OBJECTIVE: To study whether antioxidant supplementation will reduce the incidence of preeclampsia among patients at increased risk.METHODS: A randomized, placebo-controlled, double-blind clinical trial was conducted at four Brazilian sites. Women between 12 0/7 weeks and 19 6/7 weeks of gestation and diagnosed to have chronic hypertension or a prior history of preeclampsia were randomly assigned to daily treatment with both vitamin C (1,000 mg) and vitamin E (400 International Units) or placebo. Analyses were adjusted for clinical site and risk group (prior preeclampsia, chronic hypertension, or both). A sample size of 734 would provide 80% power to detect a 40% reduction in the risk of preeclampsia, assuming a placebo group rate of 21% and alpha=.05. The a level for the final analysis, adjusted for interim looks, was 0.0458.RESULTS: Outcome data for 707 of 739 randomly assigned patients revealed no significant reduction in the rate of preeclampsia (study drug, 13.8% [49 of 355] compared with placebo, 15.6% [55 of 352], adjusted risk ratio 0.87 [95.42% confidence interval 0.61-1.25]). There were no differences in mean gestational age at delivery or rates of perinatal mortality, abruptio placentae, pre-term delivery, and small for gestational age or low birth weight infants. Among patients without chronic hypertension, there was a slightly higher rate of severe preeclampsia in the study group (study drug, 6.5% [11 of 170] compared with placebo, 2.4% [4 of 168], exact P=.11, odds ratio 2.78, 95% confidence interval 0.79-12.62).CONCLUSION: This trial failed to demonstrate a benefit of antioxidant supplementation in reducing the rate of preeclampsia among'patients with chronic hypertension and/or prior preeclampsia.

The present study was undertaken to determine the importance of socioeconomic factors (family income), urban or rural family location, parity, maternal age, the presence of maternal and fetal pathologies as well as placental abnormalities on the weight and gestational age of 566 newborns. The highest incidence of newborns with low birth weight for gestational age was significantly more frequent in urban populations when the mothers were from low socio-economic levels. In mothers from low socioeconomic levels infants with low birth weight for gestational age were seen in greater proportion among primaparas and had a tendency to be higher in mothers aged less than 20 years; prematurity was highest in mothers ≥ 30 years old and significantly higher from the 8th gestation on. Maternal and fetal pathologies emphasized these characteristics and placental pathologies were not correlated with the distribution of weight and length of gestation. In mothers of high socioeconomic levels age and parity were not correlated with weight and gestational age of the newborns.

Maternal malnutrition was shown to affect early growth and leads to permanent alterations in insulin secretion and sensitivity of offspring. In addition, epidemiological studies showed an association between low birth weight and glucose intolerance in adult life. To understand these interactions better, we investigated the insulin secretion by isolated islets and the early events related to insulin action in the hind-limb muscle of adult rats fed a diet of 17% protein (control) or 6% protein [low (LP) protein] during fetal life, suckling and after weaning, and in rats receiving 6% protein during fetal life and suckling followed by a 17% protein diet after weaning (recovered). The basal and maximal insulin secretion by islets from rats fed LP diet and the basal release by islets from recovered rats were significantly lower than that of control rats. The dose-response curves to glucose of islets from LP and recovered groups were shifted to the right compared to control islets, with the half-maximal response (EC 50) occurring at 16.9 ± 1.3, 12.4 ± 0.5 and 8.4 ± 0.1 mmol/L, respectively. The levels of insulin receptor, as well as insulin receptor substrate-1 and phosphorylation and the association between insulin receptor substrate-1 and phosphatidylinositol 3-kinase were greater in rats fed a LP diet than in control rats. In recovered rats, these variables were not significantly different from those of the other two groups. These results suggest that glucose homeostasis is maintained in LP and recovered rats by an increased sensitivity to insulin as a result of alterations in the early steps of the insulin signal transduction pathway.

Iron deficiency is the commonest nutritional deficiency in the world. Although it affects adults, particularly women of reproductive age and during gestation, the most vulnerable group is children under two years of age. It affects mainly people living in developing countries, who have less access to balanced diets and health services and are exposed to precarious sanitary conditions. Iron deficiency has an impact on the immunity, adult work capacity and the cognitive development of children. Combating and preventing iron deficiency is one of the priorities in promoting public health. The strategies to achieve this end include the evaluation and correction of iron deficiency in pregnant women, the encouragement of breast feeding, oral iron supplementation in premature and newborn babies of low birth-weight, food enrichment and mobilization of the community.

Objective: To evaluate the results of the kangaroo mother method in Brazil. Methods: A prospective cohort study comparing 16 units that have or do not have the second phase of the kangaroo mother method: eight were national centers of excellence for the kangaroo mother method (study group) and eight were part of the Brazilian Neonatal Research Network (control group). A total of 985 newborn infants with birth weights of 500 to 1,749 g were enrolled. Multivariate analyses employedmultiple linear regression and Poisson regression with robust adjustment. Results: The adjusted analysis (controlled for birth weight, gestational age, Score for Neonatal Acute Physiology Perinatal Extension II, Neonatal Therapeutic Intervention Scoring System, and maternal age and educational level) demonstrated that mean length of hospital stay (p = 0.14) and intercurrent clinical conditions in the intermediate or kangaroo unit were equal for both groups. Weight (p = 0.012), length (p = 0.039) and head circumference (p = 0.006) at 36 weeks' corrected gestational age were all lower at the kangaroo units. The kangaroo units exhibited superior performance in relation to exclusive breastfeeding at discharge (69.2 vs. 23.8%, p=0.022). Conclusions: The evidence suggests that the humanization strategy adopted by the Brazilian Ministry of Health is a safe alternative to conventional treatment and a good strategy for promoting breastfeeding. Aims Maternal malnutrition by low protein diet is associated with an increased incidence of metabolic disorders and decreased male fertility in adult life. This study aimed to assess the impact of maternal protein malnutrition (MPM) on prostate growth, tissue organization and lesion incidence with aging. Main methods Wistar rat dams were distributed into two groups, which were control (NP; fed a normal diet containing 17% protein) or a restricted protein diet (RP, fed a diet containing 6% protein) during gestation. After delivery all mothers and offspring received a normal diet. Biometrical parameters, hormonal levels and prostates were harvested at post-natal days (PND) 30, 120 and 360. Key findings MPM promoted low birth weight, decreased ano-genital distance (AGD) and reduced androgen plasma levels of male pups. Prostatic lobes from RP groups presented reduced glandular weight, epithelial cell height and alveolar diameter. The epithelial cell proliferation and collagen deposition were increased in RP group. Incidences of epithelial dysplasia and prostatitis were higher in the RP offspring than in the NP offspring at PND360. Significance Our findings show that MPM delays prostate development, growth and maturation until adulthood, probably as a result of low testosterone stimuli. The higher incidence of cellular dysplasia and prostatitis suggests that MPM increases prostate susceptibility to diseases with aging. © 2013 Elsevier Inc.

O citomegalovírus é um vírus de DNA, pertencente à família Herpesviridae, subfamília Beta-herpesvirinae. Sua distribuição é universal e pode causar infecções congênitas e perinatais, assim como durante a infância e na idade adulta. É um dos principais patógenos responsáveis pela morbidade e mortalidade em pacientes imunocomprometidos. Foi estudada a incidência da infecção congênita pelo citomegalovírus, na maternidade da Fundação Santa Casa Misericórdia do Pará, no período de novembro de 1994 a maio de 1995. A amostra trabalhada constou de 663 recém-nascidos e suas respectivas mães. O peso dos recém-nascidos variou de 900 a 5450 g, com uma média de 3046g. Em 11,4% das crianças foi observado baixo peso ao nascer. A avaliação pelo isolamento do vírus da saliva dos 663 recém-nascidos, através da inoculação em células primárias de fibroblasto de prepúcio humano, mostrou 3,2% (21) de positividade. A pesquisa de anticorpos IgM específicos para o CMV, através do método ELISA, utilizando-se sangue do cordão umbilical do mesmo grupo de recém-nascidos foi positiva em 2,1% (14). Para o diagnóstico da infecção congênita pelo CMV, a análise estatística pelo Teste de McNemar dos Pares Discordantes (p)=0,0233 e Teste do Qui-Quadrado da Homogeneidade (p)<0,01 demonstrou que o isolamento do citomegalovírus da saliva foi mais sensível que a detecção de anticorpos IgM no sangue do cordão umbilical. Dos 21 recém-nascidos infectados, 28,5% (6) apresentaram nas primeiras 24 horas de vida, sintomatologia sugestiva de infecção congênita. Os sinais e sintomas encontrados foram microcefalia (4), prematuridade (3), hepatoesplenomegalia (2), pequeno para a idade gestacional (2) e icterícia precoce (1). A pesquisa de anticorpos IgG específicos para o CMV, pelo método ELISA, foi positiva em 90,2% das puérperas. Das 21 mulheres que transmitiram o vírus para o concepto, foram detectados anticorpos IgM em 4. A entrevista pessoal com as mães dos recém-nascidos revelou nível socioeconômico baixo e assistência pré-natal deficiente, com 26,4% das puérperas sem nenhuma consulta durante o período gestacional. Em nossa amostra, a idade materna variou de 12 a 42 anos, com idade média de 22,2 anos, sendo que a idade das mães dos recém-nascidos infectados foi inferior a 25 anos.

Objetivo: Identificar os determinantes da desnutrição infantil em crianças menores de dois anos de idade em populações ribeirinhas do Pará. Métodos: Estudo transversal foi desenvolvido com 203 crianças residentes em quatro comunidades ribeirinhas: Aveiro (região Sudoeste), Barcarena (região Metropolitana), Cametá (região Nordeste) e Santarém (região do Baixo Amazonas) por meio de entrevista junto ao responsável pela criança. A variável dependente foi desnutrição, considerada presente para índice estatura para idade < -1 escores z de acordo com a referência atual da Organização Mundial da Saúde. As variáveis independentes foram características: de moradia, do chefe da família, da mãe da criança, do pré-natal, do padrão alimentar da família, do nascimento da criança, dos cuidados maternos e demográficos da criança. A prevalência da desnutrição foi calculada conforme indicadores básicos, subjacentes e imediatas, considerando-se a distância entre as variáveis que compõem os indicadores e o desfecho. Análise multinível foi realizada por regressão logística tendo em conta a hierarquia das relações entre os indicadores e a desnutrição, considerando-se p<0,005. Resultados: A prevalência de desnutrição atingiu 35,0 % das crianças estudadas, variando de 28,6% em Aveiro a 43,1% em Barcarena. As variáveis que se associaram com desnutrição foram baixo peso ao nascer e maior idade da criança. A idade da criança foi o preditor da desnutrição: a chance de uma criança entre 12 e 17 meses de idade apresentar desnutrição foi 3,4 vezes maior do que a de uma criança com menos de seis meses, aumentando em cinco vezes para as crianças entre 18 e 23 meses. Conclusão: Em populações ribeirinhas, a desnutrição em menores de dois anos mostra-se ainda como um grave problema de saúde pública, possivelmente pelo maior tempo de exposição aos fatores de risco ambientais.

Technology advances and scientific studies in Neonatal Intensive Care Units (NICU) have contributed significantly to reduce mortality and morbidity of at-risk newborns (NB). However, they are more likely to present neurological and/or developmental psychomotor delay with neurological and sensory alterations. Therefore, proposals for neonatal intervention were developed with the aim of protecting the baby and offering appropriate incentives to minimize the effects of hospital intervention. To this end, programs of protective measures such as the Kangaroo Mother Care (KMC) were developed. Given the relevance of the issue described, this systematic review critically appraises articles from the national and international literature, published in recent years (from 2000 to 2011), that describe whether the KMC can be a protective factor for the development of writing in premature infants. The textual search was conducted using the Virtual Health Library (VHL), a website that covers publications worldwide, allowing access to articles from health science, including LILACS, IBECS, MEDLINE, Cochrane Library and SciELO, as database. The findings revealed that infants who participated in the KMC program showed improvements in their development and that factors such as low-birth-weight prematurity and learning disorders have close relationship with the onset of motor impairments and changes in psychomotor development. The findings showed no articles describing the KMC as a protective factor for the incidence of dysgraphia. Thus, we emphasize the importance of conducting further studies on these topics.

Maternal smoking during pregnancy causes heart changes, impaired fetal growth, spontaneous abortion and low birth weight. Even in pregnant women exposed to cigarette smoke these changes can be observed. But the exercise is to answer the increased fetal weight, besides the improvement of cardiorespiratory fitness of the pregnant and the fetus. The objective of this study was to analyze the ventricular muscle of young rats subjected to passive smoking associated with aquatic exercise. For the experiments used 24 female rats were divided into: GC (control), GE (exercise protocol in the water), GF(exposed to cigarette smoke) and GEF (exercise protocol on water and exposed to cigarette smoke). On the same day was held the 1st session of the experimental phase of the protocol of exposure to cigarette smoke, which consisted of 30 minutes twice a day, six days a week for three weeks, followed by the first session of the swimming program... Prematurity and low birth weight are public health problems. This paper aims to relate the occurrence of premature labors and low birth weight with adolescent pregnancy. We performed a literature review about the socioeconomic and cultural conditions of adolescent pregnancy and perinatal results. Through bibliographical data, we identified observational and systematic review articles that deal with the subject. The literature analysis permits us to conclude that adolescent pregnancies as well as the socioeconomic and cultural environment in which the young mother is inserted are associated with the increased frequency of prematures and low birth weight babies. The development and the implementation of effective public policies directed to that population are necessary to a better approach of the problem.
