Le taux de retour au travail après greffe est généralement bas. Or, on sait que le retour au travail après greffe améliore la qualité de vie des transplantés. Le but de notre étude était donc de comprendre les raisons possibles à ce faible taux en montrant les facteurs professionnels, individuels ou médicaux pouvant l'influencer. Parmi les 61 greffés rénaux ou hépatiques suivis au centre de transplantation d'organe (CTO) du CHUV, 39% ont repris le travail après greffe. Trois facteurs étaient significatifs de retour au travail après greffe, à savoir «travail avant greffe», «diplôme» et «âge < 45 ans». Ainsi, il est utile pour la pratique médicale de connaître les facteurs potentiels influençant le retour au travail car cela permet d'évaluer, au stade prégreffe, les chances de retour au travail et si besoin de proposer des mesures spécifiques le favorisant.

A nationwide survey was conducted in Switzerland to assess the quality level of osteoporosis management in patients aged 50 years or older presenting with a fragility fracture to the emergency ward of the participating hospitals. Eight centres recruited 4966 consecutive patients who presented with one or more fractures between 2004 and 2006. Of these, 3667 (2797 women, 73.8 years old and 870 men, 73.0 years old in average) were considered as having a fragility fracture and included in the survey. Included patients presented with a fracture of the upper limbs (30.7%), lower limbs (26.4%), axial skeleton (19.5%) or another localisation, including malleolar fractures (23.4%). Thirty-two percent reported one or more previous fractures during adulthood. Of the 2941 (80.2%) hospitalised women and men, only half returned home after discharge. During diagnostic workup, dual x-ray absorptiometry (DXA) measurement was performed in 31.4% of the patients only. Of those 46.0% had a T-score < or =-2.5 SD and 81.1% < or =-1.0 SD. Osteoporosis treatment rate increased from 26.3% before fracture to 46.9% after fracture in women and from 13.0% to 30.3% in men. However, only 24.0% of the women and 13.8% of the men were finally adequately treated with a bone active substance, generally an oral bisphosphonate, with or without calcium / vitamin D supplements. A positive history of previous fracture vs none increased the likelihood of getting treatment with a bone active substance (36.6 vs 17.9%, ? 18.7%, 95% CI 15.1 to 22.3, and 22.6 vs 9.9%, ? 12.7%, CI 7.3 to 18.5, in women and men, respectively). In Switzerland, osteoporosis remains underdiagnosed and undertreated in patients aged 50 years and older presenting with a fragility fracture.

Plusieurs études ont souligné que la qualité de l'attachement aurait un rôle à jouer dans les répercussions psychologiques d'événements potentiellement traumatiques, tels que l'abus sexuel. Cette étude se propose d'investiguer le rôle médiateur de l'attachement sur l'association entre abus sexuel, survenu dans l'enfance et/ou l'adolescence, et expériences dissociatives chez des femmes adultes. Deux groupes ont été comparés, l'un de 28 femmes ayant vécu des expériences d'abus sexuel dans l'enfance et l'autre composé de 15 femmes témoin qui n'ont pas connu ce type d'expérience. Les femmes qui ont vécu des abus sexuels pendant l'enfance obtiennent des scores significativement plus élevés de dissociation, en comparaison à des femmes témoin. Les femmes abusées ont des scores plus élevés d'attachement « insécure » et plus particulièrement « craintif» tels que définis dans le modèle de Bartholomew et Horowitz (1991). Finalement, les analyses de médiation, utilisant la procédure de Baron et Kenny (1986), indiquent que l'attachement « craintif» a un effet médiateur sur le lien entre abus sexuel et dissociation. Cette forme d'attachement représenterait ainsi un facteur de vulnérabilité pour des troubles dissociatifs chez des individus confrontés à des événements potentiellement traumatisants.

Background: Population-based cohort studies of risk factors of stroke are scarce in developing countries and none has been done in the African region. We conducted a longitudinal study in the Seychelles (Indian Ocean, east of Kenya), a middle-income island state where the majority of the population is of African descent. Such data in Africa are important for international comparison and for advocacy in the region. Methods: Three examination surveys of cardiovascular risk factors were performed in independent samples representative of the general population aged 25-64 in 1989, 1994 and 2004 (n=1081, 1067, and 1255, respectively). Baseline risk factors data were linked with cause-specific mortality from vital statistics up to May 2007 (all deaths are medically certified in the Seychelles and kept in an electronic database). We considered stroke (any type) as a cause of death if the diagnosis was reported in any of the 4 fields in the death certificates for underlying and concomitant causes of death. Results. Among the 2479 persons aged 35-64 at baseline, 280 died including 56 with stroke during follow up (maximum: 18.2 years; mean: 10.2 years). In this age range, age-adjusted mortality rates (/100'000/year) were 969 for all cause and 187 for stroke; age-adjusted prevalence of high blood pressure (≥140/90 mmHg) was 48%. In multivariate Cox survival time regression, stroke mortality was increased by 18% and 35% for a 10-mmHg increase in systolic, respectively diastolic BP (p<0.001). Stroke mortality was also associated with age, smoking ≥5 cigarettes vs. no smoking (HR: 2.4; 95% CI: 1.2-4.8) and diabetes (HR: 1.9; 1.02-3.6) but not with sex, LDL-cholesterol intake, alcohol intake and professional occupation. Conclusion. This first population-based cohort study in the African region demonstrates high mortality rates from stroke in middle-aged adults and confirms associations with high BP and other risk factors. This emphasizes the importance of reducing BP and other modifiable risk factors in high risk individuals and in the general population as a main strategy to reduce the burden of stroke.

Les conclusions que l'on peut tirer après dix mois d'une campagne de prévention intensive, sont les suivantes: -> la campagne a atteint sa cible, la population générale et les groupes exposés, après une bonne diffusion et amplification par les médias; -> elle n'a pas donné lieu à l'expression de fortes résistances ou d'oppositions; -> des phénomènes multiplicateurs (actions locales, répercussions par les leaders de groupes, etc) sont apparus et contribuent à renforcer la campagne elle-même; -> la vente de préservatifs a augmenté; -> on observe des changements d'attitudes et de comportements dans le sens d'une meilleure protection dans la population générale aussi bien que dans les groupes observés, choisis spécifiquement pour leur probabilité plus grande d'adopter des comportements à risque (jeunes, homosexuels, toxicomanes...), ce qui permet de supposer que ces tendances se généraliseront, pour autant que les campagnes se poursuivent sans relâche et sans dérapage. BACKGROUND: Polyomavirus-associated nephropathy (PVAN) is a serious complication and cause of graft loss in kidney transplant recipients. In the absence of specific antiviral drugs, early detection of the disease and reduction of immunosuppressive regimen is the cornerstone of therapy. Cidofovir, a nucleoside analogue, has been found to inhibit BK virus (BKV) replication in vitro and has been proposed as treatment of refractory PVAN at low doses; however, its efficacy has never been demonstrated in randomized controlled trials. METHODS: Cidofovir therapy (0.5 mg/kg at a 2-week interval for eight consecutive doses) was initiated in two patients with biopsy-proven PVAN and persistent BKV DNA viraemia (> or = 10,000 copies/ml despite sustained reduction of the immunosuppressive regimen). In addition to these two case reports, we performed a critical review of the literature on the use of cidofovir in PVAN. RESULTS: No significant decrease of BKV viral load in blood was observed during cidofovir therapy and in follow-up of the two patients treated with cidofovir. Our literature review identified 21 publications reporting the use of cidofovir for the treatment of PVAN. All were case reports or small series. The efficacy of cidofovir therapy could not be assessed in 17 of these publications because of lack of data or concomitant reduction of immunosuppressive regimen. The four remaining publications were case reports. CONCLUSIONS: In vitro and clinical data to support the efficacy of cidofovir in the treatment of PVAN are currently lacking. More promising compounds should be identified for further clinical studies.

BACKGROUND: The goal of this paper is to investigate the respective influence of work characteristics, the effort-reward ratio, and overcommitment on the poor mental health of out-of-hospital care providers. METHODS: 333 out-of-hospital care providers answered a questionnaire that included queries on mental health (GHQ-12), demographics, health-related information and work characteristics, questions from the Effort-Reward Imbalance Questionnaire, and items about overcommitment. A two-level multiple regression was performed between mental health (the dependent variable) and the effort-reward ratio, the overcommitment score, weekly number of interventions, percentage of non-prehospital transport of patients out of total missions, gender, and age. Participants were first-level units, and ambulance services were second-level units. We also shadowed ambulance personnel for a total of 416 hr. RESULTS: With cutoff points of 2/3 and 3/4 positive answers on the GHQ-12, the percentages of potential cases with poor mental health were 20% and 15%, respectively. The effort-reward ratio was associated with poor mental health (P < 0.001), irrespective of age or gender. Overcommitment was associated with poor mental health; this association was stronger in women (β = 0.054) than in men (β = 0.020). The percentage of prehospital missions out of total missions was only associated with poor mental health at the individual level. CONCLUSIONS: Emergency medical services should pay attention to the way employees perceive their efforts and the rewarding aspects of their work: an imbalance of those aspects is associated with poor mental health. Low perceived esteem appeared particularly associated with poor mental health. This suggests that supervisors of emergency medical services should enhance the value of their employees' work. Employees with overcommitment should also receive appropriate consideration. Preventive measures should target individual perceptions of effort and reward in order to improve mental health in prehospital care providers.

Cyst-based ecotoxicological tests are simple and low-cost methods for assessing acute toxicity. Nevertheless, only a few comparative studies on their sensitivity are known. In the present study, the suitability of the use of two freshwater Anostracan species, Streptocephalus rubricaudatus and S. texanus, was assessed. The impact of 16 priority pollutants (4 heavy metals, 11 organic, and 1 organometallic compounds) on these two species, as well as on Artemia salina (Artoxkit M), Daphnia magna (International Organization for Standardization 6341), and S. proboscideus (Streptoxkit F) was assessed. For indicative comparison, bioassays using Brachionus calyciflorus (Rotoxkit F) and Photobacterium phosphoreum (Microtox) were also performed. For heavy metals (K2Cr2O7, Cd2+, Zn2+, Cu2+), the sensitivity of the two studied Streptocephalus species was slightly higher than that of D. magna. It was significantly more elevated than for the marine A. salina. For organic and organometallic micropollutants [phenol, 3,5-dichlorophenol, pentachlorophenol (PCP), hydroquinone, linear alkylbenzene sulfonate, sodium dodecyl sulfate, tributylphosphate, dimethylphthalate, atrazine, lindane, malathion, tributyltin chloride (TBT-Cl)], the sensitivity of the 4 anostracan species was of the same order of magnitude as that of D. magna. Artemia salina was slightly less sensitive to some organic compounds (PCP, hydroquinone, TBT-Cl). The sensitivity of S. rubricaudatus to organic solvents was low. On the other hand, this anostracan was quite sensitive to NaCl. Thus, its use is restricted to freshwater samples. The evaluation of global practicability of these two tests confirms that cyst-based freshwater anostracans may be used to perform low-cost tests at a sensitivity comparable to that of D. magna (24 h immobilization test).

Sequentially along B cell differentiation, the different classes of membrane Ig heavy chains associate with the Ig alpha/Ig beta heterodimer within the B cell receptor (BCR). Whether each Ig class conveys specific signals adapted to the corresponding differentiation stage remains debated. We investigated the impact of the forced expression of an IgA-class receptor throughout murine B cell differentiation by knocking in the human C alpha Ig gene in place of the S mu region. Despite expression of a functional BCR, homozygous mutant mice showed a partial developmental blockade at the pro-B/pre-BI and large pre-BII cell stages, with decreased numbers of small pre-BII cells. Beyond this stage, peripheral B cell compartments of reduced size developed and allowed specific antibody responses, whereas mature cells showed constitutive activation and a strong commitment to plasma cell differentiation. Secreted IgA correctly assembled into polymers, associated with the murine J chain, and was transported into secretions. In heterozygous mutants, cells expressing the IgA allele competed poorly with those expressing IgM from the wild-type allele and were almost undetectable among peripheral B lymphocytes, notably in gut-associated lymphoid tissues. Our data indicate that the IgM BCR is more efficient in driving early B cell education and in mucosal site targeting, whereas the IgA BCR appears particularly suited to promoting activation and differentiation of effector plasma cells.
