OBJETIVO: Avaliar o comprimento do colo uterino por meio da ressonância magnética e comparar aos achados da ultra-sonografia transvaginal. MATERIAIS E MÉTODOS: Foram realizados exames ultra-sonográficos e de ressonância magnética do colo uterino em 20 pacientes com idade gestacional entre 19 e 30 semanas. As medidas do colo obtidas pelo exame de ressonância magnética foram aferidas por dois especialistas em diagnóstico por imagem, para calcular a variabilidade interobservador do método. RESULTADOS: O cálculo do coeficiente de correlação de Pearson entre as medidas do comprimento cervical indicou correlação significante entre os métodos (r=0,628; p<0,01). A aplicação do teste t pareado não evidenciou diferença significativa entre as medidas aferidas pela ultra-sonografia transvaginal e ressonância magnética (p=0,068). A análise da variabilidade interobservador das medidas do colo obtidas pela ressonância magnética demonstrou alta confiabilidade do método (a=0,96). CONCLUSÃO: A comparação entre os dois métodos de imagem na avaliação da biometria cervical não apresentou diferença estatística, o que reforça a aplicação do exame ultra-sonográfico. Entretanto, em situações nas quais a ultra-sonografia transvaginal apresenta contra-indicações, o exame de ressonância magnética poderá apresentar-se como segunda opção para a avaliação do comprimento cervical.

OBJETIVO: Verificar, por meio da ultra-sonografia, os parâmetros espaciais do complexo hiolaríngeo na dinâmica da deglutição e a associação entre idade e efeitos. MATERIAIS E MÉTODOS: Neste estudo quantitativo e prospectivo foram incluídos 39 indivíduos sadios, de ambos os gêneros, na faixa etária de 20 a 70 anos (média, 45,56; desvio-padrão, 14,53). Avaliou-se a biomecânica da deglutição correspondente à medida da distância entre a porção superior do osso hióide e a borda superior da cartilagem tireóide, no momento de máxima elevação laríngea. As medidas foram realizadas com a ingestão de alimentos de consistências líquida e pastosa. RESULTADOS: A variação da distância da laringe ao hióide apresentou associação positiva com a idade, somente na deglutição de alimento pastoso, consistência na qual é solicitada maior atividade de propulsão lingual do que nos líquidos. CONCLUSÃO: É possível que o aumento da distância entre o hióide e a laringe, que representa menor elevação laríngea, na deglutição de pastosos, ocorra por efeito do envelhecimento, processo no qual se verifica diminuição de reservas funcionais, mesmo em indivíduos sadios. A ultra-sonografia pode detectar comportamentos relacionados a consistências alimentares, o que mostra sua possível potencialidade diagnóstica na avaliação da deglutição.

OBJETIVO: Avaliar a reprodutibilidade do ultra-som Doppler na quantificação do volume de fluxo portal em pacientes esquistossomóticos. MATERIAIS E MÉTODOS: Estudo prospectivo, transversal, observacional e autopareado, avaliando 21 pacientes portadores de esquistossomose hepatoesplênica, submetidos a mensuração do fluxo portal pelo ultra-som Doppler por três observadores, de forma independente, sendo calculada a concordância entre estes, dois a dois, pelo coeficiente de correlação intraclasse, teste t-pareado e grau de linearidade de Pearson. RESULTADOS: A concordância interobservador foi excelente. O coeficiente de correlação intraclasse variou entre 80,6% e 93,0% (IC a 95% [65,3% ; 95,8%]), com coeficiente de correlação de Pearson variando entre 81,6% e 92,7% e sem diferença estatisticamente significante entre os observadores quanto à média do fluxo portal mensurado pelo ultra-som Doppler (p = 0,954 / 0,758 / 0,749). CONCLUSÃO: O ultra-som Doppler é um método confiável para quantificar o fluxo portal em pacientes portadores de hipertensão porta de origem esquistossomótica, apresentando boa concordância interobservador.

OBJETIVO: Utilizar a ultra-sonografia como método de avaliação do "tempo esofágico" e sua capacidade de discriminação entre as substâncias não-sólidas ingeridas (água e iogurte). MATERIAIS E MÉTODOS: Foram estudados 22 adultos jovens, sem queixa gástrica e esofágica, de ambos os sexos. Foi utilizado transdutor de ultra-som de 3,5 MHz, convexo, em modo B, colocado na região epigástrica. O intervalo de tempo esofágico foi determinado utilizando-se um cronômetro que foi acionado no momento da movimentação da glote (início da deglutição) e interrompido ao se visualizar a passagem do conteúdo deglutido no esôfago intra-abdominal. RESULTADOS: O tempo médio de trânsito para a água foi de 6,64 ± 1,83 segundos e para o iogurte foi de 8,59 ± 2,70 segundos. A análise estatística comparativa pelo teste t pareado mostrou que as médias apresentaram diferenças significativas entre as substâncias. CONCLUSÃO: O novo método experimental de avaliar o "tempo esofágico" com ultra-som é capaz de propiciar diferenças significativas do tempo necessário para um determinado alimento (líquido ou pastoso) percorrer o esôfago, esclarecendo as suspeitas clínicas e possibilitando a indicação mais precisa de exames clínicos mais complexos.

Introduction: Gamma Knife surgery (GKS) is a noninvasive neurosurgical stereotactic procedure, increasingly used as an alternative to open functional procedures. This includes the targeting of the ventrointermediate nucleus of the thalamus (e.g., Vim) for tremor. Objective: To enhance anatomic imaging for Vim GKS using high-field (7 T) MRI and Diffusion Weighted Imaging (DWI). Methods: Five young healthy subjects and two patients were scanned both on 3 and 7 T MRI. The protocol was the same in all cases, and included: T1-weighted (T1w) and DWI at 3T; susceptibility weighted images (SWI) at 7T for the visualization of thalamic subparts. SWI was further integrated into the Gamma Plan Software® (LGP, Elekta Instruments, AB, Sweden) and co-registered with 3T images. A simulation of targeting of the Vim was done using the quadrilatere of Guyot. Furthermore, a correlation with the position of the found target on SWI and also on DWI (after clustering of the different thalamic nuclei) was performed. Results: For the 5 healthy subjects, there was a good correlation between the position of the Vim on SWI, DWI and the GKS targeting. For the patients, on the pretherapeutic acquisitions, SWI helped in positioning the target. For posttherapeutic sequences, SWI supposed position of the Vim matched the corresponding contrast enhancement seen at follow-up MRI. Additionally, on the patient's follow-up T1w images, we could observe a small area of contrast-enhancement corresponding to the target used in GKS (e.g., Vim), which belongs to the Ventral-Lateral-Ventral (VLV) nuclei group. Our clustering method resulted in seven thalamic groups. Conclusion: The use of SWI provided us with a superior resolution and an improved image contrast within the central gray matter, enabling us to directly visualize the Vim. We additionally propose a novel robust method for segmenting the thalamus in seven anatomical groups based on DWI. The localization of the GKS target on the follow-up T1w images, as well as the position of the Vim on 7 T, have been used as a gold standard for the validation of VLV cluster's emplacement. The contrast enhancement corresponding to the targeted area was always localized inside the expected cluster, providing strong evidence of the VLV segmentation accuracy. The anatomical correlation between the direct visualization on 7T and the current targeting methods on 3T (e.g., quadrilatere of Guyot, histological atlases, DWI) seems to show a very good anatomical matching.

The present work describes the development of a fast and robust analytical method for the determination of 53 antibiotic residues, covering various chemical groups and some of their metabolites, in environmental matrices that are considered important sources of antibiotic pollution, namely hospital and urban wastewaters, as well as in river waters. The method is based on automated off-line solid phase extraction (SPE) followed by ultra-high-performance liquid chromatography coupled to quadrupole linear ion trap tandem mass spectrometry (UHPLC–QqLIT). For unequivocal identification and confirmation, and in order to fulfill EU guidelines, two selected reaction monitoring (SRM) transitions per compound are monitored (the most intense one is used for quantification and the second one for confirmation). Quantification of target antibiotics is performed by the internal standard approach, using one isotopically labeled compound for each chemical group, in order to correct matrix effects. The main advantages of the method are automation and speed-up of sample preparation, by the reduction of extraction volumes for all matrices, the fast separation of a wide spectrum of antibiotics by using ultra-high-performance liquid chromatography, its sensitivity (limits of detection in the low ng/L range) and selectivity (due to the use of tandem mass spectrometry) The inclusion of β-lactam antibiotics (penicillins and cephalosporins), which are compounds difficult to analyze in multi-residue methods due to their instability in water matrices, and some antibiotics metabolites are other important benefits of the method developed. As part of the validation procedure, the method developed was applied to the analysis of antibiotics residues in hospital, urban influent and effluent wastewaters as well as in river water samples

Objectives: To correlate the chronic stimulated electrode position on postoperative MRI with the clinical response obtained in PD patients. Material and Method: We retrospectively reviewed 14 consecutive parkinsonian patients who were selected for STN-DBS surgery. Coordinates were determined on an IR T2 MRI coronal section per pendicular to AC-PC plane 3 mm posterior to midcommissural point (MCP) and 12 mm lateral to the midline the inferior aspect of subthalamic region. A CRW stereotactic frame was used for the surgical procedure. A 3D IR T2 MRI was performed postoperatively to determine the location of the stimulated contact in each patient. The clinical results were assessed independently by the neurological team. Results: All but 2 patients had monopolar stimulation. The mean coordinates of the stimulated contacts were: AP ^ ÿ4:23G1:4, Lat ^ 1:12G0:15, Vert ^ ÿ4:1 G2:7 to the MCP. With a mean follow-up of 8 months, all stimulated patients had a significant clinical improvement (preop/postop «ON» UPDRS: 25:8G7:0= 23:3 G8:6; preop/postop «OFF» UPDRS: 50:2G11:4=26:0 G7:8), 60% of them without any antiparkinsonian drug. Conclusion: According to the stereotactic atlas of Schaltenbrand and Warren and the 3D shape of the STN, our results show that our targetting is accurate and almost all the stimulated contacts are comprised in the STN volume. This indicates that MRI is a safe, precise and reproducible procedure for targetting the STN. The location of the stimulated contact within the STN volume is a good predictor of the clinical results.

Si l'examen clinique revêt une importance essentielle en lymphologie et exige des praticiens expérimentés, la lymphoscintigraphie et plus récemment la lympho-fluoroscopie au vert d'indocyanine constituent des moyens d'investigation précieux dans la prévention, le diagnostic et le traitement des pathologies vasculaires lymphatiques. L'intérêt de la lymphoscintigraphie réside dans l'analyse qualitative et quantitative de la migration des macromolécules par les vaisseaux lymphatiques et l'évaluation du secteur lymphatique profond. La lympho-fluoroscopie se distingue de la lymphoscintigraphie par l'obtention d'une cartographie détaillée des vaisseaux lymphatiques superficiels et d'images dynamiques en temps réel. Elle apporte à l'angiologue et au physiothérapeute des informations irremplaçables sur leur contractilité et la présence de dérivations compensatoires à privilégier lors du drainage lymphatique manuel. Venous thromboembolism is a frequent disease with an annual incidence of 0.75-2.69/1000 reaching 2-7/1000 > 70 years. Deep vein thrombosis (DVT) and pulmonary embolism are two manifestations of the same underlying disease. Most frequent localization of DVT is at lower limbs. The diagnostic workup begins with an estimation of DVT risk, a judicious use of D-Dimers, and compression venous ultrasound depending on DVT probability. The development of direct oral anticoagulants and recent data on interventional DVT treatment, in selected cases, have widened the therapeutic spectrum of DVT. The present article aims at informing the primary care physician of the optimized workup of patients with lower limb suspicion of DVT.

BACKGROUND: No studies have identified which patients with upper-extremity deep vein thrombosis (DVT) are at low risk for adverse events within the first week of therapy. METHODS: We used data from Registro Informatizado de la Enfermedad TromboEmbólica to explore in patients with upper-extremity DVT a prognostic score that correctly identified patients with lower limb DVT at low risk for pulmonary embolism, major bleeding, or death within the first week. RESULTS: As of December 2014, 1135 outpatients with upper-extremity DVT were recruited. Of these, 515 (45%) were treated at home. During the first week, three patients (0.26%) experienced pulmonary embolism, two (0.18%) had major bleeding, and four (0.35%) died. We assigned 1 point to patients with chronic heart failure, creatinine clearance levels 30-60 mL min(-1) , recent bleeding, abnormal platelet count, recent immobility, or cancer without metastases; 2 points to those with metastatic cancer; and 3 points to those with creatinine clearance levels < 30 mL min(-1) . Overall, 759 (67%) patients scored ≤ 1 point and were considered to be at low risk. The rate of the composite outcome within the first week was 0.26% (95% confidence interval [CI] 0.004-0.87) in patients at low risk and 1.86% (95% CI 0.81-3.68) in the remaining patients. C-statistics was 0.73 (95% CI 0.57-0.88). Net reclassification improvement was 22%, and integrated discrimination improvement was 0.0055. CONCLUSIONS: Using six easily available variables, we identified outpatients with upper-extremity DVT at low risk for adverse events within the first week. These data may help to safely treat more patients at home.
