Objectives: To determine if providing informal care to a co-resident with dementia symptoms places an additional risk on the likelihood of poor mental health or mortality compared to co-resident non-caregivers.
Design: A quasi-experimental design of caregiving and non-caregiving co-residents of individuals with dementia symptoms, providing a natural comparator for the additive effects of caregiving on top of living with an individual with dementia symptoms. 
Methods: Census records, providing information on household structure, intensity of caregiving, presence of dementia symptoms and self-reported mental health, were linked to mortality records over the following 33 months. Multi-level regression models were constructed to determine the risk of poor mental health and death in co-resident caregivers of individuals with dementia symptoms compared to co-resident non-caregivers, adjusting for the clustering of individuals within households.
Results: The cohort consisted of 10,982 co-residents (55.1% caregivers), with 12.1% of non-caregivers reporting poor mental health compared to 8.4% of intense caregivers (>20 hours of care per week). During follow-up the cohort experienced 560 deaths (245 to caregivers). Overall, caregiving co-residents were at no greater risk of poor mental health but had lower mortality risk than non-caregiving co-residents (ORadj=0.93, 95% CI 0.79, 1.10 and ORadj=0.67, 95% CI 0.56, 0.81, respectively); this lower mortality risk was also seen amongst the most intensive caregivers (ORadj=0.65, 95% CI 0.53, 0.79).
Conclusion: Caregiving poses no additional risk to mental health over and above the risk associated with merely living with someone with dementia, and is associated with a lower mortality risk compared to non-caregiving co-residents.

We intended to evaluate the influence of sex mismatch between donor and recipient, which is still under much debate, on survival and comorbidities after cardiac transplantation. From November 2003 to December 2013, a total of 258 patients were transplanted in our center. From these, 200 receptors were male (77.5%) and constituted our study population, further divided into those who received the heart from a female donor (Group A) - 44 patients (22%) and those who received it from a male donor (Group B) - 156 (78%). Median follow-up was 4.2 ± 3.0 years (1-10 years). The two groups were quite comparable with each other, except for body mass index, systolic pulmonary artery pressure, and transpulmonary gradient, which were significantly lower in Group A. A low donor/recipient weigh ratio (<0.8) was avoided whenever possible. Hospital mortality was not different in the two groups. During follow-up, global survival was similar, as was survival free from acute cellular rejection and cardiac allograft vasculopathy. However, patients in Group A had decreased survival free from serious infections and malignant tumors. Allocation of female donors to male receptors can be done safely, at least in receptors without pulmonary hypertension and when an adequate donor/recipient weigh ratio is ensured.

Background: Quality of life and well-being are frequently restricted in adults with neuromuscular disorders. As such, identification of appropriate interventions is imperative. Objective: The objective of this paper was to systematically review and critically appraise quantitative studies (RCTs, controlled trials and cohort studies) of psychosocial interventions designed to improve quality of life and well-being in adults with neuromuscular disorders. Method: A systematic review of the published and unpublished literature was conducted. Studies meeting inclusion criteria were appraised using a validated quality assessment tool and results presented in a narrative synthesis. Results: Out of 3,136 studies identified, ten studies met criteria for inclusion within the review. Included studies comprised a range of interventions including: cognitive behavioural therapy, dignity therapy, hypnosis, expressive disclosure, gratitude lists, group psychoeducation and psychologically informed rehabilitation. Five of the interventions were for patients with Amyotrophic Lateral Sclerosis (ALS). The remainder were for patients with post-polio syndrome, muscular dystrophies and mixed disorders, such as Charcot-Marie-Tooth disease, myasthenia gravis and myotonic dystrophy. Across varied interventions and neuromuscular disorders, seven studies reported a short-term beneficial effect of intervention on quality of life and well-being. Whilst such findings are encouraging, widespread issues with the methodological quality of these studies significantly compromised the results. Conclusion: There is no strong evidence that psychosocial interventions improve quality of life and well-being in adults with neuromuscular disorders, due to a paucity of high quality research in this field. Multi-site, randomised controlled trials with active controls, standardised outcome measurement and longer term follow-ups are urgently required.

In support of the achievement goal theory (AGT), empirical research has demonstrated psychosocial benefits of the mastery-oriented learning climate. In this study, we examined the effects of perceived coaching behaviors on various indicators of psychosocial well-being (competitive anxiety, self-esteem, perceived competence, enjoyment, and future intentions for participation), as mediated by perceptions of the coach-initiated motivational climate, achievement goal orientations and perceptions of sport-specific skills efficacy. Using a pre-post test design, 1,464 boys, ages 10-15 (M = 12.84 years, SD = 1.44), who participated in a series of 12 football skills clinics were surveyed from various locations across the United States. Using structural equation modeling (SEM) path analysis and hierarchical regression analysis, the cumulative direct and indirect effects of the perceived coaching behaviors on the psychosocial variables at post-test were parsed out to determine what types of coaching behaviors are more conducive to the positive psychosocial development of youth athletes. The study demonstrated that how coaching behaviors are perceived impacts the athletes’ perceptions of the motivational climate and achievement goal orientations, as well as self-efficacy beliefs. These effects in turn affect the athletes’ self-esteem, general competence, sport-specific competence, competitive anxiety, enjoyment, and intentions to remain involved in the sport. The findings also clarify how young boys internalize and interpret coaches’ messages through modification of achievement goal orientations and sport-specific efficacy beliefs.

Although functional recovery could be advocated as an achievable treatment goal, many effective interventions for the treatment of psychotic symptoms, such as antipsychotic drugs, may not improve functioning. The last two decades of cognitive and clinical research on schizophrenia were a turning point for the firm acknowledgment of how relevant social cognitive deficits and negative symptoms could be in predicting psychosocial functioning. The relevance of social cognition dysfunction in schizophrenia patients’ daily living is now unabated. In fact, social cognition deficits could be the most significant predictor of functionality in patients with schizophrenia, non-redundantly with neurocognition. Emerging evidence suggests that negative symptoms appear to play an indirect role, mediating the relationship between neurocognition and social cognition with functional outcomes. Further explorations of this mediating role of negative symptoms have revealed that motivational deficits appear to be particularly important in explaining the relationship between both neurocognitive and social cognitive dysfunction and functional outcomes in schizophrenia. In this paper we will address the relative contribution of two key constructs—social cognitive deficits and negative symptoms, namely how intertwined they could be in daily life functioning of patients with schizophrenia.

Ocular rosacea is an important and underdiagnosed chronic inflammatory disorder observed in children. A clinical spectrum ranging from chronic eyelid inflammation, recurrent ocular redness, photophobia and/or hordeola/chalazions and conjunctival/corneal phlyctenules evolving to neovascularization and scarring may occur. Visual impairment and consequent amblyopia are frequent and corneal perforation although rare is the most feared complication. Ocular manifestations usually precede cutaneous lesions. Although few cases of pediatric ocular rosacea (POR) have been reported in the literature, many cases must have been underdiagnosed or misdiagnosed. The delay in diagnosis is greater than one year in the large majority of cases and may lead to serious ocular sequelae. This review aims to highlight the clinical features of POR, its epidemiology, easy diagnosis and effective treatment. We also propose new diagnostic criteria, in which at least three of the five clinical criteria must be present: (1) Chronic or recurrent keratoconjunctivitis and/or red eye and/or photophobia; (2) Chronic or recurrent blepharitis and/or chalazia/ hordeola; (3) Eyelid telangiectasia documented by an ophthalmologist; (4) Primary periorificial dermatitis and/ or primary features of rosacea; and (5) Positive familial history of cutaneous and/or ocular rosacea.

There is a growning need to address psychological health and safety in the workplace. Ergonomics tends to be widely recognized for its physical applications, such as ¨office¨ and ¨manual materials handling¨ however the other domains of specialization of ergonomics (cognitive and organizational) appear to be less well known. This study evaluates the level of understanding that professionals who practice ergonomics have of the relation between ergonomics and the control of psychosocial hazards in the workplace. A survey was distributed to ergonomics practitioners and asked them about their awareness of the relation between ergonomics and workplace psychosocial hazard control. Ergonomists and human factors specialists demonstrated a greater awareness of this relationship than other allied occupational groups that also practice ergonomics, however they indicated that there may be difficulties in the “real world” applying these areas of knowledge into practice. Participants who demonstrated a high level of awareness of the relation between ergonomics and psychosocial hazard control demonstrated stronger organizational commitment than participants with a low awareness. Ergonomics practitioners who reported having employer support for professional development also demonstrated a higher degree of awareness of the relation between ergonomics and psychosocial hazard control, as did the professionals who had been practicing in the field the longest. This research provides some insight for professional associations for Ergonomists, employers of Ergonomists, and human resource professionals about how ergonomics practitioners perceive the ergonomics field and the profession as well as their employing organization.

Purpose: as exposure to psychosocial hazard at work represents a substantial risk factor for employee health in many modern occupations, being able to accurately assess how employees cope with their working environment is crucial. As the workplace is generally accepted as being a dynamic environment consideration should be given to the interaction between employees and the acute environmental characteristics of their workplace. The aim of this study was to investigate the effects of both acute demand and chronic work-related psychosocial hazard upon employees through ambulatory assessment of heart rate variability and blood pressure. Design: a within-subjects repeated measures design was used to investigate the relationship between exposure to work-related psychosocial hazard and ambulatory heart rate variability and blood pressure in a cohort of higher education employees. Additionally the effect of acute variation in perceived work-related demand was investigated. Results: two dimensions of the Management Standards were found to demonstrate an association with heart rate variability; more hazardous levels of “demand” and “relationships” were associated with decreased SDNN. Significant changes in blood pressure and indices of heart rate variability were observed with increased acute demand.

Homeless women are one of the most vulnerable groups worldwide since they are victims of labor and sexual exploitation, abuse, discrimination and marginalization at a higher rate than the rest of the population. However, currently, Mexico lacks of an accurate definition of such social group, and their characteristics as well as the magnitude and dimensions of the phenomenon are only partially and superficially known. The purpose of this research was to develop and validate a typology of homeless women living in Mexico City. To fulfill this purpose, 300 indepth interviews were conducted and examined thoroughly with a systematic analysis of the content. The main results indicate that homeless women constitute a social group immersed predominantly in circumstances of violence, marginalization, poverty and social exclusion. Also, this collective includes two subgroups: women at risk of homelessness and women emergency sheltered, unsheltered or absolutely homeless and living in places not intended for human habitation. With this typology it is possible to define, characterize and distinguish homeless women. In addition, the phenomenon can be known with more certainty and accuracy enabling, thus, the design of effective treatment strategies.

Hintergrund: Die koronare Herzkrankheit (KHK) ist eine häufige und potenziell tödliche Erkrankung mit einer Lebenszeitprävalenz von über 20%. Allein in Deutschland wird die Zahl der durch die ischämische Herzerkrankung und des akuten Myokardinfarkts jährlich verursachten Todesfälle auf etwa 140.000 geschätzt. Ein Zusammenhang eng mit dem Lebensstil verbundener Risikofaktoren mit Auftreten und Prognose der KHK ist nachgewiesen. Durch Maßnahmen der nichtmedikamentösen Sekundärprävention wird versucht, diese Risikofaktoren positiv zu verändern sowie die KHK im Gegensatz zu palliativen interventionellen Therapiestrategien kausal zu behandeln. Zur Wirksamkeit der nichtmedikamentösen sekundärpräventiven Maßnahmen liegt eine ganze Reihe von Einzelstudien und -untersuchungen vor, eine systematische Analyse, die die Evidenz aller hauptsächlich angewandten Sekundärpräventionsstrategien zusammenfasst, fehlt unseres Wissens nach bislang jedoch. Auch eine Auswertung vorhandener Studien zur Kosten-Effektivität der Maßnahmen ist hierbei zu integieren. Fragestellung: Ziel dieses HTA-Berichts (HTA=Health Technology Assessment) ist die Erstellung einer umfassenden Übersicht der aktuellen Literatur zu nichtmedikamentösen Sekundärpräventionsmaßnahmen in der Behandlung der KHK, um diese Maßnahmen und deren Komponenten bezüglich ihrer medizinischen Wirksamkeit sowie Wirtschaftlichkeit zu beurteilen. Weiterhin sollen die ethischen, sozialen und rechtlichen Aspekte der nichtmedikamentösen Sekundärprävention und die Übertragbarkeit der Ergebnisse auf den deutschen Versorgungsalltag untersucht werden. Methodik: Relevante Publikationen werden über eine strukturierte und hochsensitive Datenbankrecherche sowie mittels Handrecherche identifiziert. Die Literaturrecherche wird in vier Einzelsuchen zu medizinischen, gesundheitsökonomischen, ethischen und juristischen Themen am 18.09.2008 durchgeführt und erstreckt sich über die vergangenen fünf Jahre. Die methodische Qualität der Publikationen wird von jeweils zwei unabhängigen Gutachtern unter Beachtung von Kriterien der evidenzbasierten Medizin (EbM) systematisch geprüft. Ergebnisse: Von insgesamt 9.074 Treffern erfüllen 43 medizinische Publikationen die Selektionskriterien, mit einem Nachbeobachtungszeitraum zwischen zwölf und 120 Monaten. Insgesamt ist die Studienqualität zufriedenstellend, allerdings berichtet nur ca. die Hälfte der Studien differenziert die Gesamtmortalität, während die übrigen Studien andere Outcomemaße verwenden. Die Wirksamkeit einzelner Sekundärpräventionsmaßnahmen stellt sich als sehr heterogen dar. Insgesamt kann langfristig eine Reduktion der kardialen sowie der Gesamtmortalität und der Häufigkeit kardialer Ereignisse sowie eine Erhöhung der Lebensqualität beobachtet werden. Vor allem für trainingsbasierte und multimodale Interventionen ist eine effektive Reduktion der Mortalität zu beobachten, während psychosoziale Interventionen besonders in Bezug auf eine Erhöhung der Lebensqualität effektiv zu sein scheinen. Für die ökonomischen Auswertungen werden 26 Publikationen identifiziert, die von ihrer Themenstellung und Studienart dem hier betrachteten Kontext zugeordnet werden können. Insgesamt kann festgestellt werden, dass sich die Studienlage zur multimodalen Rehabilitation sowohl bezüglich ihrer Menge als auch Qualität der Analysen besser darstellt, als dies für Evaluationen von Einzelmaßnahmen beobachtet werden kann. Die internationale Literatur bestätigt den multimodalen Ansätzen dabei zwar ein gutes Verhältnis von Kosten und Effektivität, untersucht jedoch nahezu ausschließlich ambulante oder häuslichbasierte Maßnahmen. Die Auswertung der Studien, die einzelne sich mit präventiven Maßnahmen in Hinblick auf ihre Kosten-Effektivität beschäftigen, ergibt lediglich positive Tendenzen für Interventionen der Raucherentwöhnung und des körperlichen Trainings. Im Hinblick auf psychosoziale Maßnahmen sowie auch die Ernährungsumstellung können aufgrund der unzureichenden Studienlage jedoch keine Aussagen über die Kosten-Effektivität getroffen werden. Insgesamt werden im Rahmen der Betrachtung sozialer Aspekte der nichtmedikamentösen Sekundärprävention elf Publikationen einbezogen. Die relativ neuen Studien bestätigen, dass Patienten mit niedrigem sozioökonomischen Status insgesamt schlechtere Ausgangsbedingungen und demnach einen spezifischen Bedarf an rehabilitativer Unterstützung haben. Gleichzeitig sind sich die Forscher jedoch uneinig, ob gerade diese Patientengruppe relativ häufiger oder seltener an den Rehabilitationsmaßnahmen teilnimmt. Bezüglich der Barrieren, die Patienten von der Teilnahme an den präventiven Maßnahmen abhalten, werden psychologische Faktoren, physische Einschränkungen aber auch gesellschaftliche und systemisch-orientierte Einflüsse genannt. Diskussion: Nichtmedikamentöse Sekundärpräventionsmaßnahmen sind sicher und in der Lage eine Reduktion der Mortalität sowie der Häufigkeit kardialer Ereignisse zu erzielen sowie die Lebensqualität zu erhöhen. Da nur wenige der methodisch verlässlichen Studien Teilnehmer über einen längeren Zeitraum von mindestens 60 Monaten nachverfolgen, müssen Aussagen über die Nachhaltigkeit als limitiert angesehen werden. Verlässliche Angaben in Bezug auf relevante Patientensubgruppen lassen sich nur sehr eingeschränkt machen ebenso wie im Hinblick auf die vergleichende Beurteilung verschiedener Maßnahmen der Sekundärprävention, da diese von eingeschlossenen Studien nur unzureichend erforscht wurden. Zukünftige methodisch verlässliche Studien sind notwendig, um diese Fragestellungen zu untersuchen und zu beantworten. Bezogen auf die Kosten-Effektivität nichtmedikamentöser sekundärpräventiver Maßnahmen kann aus den internationalen Studien eine insgesamt positive Aussage zusammengefasst werden. Einschränkungen dieser resultieren jedoch zum einen aus den Besonderheiten des deutschen Systems der stationären Rehabilitationsangebote, zum anderen aus den qualitativ mangelhaften Evaluationen der Einzelmaßnahmen. Studien mit dem Ziel der Bewertung der Kosten-Effektivität stationärer Rehabilitationsangebote sind ebenso erforderlich wie auch qualitativ hochwertige Untersuchungen einzeln erbrachter Präventionsmaßnahmen. Aus sozialer Perspektive sollte insbesondere untersucht werden, welche Patientengruppe aus welchen Gründen von einer Teilnahme an Rehabilitations- bzw. präventiven Maßnahmen absieht und wie diesen Argumenten begegnet werden könnte. Schlussfolgerung: Nichtmedikamentöse sekundärpräventive Maßnahmen sind in der Lage eine Reduktion der Mortalität und der Häufigkeit kardialer Ereignisse zu erzielen sowie die Lebensqualität zu erhöhen. Eine Stärkung des Stellenwerts nichtmedikamentöser Maßnahmen der Sekundärprävention erscheint vor diesem Hintergrund notwendig. Auch kann für einige Interventionen ein angemessenes Verhältnis von Effektivität und Kosten angenommen werden. Es besteht allerdings nach wie vor erheblicher Forschungsbedarf bezüglich der Wirksamkeitsbeurteilung nichtmedikamentöser Maßnahmen der Sekundärprävention in wichtigen Patientensubgruppen und der Effizienz zahlreicher angebotener Programme. Darüber hinaus ist weitere Forschung notwendig, um die Nachhaltigkeit der Maßnahmen und Gründe für die Nichtinanspruchnahme detailliert zu untersuchen. Vor allem gilt es jedoch den Versorgungsalltag in Deutschland, wie er sich für Ärzte, Patienten und weitere Akteure des Gesundheitswesens darstellt, zu untersuchen und den heutigen Stellenwert nichtmedikamentöser Maßnahmen aufzuzeigen.

Objectives. Recent literature indicates variance in psychosocial treatment preferences for negative symptoms of schizophrenia. Attempts at defining therapeutic aims and outcomes for negative symptoms to date have not included major stakeholder groups. The aim of the present study was to address this gap through qualitative methods. Design. Thematic Analysis was applied to qualitative semi-structured interview data to gather the opinions of people who experience negative symptoms, carers, and healthcare professionals. Participants were recruited from two mental health sites (inpatient/community) to increase generalisability of results. Ten people participated in the research. Methods. Semi-structured interview scripts were designed utilising evidence from the review in Chapter 1 of effective psychosocial intervention components for specific negative symptoms. Interviews were audio recorded and transcribed verbatim. Thematic analysis was employed to analyse data. Results. A common theme across groups was the need for a personalised approach to intervention for negative symptoms. Other themes indicated different opinions in relation to treatment targets and the need for a sensitive and graded approach to all aspects of therapy. This approach needs to be supported across systemic levels of organisation with specific training needs for staff addressed. Conclusions. There is disparity in treatment preferences for negative symptoms across major stakeholders. The findings suggest an individualised approach to intervention of negative symptoms that is consistent with recovery. Implementation barriers and facilitators were identified and discussed. There remains a need to develop a better understanding of treatment preferences for patients.
