Durante el decenio de los años 1980, muchas de las economías latinoamericanas se encontraron en una situación de profunda crisis, tipificada por altas tasas de inflación, un alto endeudamiento y tasas de crecimiento económico bajas, nulas o negativas. Problemas de esa naturaleza habían ocurrido antes, por ejemplo, en los años 1930, y en aquellos momentos la reacción de las autoridades a cargo de las políticas macroeconómicas condujo a un aumento en el papel del Estado, proteger la producción doméstica de la competencia extranjera y reglamentar la economía. Frecuentemente, la intervención del Estado alcanzó al sector transporte urbano, mediante, por ejemplo, el control férreo de las tarifas de los sistemas de tranvías, muchas veces operados por empresas de capital extranjero. El control tarifario condujo al debilitamiento financiero de las empresas, las que cayeron en los brazos abiertos de los gobiernos nacionales o municipales.

Estudos têm demonstrado alta sensibilidade da técnica da reação em cadeia de polimerase (PCR) na identificação do DNA do Mycobacterium leprae. Este estudo objetivou avaliar a sensibilidade da PCR na detecção do DNA do M. leprae em "swab" nasal de pacientes hansenianos e comparar os resultados com a baciloscopia e formas multibacilares (MBs) e paucibacilares (PBs). Foram coletadas amostras de secreção nasal de 24 pacientes hansenianos, conservadas em solução de lise um e dois. Os resultados da PCR foram altamente significativos (p<0.0000) e revelaram maior sensibilidade do que a baciloscopia, nas diversas formas clínicas. Contudo, são necessários ainda outros estudos, testando novos marcadores e conservantes, com o intuito de elevar a sensibilidade dessa técnica, em amostras de secreção nasal.

O efeito da idade relativa, que consiste na vantagem obtida pelo atleta nascido mais próximo ao início do ano de seleção, tem se mostrado uma variável importante para o processo de seleção de esportistas. Este estudo objetivou avaliar a influência da época de nascimento no tempo de reação de escolha de futebolistas presentes nas categorias de base (sub 13) de clubes profissionais futebol. Participaram 76 atletas com idade de 13,36 ± 0,45 anos. Os voluntários foram submetidos ao teste do TRE, e os resultados categorizados conforme o semestre de nascimento (S1 ou S2). Foi empregado o teste t para a comparação entre grupos, e análise de Correlação de Pearson para verificar a existência de associações entre as variáveis. Os resultados não indicaram diferenças entre os grupos no tempo de reação (r = 0,033 e p = 0,772) e no tempo de movimento (r = 0,0073 e p = 0,530). Concluiu-se que para essa população, a época de nascimento não influencia no desempenho dessa capacidade.

Las neuronas de la raíz coclear son las primeras neuronas del sistema nervioso central a recibir la información auditiva proveniente de la cóclea y se conectan con centros de integración sensoriomotora del tronco encefálico, especialmente con el núcleo reticular caudal del puente. Funcionalmente las neuronas de la raíz coclear están envueltos en el circuito elemental del reflejo auditivo de sobresalto juntamente con las células ganglionares del órgano de Corti, el núcleo reticular caudal del puente y las motoneuronas de la médula espinal. El reflejo auditivo de sobresalto presenta una serie de modulaciones como la habituación, la sensibilización, la inhibición por estímulo previo y la potenciación por un estímulo adverso. Las alteraciones en la reacción refleja de sobresalto y sus diferentes modulaciones poseen valor diagnóstico en la clínica médica de enfermedades neurodegenerativas y psiquiátricas como Parkinson y Esquizofrenia. Las modulaciones del reflejo auditivo de sobresalto ocurren mediante la influencia de diversos núcleos sobre los componentes del circuito elemental de este reflejo. El núcleo menos estudiado del circuito neural del reflejo auditivo de sobresalto es el núcleo de la raíz coclear. El patrón de conectividad eferente de las neuronas de la raíz coclear es bien conocido, sin embargo poco se conoce sobre los orígenes de sus aferencias y la identidad neuroquímica de las mismas. Estudios previos demostraron que el soma y en las dendritas de las neuronas de la raíz coclear están cubiertos por botones sinápticos de cuatro tipos, sugiriendo que existan diversos orígenes para estos terminales axonales, con características neuroquímicas propias. El conocimiento de las aferencias a las neuronas de la raíz coclear es relevante para comprender cual el papel del núcleo de la raíz coclear en las modulaciones del reflejo auditivo de sobresalto. La Encefalopatía Espongiforme Bovina (EEB) es una enfermedad degenerativa mortal y transmisible del sistema nervioso central del ganado, con largo período de incubación y clínicamente se caracteriza por sintomas nerviosos, la reacción exagerada a los estímulos externos y dificultad locomoción. La EEB es una enfermedad del grupo de las Encefalopatías Espongiformes Transmisibles (EET), y consiste en una enfermedad zoonótica transmitida por comer alimentos contaminados con una proteína llamada príon. Se discute el control de enfermedades, celebrada en la carne de vacuno refrigerada masacre a través de las Material Especificado de Riesgo (MER) que son el cerebro, cráneo, ojos, amígdalas, médula espinal, gânglio del trigémino, gânglios de la raiz dorsal y el íleon distal. Estos organismos pueden contener el agente de la EEB y transmitir la enfermedad. Los controles se basan en la ley brasileña y las normas internacionales que dictan la recogida y destrucción de estos materiales, sino también las normas para la comercialización de productos procedentes de rumiantes, así como la prohibición de alimentar rumiantes com productos de origen animal

Chronic osteomyelitis with proliferative periostitis is a kind of osteomyelitis that is characterized by a great periosteal reaction resulting of low grade stimulation, usually a caries lesion associated a periapical pathology. It affects especially children and young adults, with the mandible as the most common site, usually in the inferior aspect. Clinically presents as a painless hard swelling intra and/or extra-oral. Radiographic aspects include bone layers new formed, parallel to each other and to the underlying cortical surface, giving the characteristic appearance of "onion skin". Treatment includes elimination of the cause, with or without antibiotics, being restored the facial symmetry of patient. The present article is a clinical case report of chronic osteomyelitis with proliferative periostitis affecting a female 11 year old patient, caused by a caries lesion in the left permanent first molar, that was submissed to a exodontia and the facial symmetry of the patient was restored with management.

The aim of this study was to evaluate the biological behavior of the root canal filling sealers: Endofill, Endomethasone and Sealer 26 when in contact with the subcutaneous connective tissue of rat. For the study one control and three experimental groups were used. A total of 15 animals were divided into 5 for each period: 7, 21, and 60 days. The obtained histological sections were processed and stained using the hematoxiline & eosine technique. The histological sections were subjective and comparatively analyzed using optic microscopy. The intensity of the inflammatory reaction and the level of fibrosis of the tissue were registered. The results were registered in scores and statistical analysis by KRUSKAL-WALLIS p<0,05 and MILLER methods. The statistical analysis revealed that in the period of 60 days, there was statistical significance to group II (Endofill) between group (control) and III (Endomethasone) with mononuclear cells into connective tissue. All materials promoted inflammatory reaction more intense at 7 and 21 days with the Endomethasone showing the best results.

Objective: Identifying the main causes for underreporting of Adverse Drug Reaction (ADR) by health professionals. Method: A systematic review carried out in the following databases: LILACS, PAHO, SciELO, EMBASE and PubMed in the period between 1992 and 2012. Descriptors were used in the search for articles, and the identified causes of underreporting were analyzed according to the classification of Inman. Results: In total, were identified 149 articles, among which 29 were selected. Most studies were carried out in hospitals (24/29) for physicians (22/29), and pharmacists (10/29). The main causes related to underreporting were ignorance (24/29), insecurity (24/29) and indifference (23/29). Conclusion: The data show the eighth sin in underreporting, which is the lack of training in pharmacovigilance. Therefore, continuing education can increase adherence of professionals to the service and improve knowledge and communication of risks due to drug use.

Canine visceral leishmaniasis (CVL) is an anthropozoonosis characterized by a clinically chronic progressive disease. Non lymphoid organs are also affected, especially the kidneys. Dogs with leishmaniasis usually die with renal failure despite treatment. Haematoxylin-eosin (HE) staining in kidney tissue sections has low sensitivity for parasite identification. Immunohistochemistry (IHC) and polymerase chain reaction (PCR) are efficient methods for Leishmania sp. antigen and DNA detection in cases of low parasite burden. The present study aims to identify renal lesions of CVL and correlate them with microscopic findings determined by histochemistry, IHC and PCR. Both IHC and PCR provided similar positivity for amastigote identification, 3/20 animals (15%), thus increasing detection of the parasite in renal tissues when compared with histopathologic examination. The lesion most commonly observed with visceral leishmaniasis-positive canine kidney tissue was membranoproliferative glomerulonephritis, followed by interstitial nephritis without correlation to the number of amastigotes.

Enteric organisms, pseudomonads and other opportunistic microorganisms in the oral microbiota have been linked to serious infections in patients hospitalized in intensive care units (ICU). The present study evaluated the presence of family Enterobacteriaceae, Pseudomonas aeruginosa, and Acinetobacter baumannii in the mouth of patients in ICU, correlating it with oral and systemic conditions. Data on health, socioeconomic status, medication use, drug addiction, medical and family histories of patients held for more than 72 hours in the ICU with a diagnosis of severe infection or that developed this condition after entry in said unit were obtained. Fifty patients provided clinical samples of supragingival and subgingival biofilms, saliva and oral mucous membranes were collected, as well as respiratory secretions from patients with pneumonia, blood and urine for sepsis. The presence of target microorganisms was carried out by polymerase chain reaction (PCR) and by culture using selective media. The Chi-square and Mann-Whitney tests were used for statistical analysis, and the significance level was 5%. The intraoral clinical conditions of the patients were poor. The family Enterobacteriaceae was the most prevalent, affecting 39.5% of the supragingival biofilm samples of patients attended in ICU and 18.6% of patients in the control group, besides the rods were the only group found in extraoral samples.

The relationship between the occurrence of enterococci in the oral microbiota and serious infections in patients hospitalized in intensive care units (ICU) has been established. This study evaluated the presence of Enterococcus faecalis and other species of this genus in the mouths of patients on ICU, correlating it with oral and systemic conditions. Data on health and socioeconomic, medication use, medical and family history of patients maintained for 72 hours in the ICU, diagnosed with severe infection or who have developed this condition after the entry to the unit were obtained. Fifty patients provided intraoral and extraoral clinical samples for analysis (above and subgingival biofilm, saliva and buccal mucosa, followed by obtaining samples of respiratory secretions for patients with pneumonia, and blood and urine for sepsis). The presence of target microorganisms was performed by polymerase chain reaction (PCR) and culture using selective media. The chi-square and Mann-Whitney tests were used for statistical analysis, and the significance level was 5%. The intraoral clinical conditions of the patients showed poor. E. faecalis was significantly more frequent microorganism, followed by E. faecium. The use of broadspectrum antimicrobial action was associated with the presence of these opportunistic microorganisms. These bacteria were more frequent in patients with periodontitis or gingivitis. The results showed that enterococci associated with serious infectious processes may originate from resident microbiota of patients and its prevalence is not elevated in healthy individuals.

The use of antimicrobial agents has facilitated the dissemination of multi-resistant microorganisms, compromising dental and medical treatment. The aim of this study was to evaluate the distribution of different opportunistic microbial species in patients who suffered head and neck trauma, under temporary maintenance in nosocomial environment, particularly intensive care units, on the occurrence of such microorganisms in the oral cavity of the patients. It was selected 38 patients subjected to head and neck traumas. After emergency surgical procedures, clinical samples of saliva, sub and supragingival biofilms and mucosal surfaces were collected at two different moments: just after stabilization of the patient and soon after patients’ release from medical units. The presence of opportunistic and superinfecting microorganisms was evaluated by culture on selective and non-selective media, and the presence of the family Enterobacteriaceae, as well as genera Enterococcus, Pseudomonas, and Staphylococcus was assessed by PCR. It was found that the use of antimicrobials, even for short periods of time was sufficient to facilitate colonization by microorganisms of the families Enterobacteriaceae and Pseudomonadaceae, as well as yeasts and enterococci. These results support the concept that medical and dental teams should make a periodically change of antimicrobials used in treatment protocols in hospital for head and neck trauma patients, in order to minimize dissemination of opportunistic or superinfecting microorganisms.

Drug addiction won dramatic aspects in terms of its dimensions and the effects that it imposes. These chemical agents are able to reduce the immune reactivity and tissue repair, and enhance microbial aggression, aggravating the destruction of the periodontium and other side effects. This study aimed to evaluate the presence of key periodontal pathogens in the mouth of drug addiction patients, comparing it with individuals who do not exhibit this dependence, as well as assess the influence of oral conditions on the occurrence of such microorganisms. For this purpose, data on systemic health conditions, socioeconomic, patterns of licit or illicit drug consumption of 100 patients with chemical dependency kept in rehabilitation clinics and an equal number of non-dependent patients, who formed the control group were obtained. Intra and extraoral clinical examinations were performed and samples of supragingival and subgingival biofilm, saliva and mucous membranes were collected. The presence of the targeted microorganism was assessed by polymerase chain reaction (PCR). It was found that Porphyromonas gingivalis, Tannerella forsythia, and Treponema denticola showed close correlation with bone loss and gingival bleeding in drug addiction dependents and control group, but the oral mucous membranes and saliva of addicts showed higher occurrence of these pathogens.

Introduction: Post-marketing surveillance of drugs aims to detect problems related to safety, effectiveness and quality. The identification of adverse drug events (ADE) is made, mainly, by health professionals´ spontaneous reporting. This method allows risk communication in pharmacovigilance and contributes for market regulation. Objective: To estimate the prevalence of adverse drug reaction (ADR) and the suspicions of therapeutic failure (TF) reported by health professionals; to verify the active principle and type of drugs related to ADE, seriousness, causality, production mechanism and clinical manifestation of the events identified. METHODS: A cross-sectional study was performed in a teaching and public hospital which integrates the Sentinel Hospital Network, in 2008. ADR seriousness was classified according to intensity (mild, moderate, serious and lethal); drugs associated with ADE were categorized according to type (brand name drugs and non-brand name drugs); causality was imputed with Naranjo algorithm and the mechanism of occurrence was analyzed according to Rawlins e Thompson definitions (A or B). Results: There were 103 ADE reports in the period, of which 39 comprised TF and 64 ADR. Nurses reported the most ADE (53.4%). The majority of ADR were classified as type A (82.8%), mild (81.3%), possible (57.8%), according to causality assessment, and related to brand name drugs (20/35). Human immunoglobulin, docetaxel and paclitaxel were the drugs frequently associated with ADR. TF arising from no-brand name drugs (26/29), regarding, mainly, midazolam and ganciclovir. Conclusion: The results of the ADE report contribute for proposition of trigger tools for intensive monitoring of drug safety, as well as for the supplier qualification and for the improvement of quality products.
