Objective: Local shockwave-application (SW) has shown to improve healing of various tissues and decrease necrosis of flaps. Though, there is no data about the optimal time-point of SW-application with regard to induction of ischemia (i.e. flap elevation) and subsequent effect on flap survival. Therefore we compared 2 shock-wave protocols in a model of persistent ischemia and investigated underlying mechanisms. Methods: 18 C57BL/6-mice equipped with a skinfold chamber containing a musculocutaneous flap were assigned to 3 experimental groups: 1. One session of 500 SWimpulses at 0·15 mJ/mm2 applied 24 hrs before (preconditioning) or 2. Applied 30 min after flap elevation (treatment). 3. Untreated flaps (control). Tissue necrosis,microhemodynamics, inflammation, apoptosis and angiogenesis were assessed by intravital epi-fluorescence microscopy over 10 days. Results: SW significantly reduced flap necrosis independent from the application time-point (preconditioning: 29 ± 7%; treatment: 25 ± 7% vs. control: 47 ± 2%; d10, p<0·05). This was associated with an early increase of functional capillary density (preconditioning: 236 ± 39 cm/cm2; treatment: 211 ± 33 cm/cm2 vs. control: 141 ± 7 cm/cm2; day1, p<0·05). Arteriolar diameter, red blood cell velocity and blood flow were comparable between the 3 experimental groups. SW-application significantly decreased the ischemiainduced inflammatory response (apoptotic cell death and leukocyte-endothelial interaction: (p<0·05)). Sprouts indicating angiogenesis were observed from day 7 only after SW-application. Conclusions: SW protects ischemically challenged musculocutaneous tissue. Interestingly, postoperative SW-application is as efficient as preoperative SWapplication. The protective effect induced by mechanical stress might be based on an early recruitment of ''sleeping capillaries'' maintaining nutritive perfusion and an anti-inflammatory effect within the ischemically jeopardized tissue. SWapplication provides a non-invasive alternative to local thermic and systemic pre-treatment of endangered tissues.

BACKGROUND: Developing and updating high-quality guidelines requires substantial time and resources. To reduce duplication of effort and enhance efficiency, we developed a process for guideline adaptation and assessed initial perceptions of its feasibility and usefulness. METHODS: Based on preliminary developments and empirical studies, a series of meetings with guideline experts were organised to define a process for guideline adaptation (ADAPTE) and to develop a manual and a toolkit made available on a website (http://www.adapte.org). Potential users, guideline developers and implementers, were invited to register and to complete a questionnaire evaluating their perception about the proposed process. RESULTS: The ADAPTE process consists of three phases (set-up, adaptation, finalisation), 9 modules and 24 steps. The adaptation phase involves identifying specific clinical questions, searching for, retrieving and assessing available guidelines, and preparing the draft adapted guideline. Among 330 registered individuals (46 countries), 144 completed the questionnaire. A majority found the ADAPTE process clear (78%), comprehensive (69%) and feasible (60%), and the manual useful (79%). However, 21% found the ADAPTE process complex. 44% feared that they will not find appropriate and high-quality source guidelines. DISCUSSION: A comprehensive framework for guideline adaptation has been developed to meet the challenges of timely guideline development and implementation. The ADAPTE process generated important interest among guideline developers and implementers. The majority perceived the ADAPTE process to be feasible, useful and leading to improved methodological rigour and guideline quality. However, some de novo development might be needed if no high quality guideline exists for a given topic.

Forensic intelligence is a distinct dimension of forensic science. Forensic intelligence processes have mostly been developed to address either a specific type of trace or a specific problem. Even though these empirical developments have led to successes, they are trace-specific in nature and contribute to the generation of silos which hamper the establishment of a more general and transversal model. Forensic intelligence has shown some important perspectives but more general developments are required to address persistent challenges. This will ensure the progress of the discipline as well as its widespread implementation in the future. This paper demonstrates that the description of forensic intelligence processes, their architectures, and the methods for building them can, at a certain level, be abstracted from the type of traces considered. A comparative analysis is made between two forensic intelligence approaches developed independently in Australia and in Europe regarding the monitoring of apparently very different kind of problems: illicit drugs and false identity documents. An inductive effort is pursued to identify similarities and to outline a general model. Besides breaking barriers between apparently separate fields of study in forensic science and intelligence, this transversal model would assist in defining forensic intelligence, its role and place in policing, and in identifying its contributions and limitations. The model will facilitate the paradigm shift from the current case-by-case reactive attitude towards a proactive approach by serving as a guideline for the use of forensic case data in an intelligence-led perspective. A follow-up article will specifically address issues related to comparison processes, decision points and organisational issues regarding forensic intelligence (part II).

Tools to predict fracture risk are useful for selecting patients for pharmacological therapy in order to reduce fracture risk and redirect limited healthcare resources to those who are most likely to benefit. FRAX® is a World Health Organization fracture risk assessment algorithm for estimating the 10-year probability of hip fracture and major osteoporotic fracture. Effective application of FRAX® in clinical practice requires a thorough understanding of its limitations as well as its utility. For some patients, FRAX® may underestimate or overestimate fracture risk. In order to address some of the common issues encountered with the use of FRAX® for individual patients, the International Society for Clinical Densitometry (ISCD) and International Osteoporosis Foundation (IOF) assigned task forces to review the medical evidence and make recommendations for optimal use of FRAX® in clinical practice. Among the issues addressed were the use of bone mineral density (BMD) measurements at skeletal sites other than the femoral neck, the use of technologies other than dual-energy X-ray absorptiometry, the use of FRAX® without BMD input, the use of FRAX® to monitor treatment, and the addition of the rate of bone loss as a clinical risk factor for FRAX®. The evidence and recommendations were presented to a panel of experts at the Joint ISCD-IOF FRAX® Position Development Conference, resulting in the development of Joint ISCD-IOF Official Positions addressing FRAX®-related issues.

El present projecte pretén ser una guia per a la creació d'una petita empresa dedicada a la rehabilitació de vivendes, per una posterior venda. És en part un document de guia i suport i per altra banda un petit pla d'empresa que amb els canvis pertinents podria servir per a presentar-ho a un tercer. Com la situació actual és exageradament negativa, el marc temporal on es troba situat aquest projecte és a partir dels propers deu o quinze anys aproximadament.

Las cooperativas constituyen el eje básico de la Economía Social en el sector agrario, tanto por su importancia empresarial como por las características propias de su organización. El presente trabajo de investigación pretende mostrar la situación de la gestión en el sector de las cooperativas del aceite en Catalunya. Para ello se busca establecer una tipología concreta de las cooperativas en función de su actitud frente al sector y realizar un mapa de posicionamiento de los diferentes tipos de cooperativas en función de sus características más relevantes. En las conclusiones se citan aspectos en los que el gestor de una cooperativa debe incidir para mejorar la calidad de su gestión.

Se determinó la excreción urinaria de nitrógeno (NU, %) y de energía (EU, kcal/100g), en la última semana de vida, de 36 conejos alimentados, desde el destete (a los 28 d) hasta el sacrificio a los dos meses de edad, con tres piensos conteniendo 0, 3 y 6 % de grasa añadida (G0, G3 y G6); y administrados a dos niveles de ingestión: ad libitum (IL) y restringido (IR) al 70 % de IL. El NU y la EU estuvieron influidos positivamente tanto por el nivel de ingestión (P<0,001) como por el % de grasa ingerida (P<0,05). En estos resultados puede estar involucrado el metabolismo tan elevado de las lipoproteínas transportadoras de lípidos, implicadas en el metabolismo de las grasas, ya sean de origen alimentario o de la lipogénesis de novo. La relación entre la EU y el NU aporta valores muy superiores a los que podría corresponder a las materias orgánicas nitrogenadas, por lo que se podría pensar, como posible causa, en la intervención de materia orgánica no nitrogenada. Asimismo, la cuantificación de la EM del pienso se ve afectada por el incremento del valor calórico de la orina, con el nivel de ingestión y/o el 6 % de grasa añadida. Con ello se evidencia la desigualdad entre ED y EM de la grasa en las raciones que las incluya. Asimismo, el hecho de que la movilización grasa —bien sea de origen alimentario o de la lipogénesis— altere la excreción nitrogenada y/o energética a través de la orina, pone de manifiesto la importancia de su consideración, especialmente cuando dicha pérdida puede afectar a los rendimientos de algunas producciones animales.
