). METHODS: This study was part of the larger First Episode Psychosis Outcome Study (FEPOS) which involved a medical file audit study of all 786 patients treated at the Early Psychosis Prevention and Intervention Centre between 1998 and 2000. Of this cohort, 283 patients had an 18-month discharge diagnosis of FES and 64 had a diagnosis of FESA. DSM-IV diagnoses and clinical and functional ratings were derived and validated by two consultant psychiatrists. RESULTS: Compared to FES patients, those with FESA were significantly more likely to have a later age of onset (p=.004), longer prodrome (p=.020), and a longer duration of untreated psychosis (p<.001). At service entry, FESA patients presented with a higher illness severity (p=.020), largely due to the presence of more severe manic symptoms (p<.001). FESA patients also had a greater number of subsequent inpatient admissions (p=.017), had more severe depressive symptoms (p=.011), and higher levels of functioning at discharge. DISCUSSION: The findings support the notion that these might be considered two discernable disorders; however, further research is required to ascertain the ways and extent to which these disorders are discriminable at presentation and over time.

The genetic landscape of the European flora and fauna was shaped by the ebb and flow of populations with the shifting ice during Quaternary climate cycles. While this has been well demonstrated for lowland species, less is known about high altitude taxa. Here we analyze the phylogeography of the leaf beetle Oreina elongata from 20 populations across the Alps and Apennines. Three mitochondrial and one nuclear region were sequenced in 64 individuals. Within an mtDNA phylogeny, three of seven subspecies are monophyletic. The species is chemically defended and aposematic, with green and blue forms showing geographic variation and unexpected within-population polymorphism. These warning colors show pronounced east-west geographical structure in distribution, but the phylogeography suggests repeated origin and loss. Basal clades come from the central Alps. Ancestors of other clades probably survived across northern Italy and the northern Adriatic, before separation of eastern, southern and western populations and rapid spread through the western Alps. After reviewing calibrated gene-specific substitution rates in the literature, we use partitioned Bayesian coalescent analysis to date our phylogeography. The major clades diverged long before the last glacial maximum, suggesting that O. elongata persisted many glacial cycles within or at the edges of the Alps and Apennines. When analyzing additional barcoding pairwise distances, we find strong evidence to consider O. elongata as a species complex rather than a single species.

O objetivo deste trabalho foi caracterizar a variabilidade genética nos parentais (G0) e em três gerações consecutivas (G1, G2 e G3) de tilápia Gift (genetically improved farmed tilapia), por meio de marcadores microssatélites. Trezentos e sessenta indivíduos, provenientes do programa de melhoramento da Universidade Estadual de Maringá, foram selecionados quanto ao ganho de peso. O total de 21 alelos foi encontrado nos cinco loci microssatélites polimórficos (G12292, UNH140; G12311, UNH159; G12312, UNH160; G12314, UNH162; e G12315, UNH163), com número médio entre três e cinco alelos por locus. As frequências alélicas variaram de 0,017 (UNH160 - G2) a 0,750 (UNH160 - G0). A heterozigosidade média observada foi de 0,501, 0,391, 0,531 e 0,503 para G0, G1, G2 e G3, respectivamente. O coeficiente de endogamia médio foi 0,192 (G0), 0,401 (G1), 0,230 (G2) e 0,301 (G3). Todas as gerações apresentaram desvio no equilíbrio de Hardy-Weinberg, com desequilíbrio de ligação na maioria dos loci. Exceto para a G1, a heterozigosidade foi mantida nas gerações G2 e G3, o que indica que não há perda significativa de variabilidade genética no programa de melhoramento.

For the past decade, PET and PET/CT have been widely studied for myocardial perfusion imaging. Several studies demonstrated the incremental value of PET for the diagnostic and prognostic assessment of patients with coronary artery disease. Moreover, PET allows for non-invasively quantifying myocardial blood flow and myocardial flow reserve, that both are recognized as surrogate marker of cardiac event free survival. By enabling the exploration of epicardial disease and the microvasculature, PET constitutes a unique tool to study pathophysiogical mechanisms leading to atherosclerosis genesis. The recent emergence of high-tech hybrid machines may even provide further incremental information about coronary function and morphology. By taking the best of each modality, a better assessment of patients with coronary artery disease is expected. (C) 2011 Elsevier Masson SAS. All rights reserved.

PURPOSE: To describe the anatomical characteristics and patterns of neurovascular compression in patients suffering classic trigeminal neuralgia (CTN), using high-resolution magnetic resonance imaging (MRI). MATERIALS AND METHODS: The analysis of the anatomy of the trigeminal nerve, brain stem and the vascular structures related to this nerve was made in 100 consecutive patients treated with a Gamma Knife radiosurgery for CTN between December 1999 and September 2004. MRI studies (T1, T1 enhanced and T2-SPIR) with axial, coronal and sagital simultaneous visualization were dynamically assessed using the software GammaPlan?. Three-dimensional reconstructions were also developed in some representative cases. RESULTS: In 93 patients (93%), there were one or several vascular structures in contact, either, with the trigeminal nerve, or close to its origin in the pons. The superior cerebellar artery was involved in 71 cases (76%). Other vessels identified were the antero-inferior cerebellar artery, the basilar artery, the vertebral artery, and some venous structures. Vascular compression was found anywhere along the trigeminal nerve. The mean distance between the nerve compression and the origin of the nerve in the brainstem was 3.76±2.9mm (range 0-9.8mm). In 39 patients (42%), the vascular compression was located proximally and in 42 (45%) the compression was located distally. Nerve dislocation or distortion by the vessel was observed in 30 cases (32%). CONCLUSIONS: The findings of this study are similar to those reported in surgical and autopsy series. This non-invasive MRI-based approach could be useful for diagnostic and therapeutic decisions in CTN, and it could help to understand its pathogenesis.

PURPOSE: To investigate choroidal vascular abnormalities in peripheral exudative hemorrhagic chorioretinopathy, using dynamic ultrawide-field fluorescein angiography (FA) and indocyanine green angiography (ICGA).¦DESIGN: Prospective observational case series.¦METHODS: This institutional study comprised a consecutive series of 40 patients (48 eyes) with peripheral exudative hemorrhagic chorioretinopathy. Choroidal vascular abnormalities were assessed with dynamic ultrawide-field (150-degree) FA and ICGA, using the Staurenghi 230 SLO Retina Lens and the Heidelberg scanning laser ophthalmoscope. The main outcome measures were morphologic descriptions of structural vascular abnormalities and choroidal hemodynamics (comparison with 30 normal eyes).¦RESULTS: The peripheral mass lesions were highly exudative and hemorrhagic, and usually associated with a pigment epithelium detachment. FA revealed nonspecific alterations corresponding to the visible fundoscopic changes (window defects, blockage, staining), but no neovascular membrane. However, despite frequent masking, ICGA showed hyperfluorescent polyp-like structures in the choroid of the lesion area in 33 eyes (69%) and an abnormal choroidal vascular network in 24 eyes (50%). The abnormal choroidal vascular network filled in the arterial or early venous phase, while the polyp-like structures filled some seconds later. Optical coherence tomography revealed the typical dome-shaped elevation of the pigment epithelium over the vascular polyps. Peripheral choriocapillaris closure was observed as well as dilated shunting vessels.¦CONCLUSION: Peripheral exudative hemorrhagic chorioretinopathy shares many characteristics (polyp-like choroidal telangiectases, abnormal choroidal vascular networks, exudative and hemorrhagic presentation) with polypoidal choroidal vasculopathy. Clarification of the precise role of these abnormalities requires further studies.

Tools to predict fracture risk are useful for selecting patients for pharmacological therapy in order to reduce fracture risk and redirect limited healthcare resources to those who are most likely to benefit. FRAX® is a World Health Organization fracture risk assessment algorithm for estimating the 10-year probability of hip fracture and major osteoporotic fracture. Effective application of FRAX® in clinical practice requires a thorough understanding of its limitations as well as its utility. For some patients, FRAX® may underestimate or overestimate fracture risk. In order to address some of the common issues encountered with the use of FRAX® for individual patients, the International Society for Clinical Densitometry (ISCD) and International Osteoporosis Foundation (IOF) assigned task forces to review the medical evidence and make recommendations for optimal use of FRAX® in clinical practice. Among the issues addressed were the use of bone mineral density (BMD) measurements at skeletal sites other than the femoral neck, the use of technologies other than dual-energy X-ray absorptiometry, the use of FRAX® without BMD input, the use of FRAX® to monitor treatment, and the addition of the rate of bone loss as a clinical risk factor for FRAX®. The evidence and recommendations were presented to a panel of experts at the Joint ISCD-IOF FRAX® Position Development Conference, resulting in the development of Joint ISCD-IOF Official Positions addressing FRAX®-related issues.

O trabalho foi conduzido em casa de vegetação na Embrapa Mandioca e Fruticultura, em Cruz das Almas - BA, e objetivou-se à identificação de porta-enxertos de citros melhor adaptados ao ecossistema de Tabuleiros Costeiros, quanto a tolerância à seca. Foram estudados os limoeiros 'Cravo' e 'Volkameriano', laranjeira 'Azeda' e os híbridos trifoliados HTR - 051, TSK x CTTR - 002 e TSK x CTTR - 017. O delineamento experimental foi o inteiramente casualizado, em esquema fatorial 6 õ 9, com seis genótipos e nove tempos de avaliação. As avaliações foram realizadas na sequência: irrigação, déficit hídrico e irrigação, quando plantas possuíam de 3 a 4 pares de folhas. Foram analisadas as variáveis massas secas da raiz e da parte aérea, o potencial hídrico da planta e a transpiração da folha. Quanto à massa seca da parte aérea, os genótipos limoeiro Volkameriano e laranjeira 'Azeda' apresentaram decréscimos no período de déficit hídrico, sendo que os demais não apresentaram diferenças significativas. Na ausência de irrigação, todos os genótipos apresentaram decréscimos nos seus potenciais hídricos, enquanto apenas os híbridos apresentaram recuperação, mantendo a transpiração. Os híbridos HTR - 051 e TSK x CTTR - 017 apresentaram os melhores desempenhos para todas as variáveis estudadas, mostrando-se mais promissores como porta-enxertos de citros em condições de déficit hídrico.

Visando à identificação de genótipos de citros melhor adaptados ao ecossistema de Tabuleiros Costeiros, com potencial de uso como porta-enxertos, avaliou-se seu desempenho em relação à tolerância à seca, considerando a análise de crescimento das plantas. O trabalho foi conduzido em casa de vegetação da Embrapa Mandioca e Fruticultura Tropical, em Cruz das Almas - BA. Foram avaliados seis genótipos: limoeiros 'Cravo' e 'Volkameriano', laranjeira 'Azeda' e os híbridos trifoliados HTR - 051, TSK õ CTTR - 002 e TSK õ CTTR - 017, estes últimos obtidos pelo Programa de Melhoramento Genético de Citros da Embrapa Mandioca e Fruticultura Tropical. Plantas obtidas de sementes (pés-francos ou seedlings) foram cultivadas em substrato Plantmaxâ, irrigando-as até que apresentassem dois ou mais pares de folhas verdadeiras, quando foram transplantadas para recipientes com o mesmo substrato devidamente adubado, mantendo-se o suplemento hídrico até o início das avaliações. O delineamento experimental foi o inteiramente casualizado, em esquema fatorial 6 õ 7, sendo seis genótipos e sete tempos de avaliação. As avaliações foram realizadas sob irrigação, déficit hídrico e re-irrigação, considerando-se o acúmulo de matéria seca total (MS) e a área foliar da planta (AF) como base para a determinação dos seguintes índices fisiológicos: razão de área foliar (RAF), taxa assimilatória líquida (TAL) e taxa de crescimento relativo (TCR). Nas condições do experimento, o híbrido HTR - 051 apresentou menor sensibilidade ao déficit hídrico, o que o qualifica como porta-enxerto promissor para ambientes sujeitos a estiagem prolongada.
