Changing demographics and in particular an increasingly ageing population, in combination with improved longevity, will have a major impact on changing the face of human diseases and likewise the demand for appropriate biomaterials. The ocular surface is a multifaceted system that combines to create a unique mucosal surface, which includes the cornea, conjunctiva, sclera and lids of the eye. Physical parameters such as the eyelids and eyelashes, combined with the numerous secretory glands that produce the complex tear film, act together to protect and maintain the cornea. Unfortunately an ageing tear film and lacrimal functional unit can lead to impairment of this magnificently orchestrated structure. No single mechanism or modification is responsible but, whatever the cause, the consequence is a reduction in tear stability. An uncompromised tear film is fundamental to a healthy ocular surface. In the face of progressively changing demographics and consequent requirements for medical intervention and medical device developments, it is important to understand what effects the ageing process has on these anterior ocular structures.

With the progress of computer technology, computers are expected to be more intelligent in the interaction with humans, presenting information according to the user's psychological and physiological characteristics. However, computer users with visual problems may encounter difficulties on the perception of icons, menus, and other graphical information displayed on the screen, limiting the efficiency of their interaction with computers. In this dissertation, a personalized and dynamic image precompensation method was developed to improve the visual performance of the computer users with ocular aberrations. The precompensation was applied on the graphical targets before presenting them on the screen, aiming to counteract the visual blurring caused by the ocular aberration of the user's eye. A complete and systematic modeling approach to describe the retinal image formation of the computer user was presented, taking advantage of modeling tools, such as Zernike polynomials, wavefront aberration, Point Spread Function and Modulation Transfer Function. The ocular aberration of the computer user was originally measured by a wavefront aberrometer, as a reference for the precompensation model. The dynamic precompensation was generated based on the resized aberration, with the real-time pupil diameter monitored. The potential visual benefit of the dynamic precompensation method was explored through software simulation, with the aberration data from a real human subject. An "artificial eye'' experiment was conducted by simulating the human eye with a high-definition camera, providing objective evaluation to the image quality after precompensation. In addition, an empirical evaluation with 20 human participants was also designed and implemented, involving image recognition tests performed under a more realistic viewing environment of computer use. The statistical analysis results of the empirical experiment confirmed the effectiveness of the dynamic precompensation method, by showing significant improvement on the recognition accuracy. The merit and necessity of the dynamic precompensation were also substantiated by comparing it with the static precompensation. The visual benefit of the dynamic precompensation was further confirmed by the subjective assessments collected from the evaluation participants.

El estudio de la relación entre componentes de la dieta y la salud/enfermedad utiliza métodos de valoración de la ingesta dietética, del estatus nutricional y de marcadores de función o de efecto. En concreto, en el estudio de los carotenoides y la salud ocular, interesa el estudio de dos carotenoides sin actividad provitamina A, la luteína y la zeaxantina, por su posible papel en la optimización de la función visual y en la prevención de enfermedades crónicas asociadas a la edad, y de tres carotenoides con actividad provitamina A: -caroteno, -caroteno y -criptoxantina, por ser precursores de retinol, nutriente del que depende el ciclo visual para una visión normal. En el presente trabajo se ha llevado a cabo el estudio de los carotenoides de la dieta más relevantes para la salud ocular humana considerando de forma simultánea parámetros relacionados con la ingesta, el estatus y la función visual, así como diversas variables que pueden modificar el estatus nutricional, como son la concentración de lípidos en sangre, y la bioaccesibilidad de los carotenoides a partir de alimentos de amplio consumo...

El glaucoma engloba una serie de enfermedades que se caracterizan por la pérdida progresiva de las células ganglionares de la retina y la neuropatía del nervio óptico, que desencadenan finalmente una ceguera irreversible. La elevación de la presión intraocular (PIO) aparece como el principal desencadenante de esta patología en la mayoría de los casos, ya que produce una disminución del flujo sanguíneo hacia la retina y un aumento del estrés oxidativo. La estrategia terapéutica más común implica el uso de fármacos hipotensores, a menudo combinados, asociados a efectos no deseados. En este marco, la melatonina y sus análogos, como el 5-MCA-NAT, surgen como alternativas a los fármacos ya existentes, por sus propiedades hipotensoras y antioxidantes y, en el caso del 5-MCA-NAT, por su posible acción a largo plazo. El objetivo de esta tesis es elucidar los mecanismos de la acción hipotensora aguda ya descrita para estos compuestos, determinando además el receptor y las vías de señalización implicadas en esta respuesta a corto plazo. En este mismo sentido nos proponemos confirmar la existencia de efectos hipotensores a largo plazo, analizando esta respuesta a diferentes fármacos y esclareciendo si ésta se debe a la capacidad de estos compuestos como modificadores de la expresión génica de enzimas o receptores implicados en la regulación de la PIO...

El glaucoma es una neuropatía óptica crónica de etiología multifactorial, que provoca una pérdida progresiva e irreversible de la visión debida a la degeneración de los axones del nervio óptico y la muerte apoptótica de las células ganglionares de la retina (Downs et al., 2011; Quigley, 2011; Soto et al., 2014). Actualmente es la principal causa de ceguera irreversible en el mundo (Pascolini et al., 2012). Aunque su etiología aún no está clara, se sabe que el principal factor de riesgo para desarrollar la enfermedad es el aumento de la PIO debido a anomalías en la dinámica del humor acuoso (Casson et al., 2012). La presencia de receptores purinérgicos P2X y P2Y ha sido descrita en las principales estructuras oculares relacionadas con la producción y drenaje del humor acuoso, los procesos ciliares y la malla trabecular, que son bañadas por dicho fluido (Guzman-­‐Aranguez et al., 2013). Al mismo tiempo, se ha referido la presencia del dinucleótido Ap4A, agonista de dichos receptores, en el humor acuoso, y cómo sus niveles se veían significativamente incrementados en pacientes glaucomatosos (Castany et al., 2011). El objetivo de esta tesis ha sido evaluar la implicación de los receptores purinérgicos P2Y2 de los procesos ciliares en la hipertensión ocular. Más concretamente determinar el papel del Ap4A a través de la activación de dichos receptores sobre la movilización del ion cloruro y del agua a través de las proteínas acuaporinas (AQPs), principales componentes del humor acuoso. Por otra parte, se ha investigado el origen de los niveles incrementados de Ap4A debidos al aumento de la PIO, caracterizando uno de los posibles sensores celulares implicados en dicho proceso. Purpose: Recent studies have documented a link between axial myopia and ciliary muscle morphology; yet, the variation in biometric characteristics of the emmetropic ciliary muscle are not fully known. Ciliary muscle morphology, including symmetry, was investigated between both eyes of emmetropic participants and correlated to ocular biometric parameters. Methods: Anterior segment optical coherence tomography (Zeiss, Visante) was utilised to image both eyes of 49 emmetropic participants (mean spherical equivalent refractive error (MSE) ≥ -0.55; < +0.75 D), aged 19 to 26 years. High resolution images were obtained of nasal and temporal aspects of the ciliary muscle in the relaxed state. MSE of both eyes was recorded using the Grand Seiko WAM 5500; axial length (AXL), anterior chamber depth (ACD) and lens thickness (LT) of the right eye were obtained using the Haag-streit Lenstar LS 900 biometer. A bespoke semi-objective analysis programme was used to measure a range of ciliary muscle parameters. Results: Temporal ciliary muscle overall length (CML) was greater than nasal CML, in both eyes (right: 3.58 ± 0.40 mm and 3.85 ± 0.39 mm for nasal and temporal aspects, respectively, P < 0.001; left: 3.65 ± 0.35 mm and 3.88 ± 0.41 mm for nasal and temporal aspects, respectively, P < 0.001). Temporal ciliary muscle thickness (CMT) was greater than nasal CMT at 2 mm and 3 mm from the scleral spur (CM2 and CM3, respectively) in each eye (right CM2: 0.29 ± 0.05 mm and 0.32 ± 0.05 mm for nasal and temporal aspects, respectively, P < 0.001; left CM2: 0.30 ± 0.05 mm and 0.32 ± 0.05 mm for nasal and temporal aspects, respectively, P < 0.001; right CM3: 0.13 ± 0.05 mm and 0.16 ± 0.04 mm for nasal and temporal aspects, respectively, P < 0.001; left CM3: 0.14 ± 0.04 mm and 0.17 ± 0.05 mm for nasal and temporal aspects, respectively, P < 0.001). AXL was positively correlated with ciliary muscle anterior length (AL) (e.g. P < 0.001, r2 = 0.262 for left temporal aspect), CML (P = 0.003, r2 = 0.175 for right nasal aspect) and ACD (P = 0.01, r2 = 0.181). Conclusions: Morphological characteristics of the ciliary muscle in emmetropic eyes display high levels of symmetry between the eyes. Greater CML and AL are linked to greater AXL and ACD, indicating ciliary muscle growth with normal ocular development.

Introduction: Anterior and posterior segment eye diseases are highly challenging to treat, due to the barrier properties and relative inaccessibility of the ocular tissues. Topical eye drops and systemically delivered treatments result in low bioavailability. Alternatively, direct injection of medication into the ocular tissues is clinically employed to overcome the barrier properties, but injections cause significant tissue damage and are associated with a number of untoward side effects and poor patient compliance. Microneedles (MNs) has been recently introduced as a minimally invasive means for localizing drug formulation within the target ocular tissues with greater precision and accuracy than the hypodermic needles.  Areas covered: This review article seeks to provide an overview of a range of challenges that are often faced to achieve efficient ocular drug levels within targeted tissue(s) of the eye. It also describes the problems encountered using conventional hypodermic needle-based ocular injections for anterior and posterior segment drug delivery. It discusses research carried out in the field of MNs, to date.
Expert opinion: MNs can aid in localization of drug delivery systems within the selected ocular tissue. And, hold the potential to revolutionize the way drug formulations are administered to the eye. However, the current limitations and challenges of MNs application warrant further research in this field to enable its widespread clinical application.

Relato de quatro casos de tuberculose ocular presumida, com comprometimento do segmento posterior em três destes casos. Nos dois primeiros casos, relata-se comprometimento do segmento anterior do olho e antecedente de tuberculose, em um caso sistêmica e no outro ocular. No terceiro caso, paciente apresenta lesão coriorretiniana no olho esquerdo. No quarto caso, descrita lesão serpiginosa-like. Os pacientes evoluíram favoravelmente com o tratamento específico. As lesões oculares da tuberculose são diversas e devemos continuar atentos a esta enfermidade.

Objective: To determine the prevalence of ocular findings of the external structures and anterior segment of the eye, detected by biomicroscopic examination in schoolchildren in Natal (RN) - Brazil. Methods: After previous random selection, 1,024 pupils from elementary and secondary public and private schools in the city of Natal were evaluated from March to June 2001. All were submitted to preestablished standard research norms, consisting of identification, demographic information, ophthalmologic biomicroscopic examination, with slit lamp, performed by ophthalmologists from the “Onofre Lopes” University Hospital. Results: Alterations of the conjunctival and palpebral conditions were the most prevalent (10.4% and 6.2% respectively). Follicles (4.2%) and papillae (3.0%) were the frequent conjunctival lesions, while blepharitis (3.5%) and meibomitis (1.1%) were the most detected abnormalities in the eyelids. Upon examining the cornea, iris, lens and anterior vitreous, the most encountered findings were nubecula (0.5%), papillary membrane reliquiae (0.5%), posterior capsula opacity (0.8%) and hyaloid arteria reliquiae (2.0%). Conclusion: The most prevalent findings affecting the external structures of the eye such as eyelids and conjunctiva, consisted of blepharitis followed by follicular reaction of the conjunctiva. The most prevalent abnormalities in the cornea, iris, lens and anterior vitreous were nubecula, papillary membrane reliquiae, posterior capsular opacity and hyaloid arteria reliquiae, in that order
