OBJETIVO: Apresentar as características dos portadores e a prevalência do ectrópio em uma amostra populacional aleatória do Estado de São Paulo, Brasil. MATERIAL E MÉTODOS: Os dados foram colhidos durante a realização do Projeto de Prevenção da Cegueira da Faculdade de Medicina de Botucatu-UNESP, com amostra domiciliar sistemática aleatória. Foram avaliados 10.432 indivíduos, residentes em 11 municípios da regional de saúde localizada na região Centro-Oeste do Estado de São Paulo. Os resultados obtidos foram submetidos a análise estatística para avaliação de variáveis descritivas e da prevalência do ectrópio palpebral. RESULTADOS: Foram detectados 71 portadores de ectrópio palpebral, com prevalência estimada de 0,68%. A prevalência não foi semelhante nos Municípios estudados. A maioria dos portadores de ectrópio apresentava idade acima dos 70 anos e era do sexo masculino. Vinte e oito por cento dos portadores de ectrópio apresentavam catarata concomitantemente. CONCLUSÃO: A prevalência do ectrópio na população Centro-Oeste do Estado de São Paulo é de 0,68%. O ectrópio palpebral ocorre mais frequentemente em homens, geralmente idosos, expostos ao sol, mais na pálpebra inferior e com acuidade visual corrigida normal.

OBJETIVO: A catarata é a principal causa tratável de cegueira e deficiência visual em países subdesenvolvidos. Este estudo foi realizado para avaliar se a catarata continua sendo uma importante causa de cegueira no centro-oeste do Estado de São Paulo. MÉTODOS: Um estudo transversal, de caráter observacional, realizado em cinco cidades da região centro-oeste do Estado de São Paulo, para as quais o centro de referência é a cidade de Botucatu. A amostra estabelecida para este estudo, de forma aleatória, seria composta por 5.555 indivíduos, sendo que foram examinados 4.229 indivíduos (8.458 olhos), ou seja, 78% da amostra pretendida. Os indivíduos foram submetidos a um exame oftalmológico completo que consistia em avaliação da acuidade visual (com e sem correção), tonometria, biomicroscopia, fundoscopia e exame refracional. O diagnóstico de catarata foi dado aos indivíduos que apresentassem opacidade de cristalino na biomicroscopia, de acordo com o Sistema de Classificação de Opacidade do Cristalino II (LOCS II). Olhos apresentando deficiência visual ou cegueira, com a melhor correção e causados por catarata, foram considerados após excluir outras patologias que pudessem causar baixa da acuidade visual (AV). Olhos com deficiência visual foram considerados quando 0,05 < AV < 0,3 e olhos cegos quando AV < 0,05. Indivíduos com deficiência visual foram considerados quando 0,05 < AV < 0,3 e indivíduos cegos quando AV < 0,3, no melhor olho com a melhor correção. Foi considerado o número de olhos com catarata, o número de olhos com deficiência visual e cegueira e o número de indivíduos com deficiência visual e cegueira. RESULTADOS: A prevalência de catarata na população estudada foi de 4,94% (209 indivíduos), afetando principalmente indivíduos com mais de 50 anos (92,34%) e do sexo feminino (61,11%). A catarata foi a causa de deficiência visual para 0,96% e a causa de cegueira para 0,52% de toda a população estudada. CONCLUSÃO: Nossos resultados mostraram que a prevalência de catarata na população estudada é semelhante aos índices de países desenvolvidos.

Nineteen bovines of both sexes aging from 5 month-old to 5 year-old, were referred to a veterinary hospital. Clinical signs were observed from 6 hours to 10 days before. Thirteen animals were found in permanent lateral recumbency, five showed motor incoordination, 15 were exclusively fed on grass pasture, three showed partial loss of visual acuity, 14 were blindness, 16 showed presence of normal pupillary reflex, 16 decreased ruminal motility, 14 decreased sensorium (depression, semicoma or coma) and eight showed opisthotonos. Dehydration and dried feces were directly related to the time of evolution of the process. All the animals were administrated vitamin B1 and showed a marked improvement of the clinical status within 4 to 48 hours after treatment. The longer the time between the onset of the clinical signs and treatment, the greater the delay for the restoration of the vision. The treatment was very effective for a rapid response of the animal.

Purpose: To compare a single intraoperative sub-Tenon's capsule triamcinolone acetonicle injection with steroid drops in the treatment of ocular inflammation after cataract surgery.Design: Randomized, double-masked controlled trial.Participants: A total of 100 patients were randomized prospectively into 2 groups: 50 patients treated with 1% prednisolone eyedrops (control group A) and 50 patients treated with sub-Tenon's capsule triamcinolone (treatment group B).Methods: All patients underwent phacoemulsification and intraocular posterior lens implantation. After surgery, patients were randomized to receive either (group B) an intraoperative 40 mg triamcinolone acetonicle sub-Tenon's capsule injection or (group A) 1% prednisolone acetate eyedrops, according to the following schedule: 1 drop 4 times daily (week 1), 3 times daily (week 2), 2 times daily (week 3), once daily (week 4). To mask the study, group B received vehicle drops administered on a similar schedule, and group A received an intraoperative sub-Tenon's capsule injection of a 1 ml balanced salt solution.Main Outcome Measures: the main outcome measures included inflammation (cell, flare, ciliary flush), intraocular pressure, and lack of response.Results: Triamcinolone was shown to have anti-inflammatory efficacy clinically equivalent to conventional 1% prednisolone eyedrops in reducing intraocular inflammation, as measured by clinical methods. Triamcinolone was found to be as safe as the prednisolone in terms of adverse effects, changes in visual acuity, intraocular pressure, and biomicroscopic and ophthalmoscopic variables. on the third, seventh, fourteenth, and twenty-eighth postoperative days, a significantly lower intraocular pressure (P<0.01) was noted in the triamcinolone group than in the prednisolone group.Conclusions: A single intraoperative 40-mg triamcinolone acetonide sub-Tenon's capsule injection demonstrated a clinically equivalent therapeutic response and ocular tolerance compared with 1% prednisolone drops in controlling postoperative inflammation after uncomplicated cataract surgery and merits further investigation. Objective: To estimate the prevalence of blindness in the elderly population of Campinas, Brazil, and to describe the coverage and quality of cataract surgery services in the area. Methods: A brief assessment of cataract surgery services (using the RACSS (Rapid Assessment of Cataract Surgical Services Method) was conducted using random cluster sampling, with a sample composed of 60 clusters of 40 people aged 50 years or older. Visual acuity (VA) was measured and the lens status observed by direct visual ophthalmoscopy. From the selected sample of 2,400 subjects, 92.67% were examined. Results: Blindness (VA 3/60 with available correction) was found in 1.98 % (2.03 % among male subjects, and 1.94 % among female subjects). The prevalence of blindness varied with age, from 0.2%, in the group from 50 to 54 years, to 7.2% in those above 80. Cataract was the main cause of blindness (40.2%) followed by suspected posterior segment disorders (18.2%), diabetic retinopathy (15.9%), and glaucoma (11.4%). The cataract surgical coverage was of 93% (VA 3/60) and 82.18% when the criterion was VA 6/60 in the best eye. The main reasons the subjects did not receive surgical treatment were: fear of undergoing surgery, 11.1%; lack of awareness about the condition, 16.7%; waiting for maturity, 16.7%; and contraindication to surgery, 44.4%. Conclusion: Cataract is the major cause of blindness in Campinas. Education on eye diseases, their prevention and treatment must become part of the city's public healthcare policies.

Purpose: To evaluate the effects of panretinal photocoagulation (PRP) compared with PRP plus intravitreal bevacizumab on best corrected visual acuity (BCVA) and total area of fluorescein leakage from active new vessels (NVs) in patients with high-risk proliferative diabetic retinopathy (PDR).Methods: We carried out a prospective study of patients with high-risk PDR and no prior laser treatment who were randomly assigned to receive PRP (PRP group) or PRP plus intravitreal injection of 1.5 mg of bevacizumab (PRP-plus group). In all patients, the PRP was administered at two time-points (weeks 1 and 3), with the intravitreal bevacizumab delivered at the end of the second laser episode in the PRP-plus group. Standardized ophthalmic evaluation including Early Treatment Diabetic Retinopathy Study BCVA as well as stereoscopic fundus photography and fluorescein angiography were performed at baseline and at weeks 4, 9 (+/- 1) and 16 (+/- 2). Main outcome measures included changes in BCVA and in total area of fluorescein leakage from active NVs.Results: Twenty-two (n = 30 eyes) consecutive patients completed the 16-week follow-up. There was no significant difference between the PRP and PRP-plus groups with respect to age, gender, type or duration of diabetes, area of fluorescein leakage from active NVs or BCVA. No significant difference in BCVA was observed between the groups throughout the study period. However, the total area of actively leaking NVs was significantly reduced in the PRP-plus group compared with the PRP group at weeks 4, 9 and 16 (p < 0.001). No major adverse events were identified.Conclusions: In the short-term, the adjunctive use of intravitreal bevacizumab with PRP was associated with a greater reduction in the area of active leaking NVs than PRP alone in patients with high-risk PDR.

We retrospectively analysed 33 endophthalmitis patients treated at the Faculdade de Medicina de Botucatu. We observed that 81,8% of these patients were males and older than 60 years. Trauma was the main cause of endophthalmitis (42,4%). Endophthalmitis severity can be assessed by the fact there was no improvement on the final visual acuity.

This paper describes two solutions for systematic measurement of surface elevation that can be used for both profile and surface reconstructions for quantitative fractography case studies. The first one is developed under Khoros graphical interface environment. It consists of an adaption of the almost classical area matching algorithm, that is based on cross-correlation operations, to the well-known method of parallax measurements from stereo pairs. A normalization function was created to avoid false cross-correlation peaks, driving to the true window best matching solution at each region analyzed on both stereo projections. Some limitations to the use of scanning electron microscopy and the types of surface patterns are also discussed. The second algorithm is based on a spatial correlation function. This solution is implemented under the NIH Image macro programming, combining a good representation for low contrast regions and many improvements on overall user interface and performance. Its advantages and limitations are also presented.

Study model: observacional, retrospective. Objective: to determine the frequence of the ametropic errors and other ocular problems in children with 2 to 8 year-old at Piracicaba - SP. Patients and Method: During the school year of 2000, 1001 children enrolled at the public schools of Piracicaba - SP, age ranged from 2 to 8 years old, were referred to complete ophthalmological exam. Visual acuity was previously determined using Snellen chart, applied by school teachers. Those children presenting visual acuity equal or less than 0.8, visual complaints or visual disorders were selected to appointment. Results: 51 children (5.09%) did not attended to examination. 950 children were submitted to complete ophthalmological exam. Ametropic errors were found 70.84% of the children. The most prevalent refractive errors were Hypermetropic Astigmatism (49.62%) and Hypermetropia (32,98%). Anisometropia was found in 1.78% children. Other ocular disabilities accounted for 10.21% of the examined children, such as strabismus (3.36%), eyelid changes, allergic conjunctivitis, congenital dacryostenosis, optic atrophy, corioretinitis and congenital glaucoma. Conclusion: The frequence of ocular problems observed let us to conclude the screening programs are valid surveys on decreasing rates of preventable blindness in our country.

Objective: to evaluate the causes and the results of occlusion amblyopia treatment. Methods: a retrospective study was done including children under 12 years old. Sex, age of starting treatment, etiology, ocular deviation affection, visual acuity and time for the occlusion treatment were evaluated. Results: 158 children had all the requirements to compose the sample. 50.6% were males. 70.2% had esotropia, 13.2% exotropia, 0.7% hypertropia and 4.4% presented horizontal and vertical strabismus. 11.4% had no ocular deviation. 6.6% presented anisometropia, 0.7% congenital cataract and 6.6% other diagnoses. Visual acuity improved in 85.5%, faster in the ortophoric children. The occlusion treatment time was long in all the deviation types. Conclusion: the treatment was beneficial in improving amblyopia in the majority of the children evaluated.

Purpose: To analyze the conditions, consequences and risks of self-medication in patients attended in ophthalmology emergency room at Hospital das Clínicas de Botucatu (UNESP). Methods: Sixty patients who had used any form of self-medication were studied according: age, sex, professional activity, mainly complain, visual acuity (best-corrected vision), who administrated the self-medication (friends/parents, pharmacy clerk, advertisement, or the person himself), sort of medication or product used (eyedrops/ointments/contact lenses), cost (R$), time expenditure to medical attention, ocular complications, risk of visual loss and final diagnose. Results: The majority of patients with self-medication were male (72%). The mean age was 40,9 years (7-77 years). Patients usually used eyedrops that they had at home and delayed 3 days to the first medical evaluation. The most frequently kind of topical eyedrop used was vasoconstrictor (17%). However, many patients even knew what kind of medication they had dropped in their eyes (21%). The great majority of the patients (68%) were exposed to the risk of visual loss. According to our results, self-medication may cause visual complications in 12% of patients, in which, 42% was related to the contact lenses fit without ophthalmologic assistance. Conclusion: The majory of the patients used medication that they had in home. The topical vasoconstrictor was the most frequently used drug, however, they did not know which medication were dropping in their eyes (21%).

Purpose: To report the surgical technique and outcomes of a no-ring approach to posterior chamber intraocular lens (IOL) implantation in children and young adults with subluxation of the crystalline lens. Methods: A three-piece 5.5-mm hydrophobic acrylic IOL was implanted in which the optic and one haptic were inside the capsular bag and the other haptic was in the ciliary sulcus. The implantations were done in 13 eyes of 10 patients with lens subluxation. One haptic was fixated in the ciliary sulcus by passing it through an operculum-shaped opening in the capsular bag edge, made between 2 and 3 clock hours, clockwise from the middle point of the subluxated area. Results: The mean age of the 10 patients was 12.61 ± 8.04 years and the mean follow-up was 21.38 ± 11.29 months. The mean preoperative and postoperative best-corrected visual acuity (BCVA) was 1.15 ± 0.58 and 0.37 ± 0.17 logarithm of the minimum angle of resolution, respectively, in 11 measured eyes. The postoperative BCVA was 20/40 or better in 4 eyes (36.36%), between 20/40 and 20/60 in 6 eyes (54.54%), and between 20/80 and 20/100 in 1 eye (9.09%). The BCVA improved in all eyes. Postoperative IOL decentration between 1.0 and 1.5 mm occurred in 3 of 13 eyes (23.08%). Conclusion: The results indicate that the no-ring technique allows centration of an IOL implanted into a subluxated capsular bag without using capsule tension rings. © SLACK Incorporated.

Purpose: To assess the safety and efficacy of transitioning patients whose intraocular pressure (IOP) had been insufficiently controlled on prostaglandin analog (PGA) monotherapy to treatment with travoprost 0.004%/timolol 0.5% fixed combination with benzalkonium chloride (TTFC). Methods: This prospective, multicenter, open-label, historical controlled, single-arm study transitioned patients who had primary open-angle glaucoma, pigment dispersion glaucoma, or ocular hypertension and who required further IOP reduction from PGA monotherapy to oncedaily treatment with TTFC for 12 weeks. IOP and safety (adverse events, corrected distance visual acuity, and slit-lamp biomicroscopy) were assessed at baseline, week 4, and week 12. A solicited ocular symptom survey was administered at baseline and at week 12. Patients and investigators reported their medication preference at week 12. Results: Of 65 patients enrolled, 43 had received prior travoprost therapy and 22 had received prior nontravoprost therapy (n = 18, bimatoprost; n = 4, latanoprost). In the total population, mean IOP was significantly reduced from baseline (P = 0.000009), showing a 16.8% reduction after 12 weeks of TTFC therapy. In the study subgroups, mean IOP was significantly reduced from baseline to week 12 (P = 0.0001) in the prior travoprost cohort (19.0% reduction) and in the prior nontravoprost cohort (13.1% reduction). Seven mild, ocular, treatment-related adverse events were reported. Of the ten ocular symptom questions, eight had numerically lower percentages with TTFC compared with prior PGA monotherapy and two had numerically higher percentages with TTFC (dry eye symptoms and ocular stinging/burning). At week 12, TTFC was preferred over prior therapy for 84.2% of patients (48 of 57) by the patients themselves, and for 94.7% of patients (54 of 57) by their physicians. Conclusion: When TTFC replaced PGA monotherapy in patients whose IOP had been uncontrolled, the outcome was a significant reduction in IOP and an acceptable safety and tolerability profile. Background: Orbital infection is an uncommon devastating infection and is usually a complication of paranasal sinus infection. Without appropriate treatment, orbital infection may lead to serious complications, even death. Prompt treatment is mandatory to avoid visual loss or intracranial complications. The literature shows that initially, intravenous antibiotics should be administered, and after 48 h, if no improvement appears, the affected orbit and the sinuses must be surgically drained. The authors describe two cases of orbital cellulitis with a brief literature review. Case report: The authors describe two cases of orbital abscess caused by paranasal sinus infection. In case 1, the patient presented a decreased visual acuity associated with ophthalmoplegia of the right eye. In case 2, the patient presented a decreased visual acuity. Thus, administration of intravenous antibiotic combined with surgical drainage was performed. After surgical procedure, eye movements were normalized in case 1, and in both patients, the visual acuity returned to normal parameters. Discussion: The authors recommend early surgical drainage with parenteral antibiotic administration and careful postoperative observations by monitoring the signs and symptoms of the orbital complaint. © 2012 Springer-Verlag.

The leaf spot (Mycosphaerella leaf disease = MLD) caused by Teratosphaeria nubilosa has caused damage in eucalypt plantations in southern and southeastern Brazil. The need to assess the disease in the field to evaluate of this damage, efficiency control, evaluation of germplasm induces to the necessity of having a visual scale for evaluation of disease. The objective was to develop a diagrammatic scale for young leaves and one for adult leaves of Eucalyptus globules for MLD. To do so, the leaves collected in the field were scanned for image analysis. The damaged area, the healthy leaf area and the external area of the same scale RGB (Red, Green, Blue) were determined. Subsequently, it was determinate the levels of severity depending on the sample distribution with seven levels for young leaves and six for adult leaves. For the visual acuity test and validate the scale, the leaves were evaluated for severity, with and without scale. With this proposed scales, the assessors showed good accuracy both for young and adult leaves with R2=0,98 and R2=0,80, respectively. The importance of the development of diagrammatic scales for assessing MLD in eucalyptus must to the fact that allows quantification of the symptoms accurately and precisely.
