A situação epidemiológica da doença de Chagas no país foi substancialmente alterada como resultado das ações de controle e de mudanças ambientais, econômicas e sociais, havidas nas últimas décadas no país. A transmissão vetorial domiciliar por Triatoma infestans foi interrompida, e controlada em níveis importantes por espécies nativas de vetor. A transmissão transfusional tende ao esgotamento, desde que se logrou a triagem praticamente integral de candidatos à doação de sangue. A transmissão congênita, ainda que possível, especialmente em algumas áreas, tende também ao progressivo esgotamento em consequência do controle da transmissão vetorial e transfusional. Os mecanismos primordiais de transmissão, relacionados diretamente ao ciclo enzoótico, como a transmissão vetorial extradomiciliar ou por visitação de vetores silvestres aos domicílios, além da transmissão oral, passaram a ter relevância na produção de casos humanos de infecção por Trypanosoma cruzi. Diante deste quadro, os novos desafios a enfrentar em relação à doença de Chagas incluem I) a necessidade de se preservar os níveis de controle alcançados; II) a concepção e desenvolvimento de novas tecnologias e métodos de vigilância e controle que permitam reduzir os riscos de ocorrência de casos associados à transmissão enzoótica; e, III) a garantia de adequada atenção aos infectados e enfermos crônicos de doença de Chagas.

Interleukin 8 (CXCL8) is an autocrine chemokine specific for the chemoattraction and activation of granulocytes, NKT cells and T lymphocytes. Patients with tuberculosis and latent Mycobacterium tuberculosis infection were assessed for the spontaneous expression of CXCR1 (CD128) and CXCR2 on lymphocytes and monocytes. Compared with ex vivo profiles, increased spontaneous CXCR2 expression and normal CXCR1 expression were found on lymphocytes in two out of 59 individuals. Monocytes showed normal ex vivo profiles for both receptors. After stimulation with purified protein derivative, the in vitro levels of CXCL8 were below the median levels of all patients with prior tuberculosis. Spontaneous CXCR2 modulation did not cause notable variation in the in vitro levels of CXCL8.

A presente dissertação tem como objetivo principal a implementação de uma arquitetura baseada em algoritmos evolutivos para a sintonização dos parâmetros do controlador PID (Proporcional-Integral-Derivativo) difuso, sendo o conceito de desempenho em malha fechada explicitamente tido em conta. A sintonização dos parâmetros do controlador difuso é realizada tendo em conta um problema de otimização com restrições, em que a função de custo a ser minimizada é descrita em termos do desempenho em malha fechada, com a dinâmica do sistema a ser aproximada por um modelo não linear. Como nas metodologias de otimização existentes, a incorporação de mecanismos de adaptação referentes às funções de pertença não é comum, na presente dissertação é tido em conta, para além da usual sintonização dos fatores de escala, a sintonização dos fatores de escala e funções de pertença em simultâneo. Os resultados experimentais realizados num sistema de referência, visam demonstrar os benefícios de incorporar as funções de pertença no processo de otimização em diferido. É também utilizado um método analítico de segunda ordem como referência, por forma a comparar o desempenho de uma abordagem de otimização global contra uma de otimização local. Finalmente é implementada uma abordagem em-linha, usando o método analítico de segunda ordem, na otimização dos fatores de escala e funções de pertença.

Ileal pouch-anal anastomosis was an important advancement in the treatment of ulcerative colitis. The aim of this study was to determine whether early complications of ileal pouch-anal anastomosis in patients with ulcerative colitis are associated with poor late functional results. PATIENTS AND METHODS: Eighty patients were operated on from 1986 to 2000, 62 patients with ileostomy and 18 without. The early and late complications were recorded. Specific emphasis has been placed on the incidence of pouchitis with prolonged follow-up. RESULTS: The ileostomy was closed an average of 9.2 months after the first operation. Fourteen patients were excluded from the long-term evaluation; 6 patients were lost to regular follow-up, 4 died, and 4 patients still have the ileostomy. Of the 4 patients that died, 1 died from surgical complications. Early complications after operation (41) occurred in 34 patients (42.5%). Late complications (29) occurred in 25 patients as follows: 16 had pouchitis, 3 associated with stenosis and 1 with sexual dysfunction; 5 had stenosis; and there was 1 case each of incisional hernia, ileoanal fistula, hepatic cancer, and endometriosis. Pouchitis occurred in 6 patients (9.8%) 1 year after ileal pouch-anal anastomosis, 9 (14.8%) after 3 years, 13 (21.3%) after 5 years, and 16 (26.2%) after more than 6 years. The mean daily stool frequency was 12 before and 5.8 after operation. One pouch was removed because of fistulas that appeared 2 years later. CONCLUSIONS: Ileal pouch-anal anastomosis is associated with a considerable number of early complications. There was no correlation between pouchitis and severe disease, operation with or without ileostomy, or early postoperative complications. The incidence of pouchitis was directly proportional to duration of time of follow-up.

The impact of clinical, pathologic, and surgical variables on the postoperative morbidity, mortality, and survival of patients undergoing extended resections of colon carcinoma were evaluated. METHODS: The medical records of 95 patients who underwent extended resections for colon carcinoma between 1953 and 1996 were reviewed. In all cases, in addition to colectomy, 1 or more organs and/or structures were resected en bloc due to a macroscopically based suspicion of tumor invasion. The clinical, pathologic, and surgical parameters were analyzed. Overall survival rates were analyzed according to the method of Kaplan and Meier. Multivariate analysis was performed using the Cox proportional hazards model. RESULTS: Eighty-six patients were treated by curative surgeries and the remaining by palliative resections. Invasion of the organs and/or adjacent structures and regional lymph nodes was found microscopically in 48 and 31 patients, respectively. The median follow-up without postoperative mortality was 47.7 months. The 5-year overall survival rates was 52.6%. The 5-year overall survival rates for patients undergoing curative and palliative surgeries was 58.3% and 0%, respectively. The mean survival time in the palliative surgery group was 3.1 months. Multivariate analysis showed that Karnofsky performance status was strongly related to the risk of postoperative complications (P = .01), and postoperative deaths were associated with the type of surgery and Karnofsky performance status at the time of admission (P = .001). CONCLUSIONS: Some patients with locally advanced colon adenocarcinomas undergoing extended resections have a 5-year overall survival rates of 58.3%. Patients could benefit from palliative-intent procedures, but these measures should cautiously be indicated and avoided in patients with low Karnofsky performance status due to high rates of postoperative mortality and poor survival.

Frutos de araçá-pera (Psidium acutanguium D.C.) em estádio de amadurecimento comercial foram avaliados quanto às características físicas, fisico-químicas e químicas. Houve grande variação no peso dos frutos (47,89 a 138,34 g) e no rendimento em polpa (55,01 a 75,98%). O araçá-pera é um fruto suculento (85,85% de umidade), com baixa relação Brix/ acidez (5,88), baixo pH (3,0) e acidez elevada (1,87% de ácido citrico). Destaca-se como excelente fonte de vitamina C total (389,34 mg l00g-1 de polpa integral). O baixo grau de doçura é fator limitante para o consumo "in natura". Apresenta potencial para industrialização, e para esta finalidade, seus atributos de qualidade são a uniformidade de formato,(levemente arredondado), alto rendimento em polpa, baixo pH e elevadas concentrações de acidez e vitamina C total.

Marine ecosystem can be considered a rather exploited source of natural substances with enormous bioactive potential. In Mexico macro-algae study remain forgotten for research and economic purposes besides the high amount of this resource along the west and east coast. For that reason the Bioferinery Group of the Autonomous University of Coahuila, have been studying the biorefinery concept in order to recover high value byproducts of Mexican brown macro-algae including polysaccharides and enzymes to be applied in food, pharmaceutical and energy industry. Brown macroalgae are an important source of fucoidan, alginate and laminarin which comprise a complex group of macromolecules with a wide range of important biological properties such as anticoagulant, antioxidant, antitumoral and antiviral and also as rich source of fermentable sugars for enzymes production. Additionally, specific enzymes able to degrade algae matrix (fucosidases, sulfatases, aliginases, etc) are important tools to establish structural characteristics and biological functions of these polysaccharides. The aims of the present work were the integral study of bioprocess for macroalgae biomass exploitation by the use of green technologies as hydrothermal extraction and solid state fermentation in order to produce polysaccharides and enzymes (fucoidan and fucoidan hydrolytic enzymes). This work comprises the use of the different bioprocess phases in order to produce high value products with lower time and wastes.

The code is verified through the comparison of its predictions with analytical solutions and numerical results obtained with the differential upper-convected Maxwell model

Constitui-se o presente trabalho da transcrição integral da palestra realizada pelo Dr. Harald Sioli (1910-2004) no Seminário da Amazônia, em 10 de agosto de 1990, quando numa última viagem ao Amazonas, ao completar 80 anos de idade, foi homenageado pelos colegas e a convite do INPA falou sobre "50 anos de pesquisas em limnologia na Amazônia" que, segundo suas próprias palavras, foram "os dias mais decisivos de minha vida". O texto pode ser dividido em duas partes: a primeira se refere à história pessoal - da chegada ao Brasil, de como descobriu a Amazônia e sobreviveu, no interior do Amazonas e Pará, à 2.ª Guerra Mundial, até se tornar pesquisador do INPA e firmar o Convênio INPA/Max Planck. A segunda parte fala da sua pesquisa em limnologia, fazendo contrapontos com pesquisa e pesquisadores brasileiros e estrangeiros, os do passado e os contemporâneos, assim como da convivência com os caboclos que, no seu entender, "vivem com a floresta e não contra a floresta". A transcrição da palestra, foi feita por Elci Silva, e Antonio Alvarez e preservada por Terezinha Soares, então lotados na Assessoria de Tecnologia e Extensão - ASTE que à época organizava o Seminário da Amazônia. A publicação foi autorizada pessoalmente pelo Dr. Sioli, ainda em 2000, através de correspondência em que ele afirmava "ficarei muito satisfeito se ainda puder ser um pouco útil na luta pela sobrevivência da Amazônia, daquela minha segunda pátria". Está, enfim, sendo publicada. E ainda que Haraldi Sioli já não esteja entre nós, representa uma homenagem da Acta Amazonica à memória daquele que dedicou tantos anos ao conhecimento e à preservação da Amazônia.

The usual high cost of commercial codes, and some technical limitations, clearly limits the employment of numerical modelling tools in both industry and academia. Consequently, the number of companies that use numerical code is limited and there a lot of effort put on the development and maintenance of in-house academic based codes. Having in mind the potential of using numerical modelling tools as a design aid, of both products and processes, different research teams have been contributing to the development of open source codes/libraries. In this framework, any individual can take advantage of the available code capabilities and/or implement additional features based on his specific needs. These type of codes are usually developed by large communities, which provide improvements and new features in their specific fields of research, thus increasing significantly the code development process. Among others, OpenFOAM® multi-physics computational library, developed by a very large and dynamic community, nowadays comprises several features usually only available in their commercial counterparts; e.g. dynamic meshes, large diversity of complex physical models, parallelization, multiphase models, to name just a few. This computational library is developed in C++ and makes use of most of all language capabilities to facilitate the implementation of new functionalities. Concerning the field of computational rheology, OpenFOAM® solvers were recently developed to deal with the most relevant differential viscoelastic rheological models, and stabilization techniques are currently being verified. This work describes the implementation of a new solver in OpenFOAM® library, able to cope with integral viscoelastic models based on the deformation field method. The implemented solver is verified through the comparison of the predicted results with analytical solutions, results published in the literature and by using the Method of Manufactured Solutions.

Enquadramento: O Acidente Vascular Cerebral (AVC) acarreta múltiplas alterações no quotidiano do doente e seus familiares mas a reabilitação é considerada uma oportunidade. A Rede Nacional de Cuidados Continuados Integrados (RNCCI) facilita a reabilitação e a educação para a saúde possibilita a mudança de atitude necessária para a reabilitação. Objetivos: Conhecer e divulgar as perceções de doentes com AVC e enfermeiros relativamente às práticas de Educação para a Saúde na RNCCI. Metodologia: Qualitativa, com entrevista semi-estruturada, a 8 doentes e a 17 enfermeiros. A técnica escolhida foi a análise de conteúdo das entrevistas. Partimos de categorias definidas a priori, e emergiram outras categorias. Resultados: Os doentes atribuíram aos enfermeiros a maior parte da responsabilidade pela reabilitação. Os enfermeiros relacionaram os aspetos psicológicos e a importância do envolvimento da família com a adesão do doente ao regime terapêutico. Conclusão: Os doentes demonstram que se encontram no modelo biomédico, por outro lado os enfermeiros apontam o modelo biopsicossocial como orientador das suas práticas de Educação para a Saúde na rede.
