Introducción En esta época posterior a la guerra fría y de creciente globalización, el rol de las Naciones Unidas experimenta constantes redefiniciones y desafíos. El señor Kofi Annan, Secretario General, afirmó que las Naciones Unidas se encuentran hoy en una encrucijada, y que el momento es tan trascendente como lo fue el de su fundación, hace sesenta años. Aunque los principios fundamentales de la democracia, los derechos humanos y el estado de derecho siguen siendo universalmente aceptados y se consideran piedra angular del orden mundial, el Secretario General considera, como otros muchos, que ha llegado el momento de rediseñar su estructura básica para combinar el imperativo de la acción con la necesidad de legitimidad". En este contexto, durante la visita oficial del Secretario General a Chile en noviembre del 2003, la Comisión Económica para América Latina y el Caribe (CEPAL) fue anfitriona de la mesa redonda "El contexto global y la renovación de las Naciones Unidas", en la que participaron la señora Tarja Halonen, Presidenta de la República de Finlandia, el señor Ricardo Lagos, Presidente de la República de Chile, y el señor Annan. Entre los invitados a este acto se encontraban miembros del cuerpo diplomático en Santiago de Chile, destacados intelectuales, representantes de la sociedad civil y académicos que enriquecieron el debate con preguntas y comentarios. Los tres líderes analizaron la situación política, económica y social del mundo de hoy en el contexto del actual proceso de reforma de las Naciones Unidas. Disertaron sobre los desafíos del desarrollo, el multilateralismo, la interdependencia de los países y su necesidad de contar con una participación ciudadana creciente y coherente en los planos local, regional y mundial. Uno de los temas centrales fue el poder, y el modo en que las Naciones Unidas pueden canalizarlo y redistribuirlo con el fin de que su ejercicio sea colectivo y civilizado. Aunque es imposible resolver en una mañana o en una sola reunión estos complejos asuntos, que en ocasiones resultan abrumadores, tenemos la esperanza de que este libro proporcione inspiración, conceptos e ideas que permitan alcanzar soluciones eficaces e innovadoras en el futuro. The present work aimed to quantify the generation of group B health care`s residuals in places such as drugstores and pharmacies, in the municipality of Uberaba, MG, Brazil, as well as to identify their final destination in those places during the months of July and September, 2010. In order to also verify the medicaments generated by the whole community, some delivery campaigns were arranged to capture out-of-date medicaments at 3 drugstores. In this sense, all people who effectively looked for a pharmacy/drugstore to deposit the medicaments were invited to respond some questions, containing information about the name of the medicament, the pharmaceutical composition, the color applicable to the medicament and the expiration date. At the end of the period of research, 76 interviews were performed and 90 medicaments were observed (1.18 products per interview, in average). Results obtained suggested that pharmacies/drugstores located downtown tend to generate more medicaments than others located in the districts and periphery zones. Moreover, manipulation drugstores tend to generate more products than their conventional counterparts. Regarding the therapeutic classes, the anti-hypertensive medicaments represented the most substantial percentage of the medicaments delivered, accounting for 21,11%. However, the worst scenario tends to show that people usually do not know exactly how to discard this sort of product – about 65% of the people consulted discard chemical pharmaceutical residuals in the same place the domestic trash is thrown away – what may represent a serious risk in terms of contamination of the environment, so that a strong campaign towards the proper usage and discharge of medicaments should be strongly encouraged

Experimental tissue fusion benefits from the selective heating of superparamagnetic iron oxide nanoparticles (SPIONs) under high frequency irradiation. However, the metabolic pathways of SPIONs for tissue fusion remain unknown. Hence, the goal of this in vivo study was to analyze the distribution of SPIONs in different organs by means of magnetic resonance imaging (MRI) and histological analysis after a SPION-containing patch implantation.

BACKGROUND: Usher syndrome, a combination of retinitis pigmentosa (RP) and sensorineural hearing loss with or without vestibular dysfunction, displays a high degree of clinical and genetic heterogeneity. Three clinical subtypes can be distinguished, based on the age of onset and severity of the hearing impairment, and the presence or absence of vestibular abnormalities. Thus far, eight genes have been implicated in the syndrome, together comprising 347 protein-coding exons. METHODS: To improve DNA diagnostics for patients with Usher syndrome, we developed a genotyping microarray based on the arrayed primer extension (APEX) method. Allele-specific oligonucleotides corresponding to all 298 Usher syndrome-associated sequence variants known to date, 76 of which are novel, were arrayed. RESULTS: Approximately half of these variants were validated using original patient DNAs, which yielded an accuracy of >98%. The efficiency of the Usher genotyping microarray was tested using DNAs from 370 unrelated European and American patients with Usher syndrome. Sequence variants were identified in 64/140 (46%) patients with Usher syndrome type I, 45/189 (24%) patients with Usher syndrome type II, 6/21 (29%) patients with Usher syndrome type III and 6/20 (30%) patients with atypical Usher syndrome. The chip also identified two novel sequence variants, c.400C>T (p.R134X) in PCDH15 and c.1606T>C (p.C536S) in USH2A. CONCLUSION: The Usher genotyping microarray is a versatile and affordable screening tool for Usher syndrome. Its efficiency will improve with the addition of novel sequence variants with minimal extra costs, making it a very useful first-pass screening tool.

Objective: Identification of the ventrointermediate thalamic nucleus (Vim) in modern 3T high-field MRI for image-based targeting in deep brain stimulation (DBS) is still challenging. To evaluate the usefulness and reliability of analyzing the connectivity with the cerebellum using Q-ball-calculation we performed a retrospective analysis. Method: 5 patients who underwent bilateral implantation of electrodes in the Vim for treatment of Essential Tremor between 2011 and 2012 received additional preoperative Q-ball imaging. Targeting was performed according to atlas coordinates and standard MRI. Additionally we performed a retrospective identification of the Vim by analyzing the connectivity of the thalamus with the dentate nucleus. The exact position of the active stimulation contact in the postoperative CT was correlated with the Vim as it was identified by Q-ball calculation. Results: Localization of the Vim by analysis of the connectivity between thalamus and cerebellum was successful in all 5 patients on both sides. The average position of the active contacts was 14.6 mm (SD 1.24) lateral, 5.37 mm (SD 0.094 posterior and 2.21 mm (SD 0.69) cranial of MC. The cranial portion of the dentato-rubro-thalamic tract was localized an average of 3.38 mm (SD 1.57) lateral and 1.5 mm (SD 1.22) posterior of the active contact. Conclusions: Connectivity analysis by Q-ball calculation provided direct visualization of the Vim in all cases. Our preliminary results suggest, that the target determined by connectivity analysis is valid and could possibly be used in addition to or even instead of atlas based targeting. Larger prospective calculations are needed to determine the robustness of this method in providing refined information useful for neurosurgical treatment of tremor.
VEGF-B-induced vascular growth leads to metabolic reprogramming and ischemia resistance in the heart

Angiogenic growth factors have recently been linked to tissue metabolism. We have used genetic gain- and loss-of function models to elucidate the effects and mechanisms of action of vascular endothelial growth factor-B (VEGF-B) in the heart. A cardiomyocyte-specific VEGF-B transgene induced an expanded coronary arterial tree and reprogramming of cardiomyocyte metabolism. This was associated with protection against myocardial infarction and preservation of mitochondrial complex I function upon ischemia-reperfusion. VEGF-B increased VEGF signals via VEGF receptor-2 to activate Erk1/2, which resulted in vascular growth. Akt and mTORC1 pathways were upregulated and AMPK downregulated, readjusting cardiomyocyte metabolic pathways to favor glucose oxidation and macromolecular biosynthesis. However, contrasting with a previous theory, there was no difference in fatty acid uptake by the heart between the VEGF-B transgenic, gene-targeted or wildtype rats. Importantly, we also show that VEGF-B expression is reduced in human heart disease. Our data indicate that VEGF-B could be used to increase the coronary vasculature and to reprogram myocardial metabolism to improve cardiac function in ischemic heart disease.
