Assessment of surgical performance is a must for every surgical practice nowadays and can be done by using scientific methods imported mostly from the Quality control tools that have been in use for long in industry. Surgical performance comprises several dimensions including clinical activity (mortality and morbidity as end points), academic activities, research and, more and more, efficiency. Stable long time results (efficacy), reducing error (safety) and meeting patient expectations (patient satisfaction) are among other performance components. This paper focus on the precise definitions of mortality and morbidity related to surgical activities and on the tools to evaluate patient complexity and assess pre operative risk. Some graphic representations are suggested to compare performance profiles of surgeons and to define individual performance profiles. Strong emphasis is put on pre operative risk assessment and its crucial role to interpret divergent surgical results. Where risk assessment is not possible or is unavailable, observed / expected ratios (O/E) for a given endpoint , be it mortality, length of stay or morbidity, must be established and routinely used to refer results and to identify performance outliers. Morbidity is being pointed out as a most valuable performance indicator in surgery because it is sensitive and comprises efficiency, safety and quality, at large.

The autonomic nervous system (ANS) is known to be an important modulator in the pathogenesis of paroxysmal atrial fibrillation (PAF). Changes in ANS control of heart rate variability (HRV) occur during orthostatism to maintain cardiovascular homeostasis. Wavelet transform has emerged as a useful tool that provides time-frequency decomposition of the signal under investigation, enabling intermittent components of transient phenomena to be analyzed. AIM: To study HRV during head-up tilt (HUT) with wavelet transform analysis in PAF patients and healthy individuals (normals). METHODS: Twenty-one patients with PAF (8 men; age 58 +/- 14 yrs) were examined and compared with 21 normals (7 men, age 48 +/- 12 yrs). After a supine resting period, all subjects underwent passive HUT (60 degrees) while in sinus rhythm. Continuous monitoring of ECG and blood pressure was carried out (Task Force Monitor, CNSystems). Acute changes in RR-intervals were assessed by wavelet analysis and low-frequency power (LF: 0.04-0.15 Hz), high-frequency power (HF: 0.15-0.60 Hz) and LF/HF (sympathovagal) were calculated for 1) the last 2 min of the supine period; 2) the 15 sec of tilting movement (TM); and 3) the 1st (TT1) and 2nd (TT2) min of HUT. Data are expressed as means +/- SEM. RESULTS: Baseline and HUT RR-intervals were similar for the two groups. Supine basal blood pressure was also similar for the two groups, with a sustained increase in PAF patients, and a decrease followed by an increase and then recovery in normals. Basal LF, HF and LF/ HF values in PAF patients were 632 +/- 162 ms2, 534 +/- 231 ms2 and 1.95 +/- 0.39 respectively, and 1058 +/- 223 ms2, 789 +/- 244 ms2 and 2.4 +/- 0.36 respectively in normals (p = NS). During TM, LF, HF and LF/HF values for PAF patients were 747 +/- 277 ms2, 387 +/- 94 ms2 and 2.9 +/- 0.6 respectively, and 1316 +/- 315 ms2, 698 +/- 148 ms2 and 2.8 +/- 0.6 respectively in normals (p < 0.05 for LF and HF). During TF1, LF, HF and LF/ HF values for PAF patients were 1243 +/- 432 ms2, 302 +/- 88 ms2 and 7.7 +/- 2.4 respectively, and 1992 +/- 398 ms2, 333 +/- 76 ms2 and 7.8 +/- 0.98 respectively for normals (p < 0.05 for LF). During TF2, LF, HF and LF/HF values for PAF patients were 871 +/- 256 ms2, 242 +/- 51 ms2 and 4.7 +/- 0.9 respectively, and 1263 +/- 335 ms2, 317 +/- 108 ms2 and 8.6 +/- 0.68 respectively for normals (p < 0.05 for LF/HF). The dynamic profile of HRV showed that LF and HF values in PAF patients did not change significantly during TM or TT2, and LF/HF did not change during TM but increased in TT1 and TT2. CONCLUSION: Patients with PAF present alterations in HRV during orthostatism, with decreased LF and HF power during TM, without significant variations during the first minutes of HUT. These findings suggest that wavelet transform analysis may provide new insights when assessing autonomic heart regulation and highlight the presence of ANS disturbances in PAF.

SUMMARY The herpes simplex virus type 2 (HVS-2) is the most prevalent infection worldwide. It is a cofactor in the acquisition of human immunodeficiency virus (HIV) and the persistence of human papillomavirus (HPV). This study evaluated the prevalence of HSV-2, using the polymerase chain reaction (PCR), and associated factors in patients treated at the Federal University of Rio Grande (FURG) and Basic Health Units (BHU) in Rio Grande, Brazil. The observed prevalence of HSV-2 was 15.6%. Among the 302 women studied, 158 had received assistance in BHU and 144 were treated at FURG. The prevalence of HSV-2 in these groups was 10.8% and 20.8%, respectively, RR 1.9 and p = 0.012. Knowledge about the Pap smear, and the presence of lesions showed no association with HSV-2 infection. Multivariate analysis showed that the variable that most influenced the risk of HSV-2 infection was the presence of HIV infection, with a relative risk of 1.9 and p = 0.04. Discussion: Genital ulcers are an important entry point for HIV, and condom use is an important strategy to reduce transmission of HIV and HSV-2.

Malignant transformation of a teratoma occurs in 1-2% of cases, with carcinoma of the thyroid being extremely rare. It is usually diagnosed in the postoperative histopathological exam. We describe a case of thyroid carcinoma in a mature cystic teratoma.

RESUMO:Introdução: Reviu-se o conhecimento epidemiológico, fisiopatológico e clínico atual sobre a doença coronária, da sua génese até ao evento agudo, o Enfarte Agudo do Miocárdio (EAM). Valorizou-se, em especial, a teoria inflamatória da aterosclerose, que foi objeto de grandes desenvolvimentos na última década. Marcadores de instabilidade da placa aterosclerótica coronária: Aprofundou-se o conhecimento da placa aterosclerótica coronária instável. Descreveram-se detalhadamente os biomarcadores clínicos e laboratoriais associados à instabilidade da placa, com particular ênfase nos mecanismos inflamatórios. Objetivos:Estão divididos em dois pontos fundamentais:(1) Estudar em doentes com EAM a relação existente entre as moléculas inflamatórias: Interleucina-6 (IL-6), Fator de Necrose Tumoral-α (TNF-α) e Metaloproteinase de Matriz-3 (MMP3), não usados em contexto clínico, com um marcador inflamatório já em uso clínico: a Proteína C-Reativa ultrassensível (hs-CRP). Avaliar a relação de todas as moléculas inflamatórias com um biomarcador de lesão miocárdica: a Troponina Cardíaca I (cTnI). (2) Avaliar, no mesmo contexto de EAM, a Resposta de Fase Aguda (RFA) . Pretende-se demonstrar o impacto deste fenómeno, com repercussão clínica generalizada, no perfil lipídico e nos biomarcadores inflamatórios dos doentes. Métodos:(1) Estudo observacional prospetivo de doentes admitidos consecutivamente por EAM (grupo EAM) numa única unidade coronária, após exclusão de trauma ou infeção. Doseamento no sangue periférico, na admissão, de IL-6, TNF-α, MMP3, hs-CRP e cTnI. Este último biomarcador foi valorizado também nos valores séricos obtidos 6-9 horas depois. Procedeu-se a correlação linear (coeficiente de Pearson, de Rho-Spearman e determinação do R2) entre os 3 marcadores estudados com os valores de hs-CRP e de cTnI (valores da admissão e 6 a 9 horas após). Efetuou-se o cálculo dos coeficientes de regressão linear múltipla entre cTnI da admissão e cTnI 6-9h após, com o conjunto dos fatores inflamatórios estudados. (2) Estudo caso-controlo entre o grupo EAM e uma população aleatória de doentes seguidos em consulta de cardiologia, após exclusão de eventos cardiovasculares de qualquer território (grupo controlo) e também sem infeção ou trauma. Foram doseados os mesmos marcadores inflamatórios no grupo controlo e no grupo EAM. Nos dois grupos dosearam-se, ainda, as lipoproteínas: Colesterol total (CT), Colesterol HDL (HDLc), com as suas subfrações 2 e 3 (HDL 2 e HDL3), Colesterol LDL oxidado (LDLox),Triglicéridos (TG), Lipoproteína (a) [Lp(a)], Apolipoproteína A1 (ApoA1), Apolipoproteína B (ApoB) e Apolipoproteína E (ApoE). Definiram-se, em cada grupo, os dados demográficos, fatores de risco clássicos, terapêutica cardiovascular e o uso de anti-inflamatórios. Procedeu-se a análise multivariada em relação aos dados demográficos, fatores de risco e à terapêutica basal. Compararam-se as distribuições destas mesmas caraterísticas entre os dois grupos, assim como os valores séricos respetivos para as lipoproteínas estudadas. Procedeu-se à correlação entre as moléculas inflamatórias e as lipoproteínas, para todos os doentes estudados. Encontraram-se os coeficientes de regressão linear múltipla entre cada marcador inflamatório e o conjunto das moléculas lipídicas, por grupo. Finalmente, efetuou-se a comparação estatística entre os marcadores inflamatórios do grupo controlo e os marcadores inflamatórios do grupo EAM. Diferenças importantes entre as distribuições dos dados basais entre os dois grupos (grupo controlo vs EAM): idade (47,95±11,55 vs 68,53±2,70 anos) p<0.001; sexo feminino (18,18 vs 22,80%) p=0,076; diabetes mellitus (9,09% vs 36,84%) p=0,012; AAS (18,18 vs 66,66%) p<0,001; clopidogrel (4,54% vs 66,66%) p=0,033; estatinas (31,81% vs 66,14%) p=0,078; beta-bloqueadores (18,18% vs 56,14%) p=0,011; anti-inflamatórios (4,54% vs 33,33%) p=0,009. A regressão múltipla marcadores inflamatórios/perfil lipídico (grupo controlo) foi: hs-CRP (R=0,883, R2=0,780) p=0,022; IL-6 (R=0,911, R2=0,830) p=0,007; MMP3 (R=0,498, R2=0,248) p=0,943; TNF-α (R=0,680, R2=0,462) p=0,524. A regressão múltipla marcadores inflamatórios/perfil lipídico (grupo EAM) foi: hs-CRP (R=0,647, R2=0,418) p=0,004; IL-6 (R=0,544, R2=0,300), p=0,073; MMP3 (R=0,539, R2=0,290) p=0,089; TNF-α (R=0,595; R2=0,354) p=0,022. Da comparação entre os marcadores inflamatórios dos dois grupos resultou (mediana/intervalo interquartil, grupo controlo vs EAM): hs-CRP (0,19/0,27 vs 0,42/2,53 mg/dl) p=0,001, IL-6 (4,90/5,48 vs 13,07/26,41 pg/ml) p<0,001, MMP3 (19,70/13,70 vs 10,10/10,40 ng/ml) p<0,001;TNF-α (8,67/6,71 vs 8,26/7,80 pg/dl) p=0,805. Conclusões: (1) Nos doentes com EAM, existe correlação entre as moléculas inflamatórias IL-6, MMP3 e TNF-α, quer com o marcador inflamatório hs-CRP, quer com o marcador de lesão miocárdica cTnI. Esta correlação reforça-se para os valores de cTnI 6-9 horas após admissão, especialmente na correlação múltipla com o grupo dos quatro marcadores inflamatórios. (2) IL-6 está inversamente ligada às lipoproteínas de colesterol; hs-CRP e IL-6 têm excelentes correlações com o perfil lipídico valorizado no seu conjunto. No grupo EAM encontram-se níveis séricos mais reduzidos para as lipoproteínas de colesterol. Para TNF-α não foram encontradas diferenças significativas entre os grupos, as quais foram observadas para a IL-6 e hs-CRP (mais elevadas no grupo EAM). Os valores de MMP3 no grupo controlo estão mais elevados. ------------- ABSTRACT: Introduction: We reviewed the epidemiology, pathophysiology and current clinical knowledge about coronary heart disease, from its genesis to the acute myocardial infarction (AMI). The inflammatory theory for atherosclerosis, which has undergone considerable development in the last decade, was especially detailed. Markers of coronary atherosclerotic vulnerable plaque: The clinical and laboratory biomarkers associated with the unstable coronary atherosclerotic plaque vulnerable plaque are detailed. An emphasis was placed on the inflammatory mechanisms. Objectives: They are divided into two fundamental points: (1) To study in AMI patients, the relationship between the inflammatory molecules: Interleukin-6 (IL-6), Tumor Necrosis Factor-α (TNF-α) and Matrix metalloproteinase-3 (MMP3), unused in the clinical setting, with an inflammatory marker in clinical use: ultrasensitive C-reactive protein (hs-CRP), as well as a biomarker of myocardial injury: cardiac troponin I (cTnI). (2) To study, in the context of AMI, the Acute Phase Response (APR). We intend to demonstrate the impact of that clinical relevant phenomenon in the lipid profile and inflammatory biomarkers of our patients. Methods: (1) Prospective observational study of patients consecutively admitted for AMI (AMI group) in a single coronary care unit, after exclusion of trauma or infection. A peripheral assay at admission for IL-6, TNF-α, MMP3, hs-CRP and cTnI was performed. The latter was also valued in assays obtained 6-9 hours after admission. Linear correlation (Pearson's correlation coefficient, Spearman Rho's correlation coefficient and R2 regression) was performed between the three markers studied and the values of hs-CRP and cTnI (on admission and 6-9 hours after admission). Multiple linear regression was also obtained between cTnI on admission and 6-9h after, with all the inflammatory markers studied. (2) Case-control study between the AMI group and a random population of patients from an outpatient cardiology setting (control group). Cardiovascular events of any kind and infection or trauma were excluded in this group. The same inflammatory molecules were assayed in control and AMI groups. The following lipoproteins were also assayed: total cholesterol (TC), HDL cholesterol (HDLc) and subfractions 2 and 3 (HDL2 and HDL 3), oxidized LDL cholesterol (oxLDL), Triglycerides (TG), Lipoprotein (a) [Lp(a)], Apolipoprotein A1 (apoA1), Apolipoprotein B (ApoB) and Apolipoprotein E (ApoE). Demographics, classical risk factors, cardiovascular therapy and the use of anti-inflammatory drugs were appreciated in each group. The authors conducted a multivariate analysis with respect to demographics, risk factors and baseline therapy. The distribution of the same baseline characteristics was compared between the two groups, as well as the lipoprotein serum values. A correlation was performed between each inflammatory molecule and each of the lipoproteins, for all the patients studied. Multiple linear regression was determined between each inflammatory marker and all the lipid molecules per group. Finally, the statistical comparison between the inflammatory markers in the two groups was performed. Significant differences in the lipid pattern of the two groups (mean±SD or median/interquartile range, control group vs AMI): TC (208.45±35.03 vs 171.05±41.63 mg/dl) p<0.001; HDLc (51.50/18.25 vs 42.00/16.00 mg/dl) p=0.007; HDL2 (8.50/3.25 vs 10.00/6.00 mg/dl) p=0.292; HDL3 (41.75±9.82 vs 31.75±9.82 mg/dl) p<0.01; oxLDL (70.00/22.0 vs 43.50/21.00 U/L) p <0.001; TG (120.00/112.50 vs 107.00/86.00 mg/dl) p=0.527; Lp(a) (0.51/0.73 vs 0,51/0.50 g/L) p=0.854; apoA1 (1.38±0.63 vs 1.19±0.21 g/L) p=0.002; ApoB (0.96± 0.39 vs 0.78±0.28 g/L) p=0.004; ApoE (38.50/10,00 vs 38.00 /17,00 mg/L) p=0.574. In the linear correlations between inflammatory variables and lipid variables for all patients, we found a negative relationship between IL-6 and TC, HDLc, HDL3, ApoA1 and ApoB. The multiple linear regression inflammatory markers/lipid profile (control group) was: hs-CRP (R= 0.883, R2=0.780) p=0.022; IL6 (R=0.911, R2=0.830) p=0.007; MMP3 (R=0.498, R2=0.248) p=0.943; TNF-α (R=0.680, R2=0.462) p=0.524. For the linear regression inflammatory markers/lipid profile (AMI group) we found: hs-CRP (R=0.647, R2=0.418) p=0.004; IL-6 (R=0.544, R2=0.300) p=0.073; MMP3 (R=0.539, R2 =0.290) p=0.089; TNF-α (R=0.595, R2=0.354) p=0.022. The comparison between inflammatory markers in both groups (median/interquartile range, control group vs AMI) resulted as: hs-CRP (0.19/0.27 vs 0.42/2.53 mg/dl) p=0.001; IL-6 (4.90/5.48 vs 13.07/26.41 pg/ml) p<0.001; MMP3 (19.70/13.70 vs 10.10/10.40 ng/ml) p<0.001; TNF-α (8.67/6.71 vs 8.26/7.80 pg/dl) p=0.805. Conclusions: (1) In AMI patients there is a correlation between the inflammatory molecules IL-6, TNF-α and MMP3 with both the inflammatory marker hs-CRP and the ischemic marker cTnI. This correlation is strengthened for the cTnI at 6-9h post admission, particularly in the multiple linear regression to the four inflammatory markers studied. (2) IL-6 correlates negatively with the cholesterol lipoproteins. Hs-CRP and IL-6 are strongly correlated to the whole lipoprotein profile. AMI patients display reduced serum lipid levels. For the marker TNF-α no significant differences were found between groups, which were observed for IL-6 and hs-CRP (higher in the AMI group). MMP3 values are higher in the control group.

RESUMO: Conhecer a prevalência e perfil de consumo das populações revela-se como elementos importantes para o estabelecimento de Planos e Programas de intervenção na atenção aos problemas relacionados com o consumo do álcool. Este estudo transversal numa amostra de utentes dos cuidados primários de saúde teve por objectivo conhecer a prevalência do consumo do álcool em dois centros de saúde numa zona urbana, (Achada de Santo António e Achadinha) na capital do país e um centro de saúde numa zona rural (Picos), no interior da ilha de Santiago. O instrumento utilizado para avaliar os consumos de médio e alto risco do álcool foi o ASSIST. Foram entrevistados 302 utentes nos 3 centros de saúde. Os resultados mostraram uma prevalência ao longo da vida de 86,4% e nos últimos 3 meses de 62,3%. Mostrou que os homens tinham consumo superior às mulheres no centro de saúde de Achadinha (p value= 0.002 ) e Picos (p value=0.003). Em Achada de Santo António a diferença entre homens e mulheres não se mostrou estatisticamente significativa (p value= 0.397). Os resultados mostraram também nos dois centros associação significativa entre consumo de risco de tabaco e consumo de risco do álcool (p value=0.000 e 0.003) o que não se verificou em ASA (p value= 0.962). As variáveis idade, ocupação profissional, nível de escolaridade, não mostraram associação significativa ao consumo de risco do álcool mas mostraram associação significativa ao consumo do álcool nos últimos 3 meses. Mas o consumo do álcool nos últimos 3 meses não mostrou associação significativa ao consumo do tabaco nos últimos 3 meses. Espera-se que os resultados possam subsidiar a política de luta contra o álcool, no sentido de implementar acções que possam prevenir os danos à saúde e outras consequências resultantes do consumo abusivo do álcool. --------- ABSTRACT: To determine the prevalence and consumption profile of populations revealed as important elements for establishing plans and intervention programs in attention to problems related to alcohol consumption. This cross-sectional study on a sample of users of primary health care was aimed to determine the prevalence of alcohol consumption in two health centers in an urban area, (Achada de Santo António and Achadinha) in the capital and a health center in a rural area (Picos), in the island of Santiago’s contryside. The instrument used to assess the intake of medium and high risk alcohol was the ASSIST. We interviewed 302 users in three health centers. The results showed lifetime prevalence of 86.4% and in the last 3 months of 62.3%. It showed that men were superior to women in consumption in both health centers of Achadinha (p value=0.002) and Picos (p value=0.000). In Achada de Santo António, the difference between men and women was not statistically significant (p value = 0397). The results also showed the two centers had a significant association between risk of tobacco consumption and risk consumption of alcohol (p value = 0.000 and 0.003) which was not found in ASA (p value = 0.962). There was no association between the variables (age, professional occupation, and educational level) and risk of alcohol consumption, but they showed a significant association with alcohol consumption in the last 3 months. On the other hand, the consumption of alcohol in the last three months showed no significant association with the consumption of tobacco in the last 3 months. It is hoped that the results may support the policy against alcohol, may implement actions that can prevent damage to the health and other consequences resulting from alcohol abuse.

Este estudo determinou a riqueza de espécies e a abundância relativa dos flebotomíneos da comunidade de Lagoas, em Buriticupu, na Amazônia Maranhense. O estudo consistiu na captura de formas adultas com auxílio de armadilhas luminosas tipo CDC e armadilha de Shannon, das 18 às 6 horas, uma vez por mês, de dezembro/95 a janeiro/97, em ambiente florestal e extraflorestal (peri e intradomicílios). No total, capturou-se 9.392 espécimens (4.302 machos e 5.090 fêmeas) distribuídos em 38 espécies (1 Brumptomyia e 37 Lutzomyia). A riqueza e abundância de espécies foram maiores na mata, seguida do peri e intradomicílio. Na mata, as espécies mais freqüentes foram Lutzomyia whitmani (64,9%), L. migonei (27%) e L. serrana (3,6%). No peridomicílio, predominaram L. evandroi (55,4%), L. whitmani (33,4%) e L. migonei (6,4%) e no intra, L. evandroi (97,9%). O predomínio no ambiente silvestre resulta das potencialidades ainda encontradas nas glebas remanescentes da floresta tão explorada pelas atividades madeireira e agropecuária. Por outro lado, a peridomiciliação de 7,7% das espécies reverteu-se de grande importância epidemiológica, haja visto a existência de pacientes com leishmanioses que julgam ter adquirido a infecção nas proximidades de suas habitações.

São descritas as características clínicas e epidemiológicas de 422 casos de paracoccidioidomicose atendidos no Hospital Universitário da Universidade Federal do Mato Grosso do Sul (Campo Grande, Mato Grosso do Sul, Brasil) no período de Janeiro de 1980 a Agosto de 1999. A média de idade foi de 43,4 anos e a proporção homem: mulher foi de 10:1. Quase metade (45,5%) dos doentes era trabalhadora rural no momento do diagnóstico. A forma aguda/subaguda (tipo juvenil)(15,4% dos casos) revelou-se com importante comprometimento do sistema fagocítico-monocitário, manifestado principalmente por adenomegalia (95,4%); hepatomegalia (40%); esplenomegalia (23,1%). A forma crônica (tipo adulto)(84,6% dos casos) apresentou-se com maiores proporções de lesões em orofaringe (66,4%); rouquidão (31,4%) e tosse (50,7%). Para o diagnóstico, foram utilizados o exame micológico direto em 365 pacientes e o histopatológico em 302, com positividade de 50,7% e 97,3%, respectivamente. O tratamento antifúngico preferencial foi a associação de sulfametoxazol e trimetoprim (co-trimoxazol), utilizado em 90,3% dos doentes. Seqüelas foram observadas em 30,3% e óbito em 7,6% dos casos.

Para estudar a freqüência da infecção pelo Toxocara canis em crianças de classes sociais diferentes de Brasília, Brasil, foram testados soros de 602 crianças de ambos os sexos, de 1 a 12 anos, distribuídas em dois grupos representativos de condições socioeconômicas distintas. As amostras do primeiro grupo foram obtidas em laboratório público que atende bairros pobres. As do segundo grupo foram colhidas em laboratório privado, que serve à classe média. Os anticorpos anti-Toxocara foram detectados pelo método ELISA, com antígenos de Toxocara canis, e absorção com antígenos do Ascaris suum. A prevalência de soropositividade foi de 21,8% (66/302) no primeiro grupo e de 3% (9/300) no segundo (p<0,0001). Não houve diferença quanto ao gênero ou idade das crianças. Os resultados sugerem significativa prevalência de toxocaríase na população infantil de Brasília, com predomínio nas classes sociais menos favorecidas economicamente.

INTRODUCTION: Reduction in the vertical transmission of HIV is possible when prophylactic measures are implemented. Our objective was to determine demographic characteristics of HIV-infected pregnant women and the rate of mother-to-child transmission of HIV in Manaus, Amazonas, Brazil. METHODS: A descriptive study was conducted using notification, and investigating data from the Notifiable Diseases Data System in the Brazilian State of Amazonas, between 2007 and 2009. RESULTS: During the study period, notification was received of 509 HIV-positive pregnant women. The vertical transmission was 9.9% (95% CI: 7.2-12.6%). The mean age of women was 27 years (SD: 5.7), and the majority (54.8%) had not completed elementary school (eighth grade). Diagnosis of HIV seropositivity was made prior to pregnancy in 115 (22.6%) women, during prenatal care in 302 (59.3%), during delivery in 70 (13.8%), and following delivery in 22 (4.3%). Four hundred four of these women (79.4%) had had prenatal care, with 79.4% of patients receiving antiretroviral during pregnancy and 61.9% of the newborn infants receiving prophylaxis. In the final multivariate logistic regression model, living in urban area [OR = 0.7 (95% CI: 0.35-0.89)] and having had prenatal care [OR = 0.1 (95% CI: 0.04-0.24)] remained as protective factors against vertical HIV transmission in this population. CONCLUSIONS: The relevance of adequate compliance with the measures already established as being effective in guaranteeing a reduction in HIV transmission within the maternal and infant population should be emphasized.

INTRODUCTION: Control strategies to eliminate the transmission of Chagas disease by insect vectors have significantly decreased the number of reported acute cases in Brazil. However, data regarding the incidence and distribution of acute Chagas disease cases in the State of Pernambuco are unavailable in the literature. METHODS: A geographical information system was used to delineate the spatiotemporal distribution profile of the cases from 2002 to 2013 in 185 municipalities of Pernambuco based on the municipality where notification occurred. The results were presented in digital maps generated by the TerraView software (INPE). RESULTS: A total of 302 cases of acute disease were recorded in 37.8% of the municipalities, for a total of 0.13 cases per 1,000,000 inhabitants per year. Out of the 302 cases, 99.3% were reported between 2002 and 2006. The most affected municipalities were Carnaubeira da Penha, Mirandiba and Terra Nova. The risk maps showed a significant decrease in the number of notifications and a concentration of cases in the Midwest region. CONCLUSIONS: This study highlights a significant decrease in new cases of acute Chagas disease in Pernambuco starting in 2006 when Brazil received an international certification for the interruption of vectorial transmission by Triatoma infestans. However, control strategies should still be encouraged because other triatomine species can also transmit the parasite; moreover, other transmission modes must not be neglected.

Ventilator-dependent premature infants are often treated with dexamethasone. Several trials showed that steroids while improve pulmonary compliance and facilitate extubation, some treated infants may have adverse effects, such as alterations of growth curves. We conducted this retrospective study to evaluate the effects of steroids on mechanical ventilation, oxygen therapy, hospital length stay and mortality, in ventilator-dependent infants with bronchopulmonary dysplasia (BPD) (defined as the need of oxygen supplementation at 28 days of life). Twenty-six newborns with BPD were evaluated during 9 -- 42 days postpartum (mean = 31 days) and were divided into two groups: Group I - 14 newborns that did not receive dexamethasone, and Group II - 12 newborns that received dexamethasone at 14 --21 days of life. Dexamethasone was given at a dose of 0.25 mg per kilogram of body weight twice daily intravenously for 3 days, after which the dose was tapered. RESULTS: There were no statistically significant differences in the mean length of mechanical ventilation (Group I - 37 days, Group II - 35 days); oxygen supplementation (Group I - 16 days, Group II - 29 days); hospital stay (Group I - 72 days, Group II - 113 days); mortality (Group I - 35.7%, Group II - 41.6%). At birth, Group II was lighter (BW: Group I - 1154 grams ± 302, Group II - 791 grams ± 165; p < 0.05) and smaller (height: Group I - 37.22 cm ± 3.3, Group II - 33.5 ± 2.4; p< 0.05) than Group I. At 40 weeks, there were no statistically significant differences between groups in relation to anthropometric measurements. CONCLUSIONS: The use of corticosteroids in bronchopulmonary dysplasic infants may influence the somatic growth during its use. However, after its suspension, a recovery seems to occur, suggesting that its influence could be transitory.
