In the US, one in every eight deaths is due to an obesity-related chronic health condition (ORCHC). More than half of African American women (AAW) 20 years old or older are obese or morbidly obese, as are 63% of menopausal AAW. Many have ORCHC that increase their morbidity and mortality and increase health care costs. In 2013, 42.6 percent of AAs living in South Carolina (SC) were obese. The purpose of this cross-sectional study was to identify the cognitive, behavioral, biological, and demographic factors that influence health outcomes (BMI, and ORCHC) of AAW living in rural SC. A sample of 200 AAW (50 in each of the 4 groups of rurality by menopausal status), 18-64 years, completed the: Menopausal Rating Scale (symptoms); Body Image Assessment for Obesity (self-perception of body); Mental Health Inventory; Block Food Frequency Questionnaire; Eating Behaviors and Chronic Conditions, Traditional Food Habits, and Food Preparation Technique questionnaires – and measures for Body Mass Index. Most rural, and premenopausal AAW were single and not living with a partner. Premenopausal women had significantly higher educational levels. Sixty percent of AAW had between 1 and 5 ORCHC. Most AAW used salt based seasonings, ate deep fried foods 1 to 3 times a week, and ate outside the home 1 to 3 times a month. Few AAW knew the correct daily serving for grains and dairy, and most consumed less than the recommended daily serving of fruits, vegetables and dairy. Morbidly obese AAW used more traditional food preparation techniques than obese and normal-weight AAW. Rural, and menopausal AAW had significantly higher morbid obesity levels, consumed larger portions of meats and vegetables, and reported more body image dissatisfaction than very rural AAW, and premenopausal AAW, respectively. Controlling for socioeconomic factors the relationships between perceptions of body images, psychological distress, and psychological wellbeing remained significant for numbers of ORCHC

Issues of body image and ability to achieve intimacy are connected to body weight, yet remain largely unexplored and have not been evaluated by gender. The underlying purpose of this research was to determine if avoidant attitudes and perceptions of one’s body may hold implications toward its use in intimate interactions, and if an above average body weight would tend to increase this avoidance. The National Health and Nutrition Examination Survey (NHANES, 1999-2002) finds that 64.5% of US adults are overweight, with 61.9% of women and 67.2% of men. The increasing prevalence of overweight and obesity in men and women shows no reverse trend, nor have prevention and treatment proven effective in the long term. The researcher gathered self-reported age, gender, height and weight data from 55 male and 58 female subjects (determined by a prospective power analysis with a desired medium effect size (r =.30) to determine body mass index (BMI), determining a mean age of 21.6 years and mean BMI of 25.6. Survey instruments consisted of two scales that are germane to the variables being examined. They were (1) Descutner and Thelen of the University of Missouri’s (1991) Fear-of-Intimacy scale and (2) Rosen, Srebnik, Saltzberg, and Wendt’s (1991) Body Image Avoidance Questionnaire. Results indicated that as body mass index increases, fear of intimacy increases (p<0.05) and that as body mass index increases, body image avoidance increases (p<0.05). The relationship that as body image avoidance increases, fear of intimacy increases was not supported, but approached significance at (p<0.07). No differences in these relationships were determined between gender groups. For age, the only observed relationship was that of a difference between scores for age groups [18 to 22 (group 1) and ages 23 to 34 (group 2)] for the relationship of body image avoidance and fear of intimacy (p<0.02). The results suggest that the relationship of body image avoidance and fear of intimacy, as well as age, bear consideration toward the escalating prevalence of overweight and obesity. An integrative approach to body weight that addresses issues of body image and intimacy may prove effective in prevention and treatment.

Who analyses children’s screen content and media use in Arab countries, and with what results? Children, defined internationally as under-18s, account for some 40 per cent of Arab populations and the proportion of under-fives is correspondingly large. Yet studies of children’s media and child audiences in the region are as scarce as truly popular locally produced media content aimed at children. At the very time when conflict and uncertainty in key Arab countries have made local development and diversification of children’s media more remote, it has become more urgent to gain a better understanding of how the next generation’s identities and world-views are formed. This interdisciplinary book is the first in English to probe both the state of Arab screen media for children and the practices of Arabic-speaking children in producing, as well as consuming, screen content. It responds to the gap in research by bringing together a holistic investigation of institutions and leading players, children’s media experiences and some iconic media texts. With children’s media increasingly linked to merchandising, which favours US-based global players and globalizing forces, this volume provides a timely insight into tensions between differing concepts of childhood and desirable media messages.

Background
Temper outbursts are prevalent in individuals with PWS and are often triggered by unexpected changes to routines or plans. However, such outbursts are also common in individuals with several other neurodevelopmental disorders, including those with a diagnosis of autism spectrum disorder (ASD). We compared the profile of temper outbursts in children with PWS to that in children with ASD. We examined whether differences in the temper outburst profile predicted differences in the outcomes of two caregiver led intervention strategies aiming to reduce change triggered outbursts.
Methods and results
Thirteen 7-15 year olds with PWS – taking part in a larger study involving 60 children evidencing temper outbursts following changes – were individually matched for age to children with ASD (mean ages: 10.70; 10.76 yrs). Caregivers participated in a structured/semi-structured interview on children's outbursts; completed a web-based outburst diary over a 6 month baseline; and are currently using either a change signalling intervention to reliably warn children of forthcoming changes; or a planning ahead intervention to reduce children's exposure to unexpected changes.
As reported at interview, on average, children with PWS showed more frequent temper outbursts than those with ASD (closer to daily vs. weekly). For seven children with PWS and six with ASD, 60% or more of their temper outbursts were reported to be triggered by changes. Whilst outbursts had similar durations when triggered by changes or by other events in children with PWS; change triggered outbursts in children with ASD were generally shorter. The most commonly reported outburst components in children with PWS included indicators of heightened emotional arousal but this was not the case for children with ASD. Data on behavioural change associated with each of the intervention strategies will be discussed.
Conclusions
Change triggered temper outbursts can be a problem for children PWS and ASD, however subtle differences appear to exist in the profile of these outbursts. Some of these differences may be relevant for the expected efficacy of different behavioural intervention strategies that target outbursts.
Summary
Temper outbursts (tantrums) were compared in children with PWS or autism spectrum disorder before and during use of one of two helping strategies. Helping strategies were led by caregivers and aimed to reduce outbursts that follow changes to routines or plans by making such changes more predictable, or by reducing the quantity of changes. Characteristics of outbursts may be important to help us predict which helping strategies may be most effective.

The maintenance of normal body weight is disrupted in patients with anorexia nervosa (AN) for prolonged periods of time. Prior to the onset of AN, premorbid body mass index (BMI) spans the entire range from underweight to obese. After recovery, patients have reduced rates of overweight and obesity. As such, loci involved in body weight regulation may also be relevant for AN and vice versa. Our primary analysis comprised a cross-trait analysis of the 1000 single-nucleotide polymorphisms (SNPs) with the lowest P-values in a genome-wide association meta-analysis (GWAMA) of AN (GCAN) for evidence of association in the largest published GWAMA for BMI (GIANT). Subsequently we performed sex-stratified analyses for these 1000 SNPs. Functional ex vivo studies on four genes ensued. Lastly, a look-up of GWAMA-derived BMI-related loci was performed in the AN GWAMA. We detected significant associations (P-values <5 × 10(-5), Bonferroni-corrected P<0.05) for nine SNP alleles at three independent loci. Interestingly, all AN susceptibility alleles were consistently associated with increased BMI. None of the genes (chr. 10: CTBP2, chr. 19: CCNE1, chr. 2: CARF and NBEAL1; the latter is a region with high linkage disequilibrium) nearest to these SNPs has previously been associated with AN or obesity. Sex-stratified analyses revealed that the strongest BMI signal originated predominantly from females (chr. 10 rs1561589; Poverall: 2.47 × 10(-06)/Pfemales: 3.45 × 10(-07)/Pmales: 0.043). Functional ex vivo studies in mice revealed reduced hypothalamic expression of Ctbp2 and Nbeal1 after fasting. Hypothalamic expression of Ctbp2 was increased in diet-induced obese (DIO) mice as compared with age-matched lean controls. We observed no evidence for associations for the look-up of BMI-related loci in the AN GWAMA. A cross-trait analysis of AN and BMI loci revealed variants at three chromosomal loci with potential joint impact. The chromosome 10 locus is particularly promising given that the association with obesity was primarily driven by females. In addition, the detected altered hypothalamic expression patterns of Ctbp2 and Nbeal1 as a result of fasting and DIO implicate these genes in weight regulation.

Bakgrund: Övervikt och fetma bland barn och ungdomar har ökat kraftigt under de senaste 20 åren. I Sverige har förekomsten av fetma ökat från 1 % till 4 %. Det finns flertalet orsaker till övervikt och fetma hos barn och ungdomar. Några av dem är genetiska faktorer, livsmedelskonsumtion, miljöfaktorer samt livsstilsfaktorer. Syfte: Att beskriva de faktorer som påverkar vårdmötet mellan sjuksköterskan och föräldrar till barn och ungdomar med övervikt eller fetma. Metod: Studien har genomförts som en litteraturöversikt och bestod av 13 artiklar. 10 med kvalitativ ansats och tre med kvantitativ ansats. Dessa hämtades i databaserna Cinahl och PubMed. Resultat: Det identifierades både hinder och möjligheter som kan uppstå i vårdmötet mellan sjuksköterskan och föräldrar. Dessa hinder och möjligheter presenteras i två kategorier med totalt fem subkategorier; Skuld och skamkänslor hos föräldrarna, förnekelse hos föräldrarna angående barnets vikt, öka medvetenheten om fördelarna med viktminskning, långsiktigt förtroende underlättar vårdmötet och anpassad kommunikation i vårdmötet mellan sjuksköterska och förälder. Slutsats: Flera faktorer identifierades som har betydelse för vårdmötet mellan sjuksköterska och föräldrar. De faktorer som utgjorde hinder i vårdmötet kan båda parter påverka så att förbättringar kan ske.

This Q and A style briefing paper provides an overview of the use of body mass index (BMI) for the measurement of obesity. BMI is a person’s weight in kilograms divided by the square of their height in metres. It is one of the most commonly used ways of estimating whether a person is overweight and hence more likely to experience health problems than someone with a healthy weight. It is used to measure population prevalence of overweight and obesity. It is also a relatively easy, cheap and non-invasive method for establishing weight status. However, BMI is only a proxy for body fatness. Factors such as fitness, ethnic origin and puberty can alter the relation between BMI and body fatness and must be taken into consideration. Other measurements such as waist circumference and skin thickness can be collected to indicate a person’s weight status or body fatness. None of these is as widely used as BMI.

This report was commissioned by the Department of Health, Ireland. Obesity is a public health problem in Ireland which is largely responsible for the increasing prevalence of diet-related diseases and growing financial burden on our healthcare system. Although overweight and obesity rates may have reached a plateau in Irish adults and children, they remain at an extremely high level as 1 in 4 children areoverweight or obese and an estimated 61% of adults are overweight or obese. Urgent public health action is required to reduce the levels of obesity among our children and adults. A sustainable national intervention strategy that combines government and community-led interventions is required. These interventions need to incorporate both nutrition education and environmental modification strategies to reduce levels of obesity. International literature suggests that calorie posting has the potential to have a positive effect on the obesity crisis by encouraging people to make healthier food choices through informed consumer decisions. This evaluation focuses on the uptake of voluntary calorie posting from a national representative sample of food service businesses in Ireland and explores the attitudes of food service businesses that do and do not display calories. This evaluation will explore the most effective and efficient way of implementing mandatory calorie posting on menus in Ireland.

Excess deaths from cardiovascular disease are a major contributor to the significant reduction in life expectancy experienced by people with schizophrenia. Important risk factors in this are smoking, alcohol misuse, excessive weight gain and diabetes. Weight gain also reinforces service users’ negative views of themselves and is a factor in poor adherence with treatment. Monitoring of relevant physical health risk factors is frequently inadequate, as is provision of interventions to modify these. These guidelines review issues surrounding monitoring of physical health risk factors and make recommendations about an appropriate approach. Overweight and obesity, partly driven by antipsychotic drug treatment, are important factors contributing to the development of diabetes and cardiovascular disease in people with schizophrenia. There have been clinical trials of many interventions for people experiencing weight gain when taking antipsychotic medications but there is a lack of clear consensus regarding which may be appropriate in usual clinical practice. These guidelines review these trials and make recommendations regarding appropriate interventions. Interventions for smoking and alcohol misuse are reviewed, but more briefly as these are similar to those recommended for the general population. The management of impaired fasting glycaemia and impaired glucose tolerance (‘pre-diabetes’), diabetes and other cardiovascular risks, such as dyslipidaemia, are also reviewed with respect to other currently available guidelines. These guidelines were compiled following a consensus meeting of experts involved in various aspects of these problems. They reviewed key areas of evidence and their clinical implications. Wider issues relating to primary care/secondary care interfaces are discussed but cannot be resolved within guidelines such as these.

Background. Duodenal injuries are rare in children and classically present following a fall over the handle bar. Retroperitoneal location of the duodenum may lead to delay in diagnosis, and missed injuries are associated with increased morbidity and mortality. Case report. A 5-year-old child was admitted to the National Trauma Center, in Tirana (Albania), 28 hours after a Motor Vehicle Crash (MVC), complaining of mild abdominal pain. He was febrile (39°C) and had a white blood cells count of 18,000 mm3. On physical exam he had mild tenderness. Plain abdominal X-rays and Focused Abdominal Sonography for Trauma (FAST) were negative for free air or free fluid. The CT scan of the abdomen demonstrated free air and fluid in the retroperitoneal space. At laparatomy, a perforation of the second portion of the duodenum was found. A single layer suture repair of the duodenum with wide drainage was performed. The patient was discharged from the hospital tolerating oral feeding 8 days later. Conclusion. Duodenal injuries in children are rare. Most duodenal hematomas are managed non-operatively. This is a case of MCV with delayed presentation that was treated surgically for perforation successfully.

Résumé: Le surpoids et l’obésité dans la population pédiatrique sont des préoccupations grandissantes à l’échelle mondiale. Actuellement, au Canada, près de 21 % des jeunes Canadiens âgés de 2 à 5 ans présentent un surpoids et malheureusement, 6 % d’entre eux souffrent d’obésité. De plus, 80 % de ces enfants risquent d’être obèses à l’âge adulte, ce qui mène à plusieurs impacts sur la santé. Afin de prévenir l’obésité infantile, il est important d’identifier des facteurs de risques, notamment ceux se produisant tôt dans la vie. Plusieurs études ont démontré l’importance de l’environnement fœtal dans l’établissement de la santé métabolique à long terme. Le poids à la naissance a souvent été utilisé comme marqueur de l’exposition prénatale. Cependant, le poids à la naissance n’est qu’un marqueur grossier. L’adiposité à la naissance a été identifiée comme un facteur de risque plus important puisqu’elle permet de prédire de l’adiposité durant l’enfance. Les deux déterminants maternels majeurs de la croissance fœtale sont le statut pondéral et la glycémie maternelle. Récemment, une adipokine a été suggérée comme un déterminant potentiel dans la programmation fœtale de l’obésité. La leptine, qui est produite par les adipocytes, joue un rôle important dans la balance énergétique, mais elle semble aussi importante dans le développement de l’obésité postnatale. Durant la grossesse, le placenta produit une large quantité de leptine et la majorité est sécrétée du côté maternel. Appuyés par le fait que la leptine maternelle circulante est le reflet de la sécrétion placentaire de leptine, nous avons émis l’hypothèse que la leptine maternelle serait associée à l’adiposité du nouveau-né, et ce, indépendamment de la glycémie maternelle. Nous avons étudié la leptine durant l’hyperglycémie provoquée par voie orale (HGPO) chez les femmes enceintes au 2e trimestre. Nous avons montré, chez les femmes en surpoids ou obèse, qu’une plus haute leptine maternelle était lié à une adiposité néonatale augmentée à la naissance. D’un autre côté, chez les femmes minces, une glycémie élevée était liée à une adiposité néonatale augmentée. Ces associations sont indépendantes de la parité, du statut tabagique, du gain de poids durant la grossesse, des triglycérides maternels, du mode d’accouchement, du sexe du nouveau-né et de l’âge gestationnel à la naissance. Ces résultats suggèrent une régulation différentielle entre ces deux marqueurs métaboliques maternels et l’adiposité néonatale, selon le statut pondéral pré-grossesse.

Este estudo caracteriza a incidência do excesso de peso e da obesidade no Externato João Alberto Faria (Arruda dos Vinhos), procurando identificar as variáveis que os determinam e definir um projecto de intervenção na área da educação para a saúde no âmbito dos estilos de vida saudáveis e do combate ao excesso de peso e à obesidade. Dos 432 alunos que participaram no estudo, 53,7°/o eram do sexo feminino e 46,3% do sexo masculino. Os resultados relativos ao índice de Massa Corporal (IMC) dos alunos indicaram que, na população em estudo, 31,7% dos alunos apresentavam peso em excesso, dos quais 10,5% eram obesos. A prevalência de excesso de peso e obesidade era superior nos alunos mais novos e nas raparigas, no entanto os rapazes eram mais obesos. Em virtude dos resultados, identificaram-se como áreas de intervenção, a melhoria dos hábitos alimentares, o aumento dos níveis de actividade física e a diminuição das catividades de lazer sedentárias dos alunos, bem como intervenções ao nível das crenças, atitudes e comportamentos dos alunos e dos pais. O projecto de intervenção apresentado tem como objectivo reduzir a prevalência do excesso de peso e obesidade infantil através da aquisição e desenvolvimento de atitudes e comportamentos que visem a promoção e adopção de hábitos e estilos de vida saudáveis. É sabido que mudar atitudes e comportamentos, nomeadamente, os que dizem respeito a mudança de estilos de vida, é um processo difícil e que leva tempo, sendo fundamental uma abordagem integrada que afecte as várias dimensões das causas do problema em que se pretende intervir. ABSTRACT; This study characterizes the incidence of overweight and obesity in Externato João Alberto Faria (Arruda dos Vinhos). lts aim was to identify the variables that causes it and to define a project of intervention in the health educational field related to healthy lifestyles and against overweight and childhood obesity. 432 students participated in the survey: 53,7% females and 46,3% males. The results as far as students Body Mass Index are concerned show that within the surveyed people 31,7% were overweight and 10,5% were obese. Overweight and obesity have more evidence in the younger students and in the girls' group, despite the fact that boys were more obese. Such results lead to the intervention in certain areas: improving eating habits, increasing physical activity, diminishing sedentary hobbies and also interfering in the beliefs and attitudes of children and their parents. The intervention project presented has the following goal: to reduce the prevalence of overweight and childhood obesity through the acquisition and development of attitudes and behaviours witch aim at de promotion and adoption of healthy habits and lifestyles. Changing attitudes and behaviours related to the improving of lifestyles is difficult and takes time. Therefore, the need for an integrated approach is crucial in order to affect the several causes of the problem in witch we want to intervene.

The main aim of the research project "On the Contribution of Schools to Children's Overall Indoor Air Exposure" is to study associations between adverse health effects, namely, allergy, asthma, and respiratory symptoms, and indoor air pollutants to which children are exposed to in primary schools and homes. Specifically, this investigation reports on the design of the study and methods used for data collection within the research project and discusses factors that need to be considered when designing such a study. Further, preliminary findings concerning descriptors of selected characteristics in schools and homes, the study population, and clinical examination are presented. The research project was designed in two phases. In the first phase, 20 public primary schools were selected and a detailed inspection and indoor air quality (IAQ) measurements including volatile organic compounds (VOC), aldehydes, particulate matter (PM2.5, PM10), carbon dioxide (CO2), carbon monoxide (CO), bacteria, fungi, temperature, and relative humidity were conducted. A questionnaire survey of 1600 children of ages 8-9 years was undertaken and a lung function test, exhaled nitric oxide (eNO), and tear film stability testing were performed. The questionnaire focused on children's health and on the environment in their school and homes. One thousand and ninety-nine questionnaires were returned. In the second phase, a subsample of 68 children was enrolled for further studies, including a walk-through inspection and checklist and an extensive set of IAQ measurements in their homes. The acquired data are relevant to assess children's environmental exposures and health status.
