Penetrating hand wounds are common and these are managed by thorough debridement. However, stab wounds without evidence of divided structures are often treated with irrigation using antiseptic substances, antibiotic therapy, and immobilization. Octenisept® (Schülke & Mayr Ltd) is a widely used antiseptic agent for disinfection of acute or chronic wounds. It has a broad spectrum of antiseptic efficacy and has become an antiseptic of first choice in many hospitals. Within a few months, four patients presented to us with chronic inflammation and severe tissue necrosis after irrigation of penetrating hand wounds with Octenisept®. Repeated surgery and debridement was required in all patients. Wound healing was prolonged and patients had persisting oedema. Penetrating hand wounds must not be irrigated with Octenisept®.

Nail surgery is a special branch of hand and dermatologic surgery. It is not widely performed, and many physicians do not feel at ease to perform it. The objective of this contribution is to give a short overview of the most important surgical procedures in clinical practice. References from the literature and the author's own experiences are condensed to describe what a dermatologic practitioner with knowledge of the nail and some surgical skills can perform. Nail surgery is a precise technique that requires careful administration and attention to details. Proper patient preparation starts with a patient history to identify potential contraindications and to prevent unnecessary complications. The author recommends isopropyl alcohol scrub and chlorhexidine for disinfection and ropivacaine 1% for anesthesia. The technique used for anesthesia depends on the type of surgery. Surgical procedures are described for diagnostic biopsies, nail avulsion in general, onychogryposis, paronychia treatment, hematomas and bone fracture due to trauma, removal of subungual foreign bodies, ingrowing nails, pincer nails, warts, ungual fibrokeratomas, digital myxoid pseudocyst, subungual exostoses, and various tumors. If performed correctly with adequate skills, nail surgery will lead to functionally and aesthetically satisfying results in the majority of instances.

PURPOSE To investigate the likelihood of speaking up about patient safety in oncology and to clarify the effect of clinical and situational context factors on the likelihood of voicing concerns. PATIENTS AND METHODS 1013 nurses and doctors in oncology rated four clinical vignettes describing coworkers' errors and rule violations in a self-administered factorial survey (65% response rate). Multiple regression analysis was used to model the likelihood of speaking up as outcome of vignette attributes, responder's evaluations of the situation and personal characteristics. RESULTS Respondents reported a high likelihood of speaking up about patient safety but the variation between and within types of errors and rule violations was substantial. Staff without managerial function provided significantly higher levels of decision difficulty and discomfort to speak up. Based on the information presented in the vignettes, 74%-96% would speak up towards a supervisor failing to check a prescription, 45%-81% would point a coworker to a missed hand disinfection, 82%-94% would speak up towards nurses who violate a safety rule in medication preparation, and 59%-92% would question a doctor violating a safety rule in lumbar puncture. Several vignette attributes predicted the likelihood of speaking up. Perceived potential harm, anticipated discomfort, and decision difficulty were significant predictors of the likelihood of speaking up. CONCLUSIONS Clinicians' willingness to speak up about patient safety is considerably affected by contextual factors. Physicians and nurses without managerial function report substantial discomfort with speaking up. Oncology departments should provide staff with clear guidance and trainings on when and how to voice safety concerns.

Nosocomial infections in patients requiring renal replacement therapy have a high impact on morbidity and mortality. The most dangerous complication is bloodstream infection (BSI) associated with the vascular access, with a low BSI risk in arteriovenous fistulas or grafts and a comparatively high risk in central venous catheters. The single most important measure for preventing BSI is therefore the reduction of catheter use by means of early fistula formation. As this is not always feasible, prevention should focus on educational efforts, hand hygiene, surveillance of dialysis-associated events, and specific measures at and after the insertion of catheters. Core measures at the time of insertion include choosing the optimal site of insertion, the use of maximum sterile barrier precautions, adequate skin antisepsis, and the choice of catheter type; after insertion, access care needs to ensure hub disinfection and regular dressing changes. The application of antimicrobial locks is reserved for special situations. Evidence suggests that bundling a selection of the aforementioned measures can significantly reduce infection rates. The diagnosis of central line-associated BSI (CLABSI) is based on clinical signs and microbiological findings in blood cultures ideally drawn both peripherally and from the catheter. The prompt installation of empiric antibiotic treatment covering the most commonly encountered organisms is key regarding CLABSI treatment. Catheter removal is recommended in complicated cases or if cultures yield Staphylococcus aureus, enterococci, Pseudomonas or fungi. In other cases, guide wire exchange or catheter salvage strategies with antibiotic lock solutions may be acceptable alternatives.

INTRODUCTION During dentinogenesis, growth factors become entrapped in the dentin matrix that can later be released by demineralization. Their effect on pulpal stem cell migration, proliferation, and differentiation could be beneficial for regenerative endodontic therapies. However, precondition for success, as for conventional root canal treatment, will be sufficient disinfection of the root canal system. Various irrigation solutions and intracanal dressings are available for clinical use. The aim of this study was 2-fold: to identify a demineralizing solution suitable for growth factor release directly from dentin and to evaluate whether commonly used disinfectants for endodontic treatment will compromise this effect. METHODS Dentin disks were prepared from extracted human teeth and treated with EDTA or citric acid at different concentrations or pH for different exposure periods. The amount of transforming growth factor-β1 (TGF-β1), fibroblast growth factor 2, and vascular endothelial growth factor were quantified via enzyme-linked immunosorbent assay and visualized by gold labeling. Subsequently, different irrigation solutions (5.25% sodium hypochloride, 0.12% chlorhexidine digluconate) and intracanal dressings (corticoid-antibiotic paste, calcium hydroxide: water-based and oil-based, triple antibiotic paste, chlorhexidine gel) were tested, and the release of TGF-β1 was measured after a subsequent conditioning step with EDTA. RESULTS Conditioning with 10% EDTA at pH 7 rendered the highest amounts of TGF-β1 among all test solutions. Fibroblast growth factor 2 and vascular endothelial growth factor were detected after EDTA conditioning at minute concentrations. Irrigation with chlorhexidine before EDTA conditioning increased TGF-β1 release; sodium hypochloride had the opposite effect. All tested intracanal dressings interfered with TGF-β1 release except water-based calcium hydroxide. CONCLUSIONS Growth factors can be released directly from dentin via EDTA conditioning. The use of disinfecting solutions or medicaments can amplify or attenuate this effect.

Non routine hospital settings are those that are infrequently used in hospitals and that often do not come to mind when sanitation and disinfection practices are used. These settings are a major source of nosocomial, or hospital acquired, infections, and are often overlooked. Data on these sources are often scattered and scarce, but these sources are significant such that they warrant equal attention of commonly recognized nosocomial infection sources in order to help reduce incidence of nosocomial infections. ^

Background. Houston, Texas, once obtained all its drinking water from underground sources. However, in 1853, the city began supplementing its water from the surface source Lake Houston. This created differences in the exposure to disinfection byproducts (DBPs) in different parts of Houston. Trihalomethanes (THMs) are the most common DBP and are useful indicators of DBPs in treated drinking water. This study examines the relationship between THMs in chlorinated drinking water and the incidence of bladder cancer in Houston. ^ Methods. Individual bladder cancer deaths, from 1975 to 2004, were assigned to four surface water exposure areas in Houston utilizing census tracts—area A used groundwater the longest, area B used treated lake water the longest, area C used treated lake water the second longest, and area D used a combination of groundwater and treated lake water. Within each surface water exposure area mortality rates were calculated in 5 year intervals by four race-gender categories. Linear regression models were fitted to the bladder cancer mortality rates over the entire period of available data (1990–2004). ^ Results. A decrease in bladder cancer mortality was observed amongst white males in area B (p = 0.030), white females in area A (p = 0.008), non-white males in area D (p = 0.003), and non-white females in areas A and B (p = 0.002 & 0.001). Bladder cancer mortality differed by race-gender and time (p ≤ 0.001 & p ≤ 0.001), but not by surface water exposure area (p = 0.876). ^ Conclusion. The relationship between bladder cancer mortality and the four surface water exposure areas (signifying THM exposure) was insignificant. This result could be attributable to Houston controlling for THMs starting in the early 1980’s by using chloramine as a secondary disinfectant in the drinking water purification process.^

The study was carried out at St. Luke's Episcopal Hospital to evaluate environmental contamination of Clostridium difficile in the infected patient rooms. Samples were collected from the high risk areas and were immediately cultured for the presence of Clostridium difficile . Lack of microbial typing prevented the study of molecular characterization of the Clostridium difficile isolates obtained led to a change in the study hypothesis. The study found a positivity of 10% among 50 Hospital rooms sampled for the presence of Clostridium difficile. The study provided data that led to recommendations that routine environmental sampling be carried in the hospital rooms in which patients with CDAD are housed and that effective environmental disinfection methods are used. The study also recommended molecular typing methods to allow characterization of the CD strains isolated from patients and environmental sampling to determine their type, similarity and origin.^

OBJECTIVE. To determine the effectiveness of active surveillance cultures and associated infection control practices on the incidence of methicillin resistant Staphylococcus aureus (MRSA) in the acute care setting. DESIGN. A historical analysis of existing clinical data utilizing an interrupted time series design. ^ SETTING AND PARTICIPANTS. Patients admitted to a 260-bed tertiary care facility in Houston, TX between January 2005 through December 2010. ^ INTERVENTION. Infection control practices, including enhanced barrier precautions, compulsive hand hygiene, disinfection and environmental cleaning, and executive ownership and education, were simultaneously introduced during a 5-month intervention implementation period culminating with the implementation of active surveillance screening. Beginning June 2007, all high risk patients were cultured for MRSA nasal carriage within 48 hours of admission. Segmented Poisson regression was used to test the significance of the difference in incidence of healthcare-associated MRSA during the 29-month pre-intervention period compared to the 43-month post-intervention period. ^ RESULTS. A total of 9,957 of 11,095 high-risk patients (89.7%) were screened for MRSA carriage during the intervention period. Active surveillance cultures identified 1,330 MRSA-positive patients (13.4%) contributing to an admission prevalence of 17.5% in high-risk patients. The mean rate of healthcare-associated MRSA infection and colonization decreased from 1.1 per 1,000 patient-days in the pre-intervention period to 0.36 per 1,000 patient-days in the post-intervention period (P<0.001). The effect of the intervention in association with the percentage of S. aureus isolates susceptible to oxicillin were shown to be statistically significantly associated with the incidence of MRSA infection and colonization (IRR = 0.50, 95% CI = 0.31-0.80 and IRR = 0.004, 95% CI = 0.00003-0.40, respectively). ^ CONCLUSIONS. It can be concluded that aggressively targeting patients at high risk for colonization of MRSA with active surveillance cultures and associated infection control practices as part of a multifaceted, hospital-wide intervention is effective in reducing the incidence of healthcare-associated MRSA.^

Con el objeto de ajustar un método de evaluación a campo para facilitar la selección de clones de ajo (Allium sativum L.) con resistencia a Penicillium allii, se realizó un ensayo replicado en el cual se midió la respuesta de tres clones experimentales a la infección, en dos épocas de plantación. Los tratamientos incluyeron combinaciones de presencia o ausencia de: a) desinfección de la "semilla", b) heridas artificiales en los bulbillos, c) inoculación artificial. Los bulbillos con la hoja envolvente se desinfectaron con una solución diluida de hipoclorito de sodio. La inoculación se realizó mediante la inmersión en una solución de 106 esporas•ml-1. Las heridas se realizaron con una lanceta (blood lancet). Los bulbillos se incubaron en cámara húmeda durante 24 horas a 20 °C, antes de plantación. Las variables respuestas computadas fueron las siguientes: número de plantas muertas, y vivas con síntomas y sin síntomas, a los 152 días de plantación en la primera época y 118 días en la segunda; número de bulbos por calibres y rendimiento a cosecha. La información obtenida se analizó a través de correspondencia simple, diferencia de proporciones y análisis paramétricos. Se detectaron mayores diferencias entre los tratamientos en la tasa de sobrevivencia y en la proporción de calibres comerciales en la época tardía. No se observaron diferencias (p < 0,05) del rendimiento en la plantación temprana. La herida fue determinante en el ingreso del patógeno. Los tratamientos quedaron limitados a un testigo: desinfectado, no inoculado y sin herida, y a un tratamiento: desinfectado, con inoculación y herida. Con respecto a la época, resultó conveniente la evaluación en plantaciones tardías porque se expresan mejor las diferencias entre los tratamientos.

Un incremento de la demanda del agua, junto con el aumento de la contaminación, ha provocado que hoy en día la reutilización de las aguas depuradas sea necesaria, pero la reutilización de aguas debe garantizar y minimizar los posibles riesgos sanitarios y medioambientales que su práctica pueda provocar. En España estos parámetros se encuentran regulados por el RD 1620/2007 relativo al régimen jurídico de la reutilización de las aguas depuradas. Las aguas regeneradas son aguas que ya han sido sometidas a un tratamiento de depuración, y a las cuales se aplica un posterior tratamiento adicional o complementario que permita adecuar su calidad al uso al que vaya a destinarse. Siendo requeridos para los distintos reúsos procesos de desinfección, uno de los principales sistemas utilizados es el cloro, debido a su sencilla aplicación y costos bajos, sin tomar en cuenta la posible formación de compuestos organohalogenados potencialmente cancerígenos. Es por esto que surge la necesidad de aplicar distintos sistemas de oxidación objeto de estudio en esta tesis, como el dióxido de cloro estabilizado, ozono y los procesos avanzados de oxidación (Advanced Oxidation Processes, AOP), ozono/peróxido y uv/peróxido. En esta tesis se investiga los rendimientos que pueden alcanzar estos sistemas en la eliminación de los ácidos húmicos y los fenoles, siendo las principales sustancias formadoras de subproductos de la desinfección, así mismo, se considera necesario garantizar la desinfección del agua a través del estudio de tres grupos de microrganismos, los coliformes totales, e. coli y enterococos, siendo un punto importante el posible recrecimiento microbiológico debido a una desinfección escasa, por la permanencia en el agua de los compuestos antes mencionados, o por alguna fuente de alimento que pudieran encontrar en el sistema de distribución. Lo más importante será la calidad que se pueda alcanzar con estos desinfectantes, con el fin de obtener agua para los distintos reúsos que existen en la actualidad. Y así no limitar los alcances que puede tener la reutilización de las aguas residuales. Basándose en lo antes mencionado se procedió a realizar la caracterización del agua del rio Manzanares, con el fin de determinar la cantidad de ácidos húmicos disueltos y fenoles, obteniendo valores bajos, se decidió incorporar a las muestras de rio 5 mg/L de estos compuestos, con el fin de observar de que manera podrían interferir en la desinfección de esta agua. De esta forma se logran obtener resultados óptimos de los sistemas de desinfección estudiados, siendo el Ozono un oxidante eficiente en la desinfección de los microrganismos y en la eliminación de ácidos húmicos y fenoles con tiempos de contacto cortos, mostrando deficiencias al permitir el recrecimiento de los coliformes totales. Del sistema de oxidación avanzada UV/Peróxido se determino como un eficiente desinfectante para garantizar la inexistencia de rebrotes, al paso del tiempo. Así mismo se concluye que tiene buenos rendimientos en la eliminación del ácido húmico y los fenoles. An increase in water demand, coupled with increasing pollution, has caused today reuse of treated water is necessary, but must ensure water reuse and minimize potential health and environmental risks that their practice is cause. In Spain these parameters are regulated by Royal Decree 1620/2007 on the legal regime of the reuse of treated water. The reclaimed water is water that has already been subjected to a depuration treatment, which is applied as a subsequent further treatment that will bring quality to the use to which is to be delivered. As required for various reuses disinfection processes, one of the main systems used is chlorine, due to its simple implementation and low costs, without taking into account the possible formation of potentially carcinogenic halogenated organic compounds. That is why there is a need to apply different oxidation systems studied in this thesis, as stabilized chlorine dioxide, ozone and advanced oxidation processes (AOP), ozone/peroxide and UV/peroxide. This thesis investigates the rates can reach these systems in removing humic acids and phenols, the main substances forming disinfection byproducts, likewise, it is considered necessary to ensure water disinfection through the study of three groups of microorganisms, total coliform, e. coli and enterococci, the important point being a possible regrowth due to microbiological disinfection scarce, the water remaining on the aforementioned compounds, or a food source which may be found in the distribution system. The most important quality is that achievable with these disinfectants, with the water to obtain various reuses that exist today. And thus not limit the scope that can be reuse of wastewater. Based on the above we proceeded to perform characterization Manzanares river water, in order to determine the quantity of dissolved humic acids and phenols, obtaining low values, it was decided to incorporate river samples 5 mg / L of these compounds, in order to observe how they might interfere with the disinfection of the water. Thus optimum results are achieved for disinfection systems studied, being efficient ozone oxidant in the disinfection of microorganisms and the removal of humic acids and phenols with short contact times, showing gaps to allow regrowth total coliforms. Advanced oxidation system UV / peroxide were determined as an efficient disinfectant to ensure the absence of volunteers, the passage of time. Also it is concluded that has good yields in removing humic acid and phenols.

El injerto en hortalizas es uno de los temas de más actualidad en el panorama hortícola, no solo español, sino occidental, y recalcamos occidental, pues en muchos países que no corresponden a ese ámbito, sobre todo asiáticos: Japón, Corea, China, Filipinas, etc., esta es una técnica que cuenta con una gran difusión desde hace décadas, siendo, por ejemplo en Japón, la mayoría de sus cultivos de cucurbitáceas y solanáceas realizados con planta injertada. A finales de los noventa quedó claro que el empleo de bromuro de metilo tenía una fecha de caducidad y que las zonas que tenían una fuerte dependencia de este desinfectante de suelo debían de buscar alternativas a un plazo lo más corto posible, con un punto añadido sobre etapas anteriores, debían ser alternativas lo más respetuosas posible con el medio ambiente y que no incrementaran, de forma importante, los costes de producción. En la zona centro y concretamente en los invernaderos de la Comunidad de Madrid y zonas cercanas de Toledo y Guadalajara el pepino era y es el cultivo predominante, los horticultores empleaban el bromuro de metilo de forma sistemática para desinfectar sus suelos y la desaparición de este producto les planteaba una gran incertidumbre, lo que llevó a que desde diferentes instancias se buscaran diferentes alternativas. Tras analizar las posibilidades que se podían implementar y conocido el buen resultado que había dado el injerto en sandía en Almería, se decidió acometer los trabajos que conforman esta Tesis Doctoral, planteando en la zona, diferentes ensayos con la idea de conocer, si el injerto en pepino, con los cultivares empleados habitualmente, podía ser una alternativa real para los horticultores, tanto de Madrid, como los de las zonas cercanas de Toledo y Guadalajara. Se pretendía conocer sobre todo las repercusiones agronómicas y si esta técnica podría emplearse en solitario o era necesario complementarla con otras alternativas: desinfectantes químicos, solarización, biofumigación e incluso desinfección con vapor de agua. Los ensayos fueron realizados de forma secuencial entre el año 1999 y el 2011, comprobándose en primer lugar que el empleo de portainjertos híbridos de calabaza era posible con los cultivares de pepino corto tipo español, mayoritariamente empleados en los últimos años del siglo XX y primeros del XXI, fundamentalmente: Serena. Tras los primeros ensayos, Shintoza parecía el portainjerto híbrido de calabaza (Cucurbita maxima x C. moschata) con mejores perspectivas de empleo, pues presentaba la ventaja adicional de ser bien conocido por los semilleros que producen planta injertada al ser, en esos momentos, el portainjerto más empleado en sandía, lo que garantizaba por su lado, su empleo en pepino, y que los horticultores pudiesen disponer de planta injertada. Más adelante los trabajos se encaminaron hacia la determinación de la densidad y tipo de poda más adecuado para la planta injertada, realizándose múltiples ensayos en esta dirección, que culminaron con la conclusión de que el extravigor que los portainjertos conferían a las plantas permitía conducir estas a dos o más brazos (se suelen emplear dos, por mejor adaptación a los trabajos de manejo de la planta por parte de los agricultores), con lo que se podría disminuir la densidad de planta y por tanto ahorrar en este capítulo, cosa que preocupaba y preocupa a los agricultores. Se llegó a determinar que es posible reducir la densidad de plantación en alrededor de un 25%, estando la densidad de brazos más adecuada entre 3 y 3.5 br•m-2. Tras las primeras decisiones tomadas sobre el portainjerto y la densidad más adecuada, se continuó con el estudio de adaptación de estas propuestas a los nuevos cultivares que las empresas de semillas iban proponiendo y los agricultores adoptando. Estas acciones se complementaron con la introducción de nuevos portainjertos susceptibles de sustituir a Shintoza o rotar con él para cambiar de sistema radicular, lo que es conveniente cuando se emplean, como es el caso, portainjertos que no son resistentes a nematodos, principalmente de la especie Meloidogyne incognita, el mayor problema en la zona, debido al suelo. Cultivares como Trópico, en un primer momento, y Urano y Motril más recientemente, se adaptaron muy bien a esta técnica. Entre los portainjertos que mostraron buena adaptación a la técnica de injerto y suficientemente buena compatibilidad con la mayoría de los cultivares ensayados destacan: RS-841, Strongtosa y Camel. Azman también mostró un comportamiento relevante, pero este portainjerto no podrá ser empleado, al ser recientemente retirado del mercado por la empresa que lo obtuvo y comercializó Aunque no era el objetivo principal de esta Tesis Doctoral, se ha comprobado que puede ser interesante combinar el empleo del injerto con otras técnicas alternativas al bromuro de metilo para superar los problemas debidos a enfermedades del suelo o nematodos, pero debe seguirse trabajando pues este es un tema en continua evolución, tanto si se trata de desinfectantes, a la mayoría de los cuales les está siendo retirado el permiso para su comercialización, como de otros métodos como la biofumigación o el empleo de vapor de agua. Queda muy claro que el injerto puede considerarse entre los métodos respetuosos con el medio ambiente, si no el que más, en lo que alternativas al bromuro de metilo se refiere. También en otro momento, se comprobó que con plantas injertadas es posible reducir el aporte de nutrientes, sobre todo nitrógeno, lo que además de un ahorro supone una mejora más desde el punto de vista medioambiental. En definitiva, queda demostrado que es factible el empleo del injerto en pepino corto tipo español, que las selecciones de los híbridos entre Cucurbita maxima y C. moschata que habitualmente se están empleando en sandía son también de aplicación en estos pepinos y que su empleo puede llevarnos a producciones suficientemente remuneradoras, alargándose en muchos casos el ciclo y no modificando, de forma apreciable, la calidad. Queda también demostrado que aunque los portainjertos no sean resistentes a nematodos, su extravigor les hace desarrollarse, desde el punto de vista productivo, suficientemente, llegando por tanto, a “convivir” con ese problema. Al no ser resistentes los portainjertos, y permanecer e incluso agravarse el problema de nematodos es conveniente poder contar con diferentes portainjertos que nos permitan rotar entre ellos y utilizar diferentes sistemas radiculares que harán menos fácil el parasitismo de los nematodos, como recomiendan los nematólogos que se haga. ABSTRACT Vegetable grafting is one of the most current practices in horticulture, not only in Spain, but also in other Western and Asian countries, such as Japan, South Korea, China, the Philippines, etc. This is a decades-old, widespread technique: In fact, most cucurbit and solanaceous crops in Japan and Korea are grafted. At the end of the 1990s, it was clear that methyl bromide had an expiry date. Consequently, the areas strongly dependant on this soil disinfectant had to look for alternatives as quickly as possible. Besides, these had to be as environmentally friendly as possible and should not increase production costs significantly. The cucumber has been and still is the most important crop in greenhouses of the Comunidad de Madrid and in areas near Toledo and Guadalajara. Cucumber growers used methyl bromide systematically to disinfect the soil. The banning of this chemical product brought about uncertainty, which encouraged the search for different alternatives. After analyzing the different possibilities and taking into account the good results of watermelon grafting in Almería, it was decided to carry out the works that make up this doctoral thesis. Different trials were made in order to know if the cultivars used in cucumber grafting might be a real alternative for farmers, not only in Madrid, but also in the areas near Toledo and Guadalajara. The main aim was to assess the agronomic repercussions and whether that technique could be used alone, or if other complementary alternatives, such as chemical disinfectants, solarisation, biofumigation, or even steam disinfection, were necessary. Trials were carried out sequentially from 1999 to 2011. It was observed that the use of pumpkin hybrid rootstocks could be applied to cultivars of Spanish short cucumbers, mainly grown in the late 20th and early 21st centuries eg Serena. After the early trials, Shintoza (Cucurbita maxima x C. moschata), a pumpkin hybrid rootstock, seemed to be the best option, as it had the additional advantage of being well known by nurseries growing grafting plants. Bearing this in mind, Shintoza was then the hybrid rootstock to be used in cucumbers and consequently growers could have grafted plants at their disposal. Later on, research was focused on density and the most adequate type of pruning, by carrying out several trials. These experiments showed that, the extra vigour the rootstocks gave to the plants, allowed them to have two or three stems, (normally nurserymen use two, as it is easier for them to manage the plants). These findings would lead to the lessening the density of the plant and thus reduce costs, something which worried and still worries farmers. It was stated that it would be possible to reduce the density of the plants by about 25%, the optimum density of the stems ranging from 3 to 3.5 stem-m-2. Once decisions were taken both on the rootstock and the appropriate density, we went on to study how to apply these proposals to the new cultivars which the seed companies were proposing and the farmers were applying. These measures were complemented with the introduction of new rootstocks capable of replacing Shintoza, or rotating with it in order to change the root system. This is particularly necessary when rootstocks, non-resistant to nematodes, mainly of the species Meloidogyne incognita, are used. This is the main problem due to the soil of that area. Cultivars such as Trópico, at first, and Urano and Motril, more recently, adapted quite well to this technique. Among the rootstocks which adapted well to grafting and which were compatible with most tested cultivars, were, in particular, RS-841 Strongtosa and Camel. The behaviour of Azman was worth studying, but this rootstock was removed from the market by the company which had bought and commercialized it. Although not the main purpose of the research, it was observed that combining grafting with other alternatives to methyl bromide in order to overcome problems due to soil diseases or nematodes may be worthwhile. However, further research is needed, as this topic is in constant evolution, not only when we come to disinfectants, most of which are being affected by the removal of the permit for commercialization, but also when we refer to other techniques such as biofumigation or the use of steam. Results also showed that grafted plants may reduce the amount of fertilizers, particularly nitrogen, used: This means both saving money and the protection of the environment. We may conclude by saying that grafting Spanish short cucumbers is feasible, that the selections of the hybrids between Cucurbita maxima and C. moschata, habitually used in watermelon grafting, can also be applied to these cucumbers. It can also be concluded that the use of these grafting techniques may lead to profitable yields, by lengthening the growing cycle in many cases and by maintaining the quality to a large extent. Although these rootstocks are not resistant to nematodes, the results showed that their extra vigour enables them to develop in terms of production, and thus they cope with this problem. Since these rootstocks are not resistant to nematodes and the problem with these nematodes may even worsen, it is recommended that different types of rootstocks should be available to enable both the rotation and the use of different root systems, which will encourage the parasitism of nematodes.

La escasez de recursos, la desertización creciente y el previsible calentamiento global forman un escenario físico sin precedentes que urge la revisión de las relaciones entre arquitectura y agua en la urbanización de los paisajes secos, donde la aridez es el factor limitante principal. Cuestionar la idea de escasez, la de residuo o la de confort es el punto de partida para analizar la hidrología urbana. La condición ubicua y dinámica del agua, sus continuos cambios de estado y las implicaciones energéticas y ambientales involucradas argumentan a favor de un entendimiento integral, holístico, que aborda el diseño en relación a lo paisajístico, lo infraestructural y lo ambiental como un único ámbito de investigación, planteado en términos no solo de materia o de escasez, sino de energía. Este trabajo se interesa por el rol de la arquitectura en este proyecto conjunto, integral, del espacio físico con el ciclo hidrológico donde lo orgánico, lo geológico y lo atmosférico están ligados, y donde las categorías de lo hidrológico y lo hidráulico se confunden. Las profundas implicaciones culturales en la construcción de la naturaleza urbana y en la relación con el cuerpo humano adquieren especial notoriedad en los modelos de paisaje y de espacio público adoptados, importados desde las geografías húmedas, y en la estigmatización del agua desde los presupuestos del higienismo decimonónico, que conduce al hidrofugado general de una ciudad aséptica y estéril, que se extiende hasta las envolventes de fachadas estancas y los interiores blancos y satinados. La búsqueda de referentes salta la modernidad hacia contextos de baja energía como el oasis tradicional de las tierras áridas, que concilia la formación de un ecosistema productivo con la climatización de un espacio público exterior, o los ambientes aislados de la investigación aeroespacial, con ciclos cerrados de materia. Las condiciones del ciclo del agua, como la vinculación con el suelo y el territorio, la capacidad de disolver, de mezclarse y de lavar, su volumen variable y su papel como termorregulador señalan el interés que características físicas como la fluidez, la biodiversidad, la hidrofilia y la transpirabilidad tienen para una arquitectura con competencias hidráulicas y un espacio urbano transparente a los procesos del agua. La morfología y localización, la elección de escalas, jerarquías y relaciones entre espacios y la reformulación de los sistemas constructivos aparecen como herramientas y categorías propias desde las que proponer mejores respuestas a problemas como la deshidratación, la erosión y la contaminación. Una urbanización permeable e hidrófila que absorba el agua como un recurso valioso, nuevos ensamblajes para la bioquímica urbana (que introduzcan una idea de limpieza más próxima a fertilidad que a la desinfección), y el diseño de las condiciones atmosféricas a través de una arquitectura transpirable, que se empapa y rezuma frescor, son las claves de este nuevo proyecto. La ciudad se piensa como una síntesis multiescalar de espacios hidráulicos que aporta mayor resiliencia frente a la desertificación y las condiciones climáticas extremas, y mayor visibilidad en la escena pública al agua y a las inevitables conexiones entre ecología y economía. Pero también es una oportunidad para revisar las categorías disciplinares, para renovar las consistencias materiales, las calidades ambientales y las relaciones entre el cuerpo y el espacio. ABSTRACT The shortage of resources, foreseeable global warming and increasing desertification create an unprecedented prospect that question the existing relationships between water and architecture, in the urbanization of the arid lands. The awareness of the huge volumes of water that pierce unnoticed through urban space, their impact on the landscape as well as on environmental qualities, promote a design field where cultural, social and political considerations intersect, related to the body and the physical experience of space within the built environment. Fluidity and ubiquity, solution ability, variability and cyclical processes are characteristic of water as material, directly related with the fields of potential, chemical and thermal energy and the reality of its mass, as it occupies a changing volume in space. These are also the imposing cross sections that water introduces into the project, that argue in favor of a comprehensive and holistic understanding, of addressing design in relationship to landscape, infrastructure and environmental issues as a one single area of research. This work attempts to investigate how architecture, with its specific tools, can partake in the design of water cycle in the space, linking the organic, geological, and atmospheric, blurring the lines between hydrology and hydraulic. It aims to identify issues, within the continuous query associated with water, that deal with the architectural project and may have here better results. The deep cultural implications in the construction of urban nature and the relationship with the body, acquire special notoriety in the models of landscape and public space adopted, imported from humid geographies. Also in the stigmatization of water from the premises of nineteenth- century hygienics, which lead to the entire waterproofing of an aseptic and sterile city, to the sealed facades and white and polished interiors. The search for alternative references goes beyond modernity towards a mindset of low energy, as the traditional oasis of arid lands, which aims to reconcile the formation of a productive ecosystem with the conditioning of an outdoor public space, or the controlled environments of aerospace research, with closed cycles of matter. Fluidity, biodiversity, hydrophilicity and breathability are characteristic of an architecture with hydraulic competences. The distributing phenomenon of water, its necessary connectivity to the ground and to small cycles in the ecosystems, shows strong affinities with an infrastructural architecture, as an alternative to large-scale centralized networks. Its volume has approximated to the dimensions of the built space, promoting a new found condition of coexistence. A permeable and hydrophilic urbanization absorbs water as a valuable resource; new assemblies for urban biochemistry introduce an idea of sanitation closer to fertility than to disinfection; a breathable architecture that soaks and exudes freshness design the atmospheric conditions: these are the essential components of this new project. The city is understood as a synthesis of multi-scale hydraulic spaces that provides greater resilience against desertification and increases the visibility of water and the linkages between ecology and economy in the public scene. It is also an opportunity to review the disciplinary categories of architecture, the material consistencies, the environmental qualities and the relationship between body and space.

Los procesos de biofiltración por carbón activo biológico se han utilizado desde hace décadas, primeramente en Europa y después en Norte América, sin embargo no hay parámetros de diseño y operación específicos que se puedan utilizar de guía para la biofiltración. Además, el factor coste a la hora de elegir el carbón activo como medio de filtración impacta en el presupuesto, debido al elevado coste de inversión y de regeneración. A la hora de diseñar y operar filtros de carbón activo los requisitos que comúnmente se buscan son eliminar materia orgánica, olor, y sabor de agua. Dentro de la eliminación de materia orgánica se precisa la eliminación necesaria para evitar subproductos en la desinfección no deseados, y reducir los niveles de carbono orgánico disuelto biodegradable y asimilable a valores que consigan la bioestabilidad del agua producto, a fin de evitar recrecimiento de biofilm en las redes de distribución. El ozono se ha utilizado durante años como un oxidante previo a la biofiltración para reducir el olor, sabor, y color del agua, oxidando la materia orgánica convirtiendo los compuestos no biodegradables y lentamente biodegradables en biodegradables, consiguiendo que puedan ser posteriormente eliminados biológicamente en los filtros de carbón activo. Sin embargo la inestabilidad del ozono en el agua hace que se produzcan ácidos carboxilos, alcoholes y aldehídos, conocidos como subproductos de la desinfección. Con esta tesis se pretende dar respuesta principalmente a los siguientes objetivos: análisis de parámetros requeridos para el diseño de los filtros de carbón activo biológicos, necesidades de ozonización previa a la filtración, y comportamiento de la biofiltración en un sistema compuesto de coagulación sobre un filtro de carbón activo biológico. Los resultados obtenidos muestran que la biofiltración es un proceso que encaja perfectamente con los parámetros de diseño de plantas con filtración convencional. Aunque la capacidad de eliminación de materia orgánica se reduce a medida que el filtro se satura y entra en la fase biológica, la biodegradación en esta fase se mantienen estable y perdura a lo lago de los meses sin preocupaciones por la regeneración del carbón. Los valores de carbono orgánico disuelto biodegradable se mantienen por debajo de los marcados en la literatura existente para agua bioestable, lo que hace innecesaria la dosificación de ozono previa a la biofiltración. La adición de la coagulación con la corrección de pH sobre el carbón activo consigue una mejora en la reducción de la materia orgánica, sin afectar a la biodegradación del carbón activo, cumpliendo también con los requerimientos de turbidez a la salida de filtración. Lo que plantea importantes ventajas para el proceso. Granular activated carbon filters have been used for many years to treat and produce drinking water using the adsorption capacity of carbon, replacing it once the carbon lost its adsorption capacity and became saturated. On the other hand, biological activated carbon filters have been studied for decades, firstly in Europe and subsequently in North America, nevertheless are no generally accepted design and operational parameters documented to be used as design guidance for biofiltration. Perhaps this is because of the cost factor; to choose activated carbon as a filtration media requires a significant investment due to the high capital and regeneration costs. When activated carbon filters are typically required it is for the reduction of an organic load, removal of colour, taste and / or odour. In terms of organic matter reduction, the primary aim is to achieve as much removal as possible to reduce or avoid the introduction of disinfection by products, the required removal in biodegradable dissolved organic carbon and assimilable organic carbon to produce a biologically stable potable water which prohibits the regrowth of biofilm in the distribution systems. The ozone has historically been used as an oxidant to reduce colour, taste and odour by oxidizing the organic matter and increasing the biodegradability of the organic matter, enhancing the effectiveness of organic removal in downstream biological activated carbon filters. Unfortunately, ozone is unstable in water and reacts with organic matter producing carboxylic acids, alcohols, and aldehydes, known as disinfection by products. This thesis has the following objectives: determination of the required parameters for the design of the biological activated filters, the requirement of ozonization as a pre-treatment for the biological activated filters, and a performance assessment of biofiltration when coagulation is applied as a pretreatment for biological activated carbon filters. The results show that the process design parameters of biofiltration are compatible with those of conventional filtration. The organic matter removal reduces its effectiveness as soon as the filter is saturated and the biological stage starts, but the biodegradation continues steadily and lasts for a long period of time without the need of carbon regeneration. The removal of the biodegradable dissolved organic carbon is enough to produce a biostable water according to the values shown on the existing literature; therefore ozone is not required prior to the filtration. Furthermore, the addition of coagulant and pH control before the biological activated carbon filter achieves a additional removal of organic matter, without affecting the biodegradation that occurs in the activated carbon whilst also complying with the required turbidity removal.
