Dendritische Zellen der Haut, wie z.B. die Langerhanszellen (LC) der Epidermis, sind potente antigenpräsentierende Zellen (APC). Nach allogener Blutstammzelltransplantation (engl.: hematopoietic stemm cell transplantation, HSCT) persistieren Empfänger-APC und können Spender-T-Zellen aktivieren. Somit spielen dendritische Zellen eine kritische Rolle bei der Initiierung von akuter Transplantat-Gegen-Wirt-Reaktion (engl.: graft-versus-host-disease, GvHD).rnIn der vorliegenden Arbeit wurde ein Modellsystem entwickelt, welches humane Haut in einem Xenotransplantationsmodell nutzt, um die Wechselwirkung dieser gewebsständigen APC mit alloreaktiven T-Zellen zu untersuchen. Dafür wurden humane Resthautpräparate von subkutanem Gewebe befreit und intraskaptulär auf immunsupprimierte NOD/LtSz-scid IL2R#-null Mäuse (NSG) transplantiert. Diesen Tieren fehlen funktionale T-, B- und NK-Zellen, und sie tolerieren somit ein xenogenes Transplantat. Im Vergleich zu anderen immundefizienten Stämmen, haben sie eine erhöhte Lebenserwartung und es ist zudem möglich humane Hämatopoese durch Stammzellgabe zu etablieren.rnPublizierte Methoden der Hauttransplantation wurden für diese Arbeit optimiert und weiterentwickelt. So konnte die Erfolgsrate von 44% auf bis zu 95% gesteigert werden. Erste Untersuchungen fokussierten den Einfluss der Wundheilung auf die Verteilung dermaler Zellpopulationen, wie z.B. CD11c positive APC, und die Population der LC in der Epidermis. Während der ersten Wochen der Wundheilung war ein vorübergehendes Verschwinden der LC aus der Epidermis zu beobachten. Im Gegensatz dazu waren CD11c positive dermale Zellen permanent detektierbar. Die zu späteren Zeitpunkten festgestellte Repopulation der Epidermis mit LC unterstützt die Hypothese einer lokalen Vorläuferzelle. Die vorgelegten Daten und die lokale proliferative Aktivität dieser Zellen unterstreichen ihre Unabhängigkeit vom peripheren Blut. Versuche, eine Depletion der LC mittels UVC-Bestrahlung zu erreichen, gelangen nicht. Auch dies spricht für das Vorhandensein eines lokalen Vorläufers.rnZur Induktion von GvHD in der transplantierten Haut wurden in vitro DC des Hautspenders generiert und damit HLA-disparate T-Zellen stimuliert. Auf diese Weise sollte eine maximale Alloreaktivität gegen das Hauttransplantat generiert werden. In allen vorgestellten Systemen ließ sich nach Infusion der T-Lymphozyten in transplantierte Tiere, eine T-Zellinduzierte inflammatorische Reaktion auslösen. Optisch war eine deutliche Rötung des Transplantats feststellbar. Diese war jedoch nur in den Proben besonders deutlich, welche T-Zellen mit vorheriger in vitro Stimulation durch DC des Hautspenders erhalten hatten. Histologisch konnten Anzeichen einer Entzündung nachgewiesen werden. Neben Akanthose und Hyperparakeratose, waren deutliche T-Zellinfiltrate detektierbar. Auch Spaltbildung und Ablösung der Epidermis, sowie vereinzelte Apoptosen der epidermalen Zellen wiesen auf eine GvHD artige Entzündung hin.rnEine weitere Beobachtung nach T-Zellgabe, war die Depletion der LC aus der Epidermis. Auch konnte durch spätere T-Zellgaben keine weitere Hautrötung ausgelöst werden. Dies belegt die Funktion der LC als primäre Zielzelle der alloreaktiven T-Zellen. Unterstrichen wird dies durch Verwendung einer LC defizienten Haut, welche keine Hautrötung oder Anzeichen einer Entzündung entwickelte.rnZusammenfassend wurde für diese Arbeit ein Modellsystem entwickelt, welches es erlaubt Untersuchungen entzündlicher Hautkrankheiten unter Berücksichtigung hautständiger APC durchzuführen. Dabei kann dieses Modell in Zukunft für die Untersuchung von APC modulierenden Agenzien genutzt werden, da präklinische Modelle für spezies-spezifische Therapien bislang fehlten. Das Entstehen einer Entzündung könnte so verhindert oder eine Behandlung ermöglicht werden.

Lung cancer is one of the leading causes of cancer-related deaths in the world. Although the origin still remains to be resolved, a prevailing hypothesis implies the involvement of cancer stem cells (CSCs) responsible for tumor initiation, maintenance, and progression. Embryonic stem cell marker, OCT4, encoding the spliced variants OCT4A and OCT4B, has recently been shown to have a dual role; as a potential adult stem cell marker and as a CSC marker in germline and somatic tumors.

This phase II trial investigated rituximab and cladribine in chronic lymphocytic leukemia. Four induction cycles, comprising cladribine (0.1 mg/kg/day days 1-5, cycles 1-4) and rituximab (375 mg/m(2) day 1, cycles 2-4), were given every 28 days. Stem cell mobilization (rituximab 375 mg/m(2) days 1 and 8; cyclophosphamide 4 g/m(2) day 2; and granulocyte colony-stimulating factor 10 microg/kg/day, from day 4) was performed in responders. Of 42 patients, nine achieved complete remission (CR), 15 very good partial remission, and two nodular partial remission (overall response rate 62%). Stem cell mobilization and harvesting (> or = 2 x 10(6) stem cells/kg body weight) were successful in 12 of 20 patients. Rituximab infusion-related adverse events were moderate. The main grade 3/4 adverse events during induction were neutropenia and lymphocytopenia. Rituximab plus cladribine was effective; however, the CR rate was modest and stem cell harvest was impaired in a large number of responding patients.

Inhibitor of differentiation 1 (ID1) plays a role in cellular differentiation, proliferation, angiogenesis and tumor invasion. As shown recently, ID1 is positively regulated by the tyrosine kinase SRC in lung carcinoma cell lines and with that appears as a potential new therapeutic target in non-small cell carcinoma (NSCLC). To substantiate this hypothesis we examined ID1, SRC and matrix metalloproteinase-9 (MMP-9) immunohistochemically in human NSCLC specimens.

We have previously shown that EphB4 and ephrin-B2 are differentially expressed in the mammary gland and that their deregulated expression in the mammary epithelium of transgenic mice leads to perturbations of the mammary parenchyma and vasculature. In addition, overexpression of EphB4 and expression of a truncated ephrin-B2 mutant, capable of receptor stimulation but incapable of reverse signalling, confers a metastasising phenotype on NeuT initiated mouse mammary tumours. We have taken advantage of this transgenic tumour model to compare stem cell characteristics between the non-metastasising and metastasising mammary tumours. We analysed the expression of the proliferation attenuating p21(waf) gene, which was significantly increased in the metastasising tumours. Moreover, we compared the expression of CK-19, Sca-1, CD24 and CD49f as markers for progenitor cells exhibiting a decreasing differentiation grade. Sca-1 expressing cells were the earliest progenitors detected in the non-metastasising NeuT induced tumours. The metastasising NeuT/EphB4 tumours were enriched in CD24 expressing cells, whereas the metastasising NeuT/truncated ephrin-B2 tumours contained in addition significant amounts of CD49f expressing cells. The same cell populations were also enriched in mammary glands of single transgenic MMTV-EphB4 and MMTV-truncated ephrin-B2 females indicating that deregulated EphB4-ephrin-B2 signalling interferes with the homeostasis of the stem/progenitor cell pool before tumour formation is initiated. Since the same cell populations are enriched in the normal tissue, primary mammary tumours and metastases we conclude that these progenitor cells were the origin of tumour formation and that this change in the tumour origin has led to the acquisition of the metastatic tumour phenotype.

Retinal degenerative diseases that target photoreceptors or the adjacent retinal pigment epithelium (RPE) affect millions of people worldwide. Retinal degeneration (RD) is found in many different forms of retinal diseases including retinitis pigmentosa (RP), age-related macular degeneration (AMD), diabetic retinopathy, cataracts, and glaucoma. Effective treatment for retinal degeneration has been widely investigated. Gene-replacement therapy has been shown to improve visual function in inherited retinal disease. However, this treatment was less effective with advanced disease. Stem cell-based therapy is being pursued as a potential alternative approach in the treatment of retinal degenerative diseases. In this review, we will focus on stem cell-based therapies in the pipeline and summarize progress in treatment of retinal degenerative disease.

Preeclampsia is associated with perinatal brain injury. Autologous placenta stem cell transplantation represents a promising future treatment option for neuroregeneration. The aim of this study was to compare the neuroregenerative capacity of preeclampsia-placenta stem cells to previously characterized placentas from uncomplicated pregnancies.

Cancer most probably originates from stem/progenitor cells and exhibits a similar cell hierarchy as normal tissues. Moreover, there is growing evidence that only the stem cells are capable of metastasis formation. We have previously shown that overexpression of a dominant negative ephrin-B2 mutant interferes with mammary gland differentiation and confers a metastatic phenotype to NeuT-induced mammary tumors with an increase in cells with stem/progenitor characteristics. To investigate the role of ephrin-B2 in the control of the mammary stem cell niche, we analyzed the mammary stem and progenitor cell populations in transgenic mice overexpressing the mutant ephrin-B2. Quantification by FACS analysis revealed a significant increase of cells in the basal/alveolar cell-, the bi-potent progenitor- and the stem cell-enriched fractions. Moreover, the supposed precursors of estrogen receptor-positive cells were elevated in the stem cell-enriched fraction. In contrast, the epithelium from transgenic mice overexpressing the native ephrin-B2 gene showed an augmentation of the luminal cell- and the bi-potent progenitor-enriched fractions. Repopulation assays revealed that the epithelial cells of truncated ephrin-B2 transgenic epithelial cells have a higher regeneration capacity than those of controls and of native ephrin-B2 transgenic mice, confirming the augmentation of stem cells. Morphologically, these outgrowths exhibited impaired basal/luminal compartmentalization and epithelial polarization. These results demonstrate that deregulated ephrin-B2 expression interferes with the regulation of the stem cell niche and leads to a shift of the differentiation pathway and may thereby contribute to the acquisition of the metastatic phenotype long before carcinogenic growth becomes apparent.

In patients with acute myelogenous leukemia, published guidelines and treatment recommendations are usually the basis for starting the work-up process for allogeneic transplant. However, only consistent recommendations would allow a standardized clinical practice. We conducted a comprehensive systematic literature search to identify and evaluate the best available evidence from controlled clinical trials. In addition, recommendations given by leading organizations in the USA and Europe were analyzed. The following aspects were selected for systematic comparison: factors for risk assessment and categorization, role of type of donor, significance of allogeneic transplant in first or second complete remission and in relapse/progressive disease; and role of reduced intensity conditioning regimens. In conclusion, the recommendations for the use of allogeneic transplant given by the literature and by published guidelines are inconsistent and will need clarification.

Stem cell transplantation promises new hope for the treatment of stroke although significant questions remain about how the grafted cells elicit their effects. One hypothesis is that transplanted stem cells enhance endogenous repair mechanisms activated after cerebral ischaemia. Recognizing that bilateral reorganization of surviving circuits is associated with recovery after stroke, we investigated the ability of transplanted human neural progenitor cells to enhance this structural plasticity. Our results show the first evidence that human neural progenitor cell treatment can significantly increase dendritic plasticity in both the ipsi- and contralesional cortex and this coincides with stem cell-induced functional recovery. Moreover, stem cell-grafted rats demonstrated increased corticocortical, corticostriatal, corticothalamic and corticospinal axonal rewiring from the contralesional side; with the transcallosal and corticospinal axonal sprouting correlating with functional recovery. Furthermore, we demonstrate that axonal transport, which is critical for both proper axonal function and axonal sprouting, is inhibited by stroke and that this is rescued by the stem cell treatment, thus identifying another novel potential mechanism of action of transplanted cells. Finally, we established in vitro co-culture assays in which these stem cells mimicked the effects observed in vivo. Through immunodepletion studies, we identified vascular endothelial growth factor, thrombospondins 1 and 2, and slit as mediators partially responsible for stem cell-induced effects on dendritic sprouting, axonal plasticity and axonal transport in vitro. Thus, we postulate that human neural progenitor cells aid recovery after stroke through secretion of factors that enhance brain repair and plasticity.
