Nombres 11-12 et la question d'une rédaction deutéronomique dans le Pentateuque

(Résumé de l'ouvrage) The volume is presented to Professor C.H.W. Brekelmans in celebration of his 75th birthday. The editors were of the opinion that there could be no better way to honour Chris Brekelmans than with a collection of articles treating the "Deuteronomic" traditions found in the Book of Deuteronomy as well as in the so-called Tertrateuch, and Deuteronomistic History. In the past years, a renewed impetus has been given to the development of Deuteronomic Studies. The present Festschrift aims at contributing to the current debate which has a special interest in methodological questions regarding the identification and characterization of "Deuteronomistic" texts in the aforementioned complexes of Old Testament writings. The volume is divided into four sections. The first section offers a collection of articles which focus on the Book of Deuteronomy itself. The second section contains contributions dealing with Deuteronomic History. The third section is devoted to the relationship between the Deuteronomic - in the largest sense of the word - traditions and the Tetrateuch. The fourth and final section presents a collection of articles on a number of important issues, each of which offers its own direct or indirect contribution to the study of the Deuteronomi(sti)c literature and its relationship to the remaining texts of the Old Testament.

Dépistage du cancer du sein dans les cantons du Jura et de Neuchâtel, ainsi que dans le Jura bernois: évaluation du programme intercantonal, 2005-2010

Le Programme de dépistage du cancer du sein BEJUNE a démarré en mai 2005 dans le canton du Jura, s'est étendu au canton de Neuchâtel en mai 2007 et englobe le Jura bernois depuis janvier 2009. Ses buts sont de promouvoir, organiser, gérer et mener à bien une action de dépistage auprès de la population féminine dans la tranche d'âge de 50 à 69 ans révolus (environ 42'000 femmes). [...] Ce rapport constitue, après 5 ans et demi d'activité, la première évaluation épidémiologique du Programme de dépistage du cancer du sein BEJUNE. Cette évaluation a été réalisée par l'Unité d'épidémiologie du cancer (UEC) de l'Institut universitaire de médecine sociale et préventive de Lausanne (IUMSP) qui jouit d'une longue expérience dans le domaine. Dans ce rapport, on analyse le fonctionnement, la participation, la qualité et l'efficacité précoce du programme. Les indicateurs de performance retenus suivent les recommandations européennes et les directives émises par la Fédération suisse des programmes de dépistage du cancer du sein. Entre mai 2005 et novembre 2010, plus de 37'000 mammographies ont été réalisées dans 11 Centres/Instituts de radiologie. Les 6750 examens effectués annuellement correspondent à un volume de 420 mammographies par radiologue: 380 pour les 1e lecteurs et 1300 pour les 2e lecteurs (auxquels s'ajoutent les lectures réalisées dans le cadre d'un autre programme cantonal). L'interprète communautaire: une pièce centrale dans le puzzle de la consultation interculturelle [The community interpreter: a central piece in the puzzle of the intercultural consultation]

As a consequence of growing global migration, physicians in French speaking Switzerland often face communicational difficulties with allophone patients. This paper first discusses advantages and shortcomings of various ways of dealing with this kind of situations. The indication of using professional interpreters will be addressed, as well as some specific therapeutic, linguistic and relational features of triadic consultations involving a physician, a patient and an interpreter. Finally, useful practical information and advices are provided to clinicians in order to help them optimize their consultations with allophone patients.

Place de la laparoscopie dans le traitement du cancer de l'endomètre [Laparoscopy in the management of endometrial cancer]

Les récents progrès de la laparoscopie ont modifié l'approche chirurgicale des patientes atteintes d'un cancer de l'endomètre. Le centre Swissendos Fribourg en collaboration avec l'AGO (Groupe de travail pour la gynécologie oncologique) et l'AGE (Groupe de travail pour la gynécologie endoscopique) ont entrepris d'élaborer un consensus basé sur l'évidence pour la pratique de la laparoscopie dans le traitement du cancer de l'endomètre. L'objectif a été de définir une approche propre à la Suisse en fournissant aux praticiens une aide à la prise en charge. Recent advance in laparoscopy have changed the surgical approach of endometrial cancer patients. The Swissendos Center, Fribourg, in collaboration with AGO (Groupe de travail pour la gynécologie oncologique) and AGE (groupe de travail pour la gynécologie endoscopique) have established a consensus based on the available evidence for the use of laparoscopy in the management of patients with endometrial cancer The main objective was to define Swiss clinical practice guidelines appropriate to the country and consistent with the needs of the physicians.

Le Soutien psychiatrique Intensif dans le Milieu (SIM) à Lausanne: un projet pilote [Intensive Psychiatric Support at the Setting in Lausanne: a pilot project]

While the hospital remains an important element of the psychiatric health-care system, the fact that it is always the best place to treat acute psychotic episodes is still debated. After a brief review of the literature relative to the main existing community care models, the authors describe the development in the Department Universitaire de Psychiatrie Adulte (DUPA), of an alternative to hospitalisation for patient going through a severe acute psychiatric episode. They present three clinical situations and the aims of the research project, which will follow this pilot phase.

Rituximab dans le traitement des maladies rénales glomérulaires: indications et evidences cliniques [Rationale and clinical evidence for the use of rituximab in glomerular diseases].

Autoimmune glomerulopathies are an important cause of chronic kidney disease. Conventional treatments based on steroids, antiproliferative and cytotoxic agents are efficacious, but highly toxic. Because of their central role in the pathogenesis of autoimmunity, B cells have become an attractive therapeutic target. Rituximab is a monoclonal antibody directed against CD20 expressed on the surface of B cells, inducing profound depletion of B cells in the peripheral blood. In spite of encouraging results regarding the off-label use of Rituximab in membranous nephropathy, systemic lupus erythematosus and small vessel vasculitis, controlled, long-term data, and data with specific renal endpoints are currently lacking.
