L'oggetto del presente elaborato è un confronto tra l'opera Don Giovanni di Wolfgang Amadeus Mozart e il racconto Don Juan di Ernst Theodor Amadeus Hoffmann. A una sezione dedicata alla vita dello scrittore e alle sue opere segue un'analisi della figura del Don Giovanni nella storia, in Hoffmann e in Mozart. Un parallelo tra i due capolavori è invece il tema dell'ultimo capitolo, con un focus specifico sui personaggi, i temi, e l'interpretazione.

Il presente elaborato finale ha lo scopo di illustrare tutti i parametri necessari alla progettazione di un “buon” impianto di illuminazione stradale, dotato di tutte le nuove tecnologie, per un maggior risparmio energetico incentrato alla valorizzazione e riqualificazione urbana. L’illuminazione pubblica negli ultimi decenni ha ricoperto un’importanza sempre maggiore all’interno della città e del territorio aumentando in modo esponenziale la presenza di corpi illuminanti e formando spesso una vera e propria macchia di luce durante la notte. L’implementazione degli apparecchi di illuminazione ha portato ad una maggiore sicurezza e, a volte, un maggiore comfort nelle zone cittadine, ma anche un notevole aumento di costi energetici a carico delle amministrazioni comunali. Pertanto, alla luce degli ultimi mesi, è necessario incrementare i servizi tecnologici basati sull’efficientamento energetico degli impianti, in particolare, adottando sempre di più la sorgente a LED e sistemi di riduzione del flusso luminoso, in sostituzione alle lampade obsolete esistenti con un basso rendimento energetico. Con lo sviluppo del traffico veicolare degli ultimi anni, l’illuminazione urbana ha modificato il suo aspetto nelle città, concentrandosi sul rispetto dei valori prescritti per consentire una maggiore sicurezza e scorrevolezza del traffico motorizzato durante le ore notturne a discapito della valorizzazione urbana. Infatti, le statistiche hanno mostrato che il tasso degli incidenti notturni, ossia il rapporto tra gli incidenti e il numero di veicoli in circolazione, è più elevato rispetto agli incidenti durante le ore diurne.

O projeto de intervenção foi realizado durante o curso de especialização em saúde da família, pela equipe de saúde da família da zona rural 1 do município de Valença- PI. O presente estudo teve por objetivo avaliar o pré-natal, puerpério e saúde bucal. realizado pela equipe de saúde na UBS. O projeto de intervenção verificou que mesmo diante das dificuldades encontradas, o pré-natal é realizado de maneira satisfatória, o qual quase que a totalidade das gestantes da comunidade realizaram a quantidade de consultas preconizadas pelo Ministério da saúde, além de terem recebido todas as orientações para que ocorra um pré-natal adequado e seguro, falando um pouco mais sobre o puerpério obtivemos resultados satisfatórios, principalmente devido a atuação dos ACS que nos comunicavam, quando ocorria o parto de nossas gestantes. Com relação à saúde bucal, não tivemos muito sucesso, devido às dificuldades, pois atuamos em uma área que corresponde às dez localidades, entretanto apenas quatro delas possuem um lugar adequado para realizar o atendimento odontológico, o que acaba dificultando o acesso das gestantes a esses locais, pois ficam muito distantes das outras.Foi um trabalho que gerou um impacto positivo para todos que fazem parte da UBS e para aqueles que utilizam do serviço, pois otimizamos o serviço de pré-natal na comunidade, além de conscientizarmos todos da comunidade, sobre a importância da realização. Entretanto obtivemos algumas dificuldades em relação a abrangência da área, portanto faz-se necessário realizar um novo mapeamento geográfico da área, além de estruturar as unidades básicas de saúde para que possa ser realizado um atendimento adequado a todos os pacientes.

Resumo Pérez Amaro Liliana. Qualificação da atenção ao pré-natal e puerpério na ESF UNIÃO, Santo Ângelo, RS. 2015. Trabalho de Conclusão de Curso (Especialização em Saúde da Família) - Departamento de Medicina Social, Faculdade de Medicina, Universidade Federal de Pelotas, Pelotas, 2015. A atenção ao pré-natal e ao puerpério é de muita importância para qualquer contexto da atenção primaria à saúde, pois a mesma tem grande relevância na saúde. A ESF é a porta de entrada preferencial das gestantes, sendo o ponto de atenção estratégia para melhor acolher suas necessidades que tem como objetivo assegurar o desenvolvimento da gestação, permitindo o parto de um recém-nascido saudável, sem impacto a saúde materna, inclusive abordando aspectos psicossociais e atividades educativas e preventivas. Este trabalho de intervenção que visava a qualificação do pré-natal e do puerpério das usuárias residentes na área de abrangência da ESF União em Santo Ângelo – RS. Como objetivos a ampliação da cobertura e melhora da qualidade da assistência à estas usuárias, assim como melhorar o registro de informações, mapeamento das gestantes de risco e promover saúde delas. Para alcançar os objetivos foram realizadas ações nos quatro eixos pedagógicos: monitoramento e avaliação, organização e gestão do serviço, engajamento público e qualificação da prática clínica. Desta forma, após a intervenção 91% das gestantes estimadas estavam cadastradas no programa e 100% das puérperas com consulta até o 42º após o parto. As ações propostas e desenvolvidas nesta intervenção se constituem de ações simples, mas de grande relevância para a qualificação da assistência pré-natal e contribuíram para a melhoria dos atendimentos dispensados as mulheres gestantes e puérperas.

Quantification of dermal exposure to pesticides in rural workers, used in risk assessment, can be performed with different techniques such as patches or whole body evaluation. However, the wide variety of methods can jeopardize the process by producing disparate results, depending on the principles in sample collection. A critical review was thus performed on the main techniques for quantifying dermal exposure, calling attention to this issue and the need to establish a single methodology for quantification of dermal exposure in rural workers. Such harmonization of different techniques should help achieve safer and healthier working conditions. Techniques that can provide reliable exposure data are an essential first step towards avoiding harm to workers' health.

We assessed associations between steroid receptors including: estrogen-alpha, estrogen-beta, androgen receptor, progesterone receptor, the HER2 status and triple-negative epithelial ovarian cancer (ERα-/PR-/HER2-; TNEOC) status and survival in women with epithelial ovarian cancer. The study included 152 women with primary epithelial ovarian cancer. The status of steroid receptor and HER2 was determined by immunohistochemistry. Disease-free and overall survival were calculated and compared with steroid receptor and HER2 status as well as clinicopathological features using the Cox Proportional Hazards model. A mean follow-up period of 43.6 months (interquartile range=41.4 months) was achieved where 44% of patients had serous tumor, followed by mucinous (23%), endometrioid (9%), mixed (9%), undifferentiated (8.5%) and clear cell tumors (5.3%). ER-alpha staining was associated with grade II-III tumors. Progesterone receptor staining was positively associated with a Body Mass Index≥25. Androgen receptor positivity was higher in serous tumors. In stand-alone analysis of receptor contribution to survival, estrogen-alpha positivity was associated with greater disease-free survival. However, there was no significant association between steroid receptor expression, HER2 status, or TNEOC status, and overall survival. Although estrogen-alpha, androgen receptor, progesterone receptor and the HER2 status were associated with key clinical features of the women and pathological characteristics of the tumors, these associations were not implicated in survival. Interestingly, women with TNEOC seem to fare the same way as their counterparts with non-TNEOC.

The effectiveness of low-level laser therapy in muscle regeneration is still not well known. To investigate the effects of laser irradiation during muscle healing. For this purpose, 63 rats were distributed to 3 groups: non-irradiated control group (CG); group irradiated at 10 J/cm(2) (G10); and group irradiated at 50 J/cm(2) (G50). Each group was divided into 3 different subgroups (n=7), and on days 7, 14 and 21 post-injury the rats were sacrificed. Seven days post-surgery, the CG showed destroyed zones and extensive myofibrillar degeneration. For both treated groups, the necrosis area was smaller compared to the CG. On day 14 post-injury, treated groups demonstrated better tissue organization, with newly formed muscle fibers compared to the CG. On the 21(st) day, the irradiated groups showed similar patterns of tissue repair, with improved muscle structure at the site of the injury, resembling uninjured muscle tissue organization. Regarding collagen deposition, the G10 showed an increase in collagen synthesis. In the last period evaluated, both treated groups showed statistically higher values in comparison with the CG. Furthermore, laser irradiation at 10 J/cm(2) produced a down-regulation of cyclooxygenase 2 (Cox-2) immunoexpression on day 7 post-injury. Moreover, Cox-2 immunoexpression was decreased in both treated groups on day 14. Laser therapy at both fluencies stimulated muscle repair through the formation of new muscle fiber, increase in collagen synthesis, and down-regulation of Cox-2 expression.

The morphological criteria for identification of intercalated duct lesions (IDLs) of salivary glands have been defined recently. It has been hypothesised that IDL could be a precursor of basal cell adenoma (BCA). BCAs show a variety of histological patterns, and the tubular variant is the one that presents the strongest resemblance with IDLs. The aim of this study was to analyse the morphological and immunohistochemical profiles of IDLs and BCAs classified into tubular and non-tubular subtypes, to determine whether or not IDL and tubular BCA represent distinct entities. Eight IDLs, nine tubular BCAs and 19 non-tubular BCAs were studied. All tubular BCAs contained IDL-like areas, which represented 20-70% of the tumour. In non-tubular BCA, IDL-like areas were occasional and small (<5%). One patient presented IDLs, tubular BCAs and IDL/tubular BCA combined lesions. Luminal ductal cells of IDLs and tubular BCAs exhibited positivity for CK7, lysozyme, S100 and DOG1. In the non-tubular BCA group, few luminal cells exhibited such an immunoprofile; they were mainly CK14-positive. Basal/myoepithelial cells of IDLs, tubular BCAs and non-tubular BCAs were positive for CK14, calponin, α-SMA and p63; they were more numerous in BCA lesions. IDL, tubular BCA and non-tubular BCA form a continuum of lesions in which IDLs are related closely to tubular BCA. In both, the immunoprofile of luminal and myoepithelial cells recapitulates the normal intercalated duct. The difference between the adenoma-like subset of IDLs and tubular BCA rests mainly on the larger numbers of myoepithelial cells in the latter. Our findings indicate that at least some BCAs can arise via IDLs.

To evaluate the sparing of fertility and ovaries in women submitted to surgical treatment for benign adnexal tumors. Between February 2010 and January 2014, 206 patients were included in this observational study as they were submitted to surgical treatment for benign ovarian tumors at CAISM, a tertiary hospital. Fertility sparing surgery was defined as tumorectomy or unilateral salpingoophorectomy without hysterectomy in premenopausal women. Preservation of the ovary occurred when at least one ovary or part of it was mantained. Of the 206 women with benign tumors, 120 (58%) were premenopausal and 86 (42%) were postmenopausal. There were 36 (30%) ovarian germ cell tumors, 31 (26%) epithelial neoplasms and 11 (9%) sex-cord stromal tumors among premenopausal women. In the group of postmenopausal women, 35 (41%) epithelial neoplasms, 27 (31%) sex-cord stromal tumors and 8 (9%) ovarian germ cell tumors were identified. Among 36 women with non-neoplastic ovarian tumors, 21 (58%) had endometriomas and 8 (22%) functional cysts. Among 22 women with extra-ovarian tumors, uterine leiomyomatosis was the most frequent finding (50%). In the group of women who were ≤ 35 years old, 26 (57%) were treated by tumorectomy and 18 (39%) were submitted to unilateral salpingoophorectomy with sparing of the uterus and the contralateral ovary. Women who were ≤ 35 years old were more frequently operated by laparoscopy which was associated with a higher number of fertility sparing procedures when compared to laparotomy (p<0.01). Twenty-six (28%) women submitted to hysterectomy with bilateral salpingoophorectomy were premenopausal. Although there is a trend to perform only tumorectomy in women who are ≤ 35 years old, a significant number of young women is still treated by salpingoophorectomy. Among 36- to 45-year-old women, only 70% had their fertility spared, while 20% had both ovaries removed. However, whenever possible, we must try to preserve the ovaries, mainly in premenopausal women.

To evaluate p16(INK) (4a) immunoexpression in CIN1 lesions looking for differences between cases that progress to CIN2/3 maintain CIN1 diagnosis, or spontaneously regress. Seventy-four CIN1 biopsies were studied. In the follow-up, a second biopsy was performed and 28.7% showed no lesion (regression), 37.9% maintained CIN1, and 33.4% progressed to CIN2/3. Immunostaining for p16(INK) (4a) was performed in the first biopsy and it was considered positive when there was strong and diffuse staining of the basal and parabasal layers. Pearson's chi-square was used to compare the groups (p ≤ 0.05). The age of the patients was similar. There was no significant difference in p16(INK) (4a) immunoexpression in the groups, however, statistical analyses showed a significant association when only the progression and regression groups were compared (p = 0.042). Considering p16(INK) (4a) positivity and the progression to CIN2/3, the sensitivity, specificity, positive, and negative predictive values in our cohort were 45%, 75%, 47%, and 94%, respectively. We emphasize that CIN1 with p16(INK) (4a) staining was associated with lesion progression, but the sensitivity was not high. However, the negative predictive value was more reliable (94%) and p16(INK) (4a) may represent a useful biomarker that can identify CIN1 lesions that need particular attention, complementing morphology.

The authors present considerations about death and brain death concepts, as well the legal aspects for its diagnosis in Brazil. They also present the UNICAMP Protocol for the Diagnosis of Brain Death, revised and according with the current law, with standard techniques for the diagnostic exam. They emphasize the importance of a mature ethical position for this frequent and challenging situation.

Quimica Nova and the Journal of the Brazilian Chemical Society are two examples of successful initiatives taken by the Brazilian Chemical Society (SBQ - Sociedade Brasileira de Química), and may serve as models for the scientific societies of developing countries. Pillars of the SBQ, these two periodicals are undeniable demonstrations that idealism, utopia and dignity are the essential ingredients for transforming dreams into reality. Few believed that the Brazilian chemical community would one day have, as it does today, two scientific research periodicals indexed in the principal international data banks.
