Las Clamidias son bacterias patógenas de los animales de producción, de vida silvestre y de compañía. Además de las pérdidas económicas que producen las infecciones en los planteles de producción bovina, ovina, caprina, porcina y aves de corral, la mayoría de las especies tienen importancia zoonótica, pudiendo dar origen a infecciones graves, potencialmente letales en el ser humano. El orden Chlamydiales está integrado por bacterias que actúan como parásitos intracelulares obligados que desarrollan su ciclo de vida únicamente dentro de inclusiones citoplasmáticas. En este orden se encuentra la familia Chlamydiaceae que comprende dos géneros, Chlamydia y Chlamydophila; y las especies, Chlamydia trachomatis, C. suis, C. muridarum, Chlamydophila psittaci, C. abortus, C. felis, C. caviae, C. pecorum, y C. pneumoniae. C. psittaci causa psitacosis o clamidiosis aviar. En Argentina, los primeros casos clínicos de psitacosis fueron reportados en 1929. Los criadores de aves y quienes las poseen como mascotas, representan el grupo de mayor riesgo; pero también las personas que trabajan en pajarerías y aquellas que por su empleo se ven expuestas a contraer la enfermedad (empleados en peladeros donde se carnean y procesan pollos y otras aves para consumo, veterinarios, empleados de zoológicos, etc.). La infección en humanos se presenta como una neumonía severa; con fiebre alta, escalofríos, dolor de cabeza, mialgia y dificultad respiratoria. Ocasionalmente puede presentarse vómitos, dolor abdominal, diarrea y complicaciones como miocarditis, endocarditis, encefalitis, ictericia y fallas multiorgánicas, que pueden ser fatales sino se le administra el tratamiento adecuado. La infección en las mujeres embarazadas puede producir neumonía, hepatitis, insuficiencia renal, sepsis, parto prematuro y muerte fetal. Existen más de 465 especies de aves en las que se registró C. psittaci, incluyendo ornamentales, de corral, silvestres, acuáticas y palomas. Las patologías que pueden producir en estos animales son neumonitis, conjuntivitis, encefalomielitis, placentopatías, fetopatías, anorexia, diarrea e infecciones persistentes asintomáticas u oligosintomáticas. En bovinos, C. pecorum, C. abortus y C. psittaci producen infecciones respiratorias y genitales; que se presentan como cuadros de enteritis, artritis, encefalomielitis, endometritis e hipofertilidad. En Argentina, la infección clamidial en el ganado caprino fue asociada a daños en el tejido uterino, abortos, partos prematuros y crías débiles. En equinos, C. psittaci y C. pneumoniae producen abortos y desórdenes respiratorios, con un gran impacto en ganadería que redunda en pérdidas económicas. Considerando que existen escasos estudios eco-epidemiológicos y clínicos que reporten el estado de situación de estas infecciones en nuestro medio, es que el presente trabajo propone actualizar y profundizar el conocimiento de las especies de Clamidias de importancia médico-veterinaria presentes en la provincia de Córdoba, Argentina. El desarrollo de este proyecto aportará la implementación de técnicas que mejorarán el diagnóstico microbiológico, confirmarán los cuadros clínicos; y por lo tanto contribuirá al conocimiento de estos agentes infecciosos en nuestra región. Esta información es indispensable para los organismos responsables de la Salud Pública (Ministerios de Salud y Educación, Municipios, etc.) para que puedan obrar en consecuencia y generar sistemas de alerta temprana, tomar medidas de prevención y medidas de control frente a la presencia de un brote epidémico por alguna cepa clamidial. "Eco epidemiology of Chlamydophila psittaci, C. pecorum and C. pneumoniae: Impact on public health "The Chlamydiae are bacterial pathogens of farm animals, wildlife and pets. Besides the economic losses that occur on campuses infections of cattle, sheep, goats, swine and poultry, most species are zoonotic, infections potentially fatal can cause in humans. The order Chlamydiales is composed of the family Chlamydiaceae comprising two genera, Chlamydia and Chlamydophila, and nine species, Chlamydia trachomatis, C. suis, C. muridarum, Chlamydophila psittaci, C. abortus, C. felis, C. caviae, C. pecorum, and C. pneumoniae. The clinical manifestations in humans are associated with severe pneumonia. Also, nonspecific gastrointestinal symptoms, and complications such as myocarditis, endocarditis, encephalitis, jaundice and multiple organ failure. There are over 465 species of birds where recorded C. psittaci, including ornamental plants, poultry, wild waterfowl and pigeons. Sick birds eliminate chlamydia in all secretions, asymptomatic birds can develop the disease through contact with other infected birds. The human infection occurs by inhalation of aerosolized secretions of infected birds. The prevalence of C. psittaci records in humans, birds, horses, goats and cattle are scarce in Latin America. In Argentina it has been detected chlamydial antibody prevalence in cattle in Buenos Aires and La Pampa (5) and Rockhopper Penguins (Eudyptes chrysocome) in the town of Santa Cruz (11). Detect this pathogen in these birds and learn about potential sources of infection would be of great public health significance. To develop an efficient system of medical and veterinary surveillance is essential to have reliable diagnostic techniques for detection and identification of Chlamydia in birds, animals and humans.

Somatic post-surgical pain is invalidating and distressing to patients and carries the risk of important complications. The anterior abdominal wall is involved in most surgical procedures in general, gynecologic, obstetric, urological, vascular and pediatric surgery. Combined multimodal strategies involving nerve blocks, opiates, and non-steroidal anti-inflammatory drugs for systemic analgesia are necessary for optimal pain modulation. Anterior abdominal wall blocks, transverse abdominal plexus block, iliohypogastric and ilioinguinal nerveblock, genitofemoral nerve block and rectus sheath block have an important role as components of multimodal analgesia for somatic intraoperative and postoperative pain control. Ultrasound visualization has improved the efficacy and safety of abdominal blocks and implemented the application in the clinical setting. For this reason, they are a very important tool for all anesthesiologists who aim to treat effectively patients’ pain. This guide provides an evidence based comprehensive and necessary overview of anatomical, anesthesiological and technical information needed to safely perform these blocks.

Background: Pitavastatin is the newest statin available in Brazil and likely the one with fewer side effects. Thus, pitavastatin was evaluated in hypercholesterolemic rabbits in relation to its action on vascular reactivity. Objective: To assess the lowest dose of pitavastatin necessary to reduce plasma lipids, cholesterol and tissue lipid peroxidation, as well as endothelial function in hypercholesterolemic rabbits. Methods: Thirty rabbits divided into six groups (n = 5): G1 - standard chow diet; G2 - hypercholesterolemic diet for 30 days; G3 - hypercholesterolemic diet and after the 16th day, diet supplemented with pitavastatin (0.1 mg); G4 - hypercholesterolemic diet supplemented with pitavastatin (0.25 mg); G5 - hypercholesterolemic diet supplemented with pitavastatin (0.5 mg); G6 - hypercholesterolemic diet supplemented with pitavastatin (1.0 mg). After 30 days, total cholesterol, HDL, triglycerides, glucose, creatine kinase (CK), aspartate aminotransferase (AST), alanine aminotransferase (ALT) were measured and LDL was calculated. In-depth anesthesia was performed with sodium thiopental and aortic segments were removed to study endothelial function, cholesterol and tissue lipid peroxidation. The significance level for statistical tests was 5%. Results: Total cholesterol and LDL were significantly elevated in relation to G1. HDL was significantly reduced in G4, G5 and G6 when compared to G2. Triglycerides, CK, AST, ALT, cholesterol and tissue lipid peroxidation showed no statistical difference between G2 and G3-G6. Significantly endothelial dysfunction reversion was observed in G5 and G6 when compared to G2. Conclusion: Pitavastatin starting at a 0.5 mg dose was effective in reverting endothelial dysfunction in hypercholesterolemic rabbits.

Background: Pazopanib (PZP) may induce prolonged cardiac repolarization and proarrhythmic effects, similarly to other tyrosine kinase inhibitors. Objectives: To demonstrate PZP-induced prolonged cardiac repolarization and proarrhythmic electrophysiological effects and to investigate possible preventive effects of metoprolol and diltiazem on ECG changes (prolonged QT) in an experimental rat model. Methods: Twenty-four Sprague-Dawley adult male rats were randomly assigned to 4 groups (n = 6). The first group (normal group) received 4 mL of tap water and the other groups received 100 mg/kg of PZP (Votrient® tablet) perorally, via orogastric tubes. After 3 hours, the following solutions were intraperitoneally administered to the animals: physiological saline solution (SP), to the normal group and to the second group (control-PZP+SP group); 1 mg/kg metoprolol (Beloc, Ampule, AstraZeneca), to the third group (PZP+metoprolol group); and 1mg/kg diltiazem (Diltiazem, Mustafa Nevzat), to the fourth group (PZP+diltiazem group). One hour after, and under anesthesia, QTc was calculated by recording ECG on lead I. Results: The mean QTc interval values were as follows: normal group, 99.93 ± 3.62 ms; control-PZP+SP group, 131.23 ± 12.21 ms; PZP+metoprolol group, 89.36 ± 3.61 ms; and PZP+diltiazem group, 88.86 ± 4.04 ms. Both PZP+metoprolol and PZP+diltiazem groups had significantly shorter QTc intervals compared to the control-PZP+SP group (p < 0.001). Conclusion: Both metoprolol and diltiazem prevented PZP-induced QT interval prolongation. These drugs may provide a promising prophylactic strategy for the prolonged QTc interval associated with tyrosine kinase inhibitor use.

In this paper the A. A. report the observations about the general anesthesia by chloral hydrate on the veterinary surgery. The observations were made on emasculation practices of horses, mules and hogs. It was possible to establish the following conclusions: 1) The choral hydrate presents low cost, it harmless, and is of easy application. 2) The more recommendable dosis for equine and swine were : 12-13 g per 100 k of body weight, in destilled water solution at 30 and 20%, respectively. 3) The anaethestic was injected by intravenous way with good results; in horses and mules the applications were made in the jugular; in swine, in the anterior vena cava, as was described by Carle and Dewhirst, because it was impracticable in the ear vein. 4) The dosis applied produced deep narcosis not lasting to long and with no danger to the animal's life. 5) In the case of fattening hogs, it must be made a discount of about 40% on the body weight, to calculate dosis to be employed. 6) The tables A and B show the results, that may be considered as good.

1. A secção dos vagos na cobaya produza morte em prazos que variam de meia hora a algumas horas. 2. A morte se dá por intensas lesões pulmonares, caracterisadas por congestão e edema. 3. As cobayas introduzidas em camaras em que a atmosphera tem altas tensões de gaz carbonico, caem anesthesiadas ao fim de alguns segundos, seguindo-se logo depois a morte. Na autopsia, se encontram lesões pulmonares muito semelhantes ás que se verificam nas cobayas vagotomisadas. 4. Nas cobayas profundamente anesthesiadas pelo chloral, a sobrevida á vagotomia é muito maior que nas cobayas não anesthesiadas. Nesses casos a morte muitas vezes se dá quando o animal vem a despertar da anesthesia. 5. Foram feitas novas experiencias que demonstram produzir a novocaina uma interrupção completa de todas as fórmas de conductibilidade dos nervos. 6. Quando se produz a secção physiologica pela novocaina dos vagos na cobaya, observam-se modificações do typo respiratorio que se caracterisam por diminuição consideravel da frequencia e augmento da amplitude. A respiração não apresenta porém, o typo francamente dyspneico, istoé, ella não se faz com difficuldade. 7. A secção physiologia dos pneumogastricos permitte uma sobrevida muito maior que a secção cirurgica. 8. A morte nas condições da conclusão precedente se dá pelas mesmas lesões pulmonares que se encontram habitualmente depois de secção cirurgica, mas é produzida unicamente pela irritação dos pneumogastricos exercida pelas gotteiras de borracha e pelo algodão, depois que a novocaina é absorvida e que se dissipa a anesthesia local. 9. Quando se operam as secções dos vagos com irritações variaveis as sobrevidas são differentes. 10. A sangria immediata não tem effeito sobre o tempo de vida da cobaya vagotomisada. 11. A sangria tardia parece augmentar ligeiramente essa sobrevida. Todos os factos expostos neste trabalho demonstram: 1) De um lado a irritação sem secção dos vagos na cobaya produz congestões e edemas pulmonares que levam o animal á morte; 2) a secção sem irritação não dá logar a esses phenomenos. Dahi a conclusão principal destas pesquizas: se porventura existem excitações normalmente transmittidas pelos vagos aos centros nervosos respiratorios, não é a falta dessas excitações a causa primeira dos phenomenos produzidos pela vagotomia bilateral. Esses phenomenos são devidos a irritações do pneumogastrico. A dyspnéa encontrada nos animaes vagotomisados parece ser tambem um effeito directo ou indirecto dessas irritações. Conquanto pouco inclinados a admittir que seja essa dyspnéa a causa directa das lesões pulmonares, reconhecemos que essa questão não pode ser ainda inteiramente resolvida. As irritações dos vagos podem ter influencia sobre muitas e variadas funcções, cujas perturbações assim produzidas, têm um papel preponderante no mechanismo da morte consecutiva á dupla vagotomia. Tendo verificado uma acção hyperglobulinogena do tetrachloreto de carbono, thynol e essencia de chenopodio, quando administrados ao homem como vermifugos, procuramos averiguar qual o mechanismo destas hyperglobulias. Para isso tomamos duas substancias tambem hyperglobulinogenas (chloroformio e chloretona) e escolhemos o cão para animal de experiencia. Observamos que, com anesthesias prolongadas pelo chloroformio e pela chloretona, tal como naquellas substancias applicadas ao homem como vermicidas, apparecem intensas hyperglobulias no sangue circulante. constatamos ainda uma rapidez extraordinaria no augmento de hematias (1 a 2 horas após o inicio da anesthesia), e a existencia de um nivel maximo para este augmento, que não é ultrapassado apezar de novas anesthesias prolongadas. Entretanto, em cães esplenectomisados este effeito das anesthesias prolongadas desapparece inteiramente em sua acção sobre o sangue. em todos os casos, tanto em homem como em cães (esplenectomisados ou não), ao par da hyperglobulia, notou-se sempre estados hypotensivos as vezes bastantes accentuados. Estes resultados nos levaram a concluir que no mechanismo das hyperglobulias de origem toxica, o factor principal está em uma contracção esplenica, (provocada provavelmente pela hypotensão observada), contracção está, que resulta em um lançamento na corrente circulatoria do sangue concentrado (lama esplenica) que normalmente se acha em reserva no baço.

Studies have been carried out on the method of Britton and Silvette modified by Reinecke and Kendall, for the evaluation of cortico-adrenal extracts, based on the deposition of glycogen in the liver of adrenaletomized rats. The test was performed in a total of 180 normal and adrenalectomized rats. The extracts tested were: a) an aqueous extract of cortico-adrenal cortex prepared by the Swingle and Pfiffner technique; b) the same extract added with ascorbic acid (Supracortin Labor); c) desoxycorticosterone acetate (Percortol Ciba and Syncortyl Roussell). Male rats were used, ranging from 40-200g, fed since the 18 th days old with a special diet, in which they were maintained until the day before the injection. Adrenalectomy was performed under urethane anesthesia. The fourth day after operation, food was removed and they were fasted for 24 hours. In the morning of the fifth day, injections of the material to be assayed were given at hourly and two hours intervals, during four to eight hours. One or two hours after the last injection, the animals were sacrified, the livers removed and dropped into a hot 30% solution of potassium hydroxide, and worked by Good, Kramer and Somogyi method. The glycogen was calculated as milligrams per lOOg of body and liver weight. The results obtained are shown in the tables I, II, and III. When several dosages of the same sample of extract were made (5 animals each dose), the amount of glycogen deposited in the liver per lOOg of body and liver weight, was found to be a positive function of the dose injected. The graph 2, shows these results. The synthetic compounds were ineffective. Our results are in agreement with those of Reinecke and Kendall and of Olson et al.

Estudi elaborat a partir d’una estada al Royal Brompton Hospital, Londres, Regne Unit, durant octubre i novembre del 2006.Els beneficis de la estimulació beta-adrenèrgica en pacients amb lesió pulmonar aguda (LPA) són coneguts, però no es disposa de dades sobre el possible efecte antiinflamatori. El condensat d'aire exhalat (CAE) és una tècnica no-invasiva de recollida de mostres del tracte respiratori inferior, podent ser útil en la monitorització de patologies respiratòries. S’ha usat marcadors biològics en el CAE de pacients ventilats mecànicament amb LPA per estudiar el possible efecte antiinflamatori que el salbutamol hi podria exercir. El CAE va ser recollit abans i després de l'administració de salbutamol inahalat. Inmediatament després es va mesurar la conductivitat i el pH abans i després de la desgasificació amb heli. Es va mesurar la concentració de nitrits i nitrats. Les mostres varen ser liofilitzades i guardades a -80ºC. La concentració de leucotriè B4 es va mesurar després de la reconstitució de la mostra. Els resultats s'expressen com a mitjana (error estàndard de la mostra). No s'han detectat diferències entre els valors de CAE basals dels pacients amb LPA i els de referència de la població sana de Barcelona. Es conclou doncs que el CAE és una tècnica no invasiva que pot ser usada en la monitorització de paceints ventilats mecànicament. El salbutamol inhalat incrementa de manera significativa el pH del CAE dels paceints amb LPA, tot i que un efecte directe de la inhalació de slabutamol no pot ser desestimat.

An assessment of sewage workers' exposure to airborne cultivable bacteria, fungi and inhaled endotoxins was performed at 11 sewage treatment plants. We sampled the enclosed and unenclosed treatment areas in each plant and evaluated the influence of seasons (summer and winter) on bioaerosol levels. We also measured personal exposure to endotoxins of workers during special operation where a higher risk of bioaerosol inhalation was assumed. Results show that only fungi are present in significantly higher concentrations in summer than in winter (2331 +/- 858 versus 329 +/- 95 CFU m(-3)). We also found that there are significantly more bacteria in the enclosed area, near the particle grids for incoming water, than in the unenclosed area near the aeration basins (9455 +/- 2661 versus 2435 +/- 985 CFU m(-3) in summer and 11 081 +/- 2299 versus 2002 +/- 839 CFU m(-3) in winter). All bioaerosols were frequently above the recommended values of occupational exposure. Workers carrying out special tasks such as cleaning tanks were exposed to very high levels of endotoxins (up to 500 EU m(-3)) compared to routine work. The species composition and concentration of airborne Gram-negative bacteria were also studied. A broad spectrum of different species within the Pseudomonadaceae and the Enterobacteriaceae families were predominant in nearly all plants investigated. [Authors]

BACKGROUND: Hypotension, a common intra-operative incident, bears an important potential for morbidity. It is most often manageable and sometimes preventable, which renders its study important. Therefore, we aimed at examining hospital variations in the occurrence of intra-operative hypotension and its predictors. As secondary endpoints, we determined to what extent hypotension relates to the risk of post-operative incidents and death. METHODS: We used the Anaesthesia Databank Switzerland, built on routinely and prospectively collected data on all anaesthesias in 21 hospitals. The three outcomes were assessed using multi-level logistic regression models. RESULTS: Among 147,573 anaesthesias, hypotension ranged from 0.6% to 5.2% in participating hospitals, and from 0.3% up to 12% in different surgical specialties. Most (73.4%) were minor single events. Age, ASA status, combined general and regional anaesthesia techniques, duration of surgery and hospitalization were significantly associated with hypotension. Although significantly associated, the emergency status of the surgery had a weaker effect. Hospitals' odds ratios for hypotension varied between 0.12 and 2.50 (P < or = 0.001), even after adjusting for patient and anaesthesia factors, and for type of surgery. At least one post-operative incident occurred in 9.7% of the procedures, including 0.03% deaths. Intra-operative hypotension was associated with a higher risk of post-operative incidents and death. CONCLUSION: Wide variations remain in the occurrence of hypotension among hospitals after adjustment for risk factors. Although differential reporting from hospitals may exist, variations in anaesthesia techniques and blood pressure maintenance may also have contributed. Intra-operative hypotension is associated with morbidities and sometimes death, and constant vigilance must thus be advocated.

Drug abuse is a widespread problem affecting both teenagers and adults. Nitrous oxide is becoming increasingly popular as an inhalation drug, causing harmful neurological and hematological effects. Some gas chromatography-mass spectrometry (GC-MS) methods for nitrous oxide measurement have been previously described. The main drawbacks of these methods include a lack of sensitivity for forensic applications; including an inability to quantitatively determine the concentration of gas present. The following study provides a validated method using HS-GC-MS which incorporates hydrogen sulfide as a suitable internal standard allowing the quantification of nitrous oxide. Upon analysis, sample and internal standard have similar retention times and are eluted quickly from the molecular sieve 5Å PLOT capillary column and the Porabond Q column therefore providing rapid data collection whilst preserving well defined peaks. After validation, the method has been applied to a real case of N2O intoxication indicating concentrations in a mono-intoxication.

Cerebral metabolism is compartmentalized between neurons and glia. Although glial glycolysis is thought to largely sustain the energetic requirements of neurotransmission while oxidative metabolism takes place mainly in neurons, this hypothesis is matter of debate. The compartmentalization of cerebral metabolic fluxes can be determined by (13)C nuclear magnetic resonance (NMR) spectroscopy upon infusion of (13)C-enriched compounds, especially glucose. Rats under light α-chloralose anesthesia were infused with [1,6-(13)C]glucose and (13)C enrichment in the brain metabolites was measured by (13)C NMR spectroscopy with high sensitivity and spectral resolution at 14.1 T. This allowed determining (13)C enrichment curves of amino acid carbons with high reproducibility and to reliably estimate cerebral metabolic fluxes (mean error of 8%). We further found that TCA cycle intermediates are not required for flux determination in mathematical models of brain metabolism. Neuronal tricarboxylic acid cycle rate (V(TCA)) and neurotransmission rate (V(NT)) were 0.45 ± 0.01 and 0.11 ± 0.01 μmol/g/min, respectively. Glial V(TCA) was found to be 38 ± 3% of total cerebral oxidative metabolism, accounting for more than half of neuronal oxidative metabolism. Furthermore, glial anaplerotic pyruvate carboxylation rate (V(PC)) was 0.069 ± 0.004 μmol/g/min, i.e., 25 ± 1% of the glial TCA cycle rate. These results support a role of glial cells as active partners of neurons during synaptic transmission beyond glycolytic metabolism.

Difficult tracheal intubation assessment is an important research topic in anesthesia as failed intubations are important causes of mortality in anesthetic practice. The modified Mallampati score is widely used, alone or in conjunction with other criteria, to predict the difficulty of intubation. This work presents an automatic method to assess the modified Mallampati score from an image of a patient with the mouth wide open. For this purpose we propose an active appearance models (AAM) based method and use linear support vector machines (SVM) to select a subset of relevant features obtained using the AAM. This feature selection step proves to be essential as it improves drastically the performance of classification, which is obtained using SVM with RBF kernel and majority voting. We test our method on images of 100 patients undergoing elective surgery and achieve 97.9% accuracy in the leave-one-out crossvalidation test and provide a key element to an automatic difficult intubation assessment system.
