OBJETIVES: To detect anti-Giardia lamblia serum antibodies in healthy children attending public day care centers and to assess serological tests as tools for estimating the prevalence of G. lamblia in endemic areas. METHODS: Three separate stool specimens and filter paper blood samples were collected from 147 children ranging from 0 to 6 years old. Each stool sample was processed using spontaneous sedimentation and zinc sulfate flotation methods. Blood samples were tested by indirect immunofluorescence (IIF) and enzyme-linked immunosorbent assay (ELISA) for Giardia IgG. RESULTS AND CONCLUSIONS: Of 147 individuals tested, 93 (63.3%) showed Giardia cysts in their feces. Using IIF and ELISA, serum antibodies were detected in 93 (63.3%) and 100 (68%) samples , respectively. Sensitivity of IIF and ELISA was 82% and 72%, respectively. However, ELISA revealed to be less specific (39%) than IIF (70%). IIF also showed a higher concordance with microscopic examination than ELISA.

OBJECTIVE: To introduce a fuzzy linguistic model for evaluating the risk of neonatal death. METHODS: The study is based on the fuzziness of the variables newborn birth weight and gestational age at delivery. The inference used was Mamdani's method. Neonatologists were interviewed to estimate the risk of neonatal death under certain conditions and to allow comparing their opinions and the model values. RESULTS: The results were compared with experts' opinions and the Fuzzy model was able to capture the expert knowledge with a strong correlation (r=0.96). CONCLUSIONS: The linguistic model was able to estimate the risk of neonatal death when compared to experts' performance.

OBJETIVO: Investigar a relação entre fatores socioeconômicos, características maternas, aleitamento e hospitalização por bronquiolite no período pós-neonatal. MÉTODOS: Estudo caso-controle, aninhado a estudo de coorte de 5.304 crianças nascidas na cidade de Pelotas, RS. O estudo de coorte constitui-se de quatro subestudos, cada um com métodos e logística específicos. As mães foram entrevistadas por meio de questionário padronizado, em visitas hospitalares e domiciliares. Foram definidos como casos as crianças com idade entre 28 e 364 dias, que haviam sido hospitalizadas por bronquiolite. RESULTADOS: De 5.304 crianças da coorte, 113 (2,1%) foram hospitalizadas por bronquiolite. A análise multivariada hierarquizada, realizada pela regressão logística, mostrou os seguintes resultados: renda familiar, assim como idade gestacional, estiveram inversamente relacionadas ao risco de hospitalização por bronquiolite. O aleitamento materno desempenhou um fator protetor; crianças com tempo de aleitamento materno inferior a um mês tiveram risco 7 vezes maior de serem hospitalizadas por bronquiolite aguda nos primeiros três meses de vida. O risco de hospitalização por bronquiolite foi 57% maior naquelas expostas ao fumo materno. CONCLUSÕES: A hospitalização por bronquiolite está inversamente relacionada com renda familiar, idade gestacional e tempo de aleitamento materno, e diretamente relacionada com exposição a fumo materno. Não foi evidenciada associação com paridade ou história materna de asma brônquica.

Structures experience various types of loads along their lifetime, which can be either static or dynamic and may be associated to phenomena of corrosion and chemical attack, among others. As a consequence, different types of structural damage can be produced; the deteriorated structure may have its capacity affected, leading to excessive vibration problems or even possible failure. It is very important to develop methods that are able to simultaneously detect the existence of damage and to quantify its extent. In this paper the authors propose a method to detect and quantify structural damage, using response transmissibilities measured along the structure. Some numerical simulations are presented and a comparison is made with results using frequency response functions. Experimental tests are also undertaken to validate the proposed technique. Optical colour sensors based on multilayered a-SiC:H heterostructures can act as voltage controlled optical filters in the visible range. In this article we investigate the application of these structures for Fluorescence Resonance Energy Transfer (FRET) detection, The characteristics of a-SiC:H multilayered structure are studied both theoretically and experimentally in several wavelengths corresponding to different fluorophores. The tunable optical p-i'(a-SiC:H)-n/p-i(a-Si:H)-n heterostructures were produced by PECVD and tested for a proper fine tuning in the violet, cyan and yellow wavelengths. The devices were characterized through transmittance and spectral response measurements, under different electrical bias and frequencies. Violet, cyan and yellow signals were applied in simultaneous and results have shown that they can be recovered under suitable applied bias. A theoretical analysis supported by numerical simulation is presented.

OBJETIVO: Avaliar peso de nascimento e os escores como preditores de mortalidade neonatal em unidade de terapia intensiva neonatal, comparando os seus resultados. MÉTODOS: Foram avaliados 494 recém-nascidos admitidos em uma unidade de terapia intensiva neonatal (UTIN) de um hospital geral de Porto Alegre, RS, logo após o nascimento, entre março de 1997 e junho de 1998. Foram avaliados o peso de nascimento e os escores considerando a variável óbito durante a internação na UTI. Os critérios de exclusão foram: alta ou óbito da UTIN com menos de 24 horas de internação, recém-nascidos cuja internação não ocorreu logo após o nascimento, protocolo de estudo incompleto e malformações congênitas incompatíveis com a vida. Para avaliação do CRIB (Clinical Risk Index for Babies) foram considerados somente os pacientes com peso de nascimento inferior a 1.500 g. Foram calculadas as curvas ROC (Receiver Operating Characteristics Curve) para SNAP (Score for Neonatal Acute), SNAP-PE (Score for Neonatal Acute Physiology Perinatal Extension), SNAP II, SNAP-PE II, CRIB e peso de nascimento. RESULTADOS: Dos 494 pacientes, 44 faleceram (8,9% de mortalidade). Dos 102 recém-nascidos com peso de até 1.500 g, 32 (31,3%) faleceram. As áreas abaixo da curva ROC variaram de 0,81 a 0,94. Todos os escores avaliados mostraram áreas abaixo da curva ROC sem diferenças estatisticamente significativas. Os escores de risco de mortalidade estudados apresentaram um melhor desempenho que o peso de nascimento, especialmente em recém-nascidos com peso de nascimento igual ou menor que 1.500 g. CONCLUSÕES: Todos os escores de mortalidade neonatal apresentaram melhor desempenho e foram superiores ao peso de nascimento como medidores de risco de óbito hospitalar para recém-nascidos internados em UTIN.

OBJETIVO: Avaliar a mortalidade dos recém-nascidos de muito baixo peso em uma UTI neonatal conforme as variações do escore CRIB (Clinical Risk Index for Babies), do peso de nascimento e da idade gestacional em determinado período. MÉTODOS: O escore CRIB foi aplicado seqüencial e prospectivamente em todos os recém-nascidos com peso de nascimento <1.500 g e/ou idade gestacional <31 semanas, em maternidade de um hospital universitário de Londrina, no período de janeiro de 1997 a dezembro de 2000. Os critérios de exclusão foram: óbitos antes de 12 horas de vida, os recém- nascidos com malformações congênitas incompatíveis com a sobrevida e os recém- nascidos encaminhados de outros serviços. RESULTADOS: Foram incluídos no estudo 284 recém-nascidos. O peso médio de nascimento foi de 1.148±248 g (mediana =1.180 g); a idade gestacional média foi de 30,2±2,4 semanas (mediana =30,0) e o CRIB médio foi de 3,8±4,4 (mediana =2,0). A mortalidade neonatal foi de 23,2% diferindo conforme peso <750 g (72,7%), IG<29 semanas (57,1%) e CRIB>10 (79,4%). A curva ROC (Receiver Operator Characteristic) para os valores de CRIB, peso de nascimento e idade gestacional gerou áreas sob a curva de 0,88, 0,76 e 0,81, respectivamente. Na análise bivariada, o CRIB, peso e idade gestacional mostraram-se preditores de mortalidade, sendo o escore CRIB>4 o de melhor resultado com sensibilidade de 75,8%, especificidade de 86,7%, valor preditivo positivo de 63,3% e valor preditivo negativo de 92,2%. CONCLUSÕES: Os recém-nascidos com peso de nascimento <750 g, idade gestacional <29 semanas e escore CRIB>10 tiveram maiores taxas de mortalidade, sendo o escore CRIB>4 o que representou melhor poder preditivo quando comparado com peso ao nascer e idade gestacional.

To determine the precision and agreement of the hemoglobin (Hb) measurements in capillary and venous blood samples by the HemoCue® and an automated counter. Hb was determined by both equipaments in blood samples of 29 pregnant women. The HemoCue® showed low repeatability of Hb measurements in duplicate in capillary (CR=0.53 g/dL, CV=13.6%) and venous blood (CR=0.53 g/dL, CV=13.6%). Hb measurements in capillary blood were higher than those in venous blood (12.4 and 11.7 g/dL, respectively; p<0.05). There was high agreement between Hb in capillary blood by the HemoCue® and in venous blood by the counter (r icc=0.86; p<0.01), and also between the diagnosis of anemia by both equipments (k=0.81; p<0.01). The HemoCue® seems to be more appropriate for capillary blood and require training of the measurers.

OBJECTIVE: To comparatively detect A. actinomycetemcomitans and F. nucleatum from periodontal and healthy sites. METHODS: Subgingival clinical samples from 50 periodontitis adult patients and 50 healthy subjects were analyzed. Both organisms were isolated using a trypticase soy agar-bacitracin-vancomycin (TSBV) medium and detected by PCR. Conventional biochemical tests were used for bacteria identification. RESULTS: A. actinomycetemcomitans and F. nucleatum were isolated in 18% and 20% of the patients, respectively, and in 2% and 24% of healthy subjects. Among A. actinomycetemcomitans isolates, biotype II was the most prevalent. Primer pair AA was 100% sensitive in the detection of A. actinomycetemcomitans from both subject groups. Primers ASH and FU were also 100% sensitive to detect this organism in healthy subject samples. Primer pair FN5047 was more sensitive to detect F. nucleatum in patients or in healthy samples than primer 5059S. Primers ASH and 5059S were more specific in the detection of A. actinomycetemcomitans and F. nucleatum, respectively, in patients and in healthy subject samples. CONCLUSIONS: PCR is an effective tool for detecting periodontal pathogens in subgingival samples, providing a faster and safer diagnostic tool of periodontal diseases. The method's sensitivity and specificity is conditioned by the choice of the set of primers used.

Neonatal anthropometry is an inexpensive, noninvasive and convenient tool for bedside evaluation, especially in sick and fragile neonates. Anthropometry can be used in neonates as a tool for several purposes: diagnosis of foetal malnutrition and prediction of early postnatal complications; postnatal assessment of growth, body composition and nutritional status; prediction of long-term complications including metabolic syndrome; assessment of dysmorphology; and estimation of body surface. However, in this age group anthropometry has been notorious for its inaccuracy and the main concern is to make validated indices available. Direct measurements, such as body weight, length and body circumferences are the most commonly used measurements for nutritional assessment in clinical practice and in field studies. Body weight is the most reliable anthropometric measurement and therefore is often used alone in the assessment of the nutritional status, despite not reflecting body composition. Derived indices from direct measurements have been proposed to improve the accuracy of anthropometry. Equations based on body weight and length, mid-arm circumference/head circumference ratio, and upper-arm cross-sectional areas are among the most used derived indices to assess nutritional status and body proportionality, even though these indices require further validation for the estimation of body composition in neonates.

In order to evaluate the capacity of laser scanning cytometry (LSC) to detect acid-fast bacilli directly on clinical samples, a comparison between Kinyoun-stained smears analyzed under light microscopy and propidium iodide-auramine-stained smears analyzed by LSC was performed. The results were compared with those for culture on BACTEC MGIT 960. LSC is a new, reliable methodology to detect Mycobacteria.

OBJETIVO: Determinar os fatores responsáveis pela associação entre via de parto normal e maior mortalidade neonatal, em coorte de recém-nascidos. MÉTODOS: Estudo de coorte retrospectiva, constituído por meio do sistema de linkage a partir dos arquivos do Sistema de Informações de Nascimentos e do Sistema Informações de Mortalidade, onde foram incluídos todos os recém-nascidos de Goiânia, no ano de 2000. Foi realizada análise estratificada da via de parto e das categorias de hospital de nascimento por fatores de risco para a mortalidade neonatal, com cálculo do Risco Relativo, com nível de significância de 5%. As associações estatísticas foram analisadas utilizando o teste qui-quadrado com nível de significância de 5%. RESULTADOS: O parto normal foi mais realizado que o operatório em situações de maior risco para a morte neonatal. Os hospitais públicos, onde o parto normal foi mais freqüente, atenderam a população de maior risco para a morte neonatal. Os hospitais privados sem atendimento ao Sistema Único de Saúde realizaram a cesariana em 84,9% dos casos. Nesses serviços, o parto normal foi realizado principalmente em situações de risco para a morte neonatal como: prematuridade extrema e muito baixo peso ao nascer. CONCLUSÕES: A associação entre parto normal e maior ocorrência de óbito neonatal decorreu de viés de seleção devido à distribuição das gestantes na rede hospitalar e, ainda, da realização quase universal de cesarianas em gestações de baixo risco e do parto normal nas gestações de alto risco para a morte neonatal.

OBJECTIVE: To identify potential prognostic factors for neonatal mortality among newborns referred to intensive care units. METHODS: A live-birth cohort study was carried out in Goiânia, Central Brazil, from November 1999 to October 2000. Linked birth and infant death certificates were used to ascertain the cohort of live born infants. An additional active surveillance system of neonatal-based mortality was implemented. Exposure variables were collected from birth and death certificates. The outcome was survivors (n=713) and deaths (n=162) in all intensive care units in the study period. Cox's proportional hazards model was applied and a Receiver Operating Characteristic curve was used to compare the performance of statistically significant variables in the multivariable model. Adjusted mortality rates by birth weight and 5-min Apgar score were calculated for each intensive care unit. RESULTS: Low birth weight and 5-min Apgar score remained independently associated to death. Birth weight equal to 2,500g had 0.71 accuracy (95% CI: 0.65-0.77) for predicting neonatal death (sensitivity =72.2%). A wide variation in the mortality rates was found among intensive care units (9.5-48.1%) and two of them remained with significant high mortality rates even after adjusting for birth weight and 5-min Apgar score. CONCLUSIONS: This study corroborates birth weight as a sensitive screening variable in surveillance programs for neonatal death and also to target intensive care units with high mortality rates for implementing preventive actions and interventions during the delivery period.

The transducer consists of a semiconductor device based on two stacked -i-n heterostructures that were designed to detect the emissions of the fluorescence resonance energy transfer between fluorophores in the cyan (470 nm) and yellow (588 nm) range of the spectrum. This research represents a preliminary study on the use of such wavelength-sensitive devices as photodetectors for this kind of application. The device was characterized through optoelectronic measurements concerning spectral response measurements under different electrical and optical biasing conditions. To simulate the fluorescence resonance energy transfer (FRET) pairs, a chromatic time-dependent combination of cyan and yellow wavelengths was applied to the device. The generated photocurrent was measured under reverse and forward bias to read out the output photocurrent signal. A different wavelength-biasing light was also superimposed. Results show that under reverse bias, the photocurrent signal presents four separate levels, each one assigned to the different wavelength combinations of the FRET pairs. If a blue background is superimposed, the yellow channel is enhanced and the cyan suppressed, while under red irradiation, the opposite behavior occurs. So, under suitable biasing light, the transducer is able to detect separately the cyan and yellow fluorescence pairs. An electrical model, supported by a numerical simulation, supports the transduction mechanism of the device.
