In 2012, intramuscular midazolam appears as effective as intravenous lorezepam for the first line treatment of convulsive status epilepticus. Perampanel, a new anti-epileptic drug, will be soon available. Two oral treatments are now available for stroke prevention in atrial fibrillation setting. The methylphenidate and the Tai Chi could increase the walk capacity of patients suffering from Parkinson disease. A comprehensive cardiac work-up is essential for some congenital myopathy. A new drug against migraine seems free from vasoconstrictive effect. Antioxidants are harmful in Alzheimer disease. Some oral medication will be available for multiple sclerosis.

Demyelination as a complication of new immunomodulatory treatments.

Purpose of review: This review discusses demyelinating events of the nervous system that have been associated with new immunomodulatory treatments, in particular monoclonal antibodies (mAbs). Recent findings: Natalizumab, a mAb targeting the alpha-4 integrins, which is efficient in relapsing-remitting multiple sclerosis, has been associated with progressive multifocal leukoencephalopathy (PML). We will review the putative mechanisms linking natalizumab with JC virus, the agent of PML. Efalizumab, a mAb targeting a member of the integrin family, CD11a, was approved for the treatment of psoriasis, but had to be withdrawn in 2009 because of the occurrence of three cases of PML. Rituximab, an anti-CD20 mAb, is used in different neoplastic and autoimmune diseases and may soon enter the pharmacopeia of multiple sclerosis. It has been suggested that rituximab is a risk factor for PML; however, evidence of such a link is unclear. Antitumor necrosis factor-alpha agents are used in several autoimmune diseases. Several cases of demyelinating events of the nervous system have been reported, prompting a heightened surveillance of treated patients. Recent data are reassuring, suggesting that the incidence of such events is relatively low. Summary: Neurologists must become familiar with neurological complications of new immunomodulatory treatments, a field situated at the interface of neurology, immunology and infection.

Peut-on faire confiance à l'administration ? Une analyse du concept de confiance, appliqué au secteur public

D'une manière générale, la confiance est un « lubrifiant » (Arrow, 1974) facilitant les relations sociales (eg Luhmann, 1968, Giddens, 1984, Zucker, 1986). D'un point de vue économique, un environnement institutionnel bénéficiant d'une confiance élevée serait corrélé avec la performance de l'économie nationale (eg Dyer and Chu, 2003). La confiance, en tant que capital social, permettrait et engendrerait des comportements altruistes et coopératifs impactant et soutenant la prospérité économique d'une communauté (Fukuyama, 1995). Dans un cadre organisationnel, la confiance engendre une communication plus ouverte et facilite l'échange d'information (eg Smith and Barclay, 1997, Currall and Judge, 1995), facilite la gestion des conflits (eg Blomqvist, 2002), l'apprentissage commun (eg Bijlsma-Frankema, 2004) ou diminue les coûts d'intégration (eg Bidault and Jarillo, 1995), par exemple.

Vitamin D has a direct immunomodulatory effect on CD8+ T cells of patients with early multiple sclerosis and healthy control subjects.

Little is known on a putative effect of vitamin D on CD8+ T cells. Yet, these cells are involved in the immmunopathogenesis of MS. We assessed the cytokine profile of EBV-specific CD8+ T cells of 10 early MS patients and 10 healthy control subjects with or without 1,25(OH)(2)D(3) and found that, with 1,25(OH)(2)D(3), these cells secreted less IFN-γ and TNF-α and more IL-5 and TGF-β. CD4+ T cell depletion or even culture with CD8+ T cells only did not abolish the immunomodulatory effect of 1,25(OH)(2)D(3) on CD8+ T cells, suggesting that 1,25(OH)(2)D(3) can act directly on CD8+ T cells.

(Résumé de l'ouvrage) Neue medizinische Entwicklungen lösen zugleich Hoffnungen und Ängste aus. Immer werden dabei auch Fragen zur ethischen Verantwortbarkeit dieser neuen Möglichkeiten aufgeworfen. Im Spannungsfeld zwischen dem wissenschaftlichen Fortschritt, der gelegentlich die Grenzen erprobten medizinischen Handelns erreicht, und materiellen Allokationsfragen wird das Bedürfnis nach einer «Ethik in der Medizin» immer stärker. Das vorliegende, auf die spezifischen Bedürfnisse der Schweiz ausgerichtete Buch schliesst hier eine grosse Lücke.

Medical aspects of helicopter rescue missions involving winching of a physician

Introduction: Whereas the use of helicopters as a rapid means toreach victims and to bring them to a secure place is well-recognized,very few data are available about the value of winching physicians toprovide medical care for the victims directly on-site. We sought to studythe medical aspects of alpine helicopter rescue operations involving thewinching of an emergency physician to the victim.Methods: We retrospectively reviewed the medical reports of a singlehelicopter-based emergency medical service. Data from 1 January 2003to 31 December 2008 were analyzed. Cases with emergency callindicating that the victim was deceased were excluded. Data includedthe category (trauma or illnesses), and severity (NACA score) of theinjuries, along with the main medical procedures performed on site.Results: 9879 rescue missions were conducted between 1 January2003 and 31 December 2008. The 921 (9.3%) missions involvingwinching of the emergency physician were analysed. 840 (91%)patients suffered from trauma-related injuries. The cases of the 81 (9%)people presenting with medical emergencies were, when compared tothe trauma victims, significantly more severe according to the NACAindex (p <0.001). Overall, 246 (27%) patients had a severe injury orillness, namely, a potential or overt vital threat (NACA score between4-6, table 1). A total of 478 (52%) patients required administration ofmajor analgesics: fentanyl (443 patients; 48%), ketamine (42 patients;5%) or morphine (7 patients; 1%). The mean dose of fentanyl was 188micrograms (range 25-750, SD 127). Major medical interventions wereperformed 72 times on 39 (4%) patients (table 2).Conclusions: The severity of the patients' injuries or illnesses alongwith the high proportion of medical procedures performed directlyon-site validate emergency physician winching for advanced life supportprocedures and analgesia.

L'enfant terrible du christianisme

Les Actes des Apôtres, le seul récit de la vie de Paul en notre possession, racontent la lapidation d'Étienne sous les yeux de Saül, puis les voyages missionnaires de Saül, devenu Paul sur la route de Damas. On ne connaît de lui ni son nom complet, ni sa date de naissance, ni les circonstances exactes de sa mort, à Rome. Dans la Bible, on trouve treize lettres de Paul écrites à des amis et à des communautés chrétiennes, en Méditerranée. Mais elles ne sont pas forcément de lui. Le personnage, son itinéraire et son oeuvre, fascinent encore et divisent. Juif radical pour certains, fils de la grande culture grecque, ou enfant terrible du christianisme pour d'autres... Paul le converti est un passeur de frontières, un visionnaire. Le monde de la Bible a mené l'enquête auprès des meilleurs spécialistes contemporains.

L'éthique protestante dans la crise de la modernité. Généalogie, critique, reconstruction

L'éthicien Denis Müller présente ici une généalogie, une critique et une reconstruction théologique de l'éthique protestante. Dans un premier moment, l'auteur réfléchit à la transformation considérable qui semble affecter aujourd'hui la situation de l'éthique théologique. Dans un deuxième temps est abordée la problématique de la post-modernité dans sa dynamique de déconstruction qui affecte évidemment les modèles éthiques antérieurs. Dans une troisième partie, coeur de l'ouvrage, Denis Müller propose un modèle de reconstruction de l'éthique théologique où sont abordées des questions centrales pour toute sensibilité protestante et chrétienne: la tradition, la signification de l'herméneutique, l'usage de la Bible.
