O comprometimento pulmonar é uma das manifestações extra-articulares da artrite reumatóide e pode ser devido à própria doença ou secundário às medicações utilizadas para seu tratamento. Este trabalho relata um caso de uma paciente de 60 anos de idade com diagnóstico de artrite reumatoide que evoluiu com múltiplos nódulos pulmonares durante o tratamento com leflunomida.

The present invention relates to phthalimide derivatives of non-steroidal and/or TNF-+- modulating anti-inflammatory compounds as well as the process of obtaining the so-called derivatives, pharmaceutical compositions containing such derivatives and their uses, including use in the treatment of inflammatory diseases, especially those related to chronic inflammatory processes, such as rheumatoid arthritis and intestinal inflammatory diseases (for instance, Chron's disease) and the use of the referred to pharmaceutical compositions as antipyretic, analgesic and platelet antiaggregating medications.

This study isolated and quantified intestinal bacteria of children with cleft palate before and after palatoplasty. A prospective study was conducted from May 2007 to September 2008 on 18 children with cleft palate, aged one to four years, of both genders, attending a tertiary cleft center in Brazil for palatoplasty, to analyze the effect of surgical palate repair on the concentration of anaerobes Bacteroides sp, Bifidobacterium sp and microaerophiles Lactobacillus sp in feces of infants with cleft palate before and 24 hours after treatment with cefazolin for palatoplasty. There was significant reduction of Lactobacillus sp (p < 0.002), Bacteroides sp (p < 0.001) and Bifidobacterium sp (p = 0.021) after palatoplasty, revealing that surgery and utilization of cefazolin significantly influenced the fecal microbiota comparing collections before and after surgery. However, due to study limitations, it was not possible to conclude that other isolated factors, such as surgical stress, anesthetics and other medications used in palatoplasty might have a significant influence on the microbiota. Considering the important participation of the intestinal microbiota on both local and systemic metabolic and immunological activities of the host, professionals should be attentive to the possible influence of these changes in patients submitted to cleft repair.

Reports on the clinical course of mycophenolic acid (MPA)-related colitis in kidney transplant recipients are scarce. This study aimed at assessing MPA-related colitis incidence, risk factors, and progression after kidney transplantation. All kidney transplant patients taking MPA who had colonic biopsies for persistent chronic diarrhea, between 2000 and 2012, at the Kidney Transplantation Unit of Botucatu Medical School Hospital, Brazil, were included. Cytomegalovirus (CMV) immunohistochemistry was performed in all biopsy specimens. Data on presenting symptoms, medications, immunosuppressive drugs, colonoscopic findings, and follow-up were obtained. Of 580 kidney transplant patients on MPA, 34 underwent colonoscopy. Colonoscopic findings were associated with MPA usage in 16 patients. The most frequent histologic patterns were non-specific colitis (31.3%), inflammatory bowel disease (IBD)-like colitis (25%), normal/near normal (18.8%), graft-versus-host disease-like (18.8%), and ischemia-like colitis (12.5%). All patients had persistent acute diarrhea and weight loss. Six of the 16 MPA-related diarrhea patients (37.5%) showed acute dehydration requiring hospitalization. Diarrhea resolved when MPA was switched to sirolimus (50%), discontinued (18.75%), switched to azathioprine (12.5%), or reduced by 50% (18.75%). No graft loss occurred. Four patients died during the study period. Late-onset MPA was more frequent, and no correlation with MPA dose or formulation was found.

Background: Rheumatic diseases in children are associated with significant morbidity and poor health-related quality of life (HRQOL). There is no health-related quality of life (HRQOL) scale available specifically for children with less common rheumatic diseases. These diseases share several features with systemic lupus erythematosus (SLE) such as their chronic episodic nature, multi-systemic involvement, and the need for immunosuppressive medications. HRQOL scale developed for pediatric SLE will likely be applicable to children with systemic inflammatory diseases.Findings: We adapted Simple Measure of Impact of Lupus Erythematosus in Youngsters (SMILEY (c)) to Simple Measure of Impact of Illness in Youngsters (SMILY (c)-Illness) and had it reviewed by pediatric rheumatologists for its appropriateness and cultural suitability. We tested SMILY (c)-Illness in patients with inflammatory rheumatic diseases and then translated it into 28 languages. Nineteen children (79% female, n= 15) and 17 parents participated. The mean age was 12 +/- 4 years, with median disease duration of 21 months (1-172 months). We translated SMILY (c)-Illness into the following 28 languages: Danish, Dutch, French (France), English (UK), German (Germany), German (Austria), German (Switzerland), Hebrew, Italian, Portuguese (Brazil), Slovene, Spanish (USA and Puerto Rico), Spanish (Spain), Spanish (Argentina), Spanish (Mexico), Spanish (Venezuela), Turkish, Afrikaans, Arabic (Saudi Arabia), Arabic (Egypt), Czech, Greek, Hindi, Hungarian, Japanese, Romanian, Serbian and Xhosa.Conclusion: SMILY (c)-Illness is a brief, easy to administer and score HRQOL scale for children with systemic rheumatic diseases. It is suitable for use across different age groups and literacy levels. SMILY (c)-Illness with its available translations may be used as useful adjuncts to clinical practice and research.

Introduction: Physical inactivity is identified as the fourth greatest risk factor of mortality worldwide. Little is known about how physical inactivity alters the demand for use of primary health care services, and it is a subject which demands further investigation. Objective: This study aims to determine the influence that physical activity has on the demand for use of primary health care services. Methods: This is a retrospective and cross-sectional study. The Rio Claro Active Health Program (SARC), studied in this research, is a partnership between the Municipal Health Foundation and Universidade Estadual Paulista Julio de Mesquita Filho (UNESP), represented by the Nucleus of Physical Activity, Sport and Health (NAFES) department of Physical Education and coordinated by Prof. Dr. Eduardo Kokubun. The study was carried out on residents of Rio Claro of both sexes seen at SARC. To assess the influence that physical activity had on the demand of the use of health services a questionnaire was given to participants who had attended the program since 2009. This questionnaire contains questions concerning the level of physical activity, health service use, number of both blood pressure and glucose measurement takings, number of spontaneous and scheduled medical visits, number of medications taken, number of illnesses and hospitalizations, comparing the data found in the previous year and the year after the program began. An informed term of consent was used for research participants. Descriptive analysis was carried out, using frequency, mean, maximum and minimum standard deviation. The purpose of this study was to evaluate in vitro the antimicrobial activity of intracanal medications against Candida albicans, Enterococcus faecalis, Escherichia coli present in root canals. It was used 24 single root human teeths, contaminated for 28 days and prepared with physiological saline solution as irrigation solution. The teeth were divided into 2 groups according to the intracanal medication used: 1) 2% gel chlorhexidine, 2) sterile and pyrogen free physiological saline solution. Samples were taken of the root canals immediately after instrumentation, 7 days after intracanal medication and 7 days after removal of intracanal medication. For all samples the antimicrobial activity was performed by plating method. All results were submitted to Mann-Whitney and Dunn's test with significance of 5%. There was significant reduction of microorganisms after instrumentation and the intracanal medication of 2% gel chlorhexidine completely eliminated C. albicans and E. coli, and significantly reduced E. faecalis. It was concluded that 2% gel chlorhexidine as intracanal medication for 7 days was effective on microorganisms
