The species of the genus Kalanchoe (Crassulaceae) are widely used in Brazil as a medicinal product to treat cough, boils, gastritis and other diseases. In this scenario stands out K. brasiliensis, popularly known as coirama or saião. This paper aims at the oral exposure of mice to a hydroalcoholic extract of leaves of Kalanchoe brasiliensis. The animals (total of 100) were divided into 12 groups (6 males and 6 females) for the acute assessment; and groups of 10 for subchronic evaluation. Test groups were treated with doses of 250 mg / kg, 500 mg / kg, 1000 mg / kg and 2000 mg / kg and the control group received 0.9% saline. The animals were observed for 14 days for acute evaluation and 30 days for subchronic evaluation, and in that period analyzed the appearance of clinical signs, changes in weight and consumption of water and food. After the observation period, histopathological analysis of the organs, biochemical serum and haematological parameters for the assessment of the subchronic groups were processed. Differences and changes in body weight were not observed among the groups, nor consumption of water and food, there were no deaths among the groups in the two types of assays. Histopathological analysis showed some alterations compatible with low acute hepatic toxicity. The results of blood glucose, triglycerides, ALT, urea and creatinine showed differences between the control group and the test concentrations studied (p <0.05), but these differences do not show relevant change in the clinical picture of animals. The results showed that the extract K. brasiliensis has low acute toxicity at the doses used and no toxicity when administered for 30 days. This highlights the importance of this species as future and promising candidate for phytotherapics, so has its pharmacological trials completed.

BACKGROUND: Among the wide range of skills displayed by a medical doctor is undoubtedly the need to use cohesive and well grounded clinical reasoning in order for medical care to be indeed effective. It is in this respect that conceptual maps emerge; these are a methodological innovation that allows a comprehensive, panoramic and associative outlook of theoretical content, making it more practical and applicable to the reality of clinical observation. Promoting learning, learning resources and a feedback system between professor and students, as well as assessing and monitoring the performance of students during their academic training, are the main features of this tool. OBJETIVE: Assess the use of conceptual maps as a teaching-learning tool in the training of undergraduate medical students at Universidade Federal do Rio Grande do Norte (UFRN). METHODOLOGY: Interventional, randomized, cross-sectional study conducted with students from the 3rd and 5th periods of the medical course at UFRN, during the second semester of 2014, totaling 86 participants, divided into two groups: GI (intervention – clinical case resolution with a conceptual map) and GII (control – clinical case resolution without a conceptual map) in each period. RESULTS: The use of conceptual maps to teach liver failure syndrome resulted in a statistically significant cognitive gain for G1 students from the 5th period (GI: 6.8±1.6 and 8.0±1.5, p = 0.024; GII: 7.2±2.1 and 8.0±1.7, p = 0.125, pre and post-intermediate means, respectively), a result not observed in the period 3rd (GI: 7.7±1.3 and 8.0±1.4, p = 0.501; GII: 6.7±1.8 and 7.8±1.8; p=0.068, pre and post-intermediate means, respectively). Students in the 3 rd period gave better responses to the first clinical case, with a larger number of suitable concepts and crosslinks, when they used conceptual maps (GI: 91.3±13.15 and GII: 64.84±22.84, p=0,002). Students in the 5th period exhibited better clinical reasoning and more complete responses using the tool (p=0,01). Most of the students were not aware of the tool (53.8% from the 3rd period and 65.3% from the 5th period). Among those who knew about conceptual maps, most (59.3%) had only used them during high school, 14.8% had never used them and only seven students (25.9%) used them during the medical course. Analysis of open responses, obtained in process assessment showed clear satisfaction and enthusiasm with learning about the new tool, and frequent suggestions to use it at other moments in the course. Assessment of learning profile, using the VARK questionnaire, showed that most students from both periods exhibited a multimodal style. CONCLUSION: Despite their scant knowledge regarding the tool, good acceptability and understanding was observed in the study participants. The conceptual maps allowed cognitive gains, better responses and clinical reasoning in teaching liver failure syndrome to 5th period students.

Chronic Hepatitis C is the leading cause of chronic liver disease in advanced final stage of hepatocellular carcinoma (HCC) and of death related to liver disease. Evolves progressively in time 20-30 years. Evolutionary rates vary depending on factors virus, host and behavior. This study evaluated the impact of hepatitis C on the lives of patients treated at a referral service in Hepatology of the University Hospital Onofre Lopes - Liver Study Group - from May 1995 to December 2013. A retrospective evaluation was performed on 10,304 records, in order to build a cohort of patients with hepatitis C, in which all individuals had their diagnosis confirmed by gold standard molecular biological test. Data were obtained directly from patient charts and recorded in an Excel spreadsheet, previously built, following an elaborate encoding with the study variables, which constitute individual data and prognostic factors defined in the literature in the progression of chronic hepatitis C. The Research Ethics Committee approved the project. The results were statistically analyzed with the Chi-square test and Fisher's exact used to verify the association between variable for the multivariate analysis, we used the Binomial Logistic regression method. For both tests, it was assumed significance p < 0.05 and 95%. The results showed that the prevalence of chronic hepatitis C in NEF was 4.96 %. The prevalence of cirrhosis due to hepatitis C was 13.7%. The prevalence of diabetes in patients with Hepatitis C was 8.78 % and diabetes in cirrhotic patients with hepatitis C 38.0 %. The prevalence of HCC was 5.45%. The clinical follow-up discontinuation rates were 67.5 %. The mortality in confirmed cases without cirrhosis was 4.10% and 32.1% in cirrhotic patients. The factors associated with the development of cirrhosis were genotype 1 (p = 0.0015) and bilirubin > 1.3 mg % (p = 0.0017). Factors associated with mortality were age over 35 years, abandon treatment, diabetes, insulin use, AST> 60 IU, ALT> 60 IU, high total bilirubin, extended TAP, INR high, low albumin, treatment withdrawal, cirrhosis and hepatocarcinoma. The occurrence of diabetes mellitus increased mortality of patients with hepatitis C in 6 times. Variables associated with the diagnosis of cirrhosis by us were blood donor (odds ratio 0.24, p = 0.044) and professional athlete (odds ratio 0.18, p = 0.35). It is reasonable to consider a revaluation in screening models for CHC currently proposed. The condition of cirrhosis and diabetes modifies the clinical course of patients with chronical hepatitis C, making it a disease more mortality. However, being a blood donor or professional athlete is a protective factor that reduces the risk of cirrhosis, independent of alcohol consumption. Public policies to better efficient access, hosting and resolution are needed for this population.

La obesidad es un problema de salud pública a nivel mundial. Su prevalencia se ha multiplicado por más de dos entre 1980 y 2014. La Organización Mundial de la salud (OMS) ha estimado que 170 millones de jóvenes menores de 18 años presentan exceso de peso y ha considerado la obesidad como una epidemia a nivel mundial. La obesidad es una enfermedad crónica de origen multifactorial que viene determinada por la interacción de factores genéticos, biológicos, socioeconómicos, conductuales y ambientales actuando a través de mediadores fisiológicos nutricionales en la ingesta y gasto de energía. El sobrepeso y la obesidad son factores de riesgo relacionados con el desarrollo de diabetes, alteraciones del metabolismo lipídico, síndrome metabólico, hipertensión arterial, esteatosis hepática, litiasis biliar, alteraciones respiratorios, ortopédicas, trastornos psicológicos y de la alimentación y también en ocasiones inadaptación social del adolescente. Estos desajustes se pueden perpetuar y agravarse en la edad adulta, por lo que tanto el sobrepeso como la obesidad suponen un grave problema sanitario, económico y social, frente al cual es prioritario luchar. Numerosos estudios e investigaciones evidencian la relación existente entre la obesidad y sus inicios en la vida intrauterina, es decir, la relación entre el comportamiento nutricional y ponderal de la madre antes y/o durante el embarazo y las consecuencias en la salud de sus descendientes en la edad adulta...

El impacto que tienen los defectos congénitos sobre la salud de las personas afectadas, sus familias y sobre la sociedad en su conjunto es muy considerable. En España se estima que existe una prevalencia de anomalías cromosómicas hasta 2010 de 1,49% de los recién nacidos vivos. El Síndrome de Down (SD) es la tercera causa de defecto congénito y la primera de cromosomopatía, con una prevalencia de 23 por cada 10000 nacidos vivos. La importante morbilidad asociada en los individuos con síndrome de Down se acompaña de un alto coste económico, estimándose en 329750,63 euros por cada nuevo caso, constituyendo un cargo a lo largo de la vida de 1316 millones de euros. Por todo ello, la detección de esta alteración es la indicación más frecuente de diagnóstico prenatal invasivo. Las pruebas invasivas, como amniocentesis, biopsia de vellosidades coriales y cordocentesis, se asocian con un aumento del 1% del riesgo de aborto y, por tanto, sólo se realizan cuando se considera que hay una probabilidad elevada de que el feto tenga un defecto cromosómico, siendo por tanto fundamental una buena selección del grupo de gestantes de alto riesgo...

La tomografia a coerenza ottica (Optical Coherence Tomography, OCT) rappresenta fondamentalmente una nuova modalità di diagnostica per immagini non invasiva e ad alta risoluzione. L’OCT fornisce immagini, sotto forma di sezioni trasversali o tomografiche, delle microstrutture dei tessuti biologici tramite la misura del ritardo dell’eco e dell’intensità della luce retrodiffusa o riflessa dal campione in analisi. L’OCT è una potente tecnica poiché fornisce in tempo reale immagini in situ delle strutture tissutali normali o patologiche, con una risoluzione da 1 a 15 micron, che è da uno a due ordini di grandezza maggiore rispetto alle tecniche per immagini convenzionali come ultrasuoni, risonanza magnetica o tomografia computerizzata. Tutto questo senza la necessità di ottenere e analizzare un campione tramite biopsia e studio istopatologico. Sin dall’inizio della sua utilizzazione nel 1991, l’OCT è stata impiegata in un vasto spettro di applicazioni cliniche, principalmente l'oftalmologia. In strutture non trasparenti, diverse da quelle oculari, la profondità massima esplorabile è limitata a 2-3 mm a causa dei fenomeni di attenuazione secondari e alla dispersione della luce. Inoltre, i numerosi sviluppi fatti dalla tecnologia OCT hanno portato ad alte velocità di acquisizione e, grazie all’utilizzo di sorgenti laser di ultima generazione, risoluzioni assiali dell’ordine di 1 micrometro. Dunque, la tomografia a coerenza ottica può essere sfruttata: - Quando la biopsia escissionale è pericolosa o impossibile. - Quando si rischiano errori di campionamento, come guida alla biopsia in modo da ridurre errori nel prelievo del campione. - Come guida alla procedura d’intervento, in associazione alle procedure di cateterismo, endoscopia e laparoscopia.
