Perceber a rede estrutural formada pelos neurónios no cérebro a nível da macro escala é um desafio atual na área das neurociências. Neste estudo analisou-se a conectividade estrutural do cérebro em 22 indivíduos saudáveis e em dois doentes com epilepsia pós-traumática. Avaliaram-se as diferenças entre estes dois grupos. Também se pesquisaram diferenças a nível do género e idade no grupo de indivíduos saudáveis e os que têm valores médios mais elevados nas métricas de caracterização da rede. Para tal, desenvolveu-se um protocolo de análise recorrendo a diversos softwares especializados e usaram-se métricas da Teoria dos Grafos para a caracterização da conectividade estrutural entre 118 regiões encefálicas distintas. Dentro do grupo dos indivíduos saudáveis concluiu-se que os homens, no geral, são os que têm média mais alta para as métricas de caracterização da rede estrutural. Contudo, não se observaram diferenças significativas em relação ao género nas métricas de caracterização global do cérebro. Relativamente à idade, esta correlaciona-se negativamente, no geral, com as métricas de caracterização da rede estrutural. As regiões onde se observaram as diferenças mais importantes entre indivíduos saudáveis e doentes são: o sulco rolândico, o hipocampo, o pré-cuneus, o tálamo e o cerebelo bilateralmente. Estas diferenças são consistentes com as imagens radiológicas dos doentes e com a literatura estudada sobre a epilepsia pós-traumática. Preveem-se desenvolvimentos para o estudo da conectividade estrutural do cérebro humano, uma vez que a sua potencialidade pode ser combinada com outros métodos de modo a caracterizar as alterações dos circuitos cerebrais.

Objectives - Identify radiographers’ postures during frequent mammography procedures related to the mammography equipment and patient characteristics. Methods - A postural task analysis was performed using images acquired during the simulation of mammography positioning procedures. Simulations included craniocaudal/(CC) and mediolateral-oblique/(MLO) positioning in three different settings: radiographers and patients with similar statures, radiographers smaller than the patients and radiographers taller than the patients. Measurements of postural angles were performed by two raters using adequate software and classified according to the European Standard EN1005-4:2005 + A1:2008. Results - The simulations revealed that the most awkward posture in mammography is during the positioning of MLO projection in short-stature patients. Postures identified as causing work-related musculoskeletal disorder (WRMSD) risk were neck extension, arms elevated and the back stooped, presenting angles of 87.2, 118.6 and 63.6, respectively. If radiographers were taller than patients, then the trunk and arm postures were not acceptable. Conclusions - Working in a mammography room leads to awkward postures that can have an impact on radiographers’ health, namely WRMSDs. The results in this study showed that there are non-acceptable postures associated with frequent working procedures in mammography. MLO is the most demanding procedure for radiographer postures and may be related to WRMSDs. Mammography devices should be redesigned considering adjustability for radiographers. Main Messages: • Mammography constraints for radiographers in mammography procedures have not been well studied. • Performing mammography leads to awkward postures that can impact radiographers’ health. • MLO, the most demanding procedure for radiographers, is possibly related to WRMSDs.

OBJECTIVE: To describe the implantation and the effects of directly-observed treatment short course (DOTS) in primary health care units. METHODS: Interviews were held with the staff of nine municipal health care units (MHU) that provided DOTS in Rio de Janeiro City, Southeastern Brazil, in 2004-2005. A dataset with records of all tuberculosis treatments beginning in 2004 in all municipal health care units was collected. Bivariate analyses and a multinomial model were applied to identify associations between treatment outcomes and demographic and treatment process variables, including being in DOTS or self-administered therapy (SAT). RESULTS: From 4,598 tuberculosis cases treated in public health units administrated by the municipality, 1,118 (24.3%) were with DOTS and 3,480 (75.7%) with SAT. The odds of DOTS were higher among patients with age under 50 years, tuberculosis relapse and prior history of default or treatment failure. The odds of death were 52.0% higher among patients on DOTS as compared to SAT. DOTS modality including community health workers (CHWs) showed the highest treatment success rate. A reduction of 21.0% was observed in the odds of default (vs. cure) among patients on DOTS as compared to patients on SAT, and a reduction of 64.0% among patients on DOTS with CHWs as compared to those without CHWs. CONCLUSIONS: Patients with a "low compliance profile" were more likely to be included in DOTS. This strategy improves the quality of care provided to tuberculosis patients, although the proposed goals were not achieved.

Bisphenol A (BPA) is an endocrine disrupting chemical (EDC) whose migration from food packaging is recognized worldwide. However, the real overall food contamination and related consequences are yet largely unknown. Among humans, children’s exposure to BPA has been emphasized because of the immaturity of their biological systems. The main aim of this study was to assess the reproductive impact of BPA leached from commercially available plastic containers used or related to child nutrition, performing ecotoxicological tests using the biomonitoring species Daphnia magna. Acute and chronic tests, as well as single and multigenerational tests were done. Migration of BPA from several baby bottles and other plastic containers evaluated by GC-MS indicated that a broader range of foodstuff may be contaminated when packed in plastics. Ecotoxicological test results performed using defined concentrations of BPA were in agreement with literature, although a precocious maturity of daphnids was detected at 3.0 mg/L. Curiously, an increased reproductive output (neonates per female) was observed when daphnids were bred in the polycarbonate (PC) containers (145.1±4.3 % to 264.7±3.8 %), both in single as in multigenerational tests, in comparison with the negative control group (100.3±1.6 %). A strong correlated dose-dependent ecotoxicological effect was observed, providing evidence that BPA leached from plastic food packaging materials act as functional estrogen in vivo at very low concentrations. In contrast, neonate production by daphnids cultured in polypropylene and non-PC bottles was slightly but not significantly enhanced (92.5±2.0 % to 118.8±1.8 %). Multigenerational tests also revealed magnification of the adverse effects, not only on fecundity but also on mortality, which represents a worrying trend for organisms that are chronically exposed to xenoestrogens for many generations. Two plausible explanations for the observed results could be given: a non-monotonic dose–response relationship or a mixture toxicity effect.

OBJETIVO: Avaliar a validade concorrente e a equivalência operacional do questionário "Lista de Atividades Físicas" em crianças.MÉTODOS: O estudo complementa o processo de adaptação transcultural do Physical Activity Checklist Interview. Participaram 118 escolares de sete a dez anos matriculados do segundo ao quinto ano do ensino fundamental de São Paulo, SP, em 2009. Foram quantificados o tempo de engajamento em atividades físicas moderadas a vigorosas e em atividades sedentárias, assim como custos metabólicos total e ponderado. Adotou-se o acelerômetro como medida de atividade física. Foram quantificados a atividade física total (counts/min) e o tempo em atividade física moderada a vigorosa. A validade foi avaliada pelo coeficiente de correlação de Pearson e a equivalência operacional por meio dos dados referentes à duração e avaliação da entrevista.RESULTADOS: Os valores do coeficiente de correlação entre os resultados provenientes do questionário e do acelerômetro variaram de 0,34 a 0,40. O questionário "Lista de Atividades Físicas" superestimou o tempo em atividade física moderada a vigorosa quando comparado ao acelerômetro. A duração média da entrevista foi de 24 minutos (mínima = 13 min; máxima = 41 min; desvio-padrão = 5 min). O item da entrevista com pior resultado foi a habilidade do escolar em estimar tempo (ruim ou regular em 24,8% das entrevistas).CONCLUSÕES: Em relação à versão original, o questionário "Lista de Atividades Físicas" apresenta valores similares de indicadores de validade concorrente e de equivalência operacional, confirmando a adequação do processo de adaptação transcultural.

OBJETIVO Analisar as características das vítimas, vias e veículos envolvidos em acidentes de trânsito e os fatores de risco de acidentes com ocorrência de óbito. MÉTODOS Estudo de coorte não concorrente considerando os acidentes de trânsito em Fortaleza, CE, de janeiro de 2004 a dezembro de 2008. Foram utilizados dados do Sistema de Informação de Acidente de Trânsito de Fortaleza, do Sistema de Informações de Mortalidade, do Sistema de Informações Hospitalares e dos bancos de dados de Habilitação e Veículos do Departamento Estadual de Trânsito. Técnicas de relacionamento determinístico e probabilístico foram aplicadas para integrar as bases de dados. Efetuou-se a análise descritiva das variáveis relativas às pessoas, às vias, aos veículos e ao tempo. Foram utilizados os modelos lineares generalizados na investigação de fatores de risco para óbito por acidente de trânsito. O ajuste do modelo foi verificado pela razão de verossimilhança e análise ROC. RESULTADOS Registraram-se 118.830 acidentes no período. Predominaram colisão/abalroamento (78,1%), atropelamentos (11,9%) e choque com obstáculo fixo (3,9%) e com motocicletas (18,1%). Ocorreram óbitos em 1,4% dos acidentes. Estiveram independentemente associados ao óbito por acidente de trânsito: bicicletas (OR = 21,2; IC95% 16,1;27,8), atropelamentos (OR = 5,9; IC95% 3,7;9,2), choque com obstáculo fixo (OR = 5,7; IC95% 3,1;10,5) e acidentes com motociclistas (OR = 3,5; IC95% 2,6;4,6). Os principais fatores contribuintes foram envolvimento de uma única pessoa (OR = 6,6; IC95% 4,1;10,73), presença de condutores não habilitados (OR = 4,1; IC95% 2,9;5,5) um único veículo envolvido (OR = 3,9; IC95% 2,3;6,4), sexo masculino (OR = 2,5; IC95% 1,9;3,3), tráfego em vias de jurisdição federal (OR = 2,4; IC95% 1,8;3,7), horário madrugada (OR = 2,4; IC95% 1,8;3,0) e dia de domingo (OR = 1,7; IC95% 1,3;2,2), todas ajustadas segundo modelo log-binomial. CONCLUSÕES As ações de promoção e prevenção de acidentes no trânsito devem focar os acidentes com veículos de duas rodas, que mais frequentemente envolvem uma única pessoa, não habilitada, do sexo masculino, em horários noturnos, em finais de semana e nas vias onde se desenvolvem maiores velocidades.

Electricity markets are systems for effecting the purchase and sale of electricity using supply and demand to set energy prices. Two major market models are often distinguished: pools and bilateral contracts. Pool prices tend to change quickly and variations are usually highly unpredictable. In this way, market participants often enter into bilateral contracts to hedge against pool price volatility. This article addresses the challenge of optimizing the portfolio of clients managed by trader agents. Typically, traders buy energy in day-ahead markets and sell it to a set of target clients, by negotiating bilateral contracts involving three-rate tariffs. Traders sell energy by considering the prices of a reference week and five different types of clients. They analyze several tariffs and determine the best share of customers, i.e., the share that maximizes profit. © 2014 IEEE.

In order to correctly assess the biaxial fatigue material properties one must experimentally test different load conditions and stress levels. With the rise of new in-plane biaxial fatigue testing machines, using smaller and more efficient electrical motors, instead of the conventional hydraulic machines, it is necessary to reduce the specimen size and to ensure that the specimen geometry is appropriated for the load capacity installed. At the present time there are no standard specimen’s geometries and the indications on literature how to design an efficient test specimen are insufficient. The main goal of this paper is to present the methodology on how to obtain an optimal cruciform specimen geometry, with thickness reduction in the gauge area, appropriated for fatigue crack initiation, as a function of the base material sheet thickness used to build the specimen. The geometry is optimized for maximum stress using several parameters, ensuring that in the gauge area the stress is uniform and maximum with two limit phase shift loading conditions. Therefore the fatigue damage will always initiate on the center of the specimen, avoiding failure outside this region. Using the Renard Series of preferred numbers for the base material sheet thickness as a reference, the reaming geometry parameters are optimized using a derivative-free methodology, called direct multi search (DMS) method. The final optimal geometry as a function of the base material sheet thickness is proposed, as a guide line for cruciform specimens design, and as a possible contribution for a future standard on in-plane biaxial fatigue tests. © 2014, Gruppo Italiano Frattura. ABSTRACT OBJECTIVE To estimate the required number of public beds for adults in intensive care units in the state of Rio de Janeiro to meet the existing demand and compare results with recommendations by the Brazilian Ministry of Health. METHODS The study uses a hybrid model combining time series and queuing theory to predict the demand and estimate the number of required beds. Four patient flow scenarios were considered according to bed requests, percentage of abandonments and average length of stay in intensive care unit beds. The results were plotted against Ministry of Health parameters. Data were obtained from the State Regulation Center from 2010 to 2011. RESULTS There were 33,101 medical requests for 268 regulated intensive care unit beds in Rio de Janeiro. With an average length of stay in regulated ICUs of 11.3 days, there would be a need for 595 active beds to ensure system stability and 628 beds to ensure a maximum waiting time of six hours. Deducting current abandonment rates due to clinical improvement (25.8%), these figures fall to 441 and 417. With an average length of stay of 6.5 days, the number of required beds would be 342 and 366, respectively; deducting abandonment rates, 254 and 275. The Brazilian Ministry of Health establishes a parameter of 118 to 353 beds. Although the number of regulated beds is within the recommended range, an increase in beds of 122.0% is required to guarantee system stability and of 134.0% for a maximum waiting time of six hours. CONCLUSIONS Adequate bed estimation must consider reasons for limited timely access and patient flow management in a scenario that associates prioritization of requests with the lowest average length of stay.

Una de las actividades que realizan los programas de control de hidatidosis causada por Echinococcus granulosus en la República Argentina es la búsqueda de portadores asintomáticos de quistes hidatídicos entre la población general que habita las áreas de riesgo, mediante un ensayo inmunoenzimático (EIE) con antígeno de liquido hidatídico total (EIE-ALHT) que selecciona los posibles portadores. En base a la experiencia recogida se ha observado que dependiendo de la prevalencia del área, entre el 10% y el 30% de las personas seleccionadas por el EIE con valores de densidad óptica (DO > DO + 4S no presentan imagenes compatibles con quistes hidatídicos. El propósito del estudio fue mejorar la especificidad de la prueba. Con ese fin, se evaluó la influencia de la modificación de la oferta antigénica y el efecto de la absorción de los anticuerpos anti-componentes séricos ovinos y anti-fosforilcolina de los sueros en estudios. 114 sueros no hidatídicos seleccionados por su alto nivel de reacciones cruzadas en EIE-ALHT y 118 sueros hidatídicos se estudiaron frente a 4 fracciones antigénicas de liquido hidatídico ovino. El EIE que empleo la fracción antigénica S2B conjuntamente con la absorción previa de los sueros (EIE-S2B/A) fue el sistema que mejor discriminó los sueros hidatídicos de los no hidatídicos con altos niveles de anticuerpos no relacionados con la presencia de quistes. Se propone el empleo del sistema EIE-S2B/A para la búsqueda a campo de portadores asintomáticos de quistes hidatídicos en reemplazo del EIE-ALHT actualmente en uso. Las 4 fracciones antigénicas fueron analizadas por doble difusión y SDS-PAGE. La fracción S2B se caracterizó como enriquecida en componentes parasitarios de menos de 30 Kd entre los cuales se encontrarian el antígeno B y subunidades o fragmentos del antígeno 5.
