Background : Thrombotic antiphospholipid syndrome is defined as a complex form of thrombophilia that is developed by a fraction of antiphospholipid antibody (aPLA) carriers. Little is known about the genetic risk factors involved in thrombosis development among aPLA carriers. Methods: To identify new loci conferring susceptibility to thrombotic antiphospholipid syndrome, a two-stage genotyping strategy was performed. In stage one, 19,000 CNV loci were genotyped in 14 thrombotic aPLA+ patients and 14 healthy controls by array-CGH. In stage two, significant CNV loci were fine-mapped in a larger cohort (85 thrombotic aPLA+, 100 non-thrombotic aPLA+ and 569 healthy controls). Results : Array-CGH and fine-mapping analysis led to the identification of 12q24.12 locus as a new susceptibility locus for thrombotic APS. Within this region, a TAC risk haplotype comprising one SNP in SH2B3 gene (rs3184504) and two SNPs in ATXN2 gene (rs10774625 and rs653178) exhibited the strongest association with thrombotic antiphospholipid syndrome (p-value = 5,9 × 10−4 OR 95% CI 1.84 (1.32–2.55)). Conclusion : The presence of a TAC risk haplotype in ATXN2-SH2B3 locus may contribute to increased thrombotic risk in aPLA carriers.

Background: Maladaptive behavior has been reported as a phenotypical feature in Prader–Willi syndrome (PWS). It severely limits social adaptation and the quality of life of children and adults with the syndrome. Different factors have been linked with the intensity and form of these behavioral disturbances but there is no consensus about the cause. Consequently, there is still controversy regarding management strategies and there is a need for new data. Methods: The behavior of 100 adults with PWS attending a dedicated center was assessed using the Developmental Behavior Checklist for Adults (DBC-A) and the PWS-specific Hyperphagia Questionnaire. The DBC-A was completed separately by trained caregivers at the center and relatives or caregivers in a natural setting. Genotype, gender, age, degree of obesity and cognitive impairment were analyzed as variables with a hypothetical influence on behavioral features. Results: Patients showed a relatively high rate of behavioral disturbances other than hyperphagia. Disruptive and social relating were the highest scoring DBC-A subscales whereas anxiety/antisocial and self-absorbed were the lowest. When hospital caregiver and natural caregiver scores were compared, scores for the latter were higher for all subscales except for disruptive and anxiety/antisocial. These effects of institutional management were underlined. In the DBC-A, 22 items have descriptive indications of PWS behavior and were used for further comparisons and correlation analysis. In contrast to previous reports, rates of disturbed behavior were lower in patients with a deletion genotype. However, the behavioral profile was similar for both genotypes. No differences were found in any measurement when comparing type I and type II deletions. The other analyzed variables showed little relevance. Conclusions: Significant rates of behavioral disorders were highlighted and their typology described in a large cohort of adults with PWS. The deletion genotype was related to a lower severity of symptoms. Some major behavioral problems, such as hyperphagia, may be well controlled if living circumstances are adapted to the specific requirements of individuals with PWS.

Background: Lynch syndrome (LS) is an autosomal dominant inherited cancer syndrome characterized by early onset cancers of the colorectum, endometrium and other tumours. A significant proportion of DNA variants in LS patients are unclassified. Reports on the pathogenicity of the c.1852_1853AA>GC (p.Lys618Ala) variant of the MLH1 gene are conflicting. In this study, we provide new evidence indicating that this variant has no significant implications for LS. Methods: The following approach was used to assess the clinical significance of the p.Lys618Ala variant: frequency in a control population, case-control comparison, co-occurrence of the p.Lys618Ala variant with a pathogenic mutation, co-segregation with the disease and microsatellite instability in tumours from carriers of the variant. We genotyped p.Lys618Ala in 1034 individuals (373 sporadic colorectal cancer [CRC] patients, 250 index subjects from families suspected of having LS [revised Bethesda guidelines] and 411 controls). Three well-characterized LS families that fulfilled the Amsterdam II Criteria and consisted of members with the p.Lys618Ala variant were included to assess co-occurrence and co-segregation. A subset of colorectal tumour DNA samples from 17 patients carrying the p.Lys618Ala variant was screened for microsatellite instability using five mononucleotide markers. Results: Twenty-seven individuals were heterozygous for the p.Lys618Ala variant; nine had sporadic CRC (2.41%), seven were suspected of having hereditary CRC (2.8%) and 11 were controls (2.68%). There were no significant associations in the case-control and case-case studies. The p.Lys618Ala variant was co-existent with pathogenic mutations in two unrelated LS families. In one family, the allele distribution of the pathogenic and unclassified variant was in trans, in the other family the pathogenic variant was detected in the MSH6 gene and only the deleterious variant co-segregated with the disease in both families. Only two positive cases of microsatellite instability (2/17, 11.8%) were detected in tumours from p.Lys618Ala carriers, indicating that this variant does not play a role in functional inactivation of MLH1 in CRC patients. Conclusions: The p.Lys618Ala variant should be considered a neutral variant for LS. These findings have implications for the clinical management of CRC probands and their relatives.

Biological machines are active devices that are comprised of cells and other biological components. These functional devices are best suited for physiological environments that support cellular function and survival. Biological machines have the potential to revolutionize the engineering of biomedical devices intended for implantation, where the human body can provide the required physiological environment. For engineering such cell-based machines, bio-inspired design can serve as a guiding platform as it provides functionally proven designs that are attainable by living cells. In the present work, a systematic approach was used to tissue engineer one such machine by exclusively using biological building blocks and by employing a bio-inspired design. Valveless impedance pumps were constructed based on the working principles of the embryonic vertebrate heart and by using cells and tissue derived from rats. The function of these tissue-engineered muscular pumps was characterized by exploring their spatiotemporal and flow behavior in order to better understand the capabilities and limitations of cells when used as the engines of biological machines.

When studying physical systems, it is common to make approximations: the contact interaction is linear, the crystal is periodic, the variations occurs slowly, the mass of a particle is constant with velocity, or the position of a particle is exactly known are just a few examples. These approximations help us simplify complex systems to make them more comprehensible while still demonstrating interesting physics. But what happens when these assumptions break down? This question becomes particularly interesting in the materials science community in designing new materials structures with exotic properties In this thesis, we study the mechanical response and dynamics in granular crystals, in which the approximation of linearity and infinite size break down. The system is inherently finite, and contact interaction can be tuned to access different nonlinear regimes. When the assumptions of linearity and perfect periodicity are no longer valid, a host of interesting physical phenomena presents itself. The advantage of using a granular crystal is in its experimental feasibility and its similarity to many other materials systems. This allows us to both leverage past experience in the condensed matter physics and materials science communities while also presenting results with implications beyond the narrower granular physics community. In addition, we bring tools from the nonlinear systems community to study the dynamics in finite lattices, where there are inherently more degrees of freedom. This approach leads to the major contributions of this thesis in broken periodic systems. We demonstrate the first defect mode whose spatial profile can be tuned from highly localized to completely delocalized by simply tuning an external parameter. Using the sensitive dynamics near bifurcation points, we present a completely new approach to modifying the incremental stiffness of a lattice to arbitrary values. We show how using nonlinear defect modes, the incremental stiffness can be tuned to anywhere in the force-displacement relation. Other contributions include demonstrating nonlinear breakdown of mechanical filters as a result of finite size, and the presents of frequency attenuation bands in essentially nonlinear materials. We finish by presenting two new energy harvesting systems based on our experience with instabilities in weakly nonlinear systems.

I. The 3.7 Å Crystal Structure of Horse Heart Ferricytochrome C.

The crystal structure of horse heart ferricytochrome c has been determined to a resolution of 3.7 Å using the multiple isomorphous replacement technique. Two isomorphous derivatives were used in the analysis, leading to a map with a mean figure of merit of 0.458. The quality of the resulting map was extremely high, even though the derivative data did not appear to be of high quality.

Although it was impossible to fit the known amino acid sequence to the calculated structure in an unambiguous way, many important features of the molecule could still be determined from the 3.7 Å electron density map. Among these was the fact that cytochrome c contains little or no α-helix. The polypeptide chain appears to be wound about the heme group in such a way as to form a loosely packed hydrophobic core in the molecule.

The heme group is located in a cleft on the molecule with one edge exposed to the solvent. The fifth coordinating ligand is His 18 and the sixth coordinating ligand is probably neither His 26 nor His 33.

The high resolution analysis of cytochrome c is now in progress and should be completed within the next year.

II. The Application of the Karle-Hauptman Tangent Formula to Protein Phasing.

The Karle-Hauptman tangent formula has been shown to be applicable to the refinement of previously determined protein phases. Tests were made with both the cytochrome c data from Part I and a theoretical structure based on the myoglobin molecule. The refinement process was found to be highly dependent upon the manner in which the tangent formula was applied. Iterative procedures did not work well, at least at low resolution.

The tangent formula worked very well in selecting the true phase from the two possible phase choices resulting from a single isomorphous replacement phase analysis. The only restriction on this application is that the heavy atoms form a non-centric cluster in the unit cell.

Trata-se de estudo transversal sem modelo de intervenção, que tem como objeto a atenção à saúde na prevenção secundária da doença coronariana a pacientes com ou sem tratamento ambulatorial especializado. O objetivo primário foi: Avaliar se há diferença na atenção à saúde entre pacientes portadores de doença arterial coronariana em sua forma aguda ou crônica com ou sem acompanhamento ambulatorial especializado. Os objetivos secundários foram: a) apresentar o perfil de cada grupo de pacientes a partir de dados sócio-demográficos e econômicos; b) descrever as característica clínicas dos pacientes e identificar a que fatores de risco cada grupo de pacientes está exposto; c) descrever a regularidade na utilização de medicamentos e como se dá o acesso à mesma; d) identificar o tipo e principais dificuldades enfrentadas pelo paciente para seguir o tratamento. Para a coleta de dados foi utilizado formulário desenvolvido e previamente testado para atender os objetivos propostos para o estudo, além de informações coletadas diretamente do prontuário do paciente. A coleta de dados foi realizada em três unidades públicas de saúde localizadas no município do Rio de Janeiro. A amostra selecionada foi composta por 112 pacientes divididos igualmente entre os dois grupos existentes na pesquisa. Os dados foram transcritos para planilha do programa Statistic Package for the Social Science e análise realizada através dos testes estatísticos de diferença entre proporção, odds-ratio e qui-quadrado. Quanto ao perfil sócio-demográfico e econômico verificou-se diferença estatística significativa quanto ao grau de instrução entre os grupos I e II, com predomínio de pacientes de nível fundamental incompleto para o grupo I e médio completo no grupo II (p=0,0434), foi também verificada diferença significativa entre os grupos relacionada ao rendimento mensal, embora o maior percentual encontrado em ambos os grupos tenha sido observado na faixa de dois salários mínimos, uma vez que o grupo II apresentou maior concentração de renda entre a faixa de dois a três salários mínimos (p=0,0044). Ao que se refere aos fatores de risco para doença coronariana, observou-se diferença estatística entre os grupos para a variável tabagismo (p= 0,0001) e sedentarismo (p=0,0025). Verificou-se para pacientes do grupo I valor estatístico significativo quanto a regularidade em utilizar medicamentos (p=0,0010). Concluiu-se, portanto, que o acompanhamento de pacientes pós-síndrome coronariana aguda em ambulatório especializado de coronariopatias apresentou benefícios significativos quando comparado ao grupo de pacientes não cobertos por este tipo de assistência. Verificou-se ainda que o enfermeiro poderá contribuir e atuar amplamente para a prevenção secundária da doença coronariana, enquanto membro da equipe multidisciplinar.

A insuficiência cardíaca (IC) é uma síndrome clínica de elevada incidência e com um prognóstico ruim a longo prazo. Ela é a via final comum da maioria das doenças que acometem o coração, sendo um dos mais importantes desafios clínicos na área da saúde. O óxido nítrico (NO) representa, por intermédio de sua influência sobre o endotélio e as plaquetas, um importante papel na regulação da homeostase vascular. Este gás de meia-vida curta é sintetizado a partir do aminoácido L-arginina, pela enzima NO sintase (NOS), levando à produção de guanosina monofosfato cíclica (GMPc). Estudos mostram que anormalidades na biodisponibilidade de NO em plaquetas podem contribuir para eventos trombóticos, não tendo sido ainda avaliada na IC. Na primeira parte do estudo, o objetivo foi investigar o efeito da IC na atividade e na expressão da NOS em plaquetas, no conteúdo intraplaquetário de GMPc, na agregação plaquetária, além dos parâmetros antropométricos e da composição corporal, das variáveis bioquímicas, dos aminoácidos plasmáticos, do estresse oxidativo (plasma e plaquetas) e da concentração sistêmica de marcadores inflamatórios em 15 pacientes com IC e 15 controles saudáveis. Na segunda parte do estudo, objetivou-se avaliar os efeitos do treinamento físico (TF) regular nessas mesmas variáveis (exceto a expressão da NOS) e nas variáveis hemodinâmicas e respiratórias. Para tal, foram avaliados 15 pacientes com IC que se mantiveram sedentários e 15 pacientes com IC que realizaram 30 minutos de atividade física aeróbia e treinamento contraresistência muscular localizada com pesos livres e máquinas, três vezes por semana, durante 24 semanas. Os resultados da primeira etapa do estudo demonstraram hiperagregabilidade plaquetária induzida tanto por colágeno como por ADP, com aumento do estresse oxidativo, da atividade basal da NOS e da concentração de GMPc, estando os níveis plasmáticos de L-arginina em pacientes com IC diminuído. A expressão da iNOS, estava aumentada em plaquetas de pacientes com IC em relação aos controles saudáveis.Também foi observado aumento da resposta inflamatória, com maiores níveis sistêmicos de proteína C reativa, fibrinogênio, interleucina-6 e fator de necrose tumoral α. Após o período de TF, houve aumento do VO2 máximo e a agregação plaquetária induzida tanto por colágeno quanto por ADP estava diminuída. Ocorreu um aumento dos níveis plasmáticos de L-arginina e, uma redução da atividade da NOS após o TF. Em relação ao estresse oxidativo, tanto a produção sistêmica, quanto a intraplaquetária de substâncias reativas ao ácido tiobarbitúrico (TBARs) e a carbonilação diminuíram na presença da atividade aumentada das enzimas superóxido dismutase (SOD) e da catalase após o TF. Houve uma diminuição da resposta inflamatória, uma diminuição do colesterol total, do LDL e dos triglicerídeos e um aumento do HDL, após oTF. Nossos resultados sugerem que o TF tem efeitos antioxidantes, anti-inflamatórios e antiagregantes, que parece ser independente da produção de NO, sendo uma importante ferramenta não farmacológica no tratamento da IC

The aim of the study was to evaluate the resistance of white spot syndrome virus (WSSV) in shrimps (Penaeus monodon) to the process of cooking. The cooking was carried out at 1000C six different durations 5, 10, 15, 20, 25 and 30 min. The presence of WSSV was tested by single step and nested polymerase chain reaction (PCR). In the single step PCR, the primers 1s5 & 1a16 and IK1 & IK2 were used. While in the nested PCR, primers IK1 &IK2 – IK3 & IK4 were used for the detection of WSSV. WSSV was detected in the single step PCR with the primers 1s5 and 1a16 and the nested PCR with the primers IK1 and IK2 – IK3 & IK4 from the cooked shrimp samples. The cooked shrimps, which gave positive results for WSSV by PCR, were further confirmed for the viability of WSSV by conducting the bio-inoculation studies. Mortality (100%) was observed within 123 h of intra-muscular post injection (P.I) into the live healthy WSSV-free shrimps (P. monodon). These results show that the WSSV survive the cooking process and even infected cooked shrimp products may pose a transmission risk for WSSV to the native shrimp farming systems.
