Background: Cancer cachexia is a complex metabolic syndrome characterised by severe and progressive weight loss which is predominantly muscle mass. It is a devastating and distressing complication of advanced cancer with profound bio-psycho-social implications for patients and their families. At present there is no curative treatment for cachexiain advanced cancer therefore the most important healthcare response entails the minimisation of the psycho-social distress associated with it. However the literature suggests healthcare professionals’are missing opportunities to intervene and respond to the multi-dimensional needs of this population.

Objective:The objective of this study was to explore healthcare professionals’ response to cachexia in advanced cancer.

Methods: An interpretative qualitative approach was adopted in this study. A purposive sample of doctors, nurses, specialist nurses and dieticians were recruited from a regional cancer centre between November 2009 and November 2010. Data was collection was twofold: two multi-professional focus groups were conducted first to uncover the main themes and issues in cachexia management. This data then informed the interview schedule for the following 25 individual semi-structured interviews.

Results: Preliminary data analysis of the semi-structured interviews revealed distinct differences between disciplines in their perceptions of cancer cachexia which influenced their response to it in clinical practice. The commonality between disciplines, with the exception of palliative care, was a reliance on the biomedical approach to cancer cachexia management.

Discussion and Conclusions: Cancer cachexia is a complex and challenging syndrome which needs to be addressed from a holistic model of care to reflect the multi-dimensional needs of this patient group. The perspectives of those involved in care delivery is required in order to inform the development of interventions aimed at minimising the distress associated with this devastating syndrome.

This paper describes an end-user model for a domestic pervasive computing platform formed by regular home objects. The platform does not rely on pre-planned infrastructure; instead, it exploits objects that are already available in the home and exposes their joint sensing, actuating and computing capabilities to home automation applications. We advocate an incremental process of the platform formation and introduce tangible, object-like artifacts for representing important platform functions. One of those artifacts, the application pill, is a tiny object with a minimal user interface, used to carry the application, as well as to start and stop its execution and provide hints about its operational status. We also emphasize streamlining the user's interaction with the platform. The user engages any UI-capable object of his choice to configure applications, while applications issue notifications and alerts exploiting whichever available objects can be used for that purpose. Finally, the paper briefly describes an actual implementation of the presented end-user model. © (2010) by International Academy, Research, and Industry Association (IARIA).

Since the financial crash of 2008 monetary policy has been in a state of stasis – a condition in which things are not changing, moving, or progressing, but rather appear frozen. Interest rates have been frozen at low levels for a considerable period time. Inflation targets have consistently been missed, through phases of both overshooting and undershooting. At the same time, a variety of unconventional monetary policies involving asset purchases and liquidity provision have been pursued. Questions have been raised from a variety of sources, including various international organizations, covering distinct BIS and IMF positions about the continuing validity and sustainability of existing monetary policy frameworks, not least because inflation targeting has ceased to act as reliable guide for policy for over six years. Despite this central banks have been reluctant to debate moving to a new formal policy framework. This article argues that as an apex policy forum only the G20 leaders’ summits has the necessary political authority to call their central banks to account and initiate a wide ranging debate on the future of monetary policy. A case is made for convening a monetary policy working group to discuss a range of positions, including those of the BIS and IMF, and to make recommendations, because the G20 has been most effective in displaying international financial leadership, when leaders have convened and made use of specialist working groups.

Background: Rapid Response Systems (RRS) have been implemented nationally and internationally to improve patient safety in hospital. However, to date the majority of the RRS research evidence has focused on measuring the effectiveness of the intervention on patient outcomes. To evaluate RRS it has been recommended that a multimodal approach is required to address the broad range of process and outcome measures required to determine the effectiveness of the RRS concept. Aim: The aim of this paper is to evaluate the official RRS programme theoretical assumptions regarding how the programme is meant to work against actual practice in order to determine what works. Methods: The research design was a multiple case study approach of four wards in two hospitals in Northern Ireland. It followed the principles of realist evaluation research which allowed empirical data to be gathered to test and refine RRS programme theory [1]. This approach used a variety of mixed methods to test the programme theories including individual and focus group interviews with a purposive sample of 75 nurses and doctors, observation of ward practices and documentary analysis. The findings from the case studies were analysed and compared within and across cases to identify what works for whom and in what circumstances. Results: The RRS programme theories were critically evaluated and compared with study findings to develop a mid-range theory to explain what works, for whom in what circumstances. The findings of what works suggests that clinical experience, established working relationships, flexible implementation of protocols, ongoing experiential learning, empowerment and pre-emptive management are key to the success of RRS implementation.  Conclusion:These findings highlight the combination of factors that can improve the implementation of RRS and in light of this evidence several recommendations are made to provide policymakers with guidance and direction for their success and sustainability.References: 1.Pawson R and Tilley N. Background Rapid Response Systems (RRS) consist of four interrelated and interdependent components; an event detection and trigger mechanism, a response strategy, a governance structure and process improvement system. These multiple components of the RRS pose problems in evaluation as the intervention is complex and cannot be evaluated using a traditional systematic review. Complex interventions in healthcare aimed at changing service delivery and related behaviour of health professionals require a different approach to summarising the evidence. Realist synthesis is such an approach to reviewing research evidence on complex interventions to provide an explanatory analysis of how and why an intervention works or doesn’t work in practice. The core principle is to make explicit the underlying assumptions about how an intervention is suppose to work (ie programme theory) and then use this theory to guide evaluation. Methods A realist synthesis process was used to explain those factors that enable or constrain the success of RRS programmes. Results The findings from the review include the articulation of the RRS programme theories, evaluation of whether these theories are supported or refuted by the research evidence and an evaluation of evidence to explain the underlying reasons why RRS works or doesn’t work in practice. Rival conjectured RRS programme theories were identified to explain the constraining factors regarding implementation of RRS in practice. These programme theories are presented using a logic model to highlight all the components which impact or influence the delivery of RRS programmes in the practice setting. The evidence from the realist synthesis provided the foundation for the development of hypothesis to test and refine the theories in the subsequent stages of the Realist Evaluation PhD study [1]. Increasing consumer demand for seafood, combined with concern over the health of our oceans, has led to many initiatives aimed at tackling destructive fishing practices and promoting the sustainability of fisheries. An important global threat to sustainable fisheries is Illegal, Unreported and Unregulated (IUU) fishing, and there is now an increased emphasis on the use of trade measures to prevent IUU-sourced fish and fish products from entering the international market. Initiatives encompass new legislation in the European Union requiring the inclusion of species names on catch labels throughout the distribution chain. Such certification measures do not, however, guarantee accuracy of species designation. Using two DNA-based methods to compare species descriptions with molecular ID, we examined 386 samples of white fish, or products labelled as primarily containing white fish, from major UK supermarket chains. Species specific real-time PCR probes were used for cod (Gadus morhua) and haddock (Melanogrammus aeglefinus) to provide a highly sensitive and species-specific test for the major species of white fish sold in the UK. Additionally, fish-specific primers were used to sequence the forensically validated barcoding gene, mitochondrial cytochrome oxidase I (COI). Overall levels of congruence between product label and genetic species identification were high, with 94.34% of samples correctly labelled, though a significant proportion in terms of potential volume, were mislabelled. Substitution was usually for a cheaper alternative and, in one case, extended to a tropical species. To our knowledge, this is the first published study encompassing a large-scale assessment of UK retailers, and if representative, indicates a potentially significant incidence of incorrect product designation.

Introdução: A compreensão do papel da intimidade no cuidar é uma questão essencial para o estabelecimento das competências relacionais no processo de cuidar o utente, visto que este se centra no desenvolvimento de relações interpessoais. Na formação dos estudantes de Enfermagem, visa-se o desenvolvimento de competências, assentes na compreensão da dimensão ética dos cuidados de forma a “promover o desenvolvimento ético, com respeito pela autonomia das pessoas, pressupõe prudência, reflexão crítica, consciência de cidadania e de responsabilidade” (Bettencourt, 2008: 61). Sendo assim, o modelo de formação em enfermagem deve assentar num processo reflexivo que proporcione aos estudantes a aquisição de posturas e condutas que lhe permitam a aquisição de competências profissionais. Objetivo: Compreender quais as variáveis que são preditoras do desenvolvimento das dimensões das competências relacionais na preservação da intimidade do utente. Metodologia: Estudo descritivo – correlacional de abordagem quantitativa. A população acessível, foram os estudantes da licenciatura em enfermagem de uma Escola Superior de Enfermagem integrada numa Universidade (A) e de uma Escola Superior de Saúde integrada num Instituto Politécnico (B). A amostra foi constituída por todos os estudantes de enfermagem das referidas escolas a partir do ano em que iniciam o primeiro ensino clínico. Os instrumentos de recolha de dados foram um questionário de caracterização dos estudantes e do contexto clínico e um Inventário de Competências Relacionais de Ajuda (ICRA). Os dados foram analisados com recurso ao Software IBM® SPSS® Statistic (Statistical Package for Social Sciences) de onde se fez uma análise dedutiva. Resultados: As variáveis independentes que melhor predizem a variável dependente (Competências relacionais) são a Escola seguida do ano e do acompanhamento em ensino clínico. Após a utilização dos métodos de seleção de preditores (seleção de Forward, seleção Backward e seleção Stepwise) as nossas variáveis independentes preditoras são o ano, a escola e o tipo de acompanhamento em EC, as restantes foram excluídas (sexo, idade, estado civil e habilitações literárias). Conclusões: As variáveis preditoras evidenciam a sua importância no desenvolvimento das competências relacionais dos estudantes de enfermagem, interferindo nos seus comportamentos enquanto estudantes e futuros enfermeiros. São variáveis que de uma ou de outra forma vão influenciar como é que cada estudante se vai comportar perante as situações de cuidado, onde se salientam nomeadamente o sexo e o ano do curso, variáveis que são intrínsecas a cada estudante, enquanto a Escola e o tipo de acompanhamento em EC são extrínsecas aos estudantes. Estas últimas são, portanto, as que consideramos que poderão ser alteradas em cada escola para que os estudantes adquiram e desenvolvam este domínio das competências de forma mais harmoniosa com o cuidar em enfermagem. O presente estudo tem como objetivos analisar a estratégia competitiva das adegas cooperativas do Alentejo, através da análise das ações que têm sido por elas desenvolvidas, propor linhas adicionais de ação estratégica e perspectivar as suas possibilidades de sucesso. A inovação e a flexibilidade das adegas cooperativas para se adaptarem ao crescente dinamismo dos contextos empresariais, conduziu a uma gestão mais profissional e a encarar a estratégia como necessária para uma maior competitividade. Nesse sentido, a cooperação entre as adegas cooperativas é vista como um meio de aumentar a competitividade no mercado internacional. A investigação permitiu constatar que o sector vitivinícola a nível mundial esta na fase de maturidade, com excesso de capacidade produtiva, procura global decrescente e queda de pregos, apresenta, no entanto, alterações significativas nos hábitos de consumo. As adegas cooperativas utilizam estratégias de diferenciação, apostam em produtos de qualidade e com elevados índices de notoriedade entre os consumidores. As principais diferenças competitivas entre elas estão relacionadas com a dimensão e a relação entre esta e a capacidade de recursos para investir ao nível do Marketing-Mix por forma a construir vantagens competitivas duradouras através da notoriedade dos seus vinhos. Para além disso, verifica-se também nas adegas de maior dimensão uma gestão estratégica mais orientada para a competitividade sustentável e maior visão estratégica. O método utilizado (estudo de caso comparativo, de natureza exploratória e, essencialmente qualitativo) não permite uma generalização extensiva dos seus resultados para além das cooperativas estudadas. Os resultados devem ser entendidos, de forma ponderada, no seu contexto específico e para os casos em concreto, pois, cada organização tem características únicas que influenciam o funcionamento da gestão. ABSTRACT - The present study has has been to analyse the competitive strategy of the Alentejo cooperative cellars, through the analysis of the shares that have been developed for the cooperative cellars, propose additional lines of strategic action and preview their possibility of success. The innovation and the flexibility of the cooperative cellars to the dynamic growth of the business contexts, accelerated shattering process of the enterprise contexts, lead to a more professional management and to face the strategy as necessary for a bigger competitiveness. In this direction, the cooperation between the cooperative cellars is seen as a way to increase the competitiveness in the international market. The inquiry allowed to evidence a world industry in maturity phase, with excess of productive capacity, presents a decreasing demand, alteration of the consumption habits and fall of the prices. The cooperative cellars use differentiation strategies, bet in products of quality and with raised indices of notoriety between the consumers. The main competitive differences between them are related with the dimension and the relation between this and the capacity of resources to invest to the level of Marketing-Mix for form to construct lasting competitive advantages through the notoriety of its wines. For moreover, a strategic management more guided for the sustainable competitiveness is also verified in the cellars of bigger dimension and bigger strategic vision. Due to the applied method (comparative case study, in searching manner and, essencialy qualitative) it cannot be purposed an extensive generalization of the results to another non-studied population. The results must be understood in a balanced way, in their Enterprise Strategy: Contribution for the Strategic Analysis of the Alentejo Wines Cooperatives specific context, and for real cases, since each company has its own features that influences the management work.

Purpose – The purpose of this paper is to analyse the competitiveness of the European Union Member States of Southern Europe (France, Greece, Italy, Portugal and Spain) as tourist destinations for European Union Member States of Central and Northern Europe (Austria, Belgium, Denmark, Finland, Germany, The Netherlands, Ireland, Sweden and the UK). Design/methodology/approach – Application of the market share analysis tool, initially developed by Faulkner, using secondary data from Eurostat – statistical office of the European communities. Findings – The results obtained show that France, Greece, Italy, Portugal and Spain present distinct levels of competitiveness for the various generating countries, with changes having occurred in the period between 1999 and 2007. Originality/value – The paper offers refreshment of Faulkner's tool and an insight into tourist flows in Europe as a tool for tourism and hospitality managers.
