Objectivo: Analisar e caracterizar uma amostra de doentes de uma consulta de inflamação ocular. Material e Métodos: Análise retrospectiva de 503 consultas realizadas por um clínico entre 1 de Agosto de 2012 e 31 de Agosto de 2013 no Centro Hospitalar de Lisboa Central com recurso aos respectivos processos clínicos. Na análise da casuística da consulta foram incluídos 151 doentes. Desses, 24 padeciam de doenças auto-imunes em seguimento para monitorização de toxicidade a fármacos mas sem registo de qualquer episódio de uveíte, pelo que foram excluídos da avaliação estatística referente às uveítes. Dos 127 doentes com uveíte foram incluídos 197 olhos. Resultados: A média de idades foi de 53,8±16,5 anos, sendo 60% do sexo feminino e 40% masculino. A inflamação foi bilateral em 70 e unilateral em 57 doentes. O tipo de uveíte mais frequente foi a anterior (51,2%), seguida da panuveíte (21,3%), posterior (19,7%), intermédia (3,9%), episclerite (3,2%) e esclerite (0,8%). As etiologias foram agrupadas em: doenças sistémicas (34%), doenças infecciosas (30%), idiopáticas (27%) e patologias oculares específicas (9%). A acuidade visual média nos olhos com uveíte anterior foi 0.8, panuveíte 0.2, uveíte posterior 0.2, uveíte intermédia 0.7, episclerite e esclerite 1.0. Dos 197 olhos com uveíte, 27 (13,7%) foram submetidos a cirurgia de catarata e 5 (2,5%) a cirurgia de glaucoma. Conclusões: Apesar de se tratar de uma amostra relativamente pequena, reveste-se de importância dado ser fundamental conhecer a realidade em cada centro de referência de forma a optimizar os recursos disponíveis e a melhorar a abordagem clínica.

Foi examinado um total de 222 cães, sendo que 197 eram cães de rua apreendidos nas áreas suburbanas e rurais do Rio de Janeiro e 25 eram animais de estimação, mantidos em residências (apartamentos) nas zonas Centro e Sul da cidade. Do total de 197 cães de rua, foram encontrados 49 (24,87%) parasitados por coccidios do gênero Isospora, sendo que 6 deles (3,04%) eram parasitados por I. canis e 43 (21,83%) por I. rivolta. Nos 43 casos de parasitismo por I. rivolta foi encontrada nas fezes uma alta percentagem de esporocistos livres com 4 esporozoitos (35 casos, representando 81,4%). As fezes foram coletadas diretamente do reto e a técnica empregada para o exame do material foi a centrífugo-flutuação em sulfato de zinco. Entre os 25 cães de estimação não foi encontrado nenhum caso de parasitismo por Isospora sp. Os valores médios para os diâmetros maior e menor das formas de Isospora sp. encontradas foram de 38,63μ x 31,93μ. para oocistos de I. canis; 23μ x 18,94μ. para oocistos de I. rivolta e, 19,94μ x 10,05μ para os esporocistos de I. rivolta, encontrados livres nas fezes. Não foi diagnosticado nenhum caso de I. bigemina. Entre todos os casos positivos foram encontrados apenas 3 casos de infecção maciça nos animais. Nos 3 casos, tratavam-se de animais jovens parasitados por I. rivolta. O teste estatístico de diferença de proporções demonstrou que o sexo e a idade dos animais não tiveram influência na maior ou menor percentagem de cães positivos para Isospora sp, na presente casuística.

BACKGROUND & AIMS: Patients who underwent endoscopic gastrostomy (PEG) present protein-energy malnutrition, but little is known about Trace Elements (TE), Zinc (Zn), Copper (Cu), Selenium (Se), Iron (Fe), Chromium (Cr). Our aim was the evaluation of serum TE in patients who underwent PEG and its relationship with serum proteins, BMI and nature of underlying disorder. METHODS: A prospective observational study was performed collecting: patient's age, gender, underlying disorder, NRS-2002, BMI, serum albumin, transferrin and TE concentration. We used ferrozine colorimetric method for Fe; Inductively Coupled Plasma-Atomic Emission Spectroscopy for Zn/Cu; Furnace Atomic Absorption Spectroscopy for Se/Cr. The patients were divided into head and neck cancer (HNC) and neurological dysphagia (ND). RESULTS: 146 patients (89 males), 21-95 years: HNC-56; ND-90. Low BMI in 78. Low values mostly for Zn (n = 122) and Fe (n = 69), but less for Se (n = 31), Cu (n = 16), Cr (n = 7); low albumin in 77, low transferrin in 94 and 66 with both proteins low. Significant differences between the groups of underlying disease only for Zn (t140.326 = -2,642, p < 0.01) and a correlation between proteins and TE respectively albumin and Zn (r = 0.197, p = 0.025), and albumin and Fe (r = 0.415, p = 0.000). CONCLUSIONS: When gastrostomy was performed, patients display low serum TE namely Zn, but also Fe, less striking regarding others TE. It was related with prolonged fasting, whatever the underlying disease. Low proteins were associated with low TE. Teams taking care of PEG-patients should use Zn supplementation and include other TE evaluation as part of the nutritional assessment of PEG candidates.

Um levantamento malacológico em coleções hídricas de 13 municípios da microrregião de Belo Horizonte, MG, Brasil, foi efetuado para detectar focos de transmissão de esquistossomose e outras parasitoses. De 1990 a 1996 foram coletados 22.066 moluscos dos quais 378 (1,7%) estavam infectados com trematódeos: Biomphalaria glabrata (7.920), com Schistosoma mansoni (1,9%), com Echinostomatidae (1,2%), com Strigeidae (0,6%), com Cercaria minense (0,1%) e Derogenidae (-0,1%); B. straminea (4.093), com Strigeidae (0,6%), com Echinostomatidae (0,2%), com Clinostomatidae (-0,1%) e duas cercárias desconhecidas; B. tenagophila (1.338), com Strigeidae (0,1%); Physa marmorata (1.776), com Echinostomatidae (1,6%). Os moluscos Biomphalaria peregrina, B. occidentalis, B. schrammi, Drepanotrema depressissimum, D. lucidum, D. cimex, Physa cubensis, Lymnaea columella, Melania tuberculata, Idiopyrgus souleyetianus, Pomacea sp, Anodontites sp e Ancylidae não estavam infectados. Moluscos de 9 municípios estavam infectados com S. mansoni e de 11 com outros trematódeos.

In Pernambuco, the first dengue cases occurred in 1987. After a seven-year interval without autochthonous cases, a new epidemic occurred in 1995. Important aspects of the dengue epidemics during the period 1995-2006 have been analyzed here, using epidemiological, clinical and laboratory data. A total of 378,374 cases were notified, with 612 confirmed cases of dengue hemorrhagic fever and 33 deaths. The mortality rate was 5.4%. The incidence rate increased from 134 to 1,438/100,000 inhabitants, corresponding to the epidemics due to serotypes 2 and 3, in 1995 and 2002, respectively. Dengue mainly affected adults (20-49 years); 40.7% were male and 59.3% were female. From 2003 onwards, the number of cases among individuals younger than 15 years old increased. Out of 225 dengue hemorrhagic fever cases, 42.7% primary and 57.3% secondary infections were identified (p = 0.0279). Neurological manifestations were also observed. From 2002 onwards, serotypes 1, 2 and 3 were circulating; serotype 3 was predominant.

O Inquérito sorológico da prevalência de infecção chagásica no Brasil, 1975/1980, cujos resultados foram objeto de publicação anterior, em 1984 (Camargo e cols), é aqui detalhado em sua distribuição geográfica. Foi uma iniciativa da Superintendência de Campanhas de Saúde Pública (SUCAM/MS) e do Conselho Nacional de Pesquisas (CNPq) através de seu Programa Integrado de Doenças Endêmicas (PIDE), tendo servido para delimitar e estratificar mais precisamente a área com transmissão endêmica da doença de Chagas e orientar as ações de controle vetorial, implementadas a partir da segunda metade da década de 1970. Mostrou uma soroprevalência estimada de 4,22% da infecção chagásica para a população geral residente em área rural no país. Observe-se que não foram incluídos o Estado de São Paulo e o Distrito Federal, para onde se acreditava já haver informação suficiente e recente. Abrangeu todas as demais unidades federativas, com o exame de 1.626.745 amostras de sangue, processadas por imunofluorescencia indireta. Destas, foram consideradas válidas para efeito de processamento e análise estatística 1.352.197, procedentes de 3.026 municípios de 24 estados, segundo a divisão política de então. Os resultados no geral foram confirmatórios em relação ao que era já conhecido. Alguns achados no entanto não corresponderam ao esperado, o que foi objeto de investigação ou se soube depois justificáveis, com base em dados de entomologia e outros que serviram à interpretação dos resultados.

INTRODUCTION: This work aimed to study the community structure of sandflies, with regard to the richness, constancy, abundance, and monthly frequency of the species with a focus on the transmission of leishmaniasis. METHODS: The study was conducted in the rural villages of Bom Jardim and Santa Maria, situated on the edge of a tropical rain forest in the municipality of São Jose de Ribamar, Maranhão, Brazil. The phlebotomines were captured in the intradomiciles and peridomiciles of each village, with Centers for Disease Control (CDC) light traps set in 10 homes in each village, for 1 year, once a month, from 18h to 6h. RESULTS: We collected 1,378 individuals of 16 sandfly species. The capture success rate was higher in Bom Jardim (0.61 specimens/hour/trap) than that of Santa Maria (0.35/specimens/hour/trap). The sandflies were more abundant in the peridomiciles (86.1%) and in the rainy season (77%). Five species were considered constants (occurring in more than 50% of samples), 5 accessory (25%-50%), and 6 accidental (<25%). The most abundant species were Lutzomyia longipalpis (59.7%) and L whitmani (28%). The permutation analysis showed differences between the species composition of the villages and no separation between the intradomicile and peridomicile of each village. The species that most contributed to the dissimilarity between the light traps of the 2 villages were L. longipalpis, L. whitmani, and L. evandroi, contributing to 80.8% of the variation among groups. CONCLUSIONS: The high level of richness and abundance of species and the presence of competent vectors throughout the year and around houses justify the occurrence of leishmaniasis cases reported in the area.

The Archipelago of the Azores (Portugal) is located between 378 and 418N and 258 and 318W and crosses the Mid-Atlantic Ridge. It is the most isolated archipelago in the Atlantic, situated 1600 km west of mainland Portugal and 3500 km from the eastern coast of the United States of America. At present, the only population of seals occurring in the Portuguese territory is found on Desertas Islands, Archipelago of Madeira, where a colony of 24 Mediterranean monk seals, Monachus monachus (Hermann, 1779), still persists (Pires and Neves 2001). Nonetheless, historical accounts reported by Frutuoso (1983) dating from the early to late 1500s mention sightings of ‘‘sea wolves’’ (the old Portuguese folk term for the Mediterranean monk seal) at several sites along the Azorean Island of Santa Maria. Little is known about the occurrence of monk seals in this area over the past five centuries, but the species certainly did not escape deliberate killing by the first settlers. While the early monk seal reports by Frutuoso (1983) are the only reports referring to the presence of colonies of seals in the Azores, more recently several sightings and strandings of vagrant seals of other species have been noted.

One facet of self-transcendence is creative self-forgetfulness (CSF; tendency to be attentionally absorbed in mindaltering experiences). Proneness to mind-altering attentional absorption and other aspects of self-transcendence were previously related to vaginal intercourse frequency, sexual arousability, and female coital orgasm. Given that sexual responsiveness is enhanced by focused attention, itwas testedwhether CSF correlates with sexual responsiveness, and if maladaptive defenses, openness to experience, and testosterone explain the hypothesized relationships. One hundred thirty-nine Portuguese (98 women) provided saliva samples for testosterone determination by luminescence immunoassays before and after a romantic movie scene and reported how intensely they felt sexual desire and arousal during the movie. CSF was measured by the Temperament and Character Inventory– Revised, maladaptive defenses by the immature defenses subscale of the Defense Style Questionnaire (DSQ-40), male and female past month desire by the desire dimensions of the International Index of Erectile Function (IIEF), and Female Sexual Function Index (FSFI). Female desire and arousal during themoviewere independently predicted by CSF, openness to experience and testosterone, but not by immature defenses. Female past month desire was independently predicted by CSF, testosterone, and less immature defenses. Possible psychobiological processes linking self-transcendence and sexual responsiveness are discussed.

INTRODUCTION: Leptospirosis is a zoonosis that affects both humans and animals. Dogs may serve as sentinels and indicators of environmental contamination as well as potential carriers for Leptospira. This study aimed to evaluate the seroprevalence and seroincidence of leptospirosis infection in dogs in an urban low-income community in southern Brazil where human leptospirosis is endemic. METHODS: A prospective cohort study was designed that consisted of sampling at recruitment and four consecutive trimestral follow-up sampling trials. All households in the area were visited, and those that owned dogs were invited to participate in the study. The seroprevalence (MAT titers ≥100) of Leptospira infection in dogs was calculated for each visit, the seroincidence (seroconversion or four-fold increase in serogroup-specific MAT titer) density rate was calculated for each follow-up, and a global seroincidence density rate was calculated for the overall period. RESULTS: A total of 378 dogs and 902.7 dog-trimesters were recruited and followed, respectively. The seroprevalence of infection ranged from 9.3% (95% CI; 6.7 - 12.6) to 19% (14.1 - 25.2), the seroincidence density rate of infection ranged from 6% (3.3 - 10.6) to 15.3% (10.8 - 21.2), and the global seroincidence density rate of infection was 11% (9.1 - 13.2) per dog-trimester. Canicola and Icterohaemorraghiae were the most frequent incident serogroups observed in all follow-ups. CONCLUSIONS: Follow-ups with mean trimester intervals were incapable of detecting any increase in seroprevalence due to seroincident cases of canine leptospirosis, suggesting that antibody titers may fall within three months. Further studies on incident infections, disease burden or risk factors for incident Leptospira cases should take into account the detectable lifespan of the antibody.

Os transportes rodoviários são essenciais para a sociedade actual. Deles depende directamente a mobilidade de milhões de pessoas diariamente, assim como o funcionamento de diversos sectores económico em Portugal e no mundo. Existem diversos estudos focados na pressão ambiental associada à operação de veículos pesados. Todavia, o estudo de pressões inerentes às restantes fases do ciclo de vida permanece pouco desenvolvido carecendo de informação comparável. O objectivo da presente tese passa por avaliar o desempenho ambiental do transporte pesado rodoviário a nível nacional, através da aplicação da metodologia de análise de ciclo de vida (ACV). Na fase de quantificação do desempenho ambiental foi utilizada a metodologia EcoBlok considerando o uso de recursos e emissões de poluentes. No ano de 2013 o transporte pesado de mercadorias consumiu 12,8 hm3 eq. de água e 3 097 kt eq. de recursos; ocupou 119 km2.ano eq. de solos; emitiu 3 253 kt CO2 eq. de GEE; 121 kt NOx eq. de poluentes atmosféricos; 197 kt N eq. de poluentes para o solo e meios hídricos e 1,3 kg TCDD eq. de dioxinas e furanos. A fase de operação é a que mais contribui para o total de pressões contabilizadas no subsector mencionado. O transporte de passageiros registou, para o total da frota activa em 2013 um total de 1,7 hm3 eq. de água extraída; 221 kt eq. de recursos consumidos; 14,6 km2.ano eq. de solos usados; 234 kt CO2 eq. de GEE; 10,4 kt NOx eq. de poluentes atmosféricos; 16,9 kt N eq. de emissões para o solo e meios hídricos e 0,1 kg TCDD eq. de dioxinas e furanos emitidos. O destaque vai igualmente, para a fase de operação, a que mais contribui para a pressão total averiguada. Os resultados e conclusões obtidas ao longo do estudo podem revelar-se úteis para a concepção de políticas de transporte e tomada de decisão, na medida em que permitem a comparação com o desempenho ambiental de outros modos e sistemas de transporte.

In the stock market, information takes on special relevance, due to the market’s permanent updating and the great fluidity of information existent therein. Just as in any other negotiations, the party with the better information has a bargaining advantage, as it is able to make more advantageous business decisions. However, unlike most other markets, the proper functioning of the stock market is greatly dependent on investors’ trust in the market itself. As such, if there are investors who, due to any condition they possess or office they hold, have access to relevant information which is not accessible to the general public, distrust is bred within the market and, consequently, investment is lessened. Thus, there is a need to prevent those who hold privileged information from using it in abusive ways. In Portugal, abuse of privileged information is set out and punished criminally in Article 378. of the Portuguese Securities Code (‘Código dos Valores Mobiliários’). In this dissertation, I have set out, firstly, to analyze the inherent conditions for there to be a crime of abuse of privileged information; secondly, to analyze two well-known cases, which took place and were decided in other jurisdictions, and attempt to understand how these cases would fall under Article 378. of the Portuguese Securities Code. Whereas the first case, Chiarella v. United States, was scrutinize under Article 378 of the Portuguese Securities Code, in the second, Lafonta v. AMF, the conclusion arrived at was that the crime taken place was different. This analysis allowed, on one hand, the application to a particular case of prerequisites and concepts which were explained, at a first approach, from a more theoretical perspective; on the other hand, it also allowed the further development of specific aspects of the regime, namely the difference between an insider and a tipee, as well as to more clearly set out the limits to the precise character of the information at hand.
