The purpose of this study was to determine if race/ethnicity was a significant risk factor for hospital mortality in children following congenital heart surgery in a contemporary sample of newborns with congenital heart disease. Unlike previous studies that utilized administrative databases, this study utilized clinical data collected at the point of care to examine racial/ethnic outcome differences in the context of the patients' clinical condition and their overall perioperative experience. A retrospective cohort design was used. The study sample consisted of 316 newborns (<31 days of age) who underwent congenital heart surgery between January 2007 through December 2009. A multivariate logistic regression model was used to determine the impact of race/ethnicity, insurance status, presence of a spatial anomaly, prenatal diagnosis, postoperative sepsis, cardiac arrest, respiratory failure, unplanned reoperation, and total length of stay in the intensive care unit on outcomes following congenital heart surgery in newborns. The study findings showed that the strongest predictors of hospital mortality following congenital heart surgery in this cohort were postoperative cardiac arrest, postoperative respiratory failure, having a spatial anomaly, and total ICU LOS. Race/ethnicity and insurance status were not significant risk factors. The institution where this study was conducted is designated as a center of excellence for congenital heart disease. These centers have state-of-the-art facilities, extensive experience in caring for children with congenital heart disease, and superior outcomes. This study suggests that optimal care delivery for newborns requiring congenital heart surgery at a center of excellence portends exceptional outcomes and this benefit is conferred upon the entire patient population despite the race/ethnicity of the patients. From a public health and health services view, this study also contributes to the overall body of knowledge on racial/ethnic disparities in children with congenital heart defects and puts forward the possibility of a relationship between quality of care and racial/ethnic disparities. Further study is required to examine the impact of race/ethnicity on the long-term outcomes of these children as they encounter the disparate components of the health care delivery system. There is also opportunity to study the role of race/ethnicity on the hospital morbidity in these patients considering current expectations for hospital survival are very high, and much of the current focus for quality improvement rests in minimizing the development of patient morbidities.^

In prospective studies it is essential that the study sample accurately represents the target population for meaningful inferences to be drawn. Understanding why some individuals do not participate, or fail to continue to participate, in longitudinal studies can provide an empirical basis for the development of effective recruitment and retention strategies to improve response rates. This study examined the influence of social connectedness and self-esteem on long-term retention of participants, using secondary data from the “San Antonio Longitudinal Study of Aging” (SALSA), a population-based study of Mexican Americans (MAs) and European Americans (EAs) aged over 65 years residing in San Antonio, Texas. We tested the effect of social connectedness, self-esteem and socioeconomic status on participant retention in both ethnic groups. In MAs only, we analyzed whether acculturation and assimilation moderated these associations and/or had a direct effect on participant retention. ^ Low income, low frequency of social contacts and length of recruitment interval were significant predictors of non-completer status. Participants with low levels of social contacts were almost twice as likely as those with high levels of social contacts to be non-completers, even after adjustment for age, sex, ethnic group, education, household income, and recruitment interval (OR = 1.95, 95% CI: 1.26–3.01, p = 0.003). Recruitment interval consistently and strongly predicted non-completer status in all the models tested. Depending on the model, for each year beyond baseline there was a 25–33% greater likelihood of non-completion. The only significant interaction, or moderating, effect observed was between social contacts and cultural values among MAs. Specifically, MAs with both low social contacts and low acculturation on cultural values (i.e., placed high value on preserving Mexican cultural origins) were three and half times more likely to be non-completers compared with MAs in other subgroups comprised of the combination of these variables, even after adjustment for covariates. ^ Long term studies with older and minority participants are challenging for participant retention. Strategies can be designed to enhance retention by paying special attention to participants with low social contacts and, in MAs, participants with both low social contacts and low acculturation on cultural values. Minimizing the time interval between baseline and follow-up recruitment, and maintaining frequent contact with participants during this interval should also be is integral to the study design.^

Opioids are drugs with opium-like qualities that are either derived from opiates (drugs created from opium, such as morphine or codeine) or chemically produced. In the U.S. opiate abuse and related deaths have been increasing and traditional maintenance treatment has been Methadone with variable success. However, since 2003 synthetic Buprenorphine has been used since it is prescribed daily by physicians in pill form and should improve outcomes. Comparative studies are limited and the effect of ethnicity on treatment outcome is unknown. ^ Data collected at one clinic from December 2005 through May 2009 were used to compare the association between ethnicity and other socioeconomic variables with treatment status, and to identify factors associated with the dropout among participants. Descriptive tables and multiple logistic regression models were used to examine the data on 1,295 total participants. Of the total, 875 participants (68%) were from the Methadone subsample and 420 participants (32%) from the Buprenorphine subsample; only about 15% stayed in treatment. ^ This study showed that with either Methadone or Buprenorphine maintenance therapy, only about 15% participants stay active over 3.5 years. Methadone treated patients that stayed active in treatment were associated with Caucasian ethnicity and were more likely to be employed. With Buprenorphine maintenance treatment only age over 40 years was associated with continuing activity in the program. Further studies that examine the reasons for the high dropout status and the implication of the socioeconomic and ethnic associations found in this data may help to improve treatment outcomes.^

Objective. The prevalence of overweight and obesity differs substantially among children of different ethnic origin in the United States. The objective of this project is to estimate to what extent changes in ethnic composition since 1980 have contributed to the current general “obesity epidemic” in the childhood population of the United States.^ Methods. Populations by single year of age, 0 to 19, male and female, for Hispanics, non-Hispanic whites, and non-Hispanic blacks, from the US Census’ July estimates for 1985, 1990, 1995, 2000 and 2005 were taken and compared to the population and percentage of those groups from 1980. Age, sex, and ethnicity specific prevalence rates for overweight in 1980 were then applied to the populations by age for the specified year and differences in expected and actual overweight populations were assessed.^ Result. The results from this investigation provide estimates of the contribution that different ethnic groups have made to the overall prevalence of overweight and obesity in the childhood population of the United States. Assuming that the 1976-1980 prevalence rates had remained unchanged, and then comparing the population had there been no change in ethnic composition with the population given the actual change in ethnicity, the percentage increase was 1.06% in 1985, 1.72% in 1990, 2.57% in 1995, 3.95% in 2000, and 4.39% in 2005.^ Conclusion. The changes in ethnic composition of the population, independent of changes in ethnicity-specific prevalence, have contributed substantially to the current overall prevalence of obesity in the United States childhood population. There are a number of factors that may be responsible for the apparent susceptibility of Mexican-Americans and non-Hispanic blacks to overweight and obesity. Further research is needed on specific characteristics of those populations.^

Significant racial/ethnic differences exist in prevalence of hypertension (HTN) and non-insulin dependent diabetes mellitus (NIDDM). Hypertension is more common in diabetics than in non-diabetics, and an etiologic link between the two conditions has been proposed. Since there are few longitudinal studies of persons with both HTN and NIDDM, a retrospective cohort study was conducted to determine if ethnicity (Black, Hispanic (Mexican-American), and non-Hispanic White) was related to NIDDM incidence in a low-SES, multi-ethnic clinic population of diagnosed hypertensives. Two thousand nine hundred forty-one hypertensives free of NIDDM at baseline were followed for up to 10 years. Mean baseline age was 56 $\pm$ 12 years, M:F percent was 33:67, and Black:Hispanic:White percent was 63:17:20. There were 236 incident cases of NIDDM. In Cox proportional hazards analysis, the risk of developing NIDDM over 10 years was not related to ethnicity after controlling for significant covariates, including age, baseline blood glucose and body mass index (adjusted RR for Blacks compared to Whites =.82, 95 percent CI =.57-1.18; adjusted RR for Hispanics compared to Whites =.84, 95 percent CI =.51-1.38). This result contrasts with the increased risk of NIDDM among Blacks and Hispanics compared to Whites found in the general population. The study suggests that a diagnosis of hypertension equalizes the risk of developing NIDDM among the three ethnic groups. ^

Background. The childhood obesity epidemic has disproportionately impacted the lives of low-income, minority preschoolers and their families. Research shows that parents play a major role as "gatekeepers" who control what food is brought into the home and as role models for dietary practices. Currently, there is limited research regarding ethnic differences in families of low-income preschoolers. ^ Objective. The objective of this study was to look at ethnic differences in food availability at home among the low-income families of Hispanic and African American preschoolers attending Head Start centers in Harris County, Texas. ^ Design/Subjects. Descriptive data on food availability at home between Hispanic and African American families were used and analyzed for this study. Parents or primary caregivers (n = 718) of children enrolled at Head Start Centers in Houston, Texas completed the Healthy Home Survey. ^ Methods. In the Healthy Home Survey, participants were asked to answer open-ended questions regarding various types of foods currently available at home, such as fresh, canned or jarred, dried and frozen fruits; fresh, canned or jarred, and frozen vegetables; salty snacks, sweet snacks, candy, and soda. Descriptive analyses were conducted to identify significant differences between Hispanics and African Americans via a paired t-test to compare the means of variables between the study groups and a Pearson's chi-square or Fischer's exact (if cell size was <5) test calculated for food availability (food types) between ethnicities to determine differences in distributions. ^ Results. Although both Hispanics and African Americans reported having all categories of food types at home, there were statistically significant differences between ethnic groups. Hispanics were more likely to have fresh fruits and vegetables at home than African Americans. At the same time, more African American families reported having canned or jarred fruits and canned green/leafy vegetables than Hispanics. More Hispanic families reported having diet, regular, and both diet and regular sodas available compared to African American families. However, high percentages of unhealthy foods (including snacks and candy) were reported by both ethnicities. ^ Conclusions. The findings presented in this study indicate the implicit ethnic differences that exist in the food availability among low-income families of Hispanic and African American preschoolers. Future research should investigate the associations between food availability and children's weight status by ethnicity to identify additional differences that may exist.^

The central paradigm linking disadvantaged social status and mental health has been the social stress model (Horwitz, 1999), the assumption being that individuals residing in lower social status groups are subjected to greater levels of stress not experienced by individuals from higher status groups. A further assumption is that such individuals have fewer resources to cope with stress, in turn leading to higher levels of psychological disorder, including depression (Pearlin, 1989). Despite these key assumptions, there is a dearth of literature comparing the social patterning of stress exposure (Hatch & Dohrenwend, 2007; Meyer, Schwartz, & Frost, 2008; Kessler, Mickelson, & Williams, 1999; Turner & Avison, 2003; Turner & Lloyd, 1999; Turner, Wheaton, & Lloyd, 1995), and the distribution and contribution of protective factors, posited to play a role in the low rates of depression found among African- and Latino-Americans (Alegria et al., 2007; Breslau, Aguilar-Gaxiola, Kendler, Su, Williams, & Kessler, 2006; Breslau, Borges, Hagar, Tancredi, Gilman, 2009; Gavin, Walton, Chae, Alegria, Jackson, & Takeuchi, 2010; Williams, & Neighbors, 2006). Thus, this study sought to describe both the distribution and contribution of risk and protective factors in relation to depression among a sample of African-, European-, and Latina-American mothers of adolescents, including testing a hypothesized mechanism through which social support, an important protective factor specific to women and depression, operates. ^ Despite the finding that the levels of depression were not statistically different across all three groups of women, surprising results were found in describing the distribution of both risk and protective factors, in that results reported among all women who were mothers when analyzed masked differences within each ethnic group when SES was assessed, a point made explicit by Williams (2002) regarding racial and ethnic variations in women's health. In the final analysis, while perceived social support was found to partially mediate the effect of social isolation on depression, among African-Americans, the direct effect of social isolation and depression was lower among this group of women, as was the indirect effect of social isolation and perceived social support when compared to European- and Latina-American mothers. Or, put differently, higher levels of social isolation were not found to be as associated with more depression or lower social support among African-American mothers when compared to their European- and Latina-American counterparts. ^ Women in American society occupy a number of roles, i.e., that of being female, married or single, mother, homemaker or employee. In addition, to these roles, ethnicity and SES also come into play, such that the intersection of all these roles and the social contexts that they occupy are equally important and must be taken into consideration when making predictions drawn from the social stress model. Based on these findings, it appears that the assumptions of the social stress model need to be revisited to include the variety of roles that intersect among individuals from differing social groups. More specifically, among women who are mothers and occupy a myriad of other roles, i.e., that of being female, married or single, African- or Latina-American, mother, homemaker or employee, the intersection of all the roles and the social contexts that women occupy are equally important and must be taken into consideration when looking at both the types and distribution of stressors across women. Predictions based on simple, mutually exclusive categories of social groups may lead to erroneous assumptions and misleading results.^

En este artículo se propone la reflexión acerca de la relación entre los campos cultural y político en la definición de las orientaciones de la política cultural en la Ciudad Autónoma de Buenos Aires (CABA) Argentina, en 2000-2010. Para ello se toma el incendio del local “República de Cromañón" ocurrido en diciembre del 2004, donde murieron 194 jóvenes y un número no determinado quedó afectado física y psicológicamente, y a consecuencia del cual fue destituido el Jefe de Gobierno Aníbal Ibarra. Se considera que la productividad de dicho acontecimiento resultó en un cambio de direccionalidad en la política cultural de la Ciudad Autónoma con consecuencias que excedieron el espacio local para proyectarse en el ámbito nacional. El análisis se realizó desde la perspectiva de la relativa autonomía de los campos (Pierre Bourdieu, 1995) y a partir de la hipótesis de la “autonomía variable" (Raymond Williams, 1982). En la máxima conducción del estado local, se encontraría una suerte de dominación de las instancias políticas sobre la gestión de la política cultural, en tanto que en la implementación de los programas, los técnicos y profesionales continúan desarrollan acciones que pueden resultar relativamente autónomas de las orientaciones del Ejecutivo de la CABA.

La descentralización en el marco de las políticas neoliberales se concibió como una reforma política-administrativa para minimizar el rol del Estado postulando como bastión clave "La participación local". Sin embargo ésta sólo se queda en los ámbitos formales. El caso particular de las políticas de descentralización del riego en la provincia de Mendoza no es una excepción. Lo que nos lleva a analizar su microcosmos y las redes sociales informales que dan cuenca de la desigual participación de los distintos usuarios del agua de riego en las decisiones en torno a este recurso, según sus trayectorias y sus capitales sociales, económicos y culturales.

Círculo de Lectores, club del libro español, ha desarrollado –desde su creación en 1962 y, en particular, bajo la dirección de Hans Meinke entre 1981 y 1997– un proyecto cultural basado tanto en la calidad de la edición de obras de referencia como en la proyección social de su actividad a través de conferencias y exposiciones. Así, Meinke conformó en torno a él una red de artistas, escritores e intelectuales que le condujo a erigirse en uno de los mediadores culturales más relevantes en la España de la segunda mitad del siglo XX. Como consecuencia de una concepción de la cultura como un permanente intercambio entre sus diversas disciplinas, la realización de ediciones ilustradas constituyó uno de los pilares de su política cultural. Dentro de este terreno, fue el pintor Antonio Saura su más estrecho colaborador, no sólo en cuanto a la realización de proyectos, sino asesorando a Meinke en posibles ediciones y recomendando a otros ilustradores. Saura ilustró para Círculo de Lectores los considerados como “libros de su vida", dentro de los cuales la figura de Ramón Gómez de la Serna es una referencia ineludible, al ser su libro Ismos (1931, 1943) una de las lecturas causantes de la vocación artística del pintor. El estudio las condiciones sociales, de circulación y apropiación en Flor nueva de greguerías (1989) servirá como ejemplo del proceso de gestación de proyectos y de interacción de estas redes entre arte y edición.

El presente proyecto de investigación analiza la relación entre sociedad y política en la Argentina entre 1955 y 1976. Se trata de un período signado por una aguda crisis socio-política, una profunda pérdida de legitimidad de las instituciones y el Estado y en que la acelerada modernización socio-cultural posterior a 1955 se combinó rápidamente con una tendencia a la radicalización política. Uno de los primeros efectos de este proceso fue la recomposición de la izquierda. Del seno de sus partidos "tradicionales" (el Partido Comunista y Socialista) surgieron diversas formaciones de la "nueva izquierda", mientras que rupturas y transformaciones similares se verificaban en sectores nacionalistas, peronistas y cristianos. Estos nuevos actores sociales y políticos solieron orientarse hacia objetivos que proclamaron la "liberación nacional" y el "socialismo" y hacia estrategias revolucionarias. Nuestro trabajo se interesa centralmente en reconstruir los orígenes de las nuevas prácticas y organizaciones surgidas por entonces así como las formas de articulación entre demandas sociales y propuestas políticas, y entre elencos dirigentes y sectores populares. A su vez, examina una multiplicidad de "casos" que formaron parte de este social y políticamente variado campo, sin subsumirlo exclusivamente al accionar de las organizaciones armadas.

Nuestra investigación se centró fundamentalmente en las distintas formas en que las derechas respondieron a los conflictos suscitados en el mundo del trabajo durante el período de entreguerras. En primer lugar, advertimos que la cuestión social fue adquiriendo un rol protagónico en la agenda nacionalista tal como puede verse, por ejemplo, a través del análisis de los periódicos. En efecto, los diarios más importantes adscriptos al nacionalismo desarrollaron un discurso radical respecto a los problemas sociales e incluyeron secciones específicas para tratar estas cuestiones y expresar una posición al respecto. Las respuestas del nacionalismo argentino frente a la cuestión obrera han sido múltiples y han abarcado distintas esferas de la vida social. Lejos de esperar que la solución a los problemas sociales proviniera exclusivamente de las medidas restrictivas y represivas hacia el movimiento obrero, los nacionalistas elaboraron programas sociales, políticos, económicos y culturales que formaron parte de su proyecto de nación autoritaria y jerárquica. Los proyectos sociales y las propuestas de organización sindical fueron en gran parte inspirados por los fascismos europeos los cuales incluyeron programas de contención social dentro de un orden político totalitario. En este sentido los nacionalistas argentinos intentaron mediante sus propuestas imponer un orden que contemplara las necesidades básicas de los sectores populares y que preservara las jerarquías sociales limitando la participación política o sindical de los trabajadores y eliminando definitivamente alas fuerzas de la izquierda revolucionaria. Las organizaciones obreras nacionalistas incluyeron todo tipo de trabajadores en sus filas y procuraron captar tanto a los afiliados de los sindicatos autónomos como a los trabajadores socialistas. Algunas de estas organizaciones fueron efímeras mientras que otras tuvieron más éxito y lograron atraer adherentes. Las mismas conformaron la corriente que hemos denominado nacionalismo sindicalista, la cual desarrolló su propia doctrina social fuertemente influenciada por las encíclicas papales. Las manifestaciones nacionalistas en el espacio público porteño han sido también analizadas in extenso. Existieron distintos tipos de manifestaciones para movilizar a los seguidores del nacionalismo y para captar nuevos adherentes, especialmente aquellos provenientes de los sectores populares. Las manifestaciones se convirtieron en el escenario de las disputas ideológicas mantenidas tanto contra la política liberal como contra la revolucionaria. La "revolución nacionalista", según la formulaban sus partidarios, implicaba trascender los aspectos políticos y económicos incorporando transformaciones en otras áreas de la vida social: las costumbres, las formas de vida, los gustos culturales, los valores. Los nacionalistas advirtieron que para lograr este tipo de "revolución" debían hacer usa de los medios de comunicación masivos y diseñar proyectos para regular las industrias culturales. El objetivo de representar a los sectores populares fracasó rotundamente. El discurso nacionalista que condenaba la diversidad étnico-religiosa, que amenazaba con eliminar las distintas voces políticas existentes, y que expresaba un odio visceral a sus enemigos (ya fueran judíos, anarquistas, comunistas, o liberales) fue extremadamente desafortunado para quienes procuraron ensanchar las bases de un movimiento antidemocrático originalmente elitista que, a la luz del contexto internacional y de las condiciones locales, devino en populista

La Nación ha sido una de las más importantes construcciones políticoculturales de la Modernidad. Las premisas que le han dado mayor vitalidad han sido y son, pues, su afirmación como imaginario de identidad para los pueblos y su significación en el desarrollo de las denominadas "relaciones internacionales", en cuanto formas de expresión de las interrelaciones globales de los sectores dominantes. Este estudio se basa en el análisis de esas premisas a la luz de algunas de las nuevas perspectivas emanadas de los paradigmas político y cultural, desarrollados en las ciencias sociales a partir de las últimas décadas del siglo pasado, y aplicadas a los estudios del territorio nacional, su construcción, sus mutaciones y los imaginarios espaciales que de ellos derivan. A partir de algunas de las visiones críticas mencionadas, en la actualidad el concepto de nación parece haber entrado en un proceso de erosión o descomposición. Fenómenos sociales, económicos, políticos y culturales ligados a los procesos de reestructuración global (migraciones intercontinentales, sistemas mediáticos globales, exclusión social, discriminación étnica y religiosa, marginación y falta de participación política, nuevas formas de manifestación social...) y a la construcción de los discursos posmodernos hacen aparecer la nación como un concepto, al menos, puesto en tela de juicio. Los más audaces hablan del virtual ingreso a un mundo "posnacional". ¿Significará esto la extinción de una idea y una realidad con la que se han formado culturalmente millones de personas de numerosas generaciones en los puntos más lejanos del planeta? La respuesta deberá estar planteada en el estudio real y especializado de esos fenómenos y de las nuevas realidades que de ellos emergen.

El artículo analiza las ideas económicas de un arco de instituciones y publicaciones políticas y culturales que convergieron en la consolidación del movimiento antifascista liberal en Argentina en 1939-1943, definido por el apoyo a los Aliados en la guerra mundial y la oposición a grupos nacionalistas y antiliberales y a la administración de Ramón S. Castillo (1940-1943). En diálogo con la nueva historiografía que ha revisado el período de entreguerras en Argentina, el artículo sostiene que la defensa de las libertades políticas y culturales, centro del discurso unificador del frente antifascista, coexistía con distintas posiciones sobre el liberalismo económico y el proceso de intervención del estado en la economía desarrollado por los grupos conservadores gobernantes desde 1930. El texto pone de relieve así la existencia de coincidencias inter-partidarias y diferencias intra-partidarias sobre dichos procesos que frecuentemente eran oscurecidos por el conflicto político de esos año

Este artículo analiza las transformaciones en las formas de leer y escribir en las clases de historia, a partir del impacto de las políticas públicas de distribución de una netbook por alumno y por docente. Desde la perspectiva de la historia cultural asociada a la cultura escolar se analizan las prácticas de lectura y escritura propias de la historia como disciplina escolar y se generan numerosos interrogantes vinculados con la profundidad de los cambios por venir, el papel de los profesores en los mismos, las transformaciones de la lectura como acto social en las aulas, las nuevas vinculaciones entre lectura y escritura, la búsqueda de información histórica pertinente y confiable o el lugar de viejas tecnologías como los libros de texto. Desde allí se postula que un profesor de historia es profesor de lectura y escritura y, como hoy cambian las formas de leer y escribir, resulta preciso reinventar este rol
