El Síndrome Metabólico (SM) es un conjunto de factores de riesgo asociados con el incremento del riesgo de enfermedad cardiovascular y diabetes mellitus de tipo 2 (DM2). Dichos factores son obesidad (OB), resistencia a la Insulina (RI), hiperglucemia, dislipidemia e hipertensión (HTA) e interactúan entre sí sobre las anomalías vasculares, el estrés oxidativo, la grasa visceral, la inflamación y el cortisol en un entorno de OB y RI y bajo la influencia de las predisposiciones genéticas y condiciones ambientales, tales como hábitos de alimentación y actividad física. Según una hipótesis, la RI y la OB son los factores que más contribuyen en la manifestación de anormalidades metabólicas y las manifestaciones más tempranas del desarrollo de SM en niños. Mientras el páncreas compensa adecuadamente la RI mediante una mayor secreción de insulina, las concentraciones de glucosa en sangre se mantienen normales. Sin embargo, en algunos pacientes la capacidad de las células β del páncreas disminuye con el tiempo, lo que conduce al desarrollo de DM2. Otro factor a tener en cuenta en el diagnóstico del SM es la dislipidemia, caracterizada por el aumento de los triglicéridos (TG) y del VLDL‐colesterol, bajas cifras de HDL‐colesterol, así como por la presencia de partículas de LDL‐colesterol más pequeñas y densas de lo normal. Otro factor de riesgo de padecimiento de SM es la presión arterial elevada, que suele aparecer ligada a la presencia de OB, ya que el aumento de algunas adipoquinas que en esta se produce, entre las que se encuentran el AGE, PAI‐1, IL‐6, TNF‐α y leptina, pueden llevar a disfunción endotelial a través de la ruta del óxido nítrico. Debido a que el sobrepeso y el incremento de los valores de insulina plasmática son componentes clave del SM, es importante tener en cuenta los hábitos alimentarios y otros estilos de vida que influyen en los mismos...

Bakgrund: Diabetes mellitus typ 2 (DMT2) ökar i världen och kan leda till allvarliga fotkomplikationer. Det har påvisats brister i den förbyggande vården för att undvika fotkomplikationer. Sjuksköterskan ansvarar för att stötta personer med DMT2 och målet är att få dem att känna sig självständiga och delaktiga i sin egenvård. Syfte: Syftet med studien var att belysa omvårdnadsåtgärder och egenvård som förebygger fotkomplikationer för personer med DMT2 Metod: En litteraturstudie som utgår ifrån 20 vetenskapliga artiklar med kvantitativ uppbyggnad. Sökningarna av Artiklarna är sökta i databaserna Cinahl och Pubmed. Samtliga artiklar är kvalitetsgranskade. Resultat: Identifiering av risker är en viktig faktor i förebyggandet av fotkomplikationer. Dagliga inspektioner och undersökningar av fötterna är viktiga egenvårdsråd som sjuksköterskan bör tilldela genom individuell undervisning. Sjuksköterskan skall efter bästa förmåga stötta och motivera personer med DMT2 till god egenvård. Slutsats: Sjuksköterskan bör undervisa och ge personer med DMT2 tillräcklig kunskap, för att självständigt kunna utföra förebyggande egenvård.

Type 2 diabetes is one of the most common metabolic disorders in the world. Globally, the prevalence of this disorder is predicted to increase, along with the risk of developing diabetic related complications. One of those complications is diabetic nephropathy, defined by a progressive increase in proteinuria and a gradual decline in renal function. Approximately 25% to 30% of type 2 diabetic individuals develop this complication. However, its underlying genetic mechanisms remain unclear. Thus, the aim of this study is to contribute to the discovery of the genetic mechanisms involved in the development and progression of diabetic nephropathy, through the identification of relevant genetic variants in Portuguese type 2 diabetic individuals. The exomes of 36 Portuguese type 2 diabetic individuals were sequenced on the Ion ProtonTM Sequencer. From those individuals, 19 did not present diabetic nephropathy, being included in the control group, while the 17 individuals that presented the diabetic complication formed the case group. A statistical analysis was then performed to identify candidate common genetic variants, as well as genes accumulating rare variants that could be associated with diabetic nephropathy. From the search for common variants in the study population, the statistically significant (p-value ≤ 0.05) variants rs1051303 and rs1131620 in the LTBP4 gene, rs660339 in UCP2, rs2589156 in RPTOR, rs2304483 in the SLC12A3 gene and rs10169718 present in ARPC2, were considered as the most biologically relevant to the pathogenesis of diabetic nephropathy. The variants rs1051303 and rs1131620, as well as the variants rs660339 and rs2589156 were associated with protective effects in the development of the complication, while rs2304483 and rs10169718 were considered risk variants, being present in individuals with diagnosed diabetic nephropathy. In the rare variants approach, the genes with statistical significance (p-value ≤ 0.05) found, the STAB1 gene, accumulating 9 rare variants, and the CUX1 gene, accumulating 2 rare variants, were identified as the most relevant. Both genes were considered protective, with the accumulated rare variants mainly present in the group without the renal complication. The present study provides an initial analysis of the genetic evidence associated with the development and progression of diabetic nephropathy, and the results obtained may contribute to a deeper understanding of the genetic mechanisms associated with this diabetic complication.

La Hemoglobina Glicosilada (HbA1c) es el mejor método de control de la Diabetes Mellitus tipo I y II, ya que brinda un buen estimado glucémico de los tres meses anteriores a la prueba, revelando así el éxito o fracaso del tratamiento así como también la discontinuidad del mismo o de la dieta y medicación mandada a seguir por el médico. El objetivo de ésta investigación fue realizar la prueba de Hemoglobina Glicosilada como método para el control de diabetes en pacientes atendidos por la Asociación Salvadoreña de Diabetes en el Hospital Nacional San Pedro, departamento de Usulután. La Metodología empleada en el estudio fue de tipo Descriptivo, Prospectivo, Transversal y de Laboratorio, con una población de 50 pacientes diabéticos. Además se utilizó la guía de entrevista para identificar los factores que favorecen el buen control de la diabetes. Se les realizó la prueba de Glucosa en ayunas y Hemoglobina Glicosilada simultáneamente. Resultados obtenidos: el 82% de los pacientes diabéticos obtuvieron valores de Hemoglobina Glicosilada entre normal y controlado, dentro de los cuales el 72% son del sexo femenino y el 10% del sexo masculino. El 68% de la población tenia Glucosa en ayunas normal. Entre los factores más significativos que favorecen el buen control de la diabetes, está que el 84% cumple con el tratamiento prescrito por el médico, el 72% cumplen con el ejercicio. Conclusión: estadísticamente se comprobó que el 82% de los pacientes estaban realizando un buen control de la diabetes ya que sus valores estaban entre lo normal y controlado, el 18% de los pacientes que obtuvieron valores aumentados de Hemoglobina Glicosilada y el 32% de pacientes con valores aumentados de glucosa basal existiendo un 14% de diferencia entre los valores aumentados de glucosa basal y la Hemoglobina Glicosilada; esto se debe a que cuando se toma la muestra de glucosa basal se ve influenciada, cuando el paciente no toma las recomendaciones adecuadas y no por descuido permanente por su parte.

La presente investigación se realizó con el objetivo de determinar el efecto hipoglucemiante de un extracto etanólico de las hojas de Hamelia patens (chichipince) a tres diferentes concentraciones, las cuales fueron 100mg/kg, 250mg/kg y 500mg/kg de peso corporal. Para ello se emplearon modelos experimentales utilizándose ratones hembra albinos suizos NIH a los que se evaluaron en tres diferentes situaciones fisiológicas: a) normoglucémicos (sanos), b) Ante una sobre carga de glucosa (normoglucémicos), c) inducidos a diabetes mediante una serie de dosis bajas de Estreptozotocina (STZ). La determinación de los niveles de glucosa se realizó por medio de tiras reactivas, previo a un periodo de ayuno, extrayendo una muestra homogénea de sangre de cada animal experimental. Se encontró que el extracto etanólico de las hojas secas de Hamelia patens no presento ningún tipo de toxicidad en los sujetos experimentales durante su periodo de prueba. En cuanto a la actividad hipoglucemiante del extracto etanólico evaluado de Hamilia patens, no resulto efectivo para reducir los niveles de glucosa en sangre y por lo tanto no representa una alternativa para el tratamiento de la diabetes tipo 2

INTRODUCCIÓN: Si se valora a tiempo la Sensibilidad a la insulina, se evitara padecer diabetes tipo 2; en los adultos mayores hay cambios como el aumento de tejido adiposo y sarcopenia, relacionados con disminución de la sensibilidad a la insulina. OBJETIVO: Determinar la sensibilidad a la insulina mediante la prueba de tolerancia oral a la glucosa en la población adulta mayor del cantón Cuenca, en el año 2015. METODOLOGÍA: Estudio descriptivo en 120 adultos mayores del cantón Cuenca; 60 casos con síndrome metabólico según el criterio ATP III y 60 casos sin síndrome metabólico. Se trata de una muestra no probabilística por conveniencia debido al costo de las pruebas de laboratorio. Se tomaron dos muestras de sangre una en ayunas y otra postprandial y se dosifico glucosa e insulina. Los datos fueron analizados en SPSS 22, Excel empleando frecuencias, porcentajes, medidas de tendencia central como mediana, promedio, medidas de dispersión, desvío stándar. RESULTADOS: El 39,2 % de adultos mayores presentó insulinemia postprandial alterada. Según el método HOMA-IR el 42 % presenta baja sensibilidad a la insulina y según el método QUICKI el 91,7 % presenta sensibilidad disminuida a la insulina. La baja sensibilidad a la insulina según género, edad y estado civil no fue significativa; en cambio con el IMC elevado se tiene más probabilidad de padecer insulinorresistencia (p=0,03) .Siendo más significativo los pacientes con síndrome metabólico aumenta dos veces la probabilidad de padecer insulinorresistencia (p=0.02, OR 2.3 IC 95% 1.09 – 4.85)

No século XXI, fatores socioeconómicos dão origem a uma diminuição da atividade física (AF) (TV, internet, etc.). Estima-se que 50% da população da União Europeia (EU) tem excesso de peso ou obesidade devido a uma dieta inadequada e sedentarismo, que fazem disparar a ocorrência de doenças crónicas (cardiovasculares, músculo esqueléticas, psicológicas, diabetes tipo 2, cancro, etc.) e uma consequente ameaça para a sustentabilidade dos sistemas de saúde e segurança social. A degradação da saúde nos países desenvolvidos, derivada dos estilos de vida atuais, apresenta também alterações no modelo de vivência familiar (famílias menos numerosas e monoparentais com crescimentos na ordem dos 36%). As famílias têm ainda que lidar com a escassez de tempo, a competitividade feroz no trabalho, o stress diário e os perigos em que os elementos mais jovens do agregado familiar incorrem (consumo substâncias ilícitas, distúrbios alimentares, depressão, suicídio e isolamento social) decorrentes do uso das novas tecnologias. “Atualmente, conforme as economias crescem as pessoas param de se movimentar. É urgente, apresentar uma estrutura para a ação, para que os stakeholders, revertam a situação de modo a combater os impactos desta epidemia de inatividade física, construindo ações preventivas e inovadoras, com impacto positivo no desenvolvimento humano”. Objetivo: Pretende-se através da oferta de actividade física e desportiva (AFD) planeada para famílias, promover além da saúde e estilos de vida saudáveis e resilientes, a coesão familiar. Dar resposta científica às preocupações da UE, intervindo como medida de implementação de políticas publicas consideradas prioritárias, de promoção da AF e estilos de vida saudáveis e resilientes, para assegurar um alto nível de proteção da saúde, com repercussão na diminuição dos custos com as doenças e suas consequências.

1. Previous glucagon receptor gene (GCGR) studies have shown a Gly40Ser mutation to be more prevalent in essential hypertension and to affect glucagon binding affinity to its receptor. An Alu-repeat poly(A) polymorphism colocalized to GCGR was used in the present study to test for association and linkage in hypertension as well as association in obesity development. 2. Using a cross-sectional approach, 85 hypertensives and 95 normotensives were genotyped using polymerase chain reaction primers flanking the Alu-repeat. Both hypertensive and normotensive populations were subdivided into lean and obese categories based on body mass index (BMI) to determine involvement of this variant in obesity. For the linkage study, 89 Australian Caucasian hypertension affected sibships (174 sibpairs) were genotyped and the results were analysed using GENE-HUNTER, Mapmaker Sibs, ERPA and SPLINK (all freely available from http://linlkage.rockefeller. edu/soft/list.html). 3. Cross-sectional results for both hypertension and obesity were analysed using Chi-squared and Monte Carlo analyses. Results did not show an association of this variant with either hypertension (χ2 = 6.9, P = 0.14; Monte Carlo χ2 = 7.0, P = 0.11; n = 5000) or obesity (χ2 = 3.3, P = 0.35; Monte Carlo χ2 = 3.26, P = 0.34; n = 5000). In addition, results from the linkage study using hypertensive sib-pairs did not indicate linkage of the poly(A) repent with hypertension. Hence, results did not indicate a role far the Alu-repeat in either hypertension or obesity. However, as the heterozygosity of this poly(A) repeat is low (35%), a larger number of hypertensive sib-pairs may be required to draw definitive conclusions.

Background: Diabetic peripheral neuropathy is an important cause of foot ulceration and limb loss. This systematic review and meta-analysis investigated the effect of diabetic peripheral neuropathy on gait, dynamic electromyography and dynamic plantar pressures. Methods: Electronic databases were searched systematically for articles reporting the effect of diabetic peripheral neuropathy on gait, dynamic electromyography and plantar pressures. Searches were restricted to articles published between January 2000 and April 2012. Outcome measures assessed included spatiotemporal parameters, lower limb kinematics, kinetics, muscle activation and plantar pressure. Meta-analyses were carried out on all outcome measures reported by ≥3 studies. Findings: Sixteen studies were included consisting of 382 neuropathy participants, 216 diabetes controls without neuropathy and 207 healthy controls. Meta-analysis was performed on 11 gait variables. A high level of heterogeneity was noted between studies. Meta-analysis results suggested a longer stance time and moderately higher plantar pressures in diabetic peripheral neuropathy patients at the rearfoot, midfoot and forefoot compared to controls. Systematic review of studies suggested potential differences in the biomechanical characteristics (kinematics, kinetics, EMG) of diabetic neuropathy patients. However these findings were inconsistent and limited by small sample sizes.; Interpretation: Current evidence suggests that patients with diabetic peripheral neuropathy have elevated plantar pressures and occupy a longer duration of time in the stance-phase during gait. Firm conclusions are hampered by the heterogeneity and small sample sizes of available studies. OBJECTIVE To assess the concurrent validity of fasting indexes of insulin sensitivity and secretion in - obese prepubertal (Tanner stage 1) children and pubertal (Tanner stages 2-5) glucose tolerance test (FSIVGTT) as a criterion measure. RESEARCH DESIGN AND METHODS Eighteen obese children and adolescents (11 girls and 7 boys, mean age 12.2 +/- 2.4 years, mean BMI 35.4 +/- 6.2 kg/m(2), mean BMI-SDS 3.5 +/- 0.5, 7 prepubertal and I I pubertal) participated in the study. All participants underwent an insulin-modified FSIVGTT on two occasions, and 15 repeated this test a third time (mean 12.9 and 12.0 weeks apart). S-i measured by the FSIVGTT was compared with homeostasis model assessment (HOMA) of insulin resistance (HOMA-IR), quantitative insulin-sensitivity check index (QUICKI), fasting glucose-to-insulin ratio (FGIR), and fasting insulin (estimates of insulin sensitivity derived from fasting samples). The acute insulin response (AIR) measured by the FSIVGTT was compared with HOMA of percent beta-cell function (HOMA-beta%), FGIR, and fasting insulin (estimates of insulin secretion derived from fasting samples). RESULTS There was a significant negative correlation between HOMA-IR and S-i (r = -0.89, r = -0.90, and r = -0.81, P < 0.01) and a significant positive correlation between QUICKI and S-i (r = 0.89, r = 0.90, and r = 0.81, P < 0.01) at each time point. There was a significant positive correlation between FGIR and S-i (r = 0.91, r = 0.91, and r = 0.82, P < 0.01) and a significant negative correlation between fasting insulin and S-i (r = -90, r = -0.90, and r = -0.88, P < 0.01). HOMA-beta% was not as strongly correlated with AIR (r = 0.60, r = 0.54, and r = 0.61, P < 0.05). CONCLUSIONS HOMA-IR, QUICKI, FGIR, and fasting insulin correlate strongly with S-i assessed by the FSIVGTT in obese children and adolescents. Correlations between HOMA-β% FGIR and fasting insulin, and AIR were not as strong. Indexes derived from fasting samples are a valid tool for assessing insulin sensitivity in prepubertal and pubertal obese children.

Background: Traditionally communicable diseases were the main causes of burden in developing countries like Nepal. In recent years non-communicable diseases (NCDs), mainly cardiovascular diseases (CVDs), cancer, chronic respiratory diseases and diabetes mellitus, impose a larger disease burden compared to communicable diseases. Most elements of health and medicine policies in Nepal are still focused on communicable diseases. There is limited evidence about NCDs and NCD medicines in Nepal. Aim: To explore the gap between the burden of NCDs and the availability and affordability of NCD medicines in Nepal. Methods: Biomedical databases like Medline, Scopus, Web of Science and other online sources (including Global Burden of Diseases data) were searched for data on the burden of NCDs in term of Disability Adjusted Life Years (DALYs). The Essential Medicines List (EML) of Nepal was compared with World Health Organisation (EML) for inclusion of NCD medicines. Results: In Nepal, NCDs caused nearly 45% of the total 10.5 million DALYs in 2010. CVDs (15.2%), were the leading cause of NCDs burden followed by chronic respiratory diseases (14.7%), cancer (7.3%) and diabetes mellitus (3.2%). One hospital based national survey found that 37% of hospitalised patients had NCDs. Among them, 38% had heart disease followed by COPD (33%) , and diabetes (10%). Most (23 out of 28) non-cancer NCD medicines recommended in WHO-EML were present in Nepal's EML, theoretically indicating good availability. However, it is difficult to say whether they are accessible and affordable due to the lack of adequate data on access and pricing. Conclusion: This study gives some insight into the burden of NCDs. Although NCD medicines are available in Nepal, further research is required to determine whether they are accessible and affordable to the general population.

Objectives. To quantify the burden of disease attributable to physical inactivity in persons 15 years or older, by age group and sex, in South Africa for 2000. Design. The global comparative risk assessment (CRA) methodology of the World Health Organization was followed to estimate the disease burden attributable to physical inactivity. Levels of physical activity for South Africa were obtained from the World Health Survey 2003. A theoretical minimum risk exposure of zero, associated outcomes, relative risks, and revised burden of disease estimates were used to calculate population-attributable fractions and the burden attributed to physical inactivity. Monte Carlo simulation-modelling techniques were used for the uncertainty analysis. Setting. South Africa. Subjects. Adults ≥ 15 years. Outcome measures. Deaths and disability-adjusted life years (DALYs) from ischaemic heart disease, ischaemic stroke, breast cancer, colon cancer, and type 2 diabetes mellitus. Results. Overall in adults ≥ 15 years in 2000, 30% of ischaemic heart disease, 27% of colon cancer, 22% of ischaemic stroke, 20% of type 2 diabetes, and 17% of breast cancer were attributable to physical inactivity. Physical inactivity was estimated to have caused 17 037 (95% uncertainty interval 11 394 - 20 407), or 3.3% (95% uncertainty interval 2.2 - 3.9%) of all deaths in 2000, and 176 252 (95% uncertainty interval 133 733 - 203 628) DALYs, or 1.1% (95% uncertainty interval 0.8 - 1.3%) of all DALYs in 2000. Conclusions. Compared with other regions and the global average, South African adults have a particularly high prevalence of physical inactivity. In terms of attributable deaths, physical inactivity ranked 9th compared with other risk factors, and 12th in terms of DALYs. There is a clear need to assess why South Africans are particularly inactive, and to ensure that physical activity/inactivity is addressed as a national health priority.

Aim Retinal tissue integrity in relation to diabetic neuropathy is not known. The aim of this study was to investigate retinal tissue thickness in relation to diabetic peripheral neuropathy (DPN) with and without diabetic retinopathy (DR). Methods Full retinal thickness at the parafoveal and perifoveal macula and neuro-retinal thickness around the optic nerve head (ONH) and at the macula was examined using spectral domain optical coherence tomography. The eye on the hand-dominant side of 85 individuals with type 1 diabetes and 66 individuals with type 2 diabetes, with or without DR and DPN, were compared to the eyes (n=45) of age-matched non-diabetic controls. Diabetic neuropathy was defined as Neuropathy Disability Score (NDS) ≥3 on a scale of 0-10. A general linear model was used to examine the relationship between diabetic neuropathy and foveal, parafoveal and perifoveal retinal thickness and neuro-retinal thickness, in relation to DR status, age, gender, HbA1c levels and duration of diabetes. A p-value of <0.05 was considered statistically significant. Results Perifoveal retinal thickness is reduced with increasing severity of neuropathy, especially in the inferior hemisphere (p=0.004); this effect was not related to age (p=0.088). For every unit increase in NDS score, the inferior perifoveal retinal thickness reduced by 1.64 μm. Neuro-retinal thickness around the ONH decreased with increasing severity of neuropathy (p<0.014 for average and hemisphere thicknesses); for every unit increase in NDS, neuro-retinal thickness around the ONH reduced by 1.23 μm. Retinal thickness in the parafovea was increased in the absence of DR (p<0.017 for average and hemisphere thicknesses). Neuro-retinal thickness at the macula was inversely related to age alone (p<0.001). All retinal parameters, except the inferior perifovea, reduced with advancing age (p<0.007 for all). Conclusions Diabetic neuropathy is associated with changes in full retinal thickness and neuro-retinal layers. This may represent a second threat to vision integrity, in addition to the better-characterised retinopathy. This study provides new knowledge about the anatomical aspects of the retinal tissue in relation to neuropathy and retinopathy.
