Este trabalho teve por objetivo estudar o efeito de variações nas doses de potássio e nos níveis de estresse hídrico sobre o ajustamento osmótico e crescimento de plantas de milho (Zea mays L.). Para tanto, instalou-se um experimento em vasos que continham 40 dm³ de Terra Roxa Estruturada Latossólica textura média argilosa, em casa de vegetação telada do Departamento de Ciência do Solo, Campus de Botucatu, UNESP/SP, de dezembro de 1994 a fevereiro de 1995. O delineamento experimental utilizado foi o de blocos ao acaso em esquema fatorial (2 x 3 x 3). Os tratamentos consistiram da aplicação de duas doses (35 e 130 mg dm-3) de potássio na forma de KCl e três condições de estresse hídrico, sendo plantas sem estresse (S0) e plantas que sofreram estresse hídrico moderado (S1) ou intenso (S2), 44 dias após a emergência. O crescimento das plantas foi avaliado em três épocas, aos 55, 69 e 83 dias da emergência. Da análise geral dos resultados pôde-se inferir que as folhas das plantas de milho se ajustaram osmoticamente com relação as doses de potássio, em condições de estresse hídrico moderado. Não houve efeito dos estresses hídricos na acumulação de K, Ca e Mg nas folhas; no entanto, a maior dose de K aplicado ao solo proporcionou, em todas as épocas, maior teor deste nutriente e menores de Ca e de Mg nas folhas. Os níveis de estresse utilizados não influíram na área foliar, mas o maior teor de água no solo aumentou a produção de matéria seca das partes e, principalmente, da planta toda, mostrando ser esta última a melhor característica para avaliação do déficit de água na planta. A maior dose de K proporcionou maior área foliar e produção de matéria seca às partes e à planta toda; no entanto, não influenciou o comportamento de nenhum índice fisiológico estudado. O maior teor de água no solo mostrou menores valores de área foliar específica (AFE), razão de área foliar (RAF) e razão de massa foliar (RMF). A adição de potássio não melhorou o crescimento da cultura, quando o fornecimento de água foi limitado. No entanto, as plantas mais bem nutridas em potássio produziram mais massa, independentemente dos níveis de água utilizados, indicando melhor produção de matéria seca destas plantas.

BACKGROUND: A concentrate for bicarbonate haemodialysis acidified with citrate instead of acetate has been marketed in recent years. The small amount of citrate used (one-fifth of the concentration adopted in regional anticoagulation) protects against intradialyser clotting while minimally affecting the calcium concentration. The aim of this study was to compare the impact of citrate- and acetate-based dialysates on systemic haemodynamics, coagulation, acid-base status, calcium balance and dialysis efficiency. METHODS: In 25 patients who underwent a total of 375 dialysis sessions, an acetate dialysate (A) was compared with a citrate dialysate with (C+) or without (C) calcium supplementation (0.25 mmol/L) in a randomised single-blind cross-over study. Systemic haemodynamics were evaluated using pulse-wave analysis. Coagulation, acid-base status, calcium balance and dialysis efficiency were assessed using standard biochemical markers. RESULTS: Patients receiving the citrate dialysate had significantly lower systolic blood pressure (BP) (-4.3 mmHg, p < 0.01) and peripheral resistances (PR) (-51 dyne.sec.cm-5, p < 0.001) while stroke volume was not increased. In hypertensive patients there was a substantial reduction in BP (-7.8 mmHg, p < 0.01). With the C+ dialysate the BP gap was less pronounced but the reduction in PR was even greater (-226 dyne.sec.cm-5, p < 0.001). Analyses of the fluctuations in PR and of subjective tolerance suggested improved haemodynamic stability with the citrate dialysate. Furthermore, an increase in pre-dialysis bicarbonate and a decrease in pre-dialysis BUN, post-dialysis phosphate and ionised calcium were noted. Systemic coagulation activation was not influenced by citrate. CONCLUSION: The positive impact on dialysis efficiency, acid-base status and haemodynamics, as well as the subjective tolerance, together indicate that citrate dialysate can significantly contribute to improving haemodialysis in selected patients.

OBJECTIVE: To validate a revision of the Mini Nutritional Assessment short-form (MNA(R)-SF) against the full MNA, a standard tool for nutritional evaluation. METHODS: A literature search identified studies that used the MNA for nutritional screening in geriatric patients. The contacted authors submitted original datasets that were merged into a single database. Various combinations of the questions on the current MNA-SF were tested using this database through combination analysis and ROC based derivation of classification thresholds. RESULTS: Twenty-seven datasets (n=6257 participants) were initially processed from which twelve were used in the current analysis on a sample of 2032 study participants (mean age 82.3y) with complete information on all MNA items. The original MNA-SF was a combination of six questions from the full MNA. A revised MNA-SF included calf circumference (CC) substituted for BMI performed equally well. A revised three-category scoring classification for this revised MNA-SF, using BMI and/or CC, had good sensitivity compared to the full MNA. CONCLUSION: The newly revised MNA-SF is a valid nutritional screening tool applicable to geriatric health care professionals with the option of using CC when BMI cannot be calculated. This revised MNA-SF increases the applicability of this rapid screening tool in clinical practice through the inclusion of a "malnourished" category.

Na caracterização hídrica dos solos do Campus da Faculdade de Ciências Agrárias e Veterinárias de Jaboticabal (SP), verificou-se que o Latossolo Vermelho eutroférrico típico apresentou maior capacidade de água disponível. A retenção da água, na capacidade de campo (0,01 MPa) e ponto de murcha permanente (1,5 MPa), correlacionou-se melhor com as frações argila e silte, em relação ao teor de matéria orgânica e densidade do solo. Utilizando características hídricas, como a capacidade de água disponível, foi estabelecido por meio do balanço hídrico de Thornthwaite o regime hídrico para os solos. Verificou-se a predominância do regime hídrico ústico em todos os solos (Latossolo Vermelho Nitossolo).

BACKGROUND: Obesity is associated with vitamin D deficiency, and both are areas of active public health concern. We explored the causality and direction of the relationship between body mass index (BMI) and 25-hydroxyvitamin D [25(OH)D] using genetic markers as instrumental variables (IVs) in bi-directional Mendelian randomization (MR) analysis. METHODS AND FINDINGS: We used information from 21 adult cohorts (up to 42,024 participants) with 12 BMI-related SNPs (combined in an allelic score) to produce an instrument for BMI and four SNPs associated with 25(OH)D (combined in two allelic scores, separately for genes encoding its synthesis or metabolism) as an instrument for vitamin D. Regression estimates for the IVs (allele scores) were generated within-study and pooled by meta-analysis to generate summary effects. Associations between vitamin D scores and BMI were confirmed in the Genetic Investigation of Anthropometric Traits (GIANT) consortium (n = 123,864). Each 1 kg/m(2) higher BMI was associated with 1.15% lower 25(OH)D (p = 6.52×10⁻²⁷). The BMI allele score was associated both with BMI (p = 6.30×10⁻⁶²) and 25(OH)D (-0.06% [95% CI -0.10 to -0.02], p = 0.004) in the cohorts that underwent meta-analysis. The two vitamin D allele scores were strongly associated with 25(OH)D (p≤8.07×10⁻⁵⁷ for both scores) but not with BMI (synthesis score, p = 0.88; metabolism score, p = 0.08) in the meta-analysis. A 10% higher genetically instrumented BMI was associated with 4.2% lower 25(OH)D concentrations (IV ratio: -4.2 [95% CI -7.1 to -1.3], p = 0.005). No association was seen for genetically instrumented 25(OH)D with BMI, a finding that was confirmed using data from the GIANT consortium (p≥0.57 for both vitamin D scores). CONCLUSIONS: On the basis of a bi-directional genetic approach that limits confounding, our study suggests that a higher BMI leads to lower 25(OH)D, while any effects of lower 25(OH)D increasing BMI are likely to be small. Population level interventions to reduce BMI are expected to decrease the prevalence of vitamin D deficiency.

The treatment of multiple myeloma has undergone significant changes in the recent past. The arrival of novel agents, especially thalidomide, bortezomib and lenalidomide, has expanded treatment options and patient outcomes are improving significantly. This article summarises the discussions of an expert meeting which was held to debate current treatment practices for multiple myeloma in Switzerland concerning the role of the novel agents and to provide recommendations for their use in different treatment stages based on currently available clinical data. Novel agent combinations for the treatment of newly diagnosed, as well as relapsed multiple myeloma are examined. In addition, the role of novel agents in patients with cytogenetic abnormalities and renal impairment, as well as the management of the most frequent side effects of the novel agents are discussed. The aim of this article is to assist in treatment decisions in daily clinical practice to achieve the best possible outcome for patients with multiple myeloma.

Limbic encephalitis (LE) with waxing and waning neuropsychiatric manifestations including behavioral, personality, psychiatric, and memory changes can evolve over days to months. Many features of LE show remarkable overlap with the characteristics of mesial-temporal (limbic) status epilepticus (MTLSE or LSE). With LE, these prolonged impaired states are assumed not to be due to ongoing epileptic activity or MTLSE, because scalp EEGs usually show no epileptiform spike-wave activity; cycling behavioral and motor changes are attributed to LE; there may be little immediate improvement with antiepileptic drugs (AEDs); and of course, implanted electrodes are rarely used. Conversely, it is known that in pre-surgical patients with refractory limbic epilepsy, implanted electrodes have revealed limbic seizures that cannot be seen at the scalp. This paper assembles a chain of inferences to advance the proposition that refractory LE might represent LSE more often than is thought, and that implanted electrodes should be considered in some cases. We present two cases that suggest that LE was also LSE, one of which warranted implanted electrodes (case 1).

We investigated the relationship between being bullied and measured body weight and perceived body weight among adolescents of a middle-income sub Saharan African country. Our data originated from the Global School-based Health Survey, which targets adolescents aged 13-15 years. Student weights and heights were measured before administrating the questionnaire which included questions about personal data, health behaviors and being bullied. Standard criteria were used to assess thinness, overweight and obesity. Among 1,006 participants who had complete data, 16.5% (95%CI 13.3-20.2) reported being bullied ≥ 3 days during the past 30 days; 13.4% were thin, 16.8% were overweight and 7.6% were obese. Categories of actual weight and of perceived weight correlated only moderately (Spearman correlation coefficient 0.37 for boys and 0.57 for girls; p < 0.001). In univariate analysis, both actual obesity (OR 1.76; p = 0.051) and perception of high weight (OR 1.63 for "slightly overweight"; OR 2.74 for "very overweight", both p < 0.05) were associated with being bullied. In multivariate analysis, ORs for categories of perceived overweight were virtually unchanged while ORs for actual overweight and obesity were substantially attenuated, suggesting a substantial role of perceived weight in the association with being bullied. Actual underweight and perceived thinness also tended to be associated with being bullied, although not significantly. Our findings suggest that more research attention be given to disentangling the significant association between body image, overweight and bullying among adolescents. Further studies in diverse populations are warranted.

A qualidade física do solo sob sistemas de pastejo contínuo e rotacionado foi avaliada pelo Intervalo Hídrico Ótimo (IHO). A amostragem foi realizada na área experimental da Embrapa Gado de Corte. Foi estudado um Latossolo Vermelho distrófico. Foram avaliados quatro piquetes, sendo dois no sistema de pastejo contínuo e dois no sistema de pastejo rotacionado. No sistema de pastejo contínuo, implementado com a espécie Brachiaria decumbens cv. Basilisk, foram retiradas 30 amostras por piquete, não tendo um dos piquetes recebido adubação de manutenção (Cs), enquanto o outro havia recebido adubação bianual de manutenção (Cc). No sistema de pastejo rotacionado, implementado com a espécie Panicum maximum cv. Tanzânia, foram retiradas 30 amostras por piquete; em um dos piquetes, o resíduo pós-pastejo era mantido entre 2,0 e 2,5 t ha-1 de matéria seca total (MST) (R1), enquanto no outro era mantido entre 3,0 e 3,5 t ha-1 MST (R2). As amostras foram submetidas a um gradiente de tensão de água e, posteriormente, utilizadas nas determinações da densidade do solo (Ds), resistência do solo à penetração (RP), umidade volumétrica (tetav) e do IHO. O sistema de pastejo rotacionado apresentou piores condições físicas do solo para o crescimento vegetal, avaliadas pelo critério do IHO. Os maiores valores de Ds e menor IHO foram observados no R1, o que foi atribuído às taxas de lotação mais elevadas aplicadas neste piquete.
