Is surgery for primary hyperparathyroidism easier when methylene blue (MB) is given preoperatively? This retrospective study compares the durations of interventions for primary hyperparathyroidism carried out after i.v. MB administration to those when no MB was given. Over a period of 20 years (June 1976 to December 1996), 175 consecutive patients (56 men and 119 women, with ages ranging from 16 to 92, mean 59.6) were operated upon for primary hyperparathyrodism; 55 were operated before February 1986--the period when BM was introduced routinely, and 120 after. Thirty-two other patients were excluded from the study: 14 had had a previous cervicotomy and 18 another procedure in addition to the parathyroidectomy (usually on the thyroid gland), two conditions which prolonged the time devoted to parathyroid identification and excision. Preoperative calcemia averaged 2.97 mmol/L (2.34 to 4.59) and mean preoperative PTH was equal to 2.6 times the upper normal limit (0.5 to 24.1). Both groups were similar for as age, sex, preoperative calcium and PTH, and histologies. Methylene blue was administered intravenously (5 mg/kg diluted in 500 cc of 5% glucose) over a period of time of one hour starting two hours prior to surgery. All 175 procedures were performed by two surgeons and duration of surgery was recorded from the anesthesiologist's notes. There were 149 adenomas (85%), 24 hyperplasias (14%), a combination of both in two, and unspecified in two others. Except for a case of acute lower back pain synchronous to the injection of the dye (which was immediately stopped), MB was well tolerated. Mean duration for the 55 interventions performed without MB was 68 minutes (35 to 140, median 60), compared to 49 minutes for the 120 procedures carried out after MB had been given (20 to 155, median 45). Differences in operative, times were highly significant (p < 10(-6) and represented a gain of time of 27%. Surgery for primary hyperparathyroidism was significantly shorter when it was preceded by the administration of MB, a dye which facilitates the identification of pathologic parathyroid gland(s).

The intensity of pain perception and its sensibility to analgesic drugs is highly variable and unpredictable between individuals. Drug disposition varies during development due to the physiological maturation of enzymatic systems and physiological processes responsible for the absorption, distribution, elimination and effect at the site of action. Many of those developmental variables are not yet clearly defined, but their consideration is important for avoiding potential risks of ineffective or toxic treatment. Implications of those developmental changes for day-to-day clinical practice depend on the age of the child, on the type of drug, on the underlying disease and on the potential co-administration of other chemicals.

Introduction. - « Ostéo-Mobile Vaud » est un projet pilote visant à informer et promouvoir les mesures générales de prévention del'ostéoporose chez les femmes vaudoises de 60 ans et plus. Son butest également d'évaluer le risque de fracture dans cette populationen associant les facteurs de risque cliniques, la mesure de la DMOpar DXA de la colonne lombaire et du fémur proximal (Hologic Discovery),la recherche des fractures vertébrales préexistantes parVFA et une appréciation de la « qualité osseuse » par TBS. Une phaseprospective de 5 ans est prévue. Le Trabecular Bone Score (TBS) estun procédé qui consiste à appliquer un traitement informatique à l'image 2D de projection de la DXA basé sur la quantification desvariations locales de niveaux de gris. Un modèle mathématiquerelie le paramètre TBS avec des paramètres de microarchitecture 3Dtels la connectivité 3D, le nombre de travées et l'espace inter-trabéculaire.Ce modèle fait de TBS une mesure indirecte de microarchitectureosseuse.Résultats. - Fin juillet 2011, 510 femmes de ± 67 ans, IMC ± 26 kg/m2,ont été évaluées. Une ou plusieurs fractures de fragilité ont été rapportéeschez 72 femmes, parmi lesquelles 39 avec fractures vertébrales.TBS diminue avec l'âge (-0.005 par année, p < 0,001) etdiminue en fonction de l'IMC (- 0,011 par kg/m2, p < 0,001). La corrélationentre TBS et la DMO de la colonne lombaire est faible(r = 0,4, p < 0,001) et un grand pourcentage de la variabilité du TBSest indépendant de la DMO (> 84%). TBS discrimine les femmes avecfractures vertébrales des femmes sans fracture vertébrale et lesfemmes avec OP clinique (fractures de fragilité) de celles sans OPclinique. Ce pouvoir discriminatif est indépendant de la DMO de lacolonne lombaire ou du T-score le plus bas, après ajustement pourl'âge et l'IMC.Odd ratio (OR, 95 % IC) pour une diminution d'une déviation standarddu TBS, ajusté pour l'âge, l'IMC et la mesure de DMO :Conclusion. - TBS représente une plus-value par rapport à la DMOdans l'appréciation du risque de fracture chez les femmes vaudoisesde 60 ans et plus.
