An interdisciplinary team of a pediatric university teaching hospital in the Montreal area, who treats annually a considerable number of child burn victims, developed a behavioral observation scale on the comfort of children of 7 years of age and less during painful procedures «Échelle d’observation comportementale du confort d’enfants brûlés» (OCCEB- BECCO). The goal of this study was to initiate the validation of OCCEB-BECCO, a new tool for evaluation of comfort for child burn victims. With samples of 16 patients and 5 experts, we proceeded to the evaluation of content validity, internal consistency and criterion validity. Results have shown adequate content validity, internal consistency with (T1) r = 0,96 p < 0,0001, (T2) r = 0,95 p < 0,0001, (T3) r = 0,95 p < 0,0001 and criterion validity with Cronbach alpha at 0,82. A future study with a larger sample and on a longer period of time would be required to pursue validation of this new scale.

El objetivo principal de este trabajo es realizar una revisión teórica de los estudios que han elaborado un análisis acerca de la Inteligencia Emocional con la capacidad para afrontar situaciones generadoras de estrés. Los diferentes estudios muestran que niveles altos en Inteligencia Emocional se relacionan con estrategias de afrontamiento basadas en el análisis y resolución de conflictos, mientras que niveles bajos de inteligencia emocional se relacionan con estrategias de afrontamiento basadas en la evitación, la superstición, y la resistencia al cambio. La evidencia que arrojan los estudios indican que la inteligencia emocional es fundamental en el autocontrol emocional y en la habilidad de adaptación de los individuos para afrontar situaciones generadoras de estrés.

O presente trabalho tem como ponto de partida a questão de investigação: "Que tipo de intervenções de enfermagem podem melhorar as actividades de vida diárias dos doentes com Lúpus Eritematoso Sistémico?"; e procura identificar as necessidades sentidas por estes doentes, de forma a se poderem estabelecer estratégias para melhorar as suas actividades de vida. Desenvolveu-se uma pesquisa descritiva de carácter exploratório, com características comparativas do tipo correlacional. Constatou-se que os inquiridos consideram importante o seu acompanhamento por parte dos enfermeiros, uma vez que estes os podem ajudar a melhorar as suas actividades de vida diárias.

The Brazilian Psychiatric Reform based on the desinstitucionalization of the assistance, translated to the emphasis on community/territorial treatment and in the social inclusion of the mental suffering, promoted advances in the psychiatric restructuring. In the Rio Grande do Norte (RN), we can enumerate as advances of the Brazilian Psychiatric Reform the expansion of the mental health care chain and the implementation of some strategies that, together, aims to further the psychosocial attention of the individual with psych suffering and to reduce the indices of psychiatrics readmissions in the state. In the current Brazilian‟s mental health situation we were interesting in answered the following question: what the impact of the substitutes services‟ extension in the revolving door phenomenon? This search aims to analyze the revolving door phenomenon occurrences based on the news strategies of mental health care in the Rio Grande do Norte. This is a descriptive-exploratory study with a qualitative approach, oriented by the theoretical framework of critical-dialectical approach about the Brazilian Psychiatric Reform and using the thematic oral history as method of information collects. The search was realized on the Hospital João Machado (HJM), estate reference in psychiatric treatment, and the participants was 20 professionals that work on it. The collection of information had started after the approval of the UFRN Research Ethics Committee with the opinion number 216/2011 and CAAE number 0021.0.051.000-11 and was realized using the direct observation and semi-structured interview. The study‟s results were categorized in two categories and five subcategories of analysis. CATEGORY 1) Current situation of the mental health care chain in the RN, with the subcategories: 1.1 Impact of the new services of mental health care in the revolving door phenomenon in the RN; 1.2 Implications of the new services of mental health care in assisting user to the HJM; 1.3 Issues the permeate the mental health care chain in the RN. CATEORY 2) Main causes of the revolving door phenomenon in HJM, with the categories: 2.1 Family problems; 2.2 Lack of assistance after discharge from psychiatric hospital. In summary, we conclude that the extension of the mental health care chain contributed for the reduction of the psychiatrics re-hospitalization‟s indices in RN. However, we realized that territorial services of mental health care are not the only responsible for the revolving door phenomenon. Factors as family problems and the disarticulation of the assistance after the discharge from hospital influence on the perpetuation of hospitalizations and re-hospitalizations in the local scenario. To study the revolving door phenomenon that occur in the psychiatrics‟ assistance considering the news strategies of mental health care allowed us to approach the advances and challenges brought by the RPb and by the desinstitucionatization in the state, indicating the need for further discussions and problem-solving strategies of psychosocial care.

A proposal of formation in health/nursing built on classic science, on reducing thought and flexnerian paradigm is insufficient to comprehend and intervening with an amplified way on health needs of population, given that it is produced by fragmentation of knowledge, rationalization of thought, mechanizing and biological attitudes. It is necessary that formation in health/nursing allows the construction of effectiveness of principles and guidelines of Unique System of Health (Sistema Único de Saúde - SUS). In this context of emergency of a complex formation in health/nursing, life trajectories and formation of the nurses: Abigail Moura, Francisca Valda and Raimunda Germano are examples of transgressor and successful experiences which allow inquietude, changing and transforming formation patterns and self-formation. The present study is built from the comprehension of method as strategy , defended by Morin and complexity sciences. Has as objects building biographies of formation of these three nurses who express a formation model more totalizing and humanitarian, analyzing and discussing starting from the three biographic fragments guiding principles for the current process of formation in health/nursing. From the biographies, the courage and humbleness emerge as landmarking principles of their experiences. Humbleness neither as self-depreciation nor humiliation but as consciousness of our uncompleted and unfinished essence, acceptance of boundaries and potential and reduction of intellectual vanity. Courage, for its part, is the human pulsing, uncertain for nature, which brings us to act, to face and persevere on moments of fear and difficulties. A formation in health and nursing based on courage and humbleness allows the subjects to be taken away from indifference, arrogance, inertia, pragmatism: bets on ethic and political subjects capable of minimizing unequal, inhumane and excluding processes. This qualitative study aimed at investigating how mental health nurses acquired knowledge about the theme family during their academic training, how they see families in their daily practice, and how should professional training be considering the presence of the family in the care setting. Data were collected using semistructured questionnaires. Six nurses were interviewed. Data analysis allowed the establishment of three theme categories: Academic and professional training absence of the family theme; the nurses' perception of the family with a mentally ill member; and a new vision of the family the role of teaching. During undergraduate education, nurses acquired little knowledge about the theme family; in the institutional routine, they praise the family as a participant of care; and indicate that the need for knowledge about families is based on the interdisciplinary character of education.

It is required that patients are provided information about therapeutic possibilities, showing the risks, benefits, prognosis and costs of each possible and indicated alternative. This is an ethical and legal resolution. However, health professionals possess the clinical/technical/scientific knowledge and determine what information will be (or not) provided. The patient in question decides to undergo a treatment, providing his/her free and informed consent on the basis of the data presented. Unfortunately, some professionals may not provide all the information necessary for making an informed decision or, after obtaining the consent of the patient, may provide him information that causes the patient to give up on the treatment initially accepted. Such information, if relevant, and not a supervening fact, should have been provided initially. However, the information may not be entirely true, and bring the patient, for instance, to decide based on inadequately presented risks. The craniofacial rehabilitation of the temporomandibular joint (TMJ) by means of TMJ prosthesis, is indicated in many situations. Often, patients in need of such prostheses have aesthetic and functional problems and the rehabilitation expectations run high. This work presents a case and discusses ethical and legal issues, including the liability of partial and inadequate information to a patient.

Background: Frailty in older adults is a multifactorial syndrome defined by low metabolic reserve, less resistance to stressors, and difficulty in maintaining organic homeostasis due to cumulative decline of multiple physiological systems. The relationship between frailty and cognition remains unclear and studies about Mini-Mental State Examination (MMSE) performance and frailty are scarce. The objective was to examine the association between frailty and cognitive functioning as assessed by the MMSE and its subdomains. Methods: A cross-sectional population-based study (FIBRA) was carried out in Ermelino Matarazzo, a poor subdistrict of the city of Sao Paulo, Brazil. Participants were 384 community dwelling older adults, 65 years and older who completed the MMSE and a protocol to assess frailty criteria as described in the Cardiovascular Health Study (CHS). Results: Frail older adults had significantly worse performance on the MMSE (p < 0.001 for total score). Linear regression analyses showed that the MMSE total score was influenced by age (p < 0.001), education (p < 0.001), family income (p < 0.001), and frailty status (p < 0.036). Being frail was associated more significantly with worse scores in Time Orientation (p < 0.004) and Immediate Memory (p < 0.001). Conclusions: Our data suggest that being frail is associated with worse cognitive performance, as assessed by the MMSE. It is recommended that the assessment of frail older adults should include the investigation of their cognitive status.

Foi realizada uma revisão sistemática da literatura sobre sobrecarga de cuidadores em saúde mental. Os trabalhos foram selecionados na Biblioteca Virtual de Saúde (BVS), utilizando-se as palavras-chaves: sobrecarga do cuidador (caregiver burden). Os principais critérios para este estudo foram: artigos completos, publicados entre 2000 e 2010, nos idiomas português, inglês ou espanhol; indexados nas bases da BVS, que investigam a sobrecarga do cuidador em saúde mental, tendo cuidadores como assunto principal. Para análise foi considerado: o título, ano de publicação, objetivos, abordagem metodológica, instrumentos e principais resultados. A análise de 114 artigos na íntegra apontou como objetivos predominantes a sobrecarga entre os cuidadores informais e a validação de escalas psicométricas, destacando-se a Escala de Zarit. Alguns estudos apresentaram associação entre altos níveis de sobrecarga, ocorrência de sentimentos de culpa e sintomas depressivos. Em contrapartida indicaram intervenções psico-educacionais como positivas. Trata-se de uma temática com crescente interesse cientifico e necessidade de aprofundamento relacionado ao cuidador formal.

OBJETIVO: Descrever como são identificadas e acolhidas as necessidades de saúde mental (SM) por equipes de saúde da família, conforme a concepção de enfermeiros. MÉTODOS: Estudo exploratório, descritivo de caráter qualitativo. Utilizaram-se entrevistas semiestruturadas junto a cinco enfermeiros e a interpretação foi norteada pelas preconizações do Ministério da Saúde Brasileiro sobre a inclusão das ações de SM na atenção básica. RESULTADOS: Identificou-se que a falta de indicadores no Sistema de Informações da Atenção Básica (SIAB) afeta o planejamento das ações de SM e que outras doenças crônicas, como diabetes e hipertensão, são prioritárias para as equipes. As ações de SM vêm sendo incorporadas gradativamente no processo de trabalho das equipes de saúde da família, e a consultoria de psiquiatria exerce importante papel nisso. CONCLUSÃO: Reconhece-se a necessidade de educação permanente, revisão do SIAB e, sobretudo, criação de projetos terapêuticos sistematizados e à disposição para novos modos de cuidar.

OBJECTIVE: To compare the content covered by twelve obesity-specific health status measures using the International Classification of Functioning, Disability and Health (ICF). DESIGN: Obesity-specific health status measures were identified and then linked to the ICF separately by two trained health professionals according to standardized guidelines. The degree of agreement between health professionals was calculated by means of the kappa (kappa) statistic. Bootstrapped confidence intervals (CI) were calculated. The obesity-specific health-status measures were compared on the component and category level of the ICF. MEASUREMENTS: welve condition-specific health-status measures were identified and included in this study, namely the obesity-related problem scale, the obesity eating problems scale, the obesity-related coping and obesity-related distress questionnaire, the impact of weight on quality of life questionnaire (short version), the health-related quality of life questionnaire, the obesity adjustment survey (short form), the short specific quality of life scale, the obesity-related well-being questionnaire, the bariatric analysis and reporting outcome system, the bariatric quality of life index, the obesity and weight loss quality of life questionnaire and the weight-related symptom measure. RESULTS: In the 280 items of the eight measures, a total of 413 concepts were identified and linked to the 87 different ICF categories. The measures varied strongly in the number of concepts contained and the number of ICF categories used to map these concepts. Items on body functions varied form 12% in the obesity-related problem scale to 95% in the weight-related symptom measure. The estimated kappa coefficients ranged between 0.79 (CI: 0.72, 0.86) at the component ICFs level and 0.97 (CI: 0.93, 1.0) at the third ICF's level. CONCLUSION: The ICF proved highly useful for the content comparison of obesity-specific health-status measures. The results may provide clinicians and researchers with new insights when selecting health-status measures for clinical studies in obesity.

OBJECTIVES: To assess the bleeding on probing (BOP) tendency and periodontal probe penetration when applying various probing forces at implant sites in patients with a high standard of oral hygiene with well-maintained peri-implant tissues. MATERIAL AND METHODS: Seventeen healthy patients with excellent oral hygiene in a maintenance program after having been treated for periodontitis or gingivitis were recruited. Missing teeth had been replaced using oral implants. The BOP and probing depth (PPD) were assessed at the mid-buccal, mid-oral, mesial and distal aspects of the buccal surfaces of each implant. Moreover, contralateral teeth were designated and assessed for BOP and PPD in the same locations and at the same observation visits. At each visit, implants and contralateral teeth were randomly assigned to one of the standardized probing forces (0.15 or 0.25 N). The second probing force was applied at the repetition of the examination 7 days later. RESULTS: Increasing the probing pressure by 0.1 N from 0.15 N resulted in an increase of BOP percentage by 13.7% and 6.6% for implants and contralateral teeth, respectively. There appeared to be a significant difference of the mean BOP percentage at implant and tooth sites when a probing pressure of 0.25 N was applied. A significantly deeper mean PPD at implant sites compared with tooth sites was found irrespective of the probing pressure applied. CONCLUSIONS: The results of the present study demonstrated that 0.15 N might represent the threshold pressure to be applied to avoid false positive BOP readings around oral implants. Hence, probing around implants demonstrated a higher sensitivity compared with probing around teeth.
