Introduction: Bien que l'imatinib (Glivec®) ait révolutionné le traitement de la leucémie myéloïde chronique (LMC) et des tumeurs stromales d'origine digestive (GIST), ses relations pharmacocinétique-pharmacodynamique (PK-PD) ont été peu étudiées. De par ses caractéristiques pharmacocinétiques (PK), ce médicament pourrait toutefois représenter un candidat à un programme de suivi thérapeutique (TDM). Objectif: Cette étude observationnelle visait à explorer ces relations PK-PD, et à évaluer l'influence spécifique du génotype de la tumeur dans la population GIST. Méthode: Des données de 59 patients ont été collectées durant une étude pharmacocinétique précédente. Sur la base du modèle de population développé alors, les paramètres PK ont été obtenus par estimation bayésienne et ont permis d'estimer l'exposition au médicament (AUC; aire sous la courbe). Les paramètres se rapportant à la fraction libre de l'imatinib ont été déduits d'un modèle intégrant les taux plasmatiques d'alpha1-glycoprotéine acide. L'association entre l'AUC (ou la clairance) et la réponse ou la toxicité a été explorée par régression logistique. L'influence du génotype de la tumeur (gène KIT) sur la réponse a également été évaluée chez des patients GIST. Résultats: L'exposition du médicament totale et libre est corrélée au nombre d'effets indésirables (ex: OR 2.9 ± 0.6 pour un accroissement d'AUC d'un facteur 2; p<0.001). Une relation avec la réponse n'est par contre pas évidente (les bons répondeurs recevant souvent des doses plus faibles que les mauvais répondeurs). Cependant, chez les patients GIST, une AUC libre plus élevée prédit une meilleure réponse (OR 1.9 ± 0.6; p<0.001), notamment chez les patients présentant des mutations sur l'exon 9 du gène cible KIT (ou un gène wild-type). Un tel profile génétique est connu pour diminuer la sensibilité à l'imatinib, par opposition à des mutations sur l'exon 11. Discussion-conclusion: Ces résultats, associés à la grande variabilité PK observée, représentent des arguments pour évaluer, pour l'imatinib, le bénéfice d'un programme de TDM. Nos données suggèrent également qu'une stratification des patients selon le génotype de la tumeur est important.

A 57-year-old male with no family history was diagnosed with semantic dementia. He also showed some unusual cognitive features such as episodic memory and executive dysfunctions, spatial disorientation, and dyscalculia. Rapidly progressive cognitive and physical decline occurred. About 1.5 years later, he developed clinical features of a corticobasal syndrome. He died at the age of 60. Brain autopsy revealed numerous 4R-tau-positive lesions in the frontal, parietal and temporal lobes, basal ganglia, and brainstem. Neuronal loss was severe in the temporal cortex. Such association of semantic dementia with tauopathy and corticobasal syndrome is highly unusual. These findings are discussed in the light of current knowledge about frontotemporal lobar degeneration.

La idea de realitzar aquest projecte sorgeix d’una necessitat en el mercat de productes relacionats amb els nens i nenes petits, dins d’un rang d’edats entre 2 a 5 anys, per poder realitzar activitats de lleure a la muntanya. En ser un producte no existent s’ha hagut de fer el disseny conceptual de tot el producte buscant la solució final adient per a cada part considerada bàsica. Per assegurar-ne la fiabilitat i la seguretat del nen i del pare s’han fet els càlculs pertinents de les seccions més carregades i s’han descrit els processos de fabricació i muntatge de cada peça. La Motxilla – Carretó servirà per circular per pistes de muntanya i forestals. Tindrà dos usos, el primer per quan el nen estigui cansat de caminar i el terreny es poc accidentat,“funció carretó” i el segon ús, per quan el nen està cansat i el terreny és més accidentat, “funció cadireta”. El producte serà transformable en funció d’aquests dos usos principals

Phylogenomic databases provide orthology predictions for species with fully sequenced genomes. Although the goal seems well-defined, the content of these databases differs greatly. Seven ortholog databases (Ensembl Compara, eggNOG, HOGENOM, InParanoid, OMA, OrthoDB, Panther) were compared on the basis of reference trees. For three well-conserved protein families, we observed a generally high specificity of orthology assignments for these databases. We show that differences in the completeness of predicted gene relationships and in the phylogenetic information are, for the great majority, not due to the methods used, but to differences in the underlying database concepts. According to our metrics, none of the databases provides a fully correct and comprehensive protein classification. Our results provide a framework for meaningful and systematic comparisons of phylogenomic databases. In the future, a sustainable set of 'Gold standard' phylogenetic trees could provide a robust method for phylogenomic databases to assess their current quality status, measure changes following new database releases and diagnose improvements subsequent to an upgrade of the analysis procedure.

The prescription, widely discussed and defined conceptually in recent years in an environment of widening the spectrum of responsibilities nurse, has capacity for integration and definition in the area of accident and emergency care and carried out over the patient urgently, about emergencies and life-long commitment. Be necessary to frame throughout the legal framework, following the amendment of the Twelfth Additional Provision of Law 29/2006 of guarantees and rational use of drugs and medical devices, can be waived and implementation required, provided under model and through the nursing process and method and as an exponent and endorsementn of science and advanced clinical practice, to join the idea of interdisciplinary professional consensus that the law posed by the preparation and implementation of standardized protocols, algorithms and / or clinical practice guidelines in the context of what has come to be called "collaborative standard prescription": Prescription to the nurse in certain clinical situations in terms of a performance protocol, agreed with multidisciplinary team care health of the population (Group Protocols), which can be considered an intermediate step in the evolution towards independent nurse prescribing, providing nurses experience of a prescription under these protocols and demonstrating their capabilities.

Equality with men in the world of paid work has been a major feminist objective. Given that work in the `public' sphere has historically been shaped on the assumption that the `worker' will be male, then national employment systems which facilitate masculine employment patterns (i.e. full-time work and unbroken employment careers) might be expected to be more likely to generate gender equality. This paper compares women's employment in France (where `masculine' careers for women are common) and Britain (where part-time work and broken employment careers are more likely) at the macro, meso (occupational), and micro (individual) levels. The two occupations studied are finance and pharmacy. The evidence presented suggests that there are considerable similarities between women in the two countries at the occupational and individual level, despite national variations. In the light of this evidence, structural and individual explanations of women's employment behaviour are examined, and the continuing significance of structural constraint on the patterning of gender relations is emphasised.

Roughly fifteen years ago, the Church of Jesus Christ of Latter-day Saints published a new proposed standard file format. They call it GEDCOM. It was designed to allow different genealogy programs to exchange data.Five years later, in may 2000, appeared the GENTECH Data Modeling Project, with the support of the Federation of Genealogical Societies (FGS) and other American genealogical societies. They attempted to define a genealogical logic data model to facilitate data exchange between different genealogical programs. Although genealogists deal with an enormous variety of data sources, one of the central concepts of this data model was that all genealogical data could be broken down into a series of short, formal genealogical statements. It was something more versatile than only export/import data records on a predefined fields. This project was finally absorbed in 2004 by the National Genealogical Society (NGS).Despite being a genealogical reference in many applications, these models have serious drawbacks to adapt to different cultural and social environments. At the present time we have no formal proposal for a recognized standard to represent the family domain.Here we propose an alternative conceptual model, largely inherited from aforementioned models. The design is intended to overcome their limitations. However, its major innovation lies in applying the ontological paradigm when modeling statements and entities.

Nowadays, when a user is planning a touristic route is very difficult to find out which are the best places to visit. The user has to choose considering his/her preferences due to the great quantity of information it is possible to find in the web and taking into account it is necessary to do a selection, within small time because there is a limited time to do a trip. In Itiner@ project, we aim to implement Semantic Web technology combined with Geographic Information Systems in order to offer personalized touristic routes around a region based on user preferences and time situation. Using ontologies it is possible to link, structure, share data and obtain the result more suitable for user's preferences and actual situation with less time and more precisely than without ontologies. To achieve these objectives we propose a web page combining a GIS server and a touristic ontology. As a step further, we also study how to extend this technology on mobile devices due to the raising interest and technological progress of these devices and location-based services, which allows the user to have all the route information on the hand when he/she does a touristic trip. We design a little application in order to apply the combination of GIS and Semantic Web in a mobile device.
