OBJECTIVE: The aim of this study is to review highly cited articles that focus on non-publication of studies, and to develop a consistent and comprehensive approach to defining (non-) dissemination of research findings. SETTING: We performed a scoping review of definitions of the term 'publication bias' in highly cited publications. PARTICIPANTS: Ideas and experiences of a core group of authors were collected in a draft document, which was complemented by the findings from our literature search. INTERVENTIONS: The draft document including findings from the literature search was circulated to an international group of experts and revised until no additional ideas emerged and consensus was reached. PRIMARY OUTCOMES: We propose a new approach to the comprehensive conceptualisation of (non-) dissemination of research. SECONDARY OUTCOMES: Our 'What, Who and Why?' approach includes issues that need to be considered when disseminating research findings (What?), the different players who should assume responsibility during the various stages of conducting a clinical trial and disseminating clinical trial documents (Who?), and motivations that might lead the various players to disseminate findings selectively, thereby introducing bias in the dissemination process (Why?). CONCLUSIONS: Our comprehensive framework of (non-) dissemination of research findings, based on the results of a scoping literature search and expert consensus will facilitate the development of future policies and guidelines regarding the multifaceted issue of selective publication, historically referred to as 'publication bias'.

Dans le contexte des soins intensifs pédiatriques, la douleur est une préoccupation majeure et quotidienne pour l'équipe soignante. Les patients sont vulnérables de par leur condition et les multiples stimulations douloureuses auxquelles ils sont exposés. En dépit des données probantes démontrant l'importance d'une évaluation de la douleur, le traitement de celle-ci reste non optimal dans cette population vulnérable. Une douleur inadéquatement traitée peut causer de nombreuses complications à moyen et à long terme. L'évaluation de la douleur chez le patient ventilé et non communicant représente un défi important, rendu complexe par l'utilisation concomitante de médicaments sédatifs et analgésiques qui peuvent masquer ou altérer les comportements de la douleur. Il existe des outils d'évaluation adaptés à cette population vulnérable, mais l'interprétation des scores reste difficile. L'évaluation de la douleur demande aux infirmières des connaissances et compétences élevées à même d'être mobilisées lors d'un processus complexe lié au raisonnement clinique. Le but de cette étude descriptive et observationnelle est de déterminer les indicateurs utilisés par les infirmières expertes des soins intensifs de pédiatrie lors de l'évaluation de la douleur chez le patient ventilé et non communicant, sous analgésie et sédation. Un échantillon de convenance de dix infirmières expertes travaillant dans une unité de soins intensifs pédiatriques d'un hôpital universitaire de Suisse romande a participé à cette étude. Les données ont été récoltées par l'enregistrement de la verbalisation du raisonnement clinique au lit du patient, complété par une observation non participante et un entretien semi-structuré. Le développement d'un cadre théorique constitué d'un modèle de raisonnement clinique expert et d'une modélisation du décodage de la douleur a permis de réaliser une analyse de contenu des enregistrements. Les résultats montrent une utilisation importante des indicateurs physiologiques, en lien avec la stabilité clinique du patient qui est un critère essentiel pour la prise de décision lors de la gestion de la douleur. La difficulté à discriminer l'agitation résultant de la douleur ou d'autres causes est également omniprésente. Les expertes anticipent et préviennent la survenue de la douleur en s'appuyant sur leurs connaissances et les situations de patient déjà rencontrées. Le contexte clinique influence de manière prépondérante le raisonnement clinique et les indicateurs utilisés lors de l'évaluation et la gestion de la douleur. Celle-ci doit être évaluée de manière combinée avec la stabilité clinique du patient et son niveau de sédation. De futures recherches sont nécessaires pour confirmer ces résultats réalisés avec un échantillon de petite taille et un devis observationnel.

L'adhérence au traitement pour des enfants atteints de maladies chroniques est un vrai challenge. En effet, une mauvaise adhérence peut compromettre l'efficacité des médicaments amenant à de moins bons résultats de santé. Une consultation téléphonique infirmière, appelé Télénursing (TN), a démontré être un moyen améliorant l'adhérence. Á ce jour, peu d'études ont testé cette intervention en pédiatrie et spécifiquement chez des enfants atteints de maladies rhumatismales inflammatoires. Cette étude pilote s'insère dans un large essai randomisé et financé à la fois par le Fonds national suisse de la recherche scientifique et par le Réseau d'Etudes aux Confins de la Santé et du Social. Le but de cette étude est d'évaluer l'impact du TN sur l'adhérence des enfants atteints de maladies rhumatismales inflammatoires et leur famille ainsi que d'effectuer une analyse de puissance pour déterminer la taille de l'échantillon pour l'étude principale concernant l'adhérence. Cette étude pilote randomisée, pré-post test en simple aveugle, a été réalisée dans le cadre de la Consultation Multisite Romande de Rhumatologie Pédiatrique sur les sites de Lausanne principalement, mais aussi Sion, Neuchâtel et Aigle, sur une période de trois mois. Au total, 24 patients ont été recrutés. L'adhérence a été mesurée auprès d'un des parents et d'un enfant en utilisant deux questionnaires autoadministrés, le Parent Adherence Report Questionnaire et le Children Adherence Report Questionnaire. Les résultats des parents démontrent une tendance en faveur du TN, notamment concernant les items suivants : « le niveau de difficulté à suivre le traitement », « la fidélité au traitement » et « les rendez-vous de suivi trop espacés ». Les résultats des enfants ne permettent pas d'identifier une tendance en faveur du TN. Il ressort aussi qu'un manque d'accord entre les parents et les enfants sur l'évaluation de l'adhérence persiste. De par le devis de cette étude, les marges d'erreurs d'interprétation des résultats sont augmentées. Néanmoins, les résultats des parents indiquent qu'une intervention téléphonique infirmière permet d'avoir un impact sur l'adhérence des enfants à leur traitement et d'assurer le suivi des patients et de leur famille. Les résultats de l'étude mettent en évidence l'importance de mesurer l'adhérence dans les deux populations afin d'adapter au mieux les interventions infirmières.

In Switzerland, overcrowding in tertiary emergency departments is a frequent problem, resulting in lengthy waiting times, lower satisfaction on the part of families and a risk for patient's safety. The setting up of a nurse consultation in a university paediatric emergency centre has helped to improve the quality of care in this context.

Les problèmes de santé mentale au travail constituent un défi à la fois clinique, professionnel, économique et de santé publique. Les coûts totaux qu'ils génèrent en Suisse équivalent à 3,2 % du produit intérieur brut (PIB) suisse et ils aboutissent très souvent à un licenciement. La grande majorité des personnes sont soignées par un médecin de premier recours. L'Institut de Santé au Travail propose une consultation spécialisée dans les questions de souffrance au travail, offrant aux soignants de première ligne un avis ou un soutien pluridisciplinaire, dans une perspective collaborative des soins. Son action, adaptée aux besoins de chaque situation, va d'un avis à une orientation vers des spécialistes pouvant étoffer durablement le réseau (suivi psychiatrique, programme de soutien à l'emploi, avis juridique ou social). Mental health problems at work constitute a challenge in the clinical feld, as well in the professional, the economic and the public health perspective. The total costs they generate in Switzerland are equivalent to 3.2% of the Swiss gross domestic product and they very often lead to dismissal. The vast majority of people are treated by their primary care physician. The Institute for Work and Health features a specialized consultation on the topic of suffering at work, offering the primary care physicians a pluridisciplinary advice or support, in a collaborative care prospect. Its action, adapted to each situation's needs, goes from an advice to a referral to specialists that can strengthen the network on a long-term basis (mental health follow-up, supported employment program, legal or social advice).

This study focuses to the intersection of three sets of activities in a company: expert work, development work and supply chain management, SCM. Experts and expert work represent a set of individuals whose efficiency and impact this study is intended to improve, while development work defines the set of organizational activities to focus on. SCM as an expertise area acts as the platform on which this study is built. The study has two aims. Firstly, it aims to derive a model helping an SCM expert to increase the effectiveness of expert work in development tasks by understanding the encountered organizational situations and processes better, reflecting his/her past and future actions to organizational processes and selecting and adjusting the processes and contents of his/her work accordingly. Secondly, it aims to develop applicable approaches and methods to understand, evaluate and manage the organizational processes and situations in development work. The integrative model on approaches and methods to improve the effectiveness of development processes is split to two aggregate dimensions: technical performance of the developed solution and consumption of resources of the development process. Six potential approaches and methods aiming at helping in the management of organizational dimensions are presented in enclosed publications. The approaches focus on three subtasks of development work: decision making, implementation and change, and knowledge accumulation. The approaches and methods have been tested in case studies representing typical development processes in the area of supply chain management. As a result, four suggestions are presented. Firstly, SCM experts are advised to consider the SCM development work to be consisting of development processes. Secondly, inside these processes they should identify and evaluate the risk of difficult decision-making related to organizational factors. Thirdly, they are prompted for an active role in implementation and change, supporting the implementation through whole process. Finally, the development should be seen in a holistic view, taking into account the stage of knowledge and organizational issues related to it, and adopt a knowledge development strategy.

De vegades és difícil distingir els conceptes d’individu amb talent i individu expert. Sens dubte, això és degut a que els individus que assoleixen l’èxit en l’esport, la música, el teatre, la literatura, etc. mostren ambdues condicions: una si es vol innata, el talent, i una altra adquirida, la de ser expert. També, de vegades, desitjaríem que els talents treballessin per a ser experts, aspecte que els elevaria a un grau superior d’excel·lència. En el present article es realitza una posada al dia d’ambdós conceptes amb l’objectiu d’establir una base adequada sobre la que sostenir amb més solidesa les decisions i actuacions del formador. També es presenten algunes nocions sobre el seguiment i control dels nois que gaudeixen en les canteres esportives sempre amb l’orientació bàsica de formar individus complerts amb un rendiment òptim.

To create a more inclusive school, an increase in multidisciplinary cooperation is needed. One possible form of collaboration could encompass the special education teacher taking on the role of a consultant for other teachers in need of support in working with heterogeneous groups of pupils. Previous research shows that special education teachers see the role as consultant as diffuse and complex. The overarching aim of the present study involves deepening the knowledge on how consultation in a special educational context can be understood and developed based on teachers’ descriptions on this particular form of activity interpreted against various perspectives on consultation. The study is qualitative in nature and rests on a hermeneutic interpretive research tradition in combination with an abductive approach. The theoretical framework consists of two different approaches to consultation: the directive and the non-directive approach. The approaches differ regarding particular emphasis on advice and reflection during the consultation and with respect to who or what should be the focus of the consultation. The two approaches are here studied through various theories such as social learning theory, Bruner's theory of scaffolding, Roger’s humanist psychology, and constructivism. Semi-structured interviews were held with eighteen special education teachers (n=9) and class teachers (n=9) working in the compulsory school. The overall interpretation of the results shows that special education consultation can be understood as three different types of consultation. Consultation as counseling which harmonizes with the directive perspective on consultation is the most prominent type. In the consultation as counseling conversation, the special educational knowledge transfer is central and the focus is placed on the pupil. Although special education knowledge transfer emerges as a unique aspect of special education consultation, there are several inherent challenges in this type of consultation that can be addressed in that teachers also describe two other types of consultation. In the reflective consultation, there is a move away from the pupil focus and toward a focus on the class teacher and the use of reflection. The reflective consultation harmonizes with the non-directive approach to consultation. This type of consultation does not as of yet have a prominent place in the Finland-Swedish school context and at this stage it is not seen as a legitimate type of consultation according to the teachers’ descriptions. Despite this, certain aspects of the reflective conversation could be given more space in the development of consultation within special educational contexts. The co-operative consultation is characterized by the teachers acting as teammates and using professional exchange as a strategy for consultation. Both teachers' knowledge is seen as central, and rather than the special education teacher acting as the expert and moderator, the teachers control the consultation together and jointly move the work along. The co-operative consultation enables the focus to move from the pupil toward the context, which can lead to the development of inclusive practices. The results indicate that this type of consultation holds potential in the development of special educational consultation that takes place between equal colleagues. The co-operative consultation opens up for a third collaborative approach to consultation, where aspects of the directive and non-directive perspective can merge and develop. The thesis concludes with the proposal that special pedagogical consultation can be understood from an integrated perspective. The characteristics of the consultation can vary depending on the type of problem or situation, while co-operative consultation can be seen as the ideal as equal colleagues meet in consultation conversations. In order to develop the co-operative consultation, both teachers are required to have knowledge of consultation as a practice, to be part of a collaborative school climate, and that teachers are provided with enough time to take part in consultations.

OBJETIVOS: traduzir, adaptar culturalmente e validar o questionário International Consultation on Incontinence Questionnaire Overactive Bladder (ICIQ-OAB) para a língua portuguesa. MÉTODOS: dois tradutores brasileiros, cientes dos objetivos da pesquisa, traduziram o ICIQ-OAB para o português e as duas traduções geradas foram retrotraduzidas por outros dois tradutores ingleses. As diferenças entre as versões foram harmonizadas e pré-testadas em um estudo piloto. A versão final do ICIQ-OAB foi aplicada junto com a versão já traduzida e validada do questionário International Consultation on Incontinence Questionnaire - Short Form (ICIQ-SF) em 142 pacientes, entre homens e mulheres, com sintomas miccionais irritativos. Para validação do ICIQ-OAB foram testadas propriedades psicométricas: confiabilidade (consistência interna e teste-reteste) e validade de construto. O reteste foi realizado quatro semanas após a primeira entrevista. RESULTADOS: a confiabilidade do instrumento foi avaliada por meio do Coeficiente α Cronbach, tendo como resultado geral 0,7. O teste-reteste avaliou a estabilidade do instrumento por meio do coeficiente de correlação intraclasse e apresentou resultado de 0,91 e 0,95, quando comparados aos questionários ICIQ-OAB e ICIQ-SF, respectivamente. Comparando os instrumentos por meio do coeficiente de correlação de Pearson foi encontrado 0,7 (p=0,0001), o que confirma a validade de critério do estudo. A validade concorrente foi avaliada pela correlação entre algumas variáveis sociodemográficas e clínicas e o escore final do ICIQ-OAB. CONCLUSÃO: a versão em português do ICIQ-OAB traduzida e adaptada culturalmente para o português do Brasil apresentou confiabilidade e validade de constructo satisfatórias e foi considerada válida para avaliação dos sintomas miccionais irritativos de pacientes brasileiros de ambos os sexos.

OBJETIVO: Avaliar a correlação entre o International Consultation on Incontinence Questionnaire - Urinary Incontinence/Short Form (ICIQ-UI/SF) e a Avaliação Urodinâmica (AU) em mulheres com incontinência urinária (IU). MÉTODOS: Foram analisados retrospectivamente dados clínicos, AU e escore do ICIQ-UI/SF de 358 mulheres com IU atendidas em clínica privada. O teste de correlação entre ICIQ-UI/SF e os parâmetros urodinâmicos foi o teste de Spearman. Foi utilizada a curva ROC, com os valores de sensibilidade e especificidade dos escores do ICIQ-UI/SF apresentados pelas pacientes, para identificar o valor do questionário que determinasse a presença da alteração urodinâmica estudada. Para o cálculo do valor p foi utilizado o teste do c² ou exato de Fisher. O nível de significância foi de 5% e o software utilizado para análise foi o SAS versão 9.2. RESULTADOS: As pacientes com IU aos Esforços segundo a AU - IUE urodinâmica - representaram 67,3% do total; aquelas com IUE na AU e Hiperatividade Detrusora (HD) - IUM urodinâmica - 16,2%, e as pacientes com HD isolada - HD - 7,3% do total. As pacientes com AU normal representaram 9,2% do total da amostra. Houve associação significativa entre escore ³14 no ICIQ-UI/SF e as pacientes com IUE urodinâmica e IUM urodinâmica. Pacientes com Pressão de Perda ao Esforço (PPE) £90 cmH2O apresentaram escore ao ICIQ-UI/SF³15. O teste de Spearman mostrou correlação inversa fraca entre o escore e a PPE, porém não mostrou correlação entre esse escore e a Capacidade Cistométrica Máxima (CCM) ou com o volume vesical no primeiro desejo miccional. CONCLUSÃO: Houve associação entre o escore do ICIQ-UI/SF e IUE urodinâmica (isolada ou associada à HD); porém não houve associação com a HD isolada. Quanto menor o valor da PPE, maior o escore total do ICIQ-UI/SF. O ICIQ-UI/SF não foi capaz de discriminar o tipo de IU na população estudada.

Worry is one of the central factors in primary health care patients’ experience with their current complaint. Worry is associated with, e.g., patients’ expectations and the outcomes of doctor’s consultations. The aim of this study was to explore primary health care patients’ complaint-related worry and its changes, as well as contributing factors. Furthermore, the reasons behind patients’ pre-consultation worry and possible relief were examined. The study was conducted in a public primary health care centre in Forssa in Southern Finland. Patients, aged 18–39 years, with a current complaint were interviewed before and after a doctor’s consultation. The patients’ characteristics, perceptions of their complaint and their expectations and experiences concerning the consultation were obtained through interviews. In addition, two questionnaires were administered to measure general tendency to illness worry (IWS) and psychiatric symptoms (SCL-90). The patients’ ratings of the intensity of worry and the severity of their complaint were measured with a visual analogue scale (VAS 0–100). Changes in worry were measured by comparing pre- and post-consultation VAS ratings and asking the patients to compare their worry after the consultation with the worry they felt before it. In connection with these ratings the patients also gave reasons for their experiences in their own words. The patients’ doctors assessed the medical severity of the complaints and whether they had found a medical explanation for the complaints. Many patients were very worried before the consultation (65 % scored over 50 points on the VAS). Worry and severity ratings were associated with the duration and course of the complaint, with a general tendency to illness worry and hostility. On average, the patients were less worried after the consultation than before it. Persistent worry was associated with the patients’ uncertainty about their complaint, their perceiving it as severe, expectations for examinations and reporting symptoms of anxiety. Patients were most often worried about the nature of their complaint (e.g. duration or intensity), not knowing what was wrong, the possible harmful effects of the complaint on body functions, the complaint’s prognosis, e.g. will it get better, and their ability to function. Patients were relieved by getting an explanation or treatment or by having a positive view of the complaint’s prognosis. Patients who reported uncertainty (lack of an explanation, worry about the nature of the complaint) or worry about the complaint’s possible bodily harmfulness were relieved by getting an explanation, often accompanied with getting treatment. On the other hand, worries about the ability to function tended to persist. Doctors should bring up patients’ worries for discussion in order to be able to respond to them appropriately. Because it tends to persist, worry about the ability to function should be addressed. Uncertain patients with concerns about their complaint’s bodily harmfulness or psychological consequences need special attention from their doctor.

In this paper a computer program to model and support product design is presented. The product is represented through a hierarchical structure that allows the user to navigate across the product’s components, and it aims at facilitating each step of the detail design process. A graphical interface was also developed, which shows visually to the user the contents of the product structure. Features are used as building blocks for the parts that compose the product, and object-oriented methodology was used as a means to implement the product structure. Finally, an expert system was also implemented, whose knowledge base rules help the user design a product that meets design and manufacturing requirements.

In view of the importance of anticipating the occurrence of critical situations in medicine, we propose the use of a fuzzy expert system to predict the need for advanced neonatal resuscitation efforts in the delivery room. This system relates the maternal medical, obstetric and neonatal characteristics to the clinical conditions of the newborn, providing a risk measurement of need of advanced neonatal resuscitation measures. It is structured as a fuzzy composition developed on the basis of the subjective perception of danger of nine neonatologists facing 61 antenatal and intrapartum clinical situations which provide a degree of association with the risk of occurrence of perinatal asphyxia. The resulting relational matrix describes the association between clinical factors and risk of perinatal asphyxia. Analyzing the inputs of the presence or absence of all 61 clinical factors, the system returns the rate of risk of perinatal asphyxia as output. A prospectively collected series of 304 cases of perinatal care was analyzed to ascertain system performance. The fuzzy expert system presented a sensitivity of 76.5% and specificity of 94.8% in the identification of the need for advanced neonatal resuscitation measures, considering a cut-off value of 5 on a scale ranging from 0 to 10. The area under the receiver operating characteristic curve was 0.93. The identification of risk situations plays an important role in the planning of health care. These preliminary results encourage us to develop further studies and to refine this model, which is intended to implement an auxiliary system able to help health care staff to make decisions in perinatal care.

Patients with clinical diseases often present psychiatric conditions whose pharmacological treatment is hampered due to hazardous interactions with the clinical treatment and/or disease. This is particularly relevant for major depressive disorder, the most common psychiatric disorder in the general hospital. In this context, nonpharmacological interventions could be useful therapies; and, among those, noninvasive brain stimulation (NIBS) might be an interesting option. The main methods of NIBS are repetitive transcranial magnetic stimulation (rTMS), which was recently approved as a nonresearch treatment for some psychiatric conditions, and transcranial direct current stimulation (tDCS), a technique that is currently limited to research scenarios but has shown promising results. Therefore, our aim was to review the main medical conditions associated with high depression rates, the main obstacles for depression treatment, and whether these therapies could be a useful intervention for such conditions. We found that depression is an important and prevalent comorbidity in a variety of diseases such as epilepsy, stroke, Parkinson's disease, myocardial infarction, cancer, and in other conditions such as pregnancy and in patients without enteral access. We found that treatment of depression is often suboptimal within the above contexts and that rTMS and tDCS therapies have been insufficiently appraised. We discuss whether rTMS and tDCS could have a significant impact in treating depression that develops within a clinical context, considering its unique characteristics such as the absence of pharmacological interactions, the use of a nonenteral route, and as an augmentation therapy for antidepressants.
